지역사회 거주 노인의 자살경향성 유병률과 위험요인 (Prevalence and Risk Factors of Suicidality among Community-Dwelling Elderly)

  • Objectives : The purpose of this study was to investigate the prevalence and risk factors of suicidality among community-dwelling elderly. Methods : The participants were 2,201 elderly people whose ages were over 65. The participants were asked to complete questionnaires, including Mini International Neuropsychiatric Interview(MINI), module C, Short Geriatric Depression Scale of Korean version(SGDS-K), Korean Geriatric Anxiety Inventory(K-GAI), The Korean Health Status Measure for Elderly V 1.0, Korean version of Mini-Mental Status Examination for Dementia Screening(MMSE-DS). Their sociodemographic factors were investigated. The data were analyzed using the chi-square test and the logistic regression test to examine the relationship between suicidality and participants' risk factors. Results : The prevalence rate of suicidality was 23.3%. In multiple logistic regression, depressive symptoms (OR=3.301, 95% CI : 2.453-4.440), anxiety symptoms(OR=3.289, 95% CI : 2.515-4.303), low physical function (OR=1.606, 95% CI : 1.229-2.098), no spouse(OR=1.571, 95% CI : 1.037-1.690), elderly aged 80 years or older (OR=1.506, 95% CI : 1.094-1.740) were independently associated with suicidality. Conclusion : Suicidality in community-dwelling elderly was quite high, particularly related to depressive symptoms. The results of this study can be useful for development of community-based prevention and management programs for suicidality.

우울장애와 불안장애 환자들에서 자살관련 행동의 비교 (Comparison of Suicide-Related Behaviors between Depressive Disorder Patients and Anxiety Disorder Patients)

  • Objectives : By comparing the prevalence rates of suicide-related behaviors (suicide ideation, plan and attempt) between depressive disorder and anxiety disorder patients, we tried to find the characteristics of suicide-related behaviors in these patients. Methods : Four hundred-three patients participated in the study and the prevalence rates of suicide-related behaviors were investigated using Korean version of Mini International Neuropsychiatric Interview plus. Pearson Chi-Square test was used to find the association between depressive or anxiety disorders and suiciderelated behaviors. Results : Statistically significant differences were found between depressive disorder group and anxiety disorder group in terms of suicide ideation ($X^2$=6.173, df=1, p=0.013) and suicide attempt ($X^2$=8.008, df=1, p=0.005). We also found that patients in depressive disorder group were more likely to have suicide ideation (Odds Ratio=2.049, 95% Confidence Interval=1.155-3.635), and attempt suicide (Odds Ratio=4.970, 95% Confidence Interval=1.466-16.845) than patients in anxiety disorder group. Conclusion : These findings suggest that suicide ideation and suicide attempt rates are higher in depressive disorders than in anxiety disorders.

공황장애 호흡기 아형과 비호흡기 아형의 임상 특성 비교 (Comparison of Clinical Characteristics Between Respiratory and Non-Respiratory Subtypes of Panic Disorder)

  • Objectives : The goal of this study was to compare the clinical characteristics of panic disorder respiratory subtype(PD-R) and non-respiratory subtype(PD-NR). Methods : 84 patients with panic disorder were enrolled and divided into 2 groups, 29 PD-R and 55 PD-NR. Diagnosis of panic disorder was evaluated using Diagnostic and Statistical Manual of Mental Disorders $4^{th}$ edition and Mini International Neuropsychiatric Interview. They were also measured with Hamilton Rating Scale for Anxiety(HAM-A), Hamilton Rating Scale for Depression(HAM-D), and Panic Disorder Severity Scale (PDSS). Results : PD-R group showed significantly higher scores in PDSS than those of PD-NR group(p=.027). After controlling for the severity of panic disorder and gender, PD-R group showed higher HAM-D and somatic anxiety subscale of HAM-A than those of PD-NR group. Furthermore, results of logistic regression analysis suggested that the somatic anxiety was a possible risk factor of PD-R(OR=1.404,p=0.009). Conclusion : These results suggest that somatic anxiety and depressive symptom would be important clinical characteristics of PD-R.

