한국중년층의 유료노인요양시설에 대한 견해 (The Opinions of Middle Aged Korean about User-Paid Long-Term Care Facilities for Frail Elderly)

  • Although the proportion of frail elderly among the elderly population has been rapidly increasing, there is little preparation to provide housing alternatives for them in Korean society. Long-term care facility a housing alternative for frail elderly that enables them to receive social and medical care services that enhance the quality of their life. This research was conducted to study the opinions of middle-aged Koreans about long-term care facilities for the frail elderly. This study provides useful suggestions for the development of user-paid long-term care facilities in the near future. A survey questionnaire was administered to 600 adults in their 50s to collect data. The sample was stratified according to region, gender, and housing structure type. The results showed that most of the respondents were aware of long-term care facilities for the frail elderly; but they were relatively unwilling to live in these facilities, as compared to other housing alternatives. However, a larger number of respondents said that as they became very frail, they would be willing to live in these facilities. The respondents expressed a high level of need for user-paid long-term care facilities as an alternative to hospitalization. Also, they wanted to have a variety of long-term care facilities that are designed for different levels of disability. The education level of respondents and their spouses, average monthly income, subjective economic level, and religion were major variables that differentiated significantly the opinions about long-term care facilities for the frail elderly.

미슐랭 가이드 스타 레스토랑에 대한 기대가치가 방문의도에 미치는 영향 (The Impact of Expected Value of Michelin Guide Star Restaurants on Visit Intention)

  • Purpose: South Korea is one of the markets where diverse and innovative cuisines are being tried. Therefore, the expectations for Michelin star restaurants in South Korea are high. The expected value of a Michelin star restaurant consists of both hedonic and utilitarian aspects of the dining experience. These consumer expectations affect visit intention. Research design, data, and methodology: The survey period was from October 10, 2023 to October 18, 2023, and 302 copies were used for hypothesis verification. Result: Hedonic expected value was found to have a significant positive influence on short-term and long-term visit intention, but had no significant effect on mid-term visit intention. Utilitarian expected value was found to have a significant positive influence on short-term and mid-term visit intention, but did not appear to have a significant effect on long-term visit intentions. Short-term visit intention was found to have a significant positive influence on mid-term visit intention, and mid-term visit intention was found to have a significant positive influence on long-term visit intention. Conclusions: The theoretical implications are restaurants need to utilize third-party certification, such as the Michelin Guide. The practical implication is that Michelin star restaurants should provide expected value to increase short-term visit intention.

  • 본 논문에서는 낙상상태를 감지할 수 있는 장단기 메모리(Long Short-Term Memory)를 이용한 낙상감지 시스템에서 은닉층 노드 수 변경에 따른 영향을 소개한다. 3축 가속도 센서를 이용하여 x, y, z축 데이터를 중력 방향과 이루는 각도를 나타내는 파라미터 theta(θ)를 이용하여 훈련을 진행한다. 학습에서는 validation이 진행되어 8:2의 비율로 훈련 데이터와 테스트 데이터로 나뉘며, 효율성을 높이기 위해 은닉층의 노드 수를 변화하며 훈련을 진행한다. 노드 수가 128일 때 Accuracy 99.82%, Specificity 99.58%, Sensitivity 100%로 가장 좋은 정확도를 나타내었다.

요양병원 입원노인의 환자군 분류에 따른 자원이용수준 (Resource use of the Elderly in Long-term Care Hospital sing RUG-III)

  • Purpose: This study was to classify elderly in long-term care hospitals for using Resource Utilization Group(RUG-III) and to consider feasibility of payment method based on RUG-III classification system in Korea. Method: This study designed by measuring resident characteristics using the Resident Assessment Instrument-Minimum Data Set(RAI-MDS) and staff time. The data were collected from 382 elderly over sixty-year old, inpatient in the five long-term care hospitals. Staff time was converted into standard time based on the average wage of nurse and aids. Result: The subjects were classified into 4 groups. The group of Clinically Complex was the largest(46.3%), Reduced Physical Function(27.2%), Behavior Problem(17.0%), and Impaired Cognition(9.4%). The average resource use for one resident in terms of care time(nurses, aids) was 183.7 minutes a day. Relative resource use was expressed as a case mix index(CMI) calculated as a proportion of mean resource use. The CMI of Clinically Complex group was the largest(1.10), and then Reduced Physical Function(0.93), Behavior Problem(0.93), and Impaired Cognition(0.83) followed. The difference of the resource use showed statistical significance between major groups(p<0.0001). Conclusion: The results of this study showed that the RUG-III classification system differentiates resources provided to elderly in long-term care hospitals in Korea.

