저출산은 생산인구의 감소와 함께 보험료 부담을 가중시키는 문제 뿐 아니라, 종국에는 국가의 존립을 위협하는 묵과할 수 없는 국가적인 정책과제가 되었다. 이에 본 연구는 현 정부의 출산장려정책 중 경제적 지원인 고운맘 카드의 실효성을 알아보고 산모들의 인식조사를 바탕으로 도출된 문제점에 대한 개선안을 제언하고자 한다. 분석방법은 인식도 조사를 위해서는 빈도분석을 이용했으며, 출산장려정책에 대한 선호도를 파악하기 위해 다중응답분석, 고운맘 카드 사용에 대한 만족도와 재출산 의사정도를 알아보기 위한 카이제곱 테스트를 시행했다. 분석결과, 고운맘 카드 사용으로 인한 산모들의 만족도와 재출산 의사간에는 유의한 관련성을 보였다. 또한, 산모들은 정부기관 및 의료기관과 구전으로 전파된 내용에 의해 해당 정책에 접근하는 것으로 나타났다. 그리고 정부의 지원금액 범위 내에서 병원 청구할인서비스 정도만 활용하는 것으로 나타났다. 그러나 지원금액 및 l일 사용 한도금액과 같은 금전적 요인에서 주로 불만을 표출했다. 그에 따른 상향 조정된 희망 개선안을 요구했다. 위의 결과를 토대로, 정부는 정책시행의 궁극적 목적인 출산률의 상승을 위해 고운맘 카드를 개선 발전시켜나가야 할 것이다.
목적 본 연구는 신생아중환자실에 입원해 있는 저출생체중아를 대상으로 '어머니의 노래 들려주기 중재'의 효과를 규명하기 위해 수행되었다. 방법 서울 시내에 소재하는 일개 대학병원의 신생아중환자실에 입원한 대상자를 임의표출하여 비동등성 대조군 전후설계로 총 48명의 영아중 실험군 24명, 대조군 24명을 대상으로 하여 본 연구를 수행하였다. 결과 어머니의 노래 들려주기 중재를 제공받은 저출생체중아는 활력징후의 생리적 반응인 심박동수, 호흡수와 산소포화도에서 실험군과 대조군 사이에 통계적으로 유의한 차이가 없었다. 행동상태 점수는 두 집단 간 통계적으로 유의한 차이가 검증되었다. 즉, 노래 들려주기 중재를 제공받은 저출생체중아는 행동상태가 안정되는 결과를 보였다. 결론 본 연구를 통해 신생아중환자실에서 오랜 기간 치료를 받아야 하는 저출생체중아를 위한 긍정적인 청각자극으로서 어머니의 노래 들려주기 중재를 활용할 수 있는 객관적인 근거자료를 마련하였다는 데 의의가 있다. 또한 이러한 중재를 통해 재원기간 동안 저출생체중아의 돌봄에 어머니의 참여를 독려할 수 있을 뿐만 아니라 영아와 부모 사이의 이른 상호작용을 증진시키는 기회를 제공할 수 있다는 점에서 아동간호실무의 발전에 기여할 수 있을 것이다.

Data on 2,365 Mecheri sheep (1,201 males and 1,164 females), maintained at the Mecheri Sheep Research Station, Pottaneri, India, and recorded between 1979 and 2006, were analysed to study the growth related traits and their genetic control. The body weights at different ages (i.e. at birth, weaning (3 months), 6, 9 and 12 months) were recorded and collected from the birth and growth registers maintained in the farm. The average weights of Mecheri sheep at birth, and at 12 months of age were $2.24{\pm}0.01$ and $16.81{\pm}0.15$ kg respectively. The pre- and post-weaning average daily weight gains were $63.84{\pm}0.75$ and $29.52{\pm}0.43$ g respectively. Study revealed a significant difference with the period of lambing on body weight, weight gain and efficiency in weight gain at different stages of growth. Males were heavier and had a higher weight gain than females at almost all stages of growth and the differences tended to increase with age. The direct heritability estimates increased from birth to six months of age and then decreased. The direct heritabilities of all body weights at different stages of growth were low to moderate in magnitude and the values at birth, weaning, six, nine and 12 months of age were 0.08, 0.17, 0.21, 0.13 and 0.10 respectively. For the estimation of heritability at birth and three months body weights, the direct additive genetic and maternal additive genetic effects have to be taken into account and for the estimation of six months weight, the direct additive genetic and maternal permanent environmental effects have to be included in the model. The estimates of heritability, phenotypic and genetic correlations among the different body weights indicated that the selection for improving the body weights at different traits should be done on the basis of three or six months weight because of higher heritability estimates and having higher genetic correlations with other traits.
