Development and Evaluation of a Dysphagia Assessment Tool and an Intervention Program for the Elderly in the Long-Term Care Facilities (노인요양시설 노인의 연하곤란 사정도구, 중재프로그램 개발 및 평가)

  • The purpose of this study was to develop a dysphagia assessment tool and a intervention program for elderly in the long-term care facilities and to evaluate its effect. The dysphagia assessment tool consists of 20 items was developed through literature review, previous studies and discussion with nursing/medical members. The intervention program consisted of 72 items and participants for evaluation were 50 elders who were registered at long-term care facilities in 4 regions of Seoul. Collected data were analyzed using SPSS Statistics 18.0 and Spearman's Correlation, t-test, and $x^2$-test. After the application of the dysphagia assessment tool and intervention program, the weight in experimental group was increased (t=4.913, p=.000) and the total time of swallowing was reduced (t=-4.557, p=.000) than control group. These findings were statistically significant. Considering these results, by applying the dysphagia assessment tool and the intervention program, the lives of the elderly in long-term care facilities are expected to be improved and emergency situations such as aspiration pneumonia and asphyxia will be decreased.

Comparison of Oral Health Status of the Elderly Living in Long-Term Care Facilities and Non-resident Elderly (노인요양시설 거주 노인과 비거주 노인의 구강건강상태 비교)

  • The aim of this research was to analyze the differences for the oral health status of the elderly people living in the long-term care facilities and the non-resident elderly. The study was conducted on 469 elderly people of the same age, gender and basic living conditions. Elderly residing in long-term care facilities had higher dental caries prevalence(p=0.019), DT(p=0.001), and MT(p=0.047) than non-resident elderly, and had lower FT(p<0.001) than non-resident elderly. The elderly living in facilities were 1.93 times more likely to be caries than non-residents, and 0.73 times more likely to have a filled tooth. The probability of denture use was 0.15 times for the maxillary and 0.13 times for the mandibular. The probability of denture needs was 5.61 times for the maxillary and 5.65 times for the mandibular. All of these results showed significant differences. As a result of this, it can be used as basic data for establishing oral health policy for oral health promotion of the elderly living in Long-term care facilities.

A Study on the Spatial Structure Change of Elderly Care Facility according to Introduction of Long-Term Care Insurance (장기요양보험제도의 도입에 따른 노인요양시설의 공간구조 변화에 관한 연구)

  • Purpose : Long-term Care Insurance sets up facility standard and installation standard of aged care facilities and decides the presence of minimum number of rooms and its size in care facilities by using systematic instruments. Therefore, most aged care facilities had renovation in expansion and reconstruction following the revised regulations and even facility space structure and space composition are continuously improving. The study is to determine the purpose and trend by comparing before and after space composition of facilities which followed the implementation of Long-term Care Insurance and also to suggest hierarchical space composition suitable for aged care facilities through derived problems and to provide basic materials to plan the most appropriate facility for the aged. Methods : J-graph based on Space Syntax Theory will be schematized through in-site facility survey and before and after facility floor plan. Space composition trend will be analyzed by comparing indexes through S3 program. Results : As a result of 5 cases analysis, the following results were found; the average of whole space depth is increasing due to the Extension, the number of volunteers is decreasing and rooms for geriatric care helpers are being created due to the geriatric care helper introduction duty. Also, there are solariums being created to improve the health of the aged and dispensaries are being placed on every floor with the increase of documentary work for geriatric care helpers. With the policy implementation, care facility space composition and structure are changing with facility standard and it was analyzed that facilities were mostly put under the control of limited number of people in care room and total ground area per person. It was also found that there is increase in care space integration through before and after comparison of whole integration value. This is considered as the important result not only in facility standard satisfaction, but also in care support of geriatric care helpers and the aged, its main users. Implication : In order for elderly care facilities to have quality improvement and to develop as suitable facilities for characteristics of the aged, independent environmental facility standard preparation of elderly care facilities is needed through mutual cooperation of construction fields with regulation and policy related researches.

