A Case of Pulmonary Choriocarcinoma (폐의 융모상피암 1예)

  • Pulmonary choriocarcinoma is a very rare tumor in men. Herrein, the case of a pulmonary choriocarcinoma in 39-year-old man, and whether it had a primary nature, is reported. He denied any prior medical illness, but was admitted to our hospital with a history of a cough, and progressive dyspnea and hemoptysis 2 and 1 week duration, respectively. Chest radiographs on admission revealed a huge lung mass, 10 cm in diameter, in the left upper lung field, with left pleural effusion. Although biopsies using several diagnostic methods for the pathological confirmation were attempted, the pathology was not confirmed. Finally, the patient died after 2 months of regression. An autopsy of the lung was then performed.

Prognostic Significance of 14-3-3γ Overexpression in Advanced Non-Small Cell Lung Cancer

  • The 14-3-3 protein has been shown to be involved in the cancer process. However, there is no understanding of the relationship between 14-3-$3{\gamma}$ (14-3-3 gamma) expression and prognosis in advanced non-small cell lung cancer. In this study, we therefore investigated the association between protein levels by immunohistochemistry and clinicopathological features of advanced NSCLC patients. Survival curves were estimated using the Kaplan-Meier method and tested by log-rank. Multivariate analysis was conducted with the Cox's regression model to determine independence of factors. p values less than 0.05 were considered significant. A total 153 patients were studied, with 54.3% being stage III and 45.8% stage IV. Fifty-one cases (33.3%) were squamous cell carcinomas, and 98 cases (64.1%) were adenocarcinomas. High 14-3-$3{\gamma}$ expression was seen in 59.5% and significantly correlated with lymph node metastasis (p=0.010) and distant metastasis (p=0.017). On Kaplan-Meier analysis, high 14-3-$3{\gamma}$ expression was associated with poorer survival with a marginal trend toward significance (p=0.055). On multivariate analysis, age, treatment, and 14-3-$3{\gamma}$ expression proved to be independent prognostic parameters. In vitro experiments indicated that 14-3-$3{\gamma}$ overexpression also played a potential role in cancer invasion. In conclusion, our data suggest that 14-3-$3{\gamma}$ overexpression is associated with invasion and a poor prognosis. Therefore, 14-3-$3{\gamma}$ may be a potential prognostic marker of advanced non-small cell lung cancer.

  • Pulmonary blastoma is a primary lung tumor consisting of a mixture of immature embryonal-like mensenchymal and epithelial components and in some areas resemble embryonal lung tissue of up to 3-4 months gestational age. The tumor was first described in 1945 by Barren and Barmard and again in 1952 by Barmard, who named it an embryoma of the lung which resembled fetal lung histologically, with glandular structures lined by non-ciliated epithelium and a surrounding stroma resembling mesenchyme. In 1961 Spencer renamed them Pulmonary blastoma as he believed they represented neoplasms similar in pathogenesis to nephroblastoma. The prognosis of these rumors is poor, and the clinical course is not readily predicted from histological appearance. Untill now, more than one hundred cases have been reponed in the literature in the word and there is no report in Korea. We report a case of biphasic pulmonary blastoma, which ruptured spontaneously, in a 31-year-old young man who was presented as sharp pain under the right subscapular area.

  • Pulmonary nodular lymphoid hyperplasia is a reactive lymphoproliferative disease. It is very rare, which means that many aspects of the disease are unknown or have not been proven. Pulmonary nodular lymphoid hyperplasia can be symptomatic or asymptomatic, progressive or not, and solitary or multiple, and a surgical approach is the current treatment of choice. We present a case of pulmonary nodular lymphoid hyperplasia that was visualized as multiple ground glass opacities on a computed tomography (C T) scan, and observed for 1 year because the patient was pregnant. Over this period, the number and extent of the opacities progressed, but no symptoms were reported. A surgical biopsy was done and some remaining lesions regressed on follow-up CT scans, while others progressed, without any appearance of symptoms.

  • A 46 years old male showed radiologically a single cavitary nodular lesion in right upper lung, which extended to the regional chest wall. This finding has to be made into differential diagnosis of numerous pulmonary diseases including infections such as mycobacterial, fungal or bacterial, granulomatous diseases, and neoplasms. For the definite diagnosis, fine needle aspiration biopsy guided by biplane fluoroscopy was performed. The aspirates contained several sulfur granules, in the center of which many gram positive, filamentous organisms were compactly intermingled. Such a findings was compatible with pulmonary actinomycosis. Now the lesions is cleared out by medical treatment with amoxicillin for 3 months.