To assess the effect of an antenatal nutritional status on pregnancy outcome, especially neonatal birty weight, one-day 24hr-recall and two-day recording methods for dietary survey and interview for general and obstetric characteristics of each subject were completed and pregnancy outcome was recorded by phone after delivery. 147 pregnant women attending routinely public health centers in Ulsan were divided into 1st trimester(n=36), 2nd trimester(n=102), 3rd trimester(n=71) by LMP(Last Menstrual Period) because some subjects attended repeatedly in different trimester. The subjects were aged 27.9$\pm$2.9 as mean and the level of education was senior high school and more. 20.4% of subjects experienced spontaneous abortion and 30.0% experienced induced abortion in previous pregnancy. Mean intakes of all nutrients except ascorbic acid were significantly different but dietary composition of energy intakes was not different between trimester. Mineral of calcium, iron and zinc did not meet the RDA for pregnancy outcome was about 20%, which consists of spontaneous abortion (3.4%), caesarian section(15.6%), premature delivery(0.7%) and still births(0.7%). The mean birth weight of neonates is 3.31kg the rate of neonatal birth weight below 10th percentile was 8.4% and the rate of low birth weight(<2.5kg) was 3.1%. By analysis of nutrient factors that influence on the neonatal birth weight (NBW), iron intake correlated negatively and zinc intake correlated positively with NBW in 1st trimester but fat and iron intakes correlated with NBW positively in 3rd trimester. Prepregnancy weight, gestational age at delivery and No. of induced abortion had a positive effects on NBW and No. of spontaneous abortion and te severity of morning sickness had a negative effects on NBW.

Objective: The present study analyzed the influence of co-transferring embryos with high and low cloning efficiencies produced via somatic cell nuclear transfer (SCNT) on pregnancy outcomes in dogs. Methods: Cloned dogs were produced by SCNT using donor cells derived from a Tibetan Mastiff (TM) and Toy Poodle (TP). The in vivo developmental capacity of cloned embryos was evaluated. The pregnancy and parturition rates were determined following single transfer of 284 fused oocytes into 21 surrogates and co-transfer of 47 fused oocytes into four surrogates. Results: When cloned embryos produced using a single type of donor cell were transferred into surrogates, the pregnancy and live birth rates were significantly higher following transfer of embryos produced using TP donor cells than following transfer of embryos produced using TM donor cells. Next, pregnancy and live birth rates were compared following single and co-transfer of these cloned embryos. The pregnancy and live birth rates were similar upon co-transfer of embryos and single transfer of embryos produced using TP donor cells but were significantly lower upon single transfer of embryos produced using TM donor cells. Furthermore, the parturition rate for TM dogs and the percentage of these dogs that remained alive until weaning was significantly higher upon co-transfer than upon single transfer of embryos. However, there was no difference between the two embryo transfer methods for TP dogs. The mean birth weight of cloned TM dogs was significantly higher upon single transfer than upon co-transfer of embryos. However, the body weight of TM dogs did not significantly differ between the two embryo transfer methods after day 5. Conclusion: For cloned embryos with a lower developmental competence, the parturition rate and percentage of dogs that remain alive until weaning are increased when they are co-transferred with cloned embryos with a greater developmental competence.
Mimicking fetal nutrition is the goal of early paretneral nutrition (PN) in very low birth weight infants, however the limited metabolic capacity of immature organs raises concern about the toxicity of metabolites to the developing brain. Starting parenteral amino acids from the first day of life, with a rate of 1.0 to 1.5 g/kg/day, is generally recommended to prevent endogenous protein breakdown by maintaining a positive nitrogen balance. A greater of amino acid infusion rate in the range of the fetal transfer rate (3.5-4.0 g/kg/day) is well tolerated during the early days after birth in VLBWI, however the influence on growth and long-term neurodevelopmental outcome remains unknown. Limited data are available from controlled trials regarding the effects of early supplementation with lipid emulsions on neonatal morbidity. Considering the role of long-chain polyunsaturated fatty acids in the neurodevelopment, the choice of an optimal lipid emulsion should be based on the quality as well as the quantity of the lipid contents. Little is known about the clinical benefit of higher rates of glucose infusion by permitting high serum glucose level or co-administration with insulin.