Caregiving for the Long-Term Care Elderly Women - Focusing on Caregiving Characteristics and Depression - (장기 보호 여성노인의 수발에 관한 연구 - 수발특성과 우울을 중심으로 -)

  • The present study was initially designed to figure out the general condition of care giving system for the elderly women who need long term care and the level of their depression according to the conditions of care. And This research is intented to present appropriate policy that could help the establishment of supporting system for the fragile elderly women.1 used the data from <2001 National Study on the Needs for the Long-Term Care Elderly> by Korea Health and Population Institute. The results are as follows: First, Two third of all the respondents had serious problems (2-9 activities limits) in Instrumental Daily Living Ability(DAL). Most respondents reported “low” in satisfaction level related to receiving care, meaning the elderly had negative perception for the care from the family. The elderly expected their children to be as the primary care giver and mostly wanted to live with them in the future. Second, The majority of the long term care elderly women haven't used community service facilities very often and said they are not likely going to use the facilities in the future. Third, The respondents reported high in depression level as to lower satisfaction with their children's support, poorer health condition, more reluctant to use service facilities due to the cost, and fewer friends and neighbors resources around them. Therefore I could say that negative factors for the elderly women's psychological health were having unsatisfactory relationship with intimate people, developing physical illness, being in economic difficulties. That is, receiving less help from close family members, shrinking social network, and experiencing economic hardship would have negative effects on elderly women's psychological health. In the basis of these results, I suggest that in the mean time we shouldn't overlook the importance of the private support when we develop the public elderly support system.

The Relationship between the Quality of Life, Care Services, and Family Support of Elderly Residents in Long-Term Care Facilities (노인요양 시설 노인의 삶의 질과 시설 서비스, 가족 지지의 관계)

  • The aim of this study is to identify the influence of care services on the quality of life of elderly residents in long-term care facilities, and to determine the mediating effect of family support. The subjects of the study were 192 older adults living in long-term care facilities in the Daegu and Gyeongsangbuk-do region. The collected data were analyzed using the SPSS 22.0 program, and the methods used include descriptive statistics, t-test, ANOVA, Pearson correlation analysis, and regression analysis. The results of the study found that the subjects' quality of life differed depending on their age, education, income level of their children, reasons for admission, and length of stay. The factors that influenced their quality life included age, education, the income level of their children, reasons for admission, length of stay, and care services. The study also revealed that family support was the mediating effect in the relationship between care services and quality of life. The results of the study will provide insight in establishing a nursing intervention plan to improve the quality of life of elderly residents in care facilities.

Factors Affecting Fatigue of Care Helpers in Long-term Care Facilities (장기요양시설 요양보호사의 피로 영향요인)

  • Purpose: This study aims to investigate the factors affecting fatigue of care helpers in long-term care facilities. Methods: The subjects consisted of 131 care helpers at six long-term care facilities in Chungcheongnam-do. A survey was conducted between July and September 2019 to collect data, which were analyzed using t-test, ANOVA, Pearson's correlation coefficients, and multiple regression analysis.Results: An increase in fatigue of care helpers owing to taking care of over 10 patients a day, increase in emotional labor and sleep disturbance, and a decrease in economic status was observed. Their fatigue decreased when there was an increase in their subjective health status and job satisfaction. Work-related stress (β=.37, p<.001), emotional labor (β=.21, p=.014), and sleep disturbance (β=.18, p=.026) significantly predicted fatigue. These factors explained 37% of the total variance in fatigue (F=10.48, p<.001).Conclusion: In addition to improving the work environment, interventions that alleviate emotional labor and improve sleep quality, are needed to lower the fatigue of care helpers.