A Study on Factors Affecting Suicidal Tendency of the Elderly Living Alone on Care Services

  • Objectives The high suicide rate has risen as a main concern in South Korea. Given the complexity of the mechanism resulting in complete suicide, studies targeting various populations are needed for broader understanding of its risk factors. This study aims to analyze the factors affecting the suicidal tendency in the population of the elderly living alone in Seoul depending on basic elderly-care services. Methods A total number of 415 people participated in the study. Home-visit interviews were administered by trained interviewers. Suicidal tendency was measured by the Korean version of mini international neuropsychiatric interview (K-MINI). Health-related quality of life was measured by a brief version of the World Health Organization Quality-of-life Scale (WHOQOL-BREF). Alcohol problem was assessed by the Korean version of the alcohol use disorders identification test (AUDIT-K). Among total subjects of 415 interviewees, the actual responses of 408 people were used for the final analysis. Results The result showed that the suicide risk was high in persons isolated from social support [odds ratio (OR) = 4.49], having depression (OR = 14.85), and having low quality of life (OR = 4.39). Conclusions We found that social support, depression and health-related quality of life are associated with suicidal tendency in the elderly living alone on basic services. Our evidence will contribute to suicide prevention policy for the elderly living alone on care services.

거주형태에 따른 노인 우울증과 자살경향성 비교 연구 (Geriatric Depression and Suicidality According to Residence Type among the Elderly in a Rural City Area)

  • 본 연구는 이천시에 거주하는 65세 이상의 인구를 대상으로, 우울증의 유병률과 자살경향성, 우울증과 자살경향성에 영향을 미치는 위험요인을 살펴본 연구이다. 전체적으로 우울증의 유병률은 28% 정도로 파악되어 실제로 65세 이상의 연령대에서 상당수의 노인들이 임상적으로 의미 있는 우울증을 경험하고 있는 것으로 관찰되었다. 또한 자살경향성은 전체 조사대상자의 약 20%에서 자살생각, 자살의도, 자살계획, 자해의도, 자살시도 중 한 가지 이상을 지난 한 달 동안 경험했던 것으로 나타났다. 거주 형태별로 구분하여 비교 분석한 결과, 혼자 생활하거나 시설에서 생활하는 노인들에서 동거가족이 있는 노인들보다 우울증의 유병률이 높고, 증상도 심한 것으로 나타났으며, 이들에게서 자살경향성 역시 높은 것으로 파악되어 이들 고위험군에 대한 보다 집중적인 우울증의 조기 발견 및 자살예방사업이 필요할 것으로 판단되었다. 위험요인 분석에서는 고령, 시설거주, 낮은 사회적 지지 수준은 우울증의 위험요인으로, 독거생활, 시설거주, 낮은 사회적 지지 수준, 두통의 기왕력은 자살경향성의 위험요인인 것으로 파악되었다.

공황장애의 발병연령에 따른 정신과적 공존질환의 차이 (Difference in Psychiatric Comorbidity of Panic Disorder According to Age of Onset)

  • Objectives : It is reported that panic disorder is frequently comorbid with other psychiatric illnesses. The aim of this study was to investigate differences of psychiatric comorbidity according to age of onset of panic disorder. Methods : Three hundred-two patients participated in the study. All the patients were evaluated by clinical instruments for the assessment the presence of other comorbid psychiatric disorders and various clinical features; Korean version of Mini International Neuropsychiatric Interview, Self-report questionnaires(Beck Anxiety Inventory, Beck Depression Inventory, Anxiety Sensitivity Index and State-Trait Anxiety Inventory) and clinical rating scale (Hamilton Anxiety Scale, Hamilton Depression Scale and Global Assessment of Functional score). Chi-square test was used to determine the difference between early onset and late onset panic disorder. Results : Forty percent of panic patients were found to have at least one comorbid psychiatric diagnosis. There were no differences among the groups divided by number of comorbidity in sex, agoraphobia comorbidity, duration of panic disorder, except onset age of panic disorder. Early onset group had more comorbidy with social phobia, agoraphobia, PTSD. We also found that Early onset panic disorder patients were more likely to experience derealization, nausea, parethesia than late onset panic disorder patients. Conclusion : The results of our study are in keeping with previous data from other parts of the world. Our finding suggest that earier onset of panic disorder related to more psychiatric comorbidity.

일 대학병원 외래 내원 사회공포증 환자에서 증상적 특성의 남녀차이 (Gender Differences in the Symptomatic Characteristics of Social Phobia Patients in One University Hospital)