목적: 한국에서는 최근 저 출산율의 지속과 더불어 미숙아 및 저체중출생아의 출생 빈도가 증가하는 경향을 보인다. 이러한 변화에도 불구하고 지속적으로 신생아중환자실의 병상수가 부족한 상태이다. 이 연구의 목적은 2009년 국가통계포털의 통계청 자료에 의거한 총 출생아 수, 미숙아, 저출생체중아 수 NICU 등록 병상 수를 행정지역별로 검토하여 보다 나은 NICU 운영 및 미숙아 신생아의 생존 향상에 도움을 주고자 하였다. 방법: 한국통계포탈에 공개된 2009년도 인구통계 중 미숙아수, LBWI, 출생통계부분, 총 출생 수, 미숙아 수, LBWI 수, VLBWI 수를 얻었고, 전국 및 지역별 NICU 병상 수의 자료는 한국 건강보험심사평가원에서 획득하였다. 한국의 행정구역 지역별, 미숙아 수, LBWI 수, VLBWI수에 따른 병상수의 비와 각 평균에 대한 부족 지역 등을 조사하였다. 결과: 2009년 전체 출생 수 444,849명 중 미숙아 25,374명(5.7%), LBWI 21,954명(4.9%) 이었으며 미숙아나 LBWI 의 빈도가 평균보다 높은 지역은 부산, 인천, 광주였다. 2009년 NICU 총 병상 수는 1,284병상 이었으며 1병상 당 미숙아나 LBWI의 비율이 평균보다 높은 지역은 인천, 대구, 울산 이었다. 2009년 조사된 NICU 병상보유율은 87.5% (1,284/1,486)로 2005년보다는 증가하였으나 12.5%인 184병상이 여전히 부족하다. 특히 경기도는 159병상이 부족하였다. 결론: 신생아는 환자의 특성상 출생 후 이동의 어려움이 있어 즉각적이고 지속적인 처치가 중요하다. 이를 위해 전국적으로 균형 잡힌 NICU의 지역화가 필수적이다. 본 연구를 통해 NICU의 현황 및 시도별 단위에 대한 부족 병상 수에 대한 기본 자료를 제시하는 바이다.
Recently, low birthrate has been recognized as a social problem, which is expressed as catastrophy of population. There are several reasons why low birth rate has occurred directly and indirectly, generating total economic and social burden for our society. That is, low birthrate should not be simply regarded as reduction of population. The present study identified the reasons of low birthrate and the current status of infertility, and examined practical and theoretical issues in terms of low birthrate. It is our contention that low birthrate and infertility is one of the most important governmental issue to enhance quality of life of the public. Thus, integrated and systematic policy must be developed and implemented to effectively meet the issues.
The adolescent birth rate has doubled in the recent decade. As a result, the Korean government has implemented support for adolescent single-parent families by expanding the scope of coverage of the Single-parent Family Support Act in 2010. In order to understand whether experiences of facility entry programs and the Single-parent Family Support Act for adolescent single-parents were helpful or not, this study verified the relationship to mental health. This study utilized a part of the National Youth Policy Institute's 'Research on the Actual Condition of the Adolescent Pregnancy, Birth and Rearing Children.' Participants of the study were 218 adolescent single-parents at the age of 24 and younger. This study used the research methods of descriptive statistical analysis, ${\chi}^2$test, t-test, and one-way analysis of variance. The findings and discussion of this study are as below: Firstly, the depression level of adolescent single-parents who had been raised by grandparents was higher than other family types to a meaningful standard and adolescent single-parent's cognition of the original family's economic level was related to self-esteem and parenting efficacy. Secondly, the practical single-parent family support policy for adolescent single-parent's pregnancy, birth and rearing children has shown a higher reception rate, and medical service of facility entry programs has a higher satisfaction level. However, some of the facility entry programs and the Single-parent Family Support Act have a low reception rate and it should be considered whether to maintain them or not.Lastly, parts of facility entry programs and the Single-parent Family Support Act for adolescent single-parents have increased self-esteem and parenting efficacy and reduced parenting stress and depression's levels.
Korean government enacted the 1st Basic Plan for Low Fertility and Population Ageing in 2005 due to the long-lasting ultra-low birth rate. Based on the Act, Korean government set up a step-by-step strategic goal and establish a basic plan, every five years, to achieve the sustainable development of society corresponding to low fertility and aging society. Over the past decade, the 1st Basic Plan for Low Fertility and Population Ageing (2006-2010) and the 2nd Basic Plan for Low Fertility and Population Ageing (2011-2015) was executed. This year, based on the achievements and limitations of the 1st and 2nd Basic Plan, the 3rd Basic Plan for Low Fertility and Population Ageing begins. In this manuscript, I discuss the background of the 3rd Basic Plan and its significance in oral health area.
