The Effect of Protective Socks with Functional Insoles on Plantar Foot Pressure in Diabetes Patients

  • Purpose: The most common cause of plantar ulceration is an excessive plantar pressure in patients with peripheral neuropathy. Foot orthosis and therapeutic footwear have been used to decrease the plantar pressure and prevent the plantar ulceration in in diabetes patients. We investigated whether protective sock with functional insoles reduce plantar pressure while walking in 17 diabetes patients. Methods: An in-shoe measurement device was used to measure the peak plantar pressure while walking. Peak plantar pressure data were collected while walking under two conditions: 1) wearing diabetic sock and 2) wearing the protective sock with functional insoles. Each subject walked 3 times in 10-m corridor under three conditions, and data were collected in 3 steps in the middle of corridor with in right and left feet, respectively. Pared t-test was used to compare the peak plantar pressures in three plantar areas under these two conditions. Results: The protective sock with functional insoles significantly reduced the peak plantar pressure on the lateral rearfoot, but significantly increased the peak plantar pressure on the middle forefoot, and medial midfoot (p<0.05). However, there were not significant in medial and lateral forefoot, lateral midfoot, and medial rearfoot between diabetic sock and the protective sock conditions (p>0.05). Conclusion: The protective sock with functional insoles reduced plantar pressures in the rearfoot and supported the medial longitudinal arch. However, it is necessary to change the position of metatarsal pad in the insole design of forefoot area to prevent diabetic foot ulceration.

제1유구치의 조기 상실로 인한 공간 변화에 대한 3차원적 분석 (THREE-DIMENSIONAL SPACE CHANGES AFTER PREMATURE LOSS OF THE PRIMARY FIRST MOLAR: A LONGITUDINAL STUDY)

  • 본 연구는 제1유구치의 조기 상실 시공간 변화에 대한 분석을 위하여 3-Dimensional Laser Scanner를 이용하여 그 유용성을 평가하고자 하는 예비논문이다. 6세-8세 사이의 제1대구치가 맹출하여 확실한 교합 관계를 이루는 혼합치 열기 어린이 중 편측 제1유구치의 조기 발치가 필요한 6명의 발치 전 후의 모형을 이용하여 다음과 같은 결과를 얻었다. 1. 유구치 공간변화는 상악은 실험군과 대조군 모두 현저한 감소를 보이지 않았고, 하악은 실험군에서 3명 중 2명에서 공간감소가 관찰되었다. 2. 치열궁 너비, 치열궁 둘레는 초기모형과 최종모형에서 비슷한 값을 보였다. 3. 협설측 경사도 변화는 유견치에서는 상하악 모두 특별한 경향을 보이지 않았으며, 제2유구치는 상악은 실험군과 대조군이 유사하였고, 하악에서는 3명중 2명에서 실험군이 더 큰 설측경사도 변화를 보였다. 제1대구치는 하악에서 실험군이 더 큰 설측경사도 변화가 관찰되었다. 4. 근원심 경사도 변화 측정에서 유견치는 상하악 모두 특별한 경향이 관찰되지 않았고 제2 유구치의 경우 하악에서 실험군이 더 큰 근심경사도 증가가 보였다. 제1대구치는 상악에서 실험군, 대조군 모두 원심 경사도가 증가되었다.

스터럽이 없는 고강도 콘크리트 보의 전단강도 예측을 위한 새로운 예측식의 제안 (New Approach for Shear Capacity Prediction of High Strength Concrete Beams without Stirrups)

