일부 도시지역 장기요양급여 인정 노인들의 우울수준과 관련 요인 (Depression Symptoms and its Related Factors among the Elderly People Affiliated with Long-term Care Insurance Services in Urban Areas)

  • 본 연구는 장기요양보험급여를 받고 있는 도시지역 노인들의 장기요양등급에 따른 우울수준을 파악해 보고, 우울수준에 관련된 요인을 규명해 보고자 시도하였다. 조사대상은 대전광역시 5개구에서 장기요양보험급여를 받고 있는 노인 338명을 대상으로 하였다. 조사는 2012년 3월 1일부터 5월 31일까지의 기간 동안에 표준화된 무기명식 면접조사용 설문지를 사용하여 면접조사 하였다. 연구결과, 조사대상자의 CES-D에 의한 우울수준은 정상군(CES-D 21점미만)이 21.6%, 우울군(CES-D 21점 이상)이 78.4%이었다. 장기요양등급에 따른 우울군(CES-D 21점 이상)의 분포는 장기요양 1등급에서 83.6%, 2등급에서 82.1%, 3등급에서 67.0%로 장기요양 등급이 높을수록 우울군의 분포도 유의하게 높은 것으로 나타났다. 우울 수준과 제 요인과의 관련성을 알아보기 위해 다중로지스틱회귀분석 결과 우울수준에 대한 교차비는 65-74세 연령군보다 75세이상 연령군에서, 주관적인 건강상태가 좋다는 군보다 좋지 않다는 군에서, 취미활동을 자주한다는 군보다 거의하지 않는다는 군에서 유의하게 증가하였으며, 월수입 50만원 이하군보다 50만원 이상 군에서 유의하게 감소하였다. 이 같은 결과는 우울에 관련된 요인이 개인의 기본 속성, 경제 상태, 신체의 건강상태나 활동성 등의 다양한 요인과 관련성이 있다는 것을 시사하고 있다.

아동병원 간호사의 의사소통능력, 자기효능감이 간호사-환아부모 파트너십에 미치는 영향 (The Influence of Communication Competence and Self-Efficacy of Nurses at Children's Hospitals on Partnership with Parents)

  • Purpose: This study aimed to identify the influence of communication competence and self-efficacy of nurses at children's hospitals on the nurse-parent partnership. Methods: Data were collected by surveying (from July 4th to August 4th, 2019) 162 nurses who had 6 or more months of experience across 15 children's hospitals in G City and B Metropolitan City. Using SPSS 23.0 for Windows for data analysis, t-tests, analysis of variance, Bonferroni tests, Pearson's correlation coefficient, and multiple regression analysis were performed. Results: The nurse-parent partnership showed significantly positive correlations with nurses' communication competence and with self-efficacy. There was a significantly positive correlation between nurses' communication competence and self-efficacy. The factors influencing the nurse-parent partnership were nurses' communication competence, job satisfaction, and self-efficacy; the total explanatory power of these variables was 47.0%. Conclusion: Since the communication competence of nurses at children's hospitals was identified as a major factor influencing the nurse-parent partnership, it may be meaningful to conduct basic research aimed at developing strategies to improve this partnership at children's hospitals.

  • Even though the interest to the mental hospital is recently increased, the mental hospitals are not well developed due to the pending arrangement of the concept of returning to the society and the system of local communities. Also, the hospitalism due to the isolation of patients and the long term stay in hospitals has become an issue. Despite the present circumstances, the prototype of mental health care in many field are investigated and progressively fixed. This study attempts to examine the current status of domestic and foreign mental institution and to establish the design guideline of the wards of the mental hospitals.

요양병원 간호사를 위한 전문직 자아개념 (Nurses' Self-Concept Instrument, NSCI) 도구의 타당도와 신뢰도 (Validity and Reliability of the Nurses' Self-Concept Instrument (NSCI) for Registered Nurses in Long-term Care Hospitals)

  • Purpose: The purpose of this study was to evaluate the validity and reliability of the Korean Version of the Nurses' Self-Concept Instrument (NSCI) geriatric hospital nurses in Korea. Methods: Bilingual nursing professionals performed translation and back-translation. Reliability and validity of the content and construction of the instrument were confirmed. Internal consistency reliability was determined. Construction and concurrent validity were verified using factor analysis and correlation coefficients. Results: The total 14 items for the Korean version of the Nurses' Self-Concept Instrument (NSCI) were retained through item analysis. In explanatory factor analysis, four subcategories were proposed with their names of each factor: 'Leadership', 'Staff relation', 'Knowledge', and 'Care'. The four factors accounted for 78.81% of the variances. The Cronbach's ${\alpha}$ regarding internal consistency were .77~.91 for the NSCI subscales. Correlation among four subcategories ranged .62~.84. Conclusion: The findings show that the Korean version of the Nurses' Self-Concept Instrument is reliable and valid for measuring professional Self-Concept of geriatric hospital nurses in Korea.