  • Government came to enforce long-term care insurance system in preparation for the rapid aging society. Whether this system is successful or not depends on the professionalism of long-term care-givers who are professional population in charge of care service. Currently in the early stage of enforcement, such problems as a race cutting fee resulted from numerical increase of educational facilities, insolvent operation, degradation of education level resulted from unprofessional instructor, are pointed out. As a mean of manpower-training on long-term care insurance system, this study is to research public-private-university network model of the Academy of Continuing Education attached to Ko Sin University which is the case of Yong Do Gu Busan city. Networking between the vision and development strategy of Yong Do Gu on continuing education city, education system on community manpower-training supported by Ko Sin University, and service field of welfare for the elderly can not only contribute to the professionalism of long-term care-givers but also play an ideal role in manpower-utilization and arrangement of community. Through this networking, high quality of education level and circumstance, using the existing infra, manpower-training and utilization for continuing education of Yong Do Gu can be accomplished. Additionally, the connection with facilities related with welfare for the elderly can contribute to professionalism and accountability of manpower-networking.

Current Status of Rehabilitation Services in Long-Term Care Hospitals for Health Promotion of Jeollabuk-do Residents (전라북도민 건강증진을 위한 요양병원 재활서비스 현황)

  • As the super-aged society is imminent, the number of people with senile diseases is also increasing, resulting in a lack of facilities and policies as a social problem. National Health Insurance Service is being talked about as a realistic alternative to medical coverage. Long-term care hospitals are classified into acute-stage hospitals and long-term care facilities and are acting as an alternative to reducing medical expenses. However, in long-term care hospitals, which are the core of maintenance rehabilitation, the quality of service issues have been raised seriously. Currently, there is a problem of mass production of refugees from rehabilitation due to a problem of the medical system. In particular, in the preceding study in 2015, the status of long-term care hospitals in Jeollabuk-do was the worst. Therefore, using public data, the general status of long-term care hospitals in Jeollabuk-do, status of frequent disease, status of rehabilitation services, and status of occupational therapy services are to be explored.

An Efficiency Analysis of the Long-term Care Facilities Using DEA Model (자료포락분석을 이용한 노인요양시설 효율성 연구)

  • The aim of this study is to analysis the efficiency of the Long-term Care facilities as well as make counterproposals to conduct of efficient management for the Long-term Care facilities. The data of professionals states of Long-term Care facilities and wages state of Long-term Care facilities from regions in 2014. To analyse the data, the number of professionals and facilities are an input variable whereas the size of number and wages are the output variables. The results showed as below. First, according to the CCR test, Kangwon, Gyeonggi, GyeongNam, Deajeon, Seoul, Ulsan, Incheon, Jeju, and Chong Buk showed significance, but Daegu and Busan showed no significance. Second, the BCC result showed that Kangwon, Gyeonggi, GyeongNam, Deajeon, Seoul, Ulsan, Incheon, Jeju, and Chong Buk has efficiency whereas Daegu and Busan has no efficiency. The result of excess efficieincy analysis confirmed 133.5% in Jeju as the highest area, 37.54% of the highest efficiency of the care provider, and 28.61% of imporvable possibility with doctor's number. The realization of increasing numbers, the ensure of the doctor's number, and consolidating of the care provider's the espertise are required for the future.

Development and Effects of a Coping Skill Training Program for Caregivers in Feeding Difficulty of Older Adults with Dementia in Long-Term Care Facilities (요양시설 요양보호사를 위한 치매노인의 식사행동장애 대처기술훈련 프로그램 개발 및 효과)

  • Purpose: We developed and tested the effects of a coping skill training program for caregivers in feeding difficulty among older adults with dementia in long-term care facilities. Methods: A non-equivalent control group pretest-posttest design was used. The subjects comprised 34 caregivers (experimental group: 17, control group: 17) and 40 older adults with dementia (experimental group: 20, control group: 20). The developed program was delivered in 4-hour sessions over 6 weeks (including 2 weeks of lectures and lab practice on feeding difficulty coping skills, and 4 weeks of field practice). Data were collected before, immediately after, and 4 weeks after the program (January 3 to April 6, 2016). The data were analyzed using t-test and repeated measures ANOVA using SPSS/WIN 20.0. Results: Compared to their counterparts in the control group, caregivers in the experimental group showed a significantly greater improvement in feeding knowledge and feeding behavior, while older adults with dementia showed greater improvements in feeding difficulty and Body Mass Index. Conclusion: The study findings indicate that this coping skill training program for caregivers in feeding difficulty is an effective intervention for older adults with dementia in long-term care facilities.