  • 보의 전단 메커니즘을 구성하는 보 작용과 아치 작용은 전단경간비(a/d)에 따라 그 존재 비율이 바뀔 뿐, 항상 공존한다. 그러나 대부분의 제안식들은 이를 함께 고려하지 않으며, 단순히 a/d=2.5를 기준으로 식의 형태를 나누어 제시하는 것에 그치고 있다. 또한 현 설계 기준은 과거의 일반 강도 콘크리트를 기준으로 설립되었기 때문에, 현재 사용 추세가 점차 증가하고 있는 고강도 콘크리트에 적용하기에는 다소 한계점이 있다. 이러한 문제점을 보완하고자, 본 연구에서는 a/d의 범위에 구애받지 않으며 고강도의 콘크리트에서도 사용이 가능한 전단 강도 예측식을 제안하였다. 보 작용과 아치 작용을 동시에 고려하기 위해 Jenq과 Shah 모델을 바탕으로 축방향에 따른 철근의 응력 변화를 산정하여, 단면의 모멘트로부터 전단 강도를 산출하였다. 보다 정확한 예측식의 제안을 위하여 장부 작용과 전단 마찰과 같은 2차적인 작용들 역시 제안식에 포함시켰으며, 크기 효과도 반영하였다. 식의 상수를 결정하고 식의 형태를 간략화하기 위해 다량의 실험 자료를 이용하여 회귀분석을 실시하였다. 제안식을 콘크리트의 압축강도($f'_c$), 전단경간비(a/d), 철근비($\rho$), 보의 유효 깊이(d), 철근의 항복강도($f_y$)의 변수를 기준으로 기존의 식들과 비교 검증한 결과, 제안식이 보다 정확하고 합리적인 예측을 하는 것으로 판명되었다. 또한 제안식의 변수별 거동 양상 역시 실험값과 비교적 일치하는 것으로 나타났다.

산지하천의 스텝-풀 연속체 복원설계 (Restoration design of step-pool sequence in mountain streams)

  • 본 연구의 목적은 하천복원에 스텝-풀 연속체를 적용하기 위해 기존 연구결과에서 스텝-풀의 기하구조, 형성 과정, 수리 기능, 복원 및 생태 기능과 관련된 사항을 검토하여 설계기준 및 세부설계 모델을 제시하는 것이다. 분석 및 검토결과를 바탕으로 스텝 간격과 하폭의 비, 단위스텝경사와 하상경사의 비, 및 스텝 높이와 입경 비로서 스텝-풀의 구조 및 규모에 대한 설계기준을 제시하였고, 스텝의 구조적 안정성 확보를 위해서는 키스톤 이론을 근거로 전도에 대한 안정성 해석방법을 설계기준으로 제시하였다. 세부설계 개념으로서 스텝-풀의 평면, 종단 및 횡단 구성에 키스톤과 아치스톤에 대한 겹침구조를 적용한 배치 모델을 제안하였다.

터널 내 콘크리트포장 표면처리공법 별 탑승자의 주행쾌적성 평가 (Evaluation on Riding comfort of A Passenger with Various Surface Textures of Concrete Pavement in Tunnels)

  • PURPOSES : The purpose of this article is to compare and evaluate the riding comfort of a passenger in tunnels depending on different surface textures of concrete pavement. METHODS : Evaluation of riding comfort is conducted at 17 sections, which have different surface texture such as transverse tinned(TT), longitudinal tinned(LT) and diamond grinded(DG). A triaxial accelerometer was set up on the passenger seat surface of the test vehicle to measure vibrations of an occupant, then the effects of vibration on comfort and health were evaluated by ISO 2631. And microphones were installed at passenger's ears height to measure sound pressure level(SPL) in the test vehicle. Additionally, a surface microphone was installed on the inside of wheel arch to evaluate noise between tire and pavement by NCPX method. All tests were conducted cruising at 100km/h. RESULTS : The results of all tests are as follows. First, both vibration magnitudes for comfort and for health in LT and DG sections are almost same and they represent lower than those in TT sections. Second, the average SPL of DG shows the lowest decibels among them. And third, it is founded that interior noise is significantly affected by noise between tire and pavement. CONCLUSIONS : It may be concluded that DG can provide more excellent riding comfort to passenger than LT or TT. Therefore, it is necessary to consider applying DG to existing pavement surface to improve surface condition when the driving environment especially requires riding comfort like a long tunnel.