  • In order to evaluate the antiallergic effects of Kamiohihyosan(KCHS), studies were done. We measured the cytotoxic activity for lung fibroblast cell, cytokines transcript expression, production of IL-4, IL-10, IFN-γ, proliferation of B cell in anti-CD40mAb plus rIL-4 stimulated murine splenic B cells. The results were obtained as follows: KCHS was not showed cytotoxicity in the fibroblast lung cell, KCHS increased the gene synthesis of INF-γ, TNF-α, IL1-β, IL-6, IL-10(m-RNA), KCHS decreased the gene synthesis of IL-4, IL-5, TGF-β(m-RNA), KCHS decreased the appearance of IL-4, IgE significantly, KCHS increased the appearance of IL-10, IFN-γ significantly, KCHS decreased the proliferation of B cell significantly, The facts above prove that KCHS is effective against the allergy. Thus, I think that we should study on this continuously.

Effects of Wikyung-Tang on the Lipopolysaccharide-Induced Acute Lung Injury in Mice (위경탕(葦莖湯)이 LPS로 유발된 급성 폐손상에 대한 영향)

  • Wikyung-Tang(WKT) is herbal medication used in abcess-causing respiratory disease. Previous in vitro study demonstrates that WKY presents anti-proliferative effects in A549 cells. Here we show that WKY protects mice against lipopolysaccharide(LPS)-induced acute lung injury (ALI). We pretreated mice orally with WKY(2.34 and 5.85 g/kg body weight) 1, 24 and 48 hours before intratracheal administration of LPS. For same condition, control group was pretaken orally distilled water before LPS administration. 24 hours after LPS intratracheal instillation, bronchoalveolar lavege fluids(BALF) was obtained to measure protein and proinflammatory cytokines(TNF-${\alpha}$, IL-$1{\beta}$, IL-6). Protein and proinflammatory cytokines in BALF of WKT treated groups were totally decreased. Statistically, Protein, TNF-${\alpha}$ and IL-$1{\beta}$ of high concentrate WKT treated group decreased significantly compared with control group. In conclusion, WKY had some anti-inflammatory effect in a clinically relevant model of ALI. these results indicated that WKY was effective in inhibiting ALI and might act as a potential therapeutic reagent for treating ALI in the future.

  • Pulmonary mucormycosis is an uncommon opportunistic fungal infection associated with diabetis mellitus, leukemia, lymphoma & other debilitating diseases. Their clinical presentations depend on the associated underlying disorders. Generally patients with leukemia and lymphoma often have rapidly progressive clinical course, and may cause diffuse parenchymal disease refractory to medical and surgical therapies. However, some diabetics with pulmonary mucormycosis have a striking tendency to develop a localized endobronchial disease in major airway which is amenable to therapy with surgery and Amphotericin B. We have experienced a case of endobronchial mucormycosis complicating lung abscess associated with diabetic ketoacidosis, which was cured without specific antifungal therapy or surgery. A brief review of the literature was given.

The Radix Asparagi is used in a range of $2g{\sim}16g$ per dose with 4g being the typical dose for most treatments. 5. The Radix Asparagi is used as a fundamental prescription with several herb remedies for the pathology. The Radix Asparagi has been used to moisten the dryness and to keep the balance among fever of heart, lung, and kidney. According to the results. I suggest to use Radix Asparagi in a various pathogenic fields. The Radix Asparagi is able to use to improve not only fatigue. but also diabetes, cataract, and debility of blood.

  • The anticarcinogenic effects of allyl methyl trisulfide(AMT) and methyl propyl disulfide(MPD) were studied in a 28 weeks rat multi-organ carcinogenesis model. Tumor incidence rate was decreased by AMT or MPD treatment comparing with the positive control. AMT treatment significantly decreased the incidence of neoplastic and preneoplastic lesions in the kidney thyroid gland urinary bladder alimentary tract lung and Zymbal's gland. MPD also inhibited incidence of noplastic and preneoplastic lesions in the liver kidney urinary bladder alimentary tract lung and Zymbal's gland but increased that in the thyroid gland. GST-p positive foci in the liver were slightly decreased by AMT or MPD. There was no significant histopathological lesions in AMT or MPD treated group without pretreatment of carcinogens.