한국 보건진료원 제도의 시작 (Beginnings of the Community Health Practitioner (CHP) System in Republic of Korea)

  • Purpose: This research was done do identify and analyze the beginnings of the community health practitioner system in the Republic of Korea (ROK) around 1980. Method: Primary sources were collected and analyzed, mainly newspapers around 1980, the act for health service for rural areas, and other relative publications. Results: The government of the ROK was trying to solve the problem of doctorless villages and regarded the introduction of primary health care (PHC) services using registered nurses (RNs) to be an economic solution to this problem. The Korean Nurses' Association presented 'a plan for community health service' to the government party and medical association in 1976. In this plan, RNs would provide primary care at the sub-county (myun) level, and hospital would provide secondary care. The Korean Public Health Development Research Center was awarded the project 'RNs and nurse aids as CHP for primary care service and their training'. In 1977, 25 RNs began to work as PHC in 3 areas, and interim findings showed that RNs were very capable of doing PHC. The Ministry of Health and Welfare announced long term plans for health and welfare administration including a tertiary health care delivery system. RNs after training were posted to rural areas with no medical services to do medical treatment for mild cases. The Act for health services for rural areas was enacted on December 31, 1980. Enforcement Ordinance and Enforcement Regulations were enacted in 1981. In 1981, 257 CHP were selected, trained, and deployed. In 1983, the president of the ROK announced continuation of the CHP system for residents of medically vulnerable areas. The number of CHP increased from 257 in 1981 to 2038 in 1989.

진료환경개선을 위한 우선적 전략과제 설정 및 그 적용 (A Strategic Quality Initiative and Its Opportunities to Improve Healthcare Environment)

  • Background : Strategic planning is an organizationwide or systemwide, ongoing look into the future usually of 2~3 years, based on objective analysis of the current environment and trends, but it can incorporate both short-term and long-term goals. The strategic planning process includes external analysis, internal analysis, issue analysis, development of mission, vision and values, and lastly development of organizational goals and objectives. As a part of the strategic quality planning process, certain service lines, important organizationwide functions, or key processes supporting these functions can be prioritized to expedite and roll out certain strategic goals. This is called strategic quality initiatives. Methods : We organized a quality improvement team, a subgroup of 21st century vision planning corps of our medical center, and pursued QI activities for improvement of healthcare environment, particularly in the admission setting. We developed a strategic quality initiative based on the results of patient satisfaction surveys, and carried out functions of self-directed work team. Results : The strategic goal was to be the benchmark for peer group hospitals in Korea for providing cost-effective best-practice. The QI team included 3 medical doctors, 1 nurse, 1 social worker, and 1 QI consultant as well as many operational members to support services and quality initiatives met every Tuesday for 18 weeks. Outcome objectives were to improve patient satisfaction score. The issues included in the objectives were comfort, temperature, noise, cleanliness of the admission wards, quality and education of patient meals, matters regarding the admission process, and an appurtenant facility such as restaurant or convenience store. Every issue was discussed and recommendations, conclusions and opportunities were implemented. Conclusions : By developing a strategic quality initiative as a part of the strategic quality planning process, and pursuing a self-directed work team, certain sen/ice lines, important organizationwide functions, or key processes supporting these functions can be improved effectively within a short period. Strategic quality initiatives serve to support, or roll out, certain strategic goals that are relevant to performance improvement and development of specific measurable outcome objectives, and associated performance measure for each initiative. Each strategic quality initiative should include a statement of intent outcome objectives, and performance measures. We will come back with follow up of the strategic quality initiative, for improvement of healthcare environment, and results of patient satisfaction re-survey.

정맥주입 전문간호사가 삽입한 말초삽입형 중심정맥관(PICC) 사용 결과에 대한 후향적 분석 (Analysis of PICC Inserted Patient Data in a Hospital by IV CNS-Driven Intervention)

  • Background : Intravenous(IV) access is becoming an increasingly important part of health care today. The current drive for clinical effectiveness and cost-effective health care serves to increase the need for reliable vascular access. Venous access devices were developed to overcome problems associated with limited peripheral access and frequent venipuncture in patients with long-term therapy. Although the peripherally inserted central catheter(PICC) have become popular during recent years in USA, its procedure is rare in Korea. Purpose : The goal of this study was to analyze the PICC inserted patient data by IV CNS intervention. Method : A Total of 62 PICCs were inserted into 51 patients by the IV CNS during a 10-month period form November, 14, 2002, to October 2, 2002. Data was obtained retrospectively through chart review. Result : The patient population included 34(54.8%) men and 28(45.2%) women, with a mean age 50.6 years. The main indication for PICC placement was to access vein in poor peripheral venous status(40.3%). The mean served interval for PICC insertions was 16.7 days(range, $2{\sim}61$ days). The reasons for removal were completed therapy in 18 cases(29.0%), patient death in 13 cases(21.0%), and mechanical or functional PICC problem in 10cases(16.1%). The three PICCs removed for presumed infection, and one had only positive tip cultures(0.2%). Conclusion : PICCs are rapidly growing popularity and required an extended course of IV therapy.