성토지지말뚝에 작용하는 연직하중에 대한 모형실험 (Model Tests for Vertical Loads Acting on Embankment Piles)

  • 성토지지말뚝 상부의 성토지반내에서 발생되는 지반아칭효과에 의하여 성토하중이 말뚝에 전달되는 효과를 조사하면서 제안된 이론해석의 신뢰성을 확인하기 위하여 일련의 모형실험을 수행하였다. 본 모형실험에서 말뚝은 성토 아래에 일렬로 수열의 줄말뚝으로 설치하였으며, 말뚝캡보는 성토의 길이방향에 직각방향으로 말뚝두부에 설치하였다. 성토재의 하중전달에 가장 큰 영향을 미치는 요인으로는 말뚝캡보사이의 간격과 성토고를 들 수 있다. 이전의 이론적인 연구에 의해 제안된 지반아치의 반경보다 약 33%정도 큰 최소소요성토고보다 높게 성토를 실시할 경우 지반아치는 완벽하게 발생될 뿐만 아니라, 실험치와 이론치는 잘 일치함을 모형실험결과 확인할 수 있다. 모형말뚝캡보에 작용하는 성토하중의 분담률은 말뚝캡보사이의 간격이 증가함에 따라 감소하는 반면, 성토고가 높아짐에 따라 증가하였다. 따라서, 설계시 말뚝의 성토하중지지효과를 극대화시키기 위해서는 성토고를 충분히 높게한 상태에서 말뚝캡보의 간격비를 감소시켜야 한다. 여기서 말뚝캡보의 간격비를 감소시키려면 말뚝캡보사이의 간격을 감소시키거나 말뚝캡보의 폭을 증가시켜야 한다.

과도하게 엎침된 발을 가진 뇌졸중 환자에게 적용된 수정 발바닥활 지지 테이핑의 효과 (Effects of Modified Low-Dye Taping on Stroke Patients with an Excessive Pronated Foot)

  • PURPOSE: Stroke patients may develop an over-pronated foot, resulting in a lower medial longitudinal arch. This can lead to a structural change of the foot due to deformation of the musculoskeletal system. The purpose of this study was to examine the effects of modified low-dye taping on stroke patients with an excessively pronated foot. The effect of the taping on the foot after light daily activity was also examined. METHODS: The subjects consisted of 21 stroke patients with an excessively pronated foot, as measured by the navicular drop test. First, their navicular heights were measured at a relaxed standing position (measure 1) and while standing in the subtalar neutral position (measure 2). Modified low-dye taping was applied to each subject's affected foot and the navicular height was then measured for the standing posture (measure 3). Finally, each subject walked around for 10 minutes and the navicular height was measured again (measure 4). RESULTS: Statistical analyses showed that the navicular height value at the relaxed standing position (measure 1) was significantly lower than for the other 3 measurements. That is, the modified low-dye taping was effective in maintaining a subtalar neutral position, even after a 10-minute walk, for stroke patients with an excessively pronated foot. CONCLUSION: The results suggest that modified low-dye taping applied to stroke patients with an excessively pronated foot could be an effective way to place the subtalar joint in a neutral position, and that its effect can be sustained for light daily activities.

소아 편평족 환자에서 Pressure Based Customized 3-Dimensional Printing Insole의 유용성 (Effect of Pressure Based Customized 3-Dimensional Printing Insole in Pediatric Flexible Flat Foot Patients)