  • Objectives : Gender differences in demographic characteristics, symptom presentation, and severity may influence the selection of target behaviors as well as the course and outcome of therapeutic interventions for patients with social phobia. The purpose of this study was to explore the gender differences in the symptomatic characteristics of Korean patients with social phobia. Methods : A total of 256 outpatients with social phobia at Kangbuk Samsung Hospital were included in this study. All subjects were evaluated using the Korean version of the MINI International Neuropsychiatric Interview Plus. Potential differences in demographic characteristics, severity of anxiety, and situational fear and avoidance were examined. Self-report questionnaires and interviewer-administered instruments, including the Social Phobia Scale (SPS), Social Phobia subscale of the Fear Questionnaire (FQ-social), Appraisal of Social Concerns (ASC), and Liebowitz Social Anxiety Scale (LSAS), were used to evaluate the severity of the patients' symptoms. Results : There were no gender differences in demographic characteristics, age, age at onset of social phobia, total SPS score, FQ-Social score, ASC score, and LSAS score. In addition, there was no gender difference in the severity of fear in specific social situations. However, women reported significantly higher total scores on the social helplessness category of the ASC than men (p=0.009). Conclusion : Previous studies reported that the severity of social phobia symptoms is greater in women than in men, but our results suggest that there are no significant gender differences in the severity of social phobia symptoms. Thus, gender differences among patients with social phobia are discussed in the context of traditional sex-role expectations.

Clinical Utility and Cut-Off Scores of the Korean Adult Attention-Deficit/Hyperactivity Disorder Rating Scale

  • Objectives: This study was conducted to re-validate the clinical efficacy of the Korean Adult Attention-Deficit/Hyperactivity Disorder (ADHD) Rating Scale (K-AARS), which is a self-report scale for ADHD in adults, and to determine the clinical utility and cut-off scores of K-AARS. Methods: The participants were 135 drug naïve adults with ADHD and 144 healthy controls. To diagnose ADHD based on the diagnostic criteria of the Diagnostic and Statistical Manual of Mental Disorders, 5th Edition, two board-certified pediatric psychiatrists interviewed the participants and completed the Mini International Neuropsychiatric Interview. K-AARS was applied to all participants. K-AARS comprises six clinical subscales, one impairment subscale, and one driving behavior subscale. The receiver operating characteristic analysis was conducted to calculate the cut-off scores of K-AARS. Results: All subscale scores, including six clinical subscale, impairment subscale, and driving behavior subscale scores, were found to be significant in distinguishing adults with ADHD from healthy controls. The sensitivity and specificity of the six clinical subscales were 63.0-77.0% and 66.7-79.9%, respectively. The combined total score of the six clinical subscales, had a sensitivity of 80.0% and specificity of 79.9%. Conclusion: The discriminative power of K-AARS for the diagnosis of ADHD in adults was excellent, and K-AARS and the empirical diagnosis of adults can be useful in diagnosing ADHD in adulthood.

Associations Between Parental Depression and Early Childhood Development in Indonesia: A Cross-sectional Study

  • Objectives: This study aimed to determine the associations between parental depression and early childhood development among children aged 36 months to 59 months in Indonesia. Methods: From Indonesia's Basic Health Survey (RISKESDAS) 2018, this study included 6433 children aged 36 months to 59 months and their parents. Maternal and paternal depression was examined using the Mini International Neuropsychiatric Interview survey instrument, which was previously translated into Indonesian. The study also used the Early Child Development Index to measure child development and its 4 domains (cognitive, physical, socio-emotional, and learning). Multivariate logistic regression analysis was performed to determine the association between parental depression and early childhood development. Results: Overall, 10.3% of children aged 36 months to 59 months were off-track for development. After adjusting for biological, parental, and social characteristics, children born to parents with depression were found to be 4.72 times more likely to be off-track for development (95% confidence interval, 1.83 to 12.15). Conclusions: Children of depressed parents were more likely to be off-track for development. The findings highlight the need for early diagnosis and timely intervention for parental depression to promote early childhood development.

조현병의 질환단계에 따른 자살경향성과 정신병리의 관계 (Suicidality and Related Psychopathology across Different Stages of Schizophrenia)

  • Objectives: This study aimed to investigate suicidal behaviors and the related psychopathology across the different stages of schizophrenia. Methods: We recruited 131 patients with schizophrenia and categorized them into two groups, according to the duration of illness (DI) as follows: ≤10 years (n=39) and >10 years (n=92). Psychopathology and suicidality were assessed using the 18-item Brief Psychiatric Rating Scale (BPRS-18) and the suicidality module from the Mini-International Neuropsychiatric Interview, respectively. Results: One-quarter of the patients with a DI ≤10 years and nearly one-sixth of the patients with a DI >10 years experienced suicidal behaviors in the previous month. Suicidality scores were significantly associated with the "affect" factor scores of the BPRS-18 in patients with a DI ≤10 years (β=0.55, p=0.003) and with the "resistance" factor scores in patients with a DI of >10 years (β=0.29, p=0.006). Conclusion: The present study demonstrated that psychopathological factors were differentially associated with suicidality in patients with schizophrenia according to the illness stage. Our findings suggest that for effective suicide prevention, different approaches are required for the management of each stage of schizophrenia.