요양병원 인증제 평가를 경험한 간호사들의 직무 스트레스 및 소진이 이직의도에 미치는 영향 (Factors Influencing Turnover Intention of Nurses after Evaluation for Certification at Geriatric Hospitals: Focused on Job Stress and Burnout)

  • 본 연구는 요양병원 인증제 평가를 경험한 간호사의 직무스트레스 및 소진이 이직의도에 미치는 영향에 대해 파악하기 위한 서술적 상관관계 연구이다. K대학교 기관생명윤리위원회에서 승인을 받았으며, 자료수집은 2015년 4월 1일부터 8월 31일까지이며, 자료는 SPSS/WIN 18.0 프로그램을 이용하여 분석하였다. 연구 대상자는 B광역시와 경상남 북도 소재한 15개 요양병원에서 205명의 간호사를 대상으로 하였다. 직무스트레스 평균은 5점 만점에 3.83, 소진의 평균은 5점 만점에 2.92이었으며, 이직의도 평균은 5점 만점에 3.12였다. 이직의도와 직무스트레스(r=.356, p<.001), 소진(r=.729, p<.001)은 유의한 상관관계가 있었으며, 소진은 이직의도에 53%의 설명력을 보였다(${\beta}=.729$). 이 연구의 결론을 통해 요양병원 인증제를 경험한 요양 병원 간호사의 이직의도를 감소시키기 위해 주요한 요인인 소진을 낮추기 위한 중재 프로프램 개발이 지속적으로 수행되어야 할 것이다.

요양병원 간호사의 간호행위와 위임실태 (Nursing Activities and Delegation in Long-term Care Settings)

  • 본 연구는 요양병원 간호사의 간호행위와 위임실태를 파악하여 의료서비스의 질향상에 기초적 자료를 제공하고자 하였다. 연구 대상자는 요양병원 3년 이상의 근무경력을 가진 간호사 140명을 대상으로 2013년 7월 26일부터 12월 23일까지 자료수집하였다. 수집된 자료는 SPSS 20.0 통계프로그램의 빈도분석, 평균과 표준편차, Independentent t-test와 One way ANOVA로 분석하였다. 본 연구결과 간호행위 위임실태로는 간호행위 위임절차의 프로토콜이 없는 경우가 프로토콜이 있는 경우보다 높게 나타났다. 이 연구를 통해 요양병원 간호사의 간호행위는 위임에 대한 명시된 기준이나 절차가 없는 상황에서는 의료서비스의 질을 떨어뜨리는 주요요인이 될 수 있으므로 간호업무위임의 법칙, 제도적 근거와 간호업무 위임행위에 대한 절차와 방법, 지침이 마련되어짐이 요구된다.

노인요양병원 간호사의 욕창간호지식, 욕창예방 간호수행 및 간호수행 장애요인에 관한 연구 (A Study on the Knowledge of Nurses, Performance and Preventive Practice of Pressure Ulcer among Nurses in Long-Term Care Hospital)

  • 본 연구는 노인요양병원 간호사의 욕창간호지식과 욕창예방 간호수행과의 관계를 파악하기 위한 서술적상관조사 연구이다. 2014년 3월 1일부터 7월 1일까지 노인요양병원에 근무하고 있는 간호사 317명을 대상으로 분석하였으며 욕창간호지식, 욕창예방 간호수행, 욕창예방 장애요인에 관한 구조화된 설문지를 배부하여 자가 보고법으로 작성하였고 수집된 자료는 기술통계로 분석하였다. 일반적 특성에 따른 욕창간호지식은 임상경력, 교육내용, 교육시간, 교육 횟수에서는 유의한 차이를 보였고(p<.05), 욕창예방 간호수행에서는 교육 횟수에서만 유의한 차이를 보였다(p=.04). 욕창지식정도와 욕창예방 간호수행에서는 음의 상관관계를 나타냈으며(rho=.137, p<.05), 욕창예방 간호수행 장애요인으로는 인력부족이 57.7%로 나타났다. 따라서 욕창간호 예방수행의 질적 향상을 위해서는 욕창예방 간호지식을 올리는 것보다는 욕창예방 간호교육의 횟수를 증가시키는 것과 적정인력이 근무할 수 있는 제도적 정책이 필요하다.