  • Purpose: A flatfoot that fails to form a longitudinal foot arch is a common lower limb deformity in children. This study evaluated the structural and functional effects of the insole for pediatric flexible flat foot (PFFF). Materials and Methods: Twenty-nine PFFF patients (20 boys and 9 girls, 58 feet) with bilateral symptomatic flatfoot deformities between February 2017 and May 2019 were included in this study. Sixteen patients (32 cases, study group) were treated with a pressured based 3-dimensional printing insole, and 13 patients (26 cases, control group) were followed up regularly without any treatment. Flatfoot was diagnosed by a lateral talo-first metatarsal angle of more than 4° in convex downward and talocalcaneal angles of more than 30° and a calcaneal pitch of less than 20°. The foot pressures, including the midfoot pressure, total foot pressure, and the ratio of the midfoot pressure to the total foot pressure, were evaluated by pedobarography. The clinical scores were assessed using the visual analogue scale (VAS), American Orthopaedic Foot and Ankle Society (AOFAS), and Pediatrics Outcomes Data Collection Instrument (PODCI) scores. Results: The mean age of the study group was 9.16 years, and the mean age of the control group was 7.73 years. The mean follow-up period was 16 months. The change in the lateral talocalcaneal angle was -4.664°±1.239° in the study group and -0.484°±1.513° in the control group. A significant difference in the amount of change of the lateral talocalcaneal angle was observed between the two groups (p=0.034). The midfoot pressures were similar in the two groups. Conclusion: Pressure based customized 3-dimensional printing insole in PFFF may have some effect on the hindfoot bony alignment, but it does not affect the changes in midfoot pressure.

Relationship Between Electromyographic Activity of the Abductor Hallucis and the Pressure of a Pinch Gauge During Short Foot Exercise

  • The aims of this study were to investigate the correlation between the electromyographic (EMG) activity of the abductor hallucis (AbdH) and the amount of pressure measured by a pinch gauge (PG), and to compare the EMG activity of AbdH and the pressure measured by the PG during short foot (SF) exercise in subjects with pes planus and in subjects with a neutral foot alignment. Fourteen subjects were recruited for this study (pes planus group=7; neutral foot alignment group=7). A surface EMG was used to collect AbdH activity, and a PG was positioned under the first metatarsophalangeal joint to measure the pressure produced by the first metatarsal head during the SF exercise. The AbdH activity and the pressure measured by the PG showed a positive good correlation (r=.80, p<.05). The EMG activity of the AbdH and the pressure measured by the PG were significantly lower for subjects with pes planus than for subjects with a neutral foot alignment (p<.05). Based on these findings, the PG can be recommended as an effective instrument for evaluating the performance of the AbdH. It may also be beneficial for monitoring how well the SF exercise is performed, and for providing visual feedback to patients with pes planus during SF exercise in a clinical setting.

Comparison of Flatfoot on the Activities of Medial and Lateral Plantar Flexor Muscle During Heel Rising

  • Background: Flat feet can be identified by assessing the collapse of the medial longitudinal arch (MLA) and these conditions can trigger epidemiological changes in the feet. Many of previous studies compared the muscle activity of lower body in terms of intervention and dynamics to treat the structural defect of flat feet. However, few studies have investigated or analyzed the muscle activity of gastrocnemius muscle in the subjects with flat feet. Objects: This study investigated the differences in changes of medial and lateral plantar flexors in subjects with flat feet during bipedal heel-rise (BHR) task and analyzed the differences in muscle activity between two groups by measuring the electromyography (EMG) of abductor hallucis (AH), tibialis anterior (TA), medial gastrocnemius (MG), lateral gastrocnemius (LG). Methods: A total of Twenty one adult females participated in this experiment. Subjects were assigned to groups according to the navicular drop test. The task was applied to the leg, where the heel lifting action prevailed. The muscle activity of the medial and lateral feet plantar flexors was evaluated, and the % maximum voluntary isometric contractions (%MVIC) of these were compared. Results: For the difference between groups the muscle activity (%MVIC) of LG muscle was statistically significantly low in flat feet group compared to healthy feet group (flat feet: 64.57, healthy feet: 90.17; p < 0.05). Conclusion: The results of this study will contribute to identifying the muscle activities of medial and lateral feet plantar flexors among subjects with flat feet, which can cause abnormal epidemiological changes in the feet.