저소득층 재가노인의 기능상태와 요구되는 요양서비스 유형 분석 (Functional Status and Long-Term Care Services for the Community-Dwelling Low-Income Elderly)

  • Purpose: This study was conducted to explore the functional status and long-term care services for the community-dwelling low-income elderly. Method: A descriptive research design was used in this study. The functional status of the participants was obtained using Minimum Data Set-Home Care Version 2.0 and the long-term care services were identified via Michigan's choice. Total of 154 persons aged 65 years or older completed Korean Minimum Data Set-Home Care Version 2.0 on the community dwelling low-income elderly. Results: The average of Activities of Daily Living was 4.19, and the range was 0-55, while the average of Instrument of Activities of Daily Living was 4.85 and the range was 0-56. Among the subjects, 46.1% belonged to the Information and Referral group and 1.3% to the Nursing Home group. Severe daily pain was reported by 14.9%, and 76.6% of the participants had impaired vision. The Activities of Daily Living was difference according to living with, education, vision, and depression. The long-term care services differed according to gender, pain, vision, hearing, and depression. Conclusion: The support policy for the elderly needed to focus on impaired visual and depression to enhance the activities of daily living. Moreover, there is a need for the Information and Referral group to arrange and develop nursing intervention resources.

장기요양시설노인의 좋은 서비스 경험에 관한 개념도 연구 (A Concept Mapping Study of Good Service Experience among the Elderly Residents of Long-term Care Facilities)

  • Purpose: The purpose of this study was to explore the reported good service experiences from the perspective of elderly residents of long-term care facilities. Methods: Of those residents who are 65 years old or older, 14 residents whose length of stay were one month or longer and scores of the K-Mini Mental State Examination were 15 or higher were interviewed. The interview data formed the basis for the empirical statements about the reported nature of patients' experiences as residents of long-term care facilities. These data were used in concept mapping. Results: Through multidimensional scaling analysis and hierarchical cluster analysis, 62 core statements, two dimensions, and six clusters of good service experiences were derived. The two dimensions were classified as 'care centered-participation centered services' and as 'physical-emotional services.' Six cluster themes emerged as good service experiences: 'safety of care and treatment', 'responsible and supportive staff', 'comfort of living environment', 'mental well-being', and 'respect and communication'. Conclusion: The result of the study provides information about what experiences are important to older adults with cognitive impairment. The concept map can be used to develop a patient experience index for the elderly residents of long-term care facilities.

한국의 장기요양서비스에 대한 RUG-III의 적용가능성 (On the Feasibility of a RUG-III based Payment System for Long-Term Care Facilities in Korea)

  • Purpose: The purpose of this study was to classify the elderly in long-term care facilities using the Resource Utilization Group(RUG-III) and to examine the feasibility of a payment method based on the RUG-III classification system in Korea. Data was collected from 530 elderly residents over sixty, residing in long-term care facilities. Resource use for individual patients was measured by a wage-weighted sum of staff time and the total time spent with the patient by nurses, aides, and physiotherapists. Result: The subjects were classified into 4 groups out of 7 major groups. The group of Clinically Complex was the largest (46.3%), and then Reduced Physical Function(27.2%), Behavior Problems (17.0%), and Impaired Cognition (9.4%) followed. Homogeneity of the RUG-III groups was examined by total coefficient of variation of resource use. The results showed homogeneity of resource use within RUG-III groups. Also, the difference in resource use among RUG major groups was statistically significant (p<0.001), and it also showed a hierarchy pattern as resource use increases in the same RUG group with an increase of severity levels(ADL). Conclusion: The results of this study showed that the RUG-Ill classification system differentiates resources provided to elderly in long-term care facilities in Korea.

요양병원 간호사의 감정노동과 직무몰입이 이직의도에 미치는 영향 (The Effects of Emotional Labor and Job Involvement on Turnover Intention of Nurses in Long-term Care Hospitals)

  • Purpose: The purpose of this study was to identify the influencing factors on turnover intention of nurses in long-term care hospitals. Methods: Data were collected from 210 nurses in 11 long-term care hospitals in B city. Data were analyzed using descriptive statistics, t-test, ANOVA, Pearson's correlation coefficient, and Stepwise multiple regression. Results: Turnover intention was significantly correlated with emotional labor(r=.35, p<.001) and job involvement (r=-.38, p<.001). In a multiple regression, emotional labor (${\beta}=.31$, p<.001), age (${\beta}=-.27$, p<.001), job satisfaction (${\beta}=.24$, p<.001), job involvement (${\beta}=-.23$, p<.001), and number of patients assigned(${\beta}=.14$, p=.009) were associated with turnover intention. These factors attributed to 41.2% of the total variance in turnover intention. Conclusion: Considering the results of this study, proactive educational and/or human resource management interventions need to be developed especially for those younger nurses in order to reduce emotional labor as well as to promote job satisfaction and job involvement of nurses in long-term care hospitals.

Effects of Compassion Satisfaction, Compassion Fatigue, Burnout on the Attitude towards Terminal Care among Care Providers in Long- term Care Facilities

  • The purpose of this study was to investigate the effects of compassion satisfaction, compassion fatigue, burnout on the attitude towards terminal care among care providers in long-term care facilities. In order to achieve the purpose of the study, data was collected from 126 care providers in long-term care facilities in Chungbuk and Kangwon province. Data were collected with self-administrated questionnaires and analyzed by hierarchical multiple regression. Findings of this study is as follows. Firstly, attitude towards terminal care was positively affected by compassion satisfaction. Secondly, the attitude towards terminal care was negatively affected by compassion fatigue. Thirdly, the attitude towards terminal care was negatively affected by burnout. In conclusion, these findings provide strong empirical evidence for the importance of compassion fatigue, compassion satisfaction and burnout in explaining attitude towards terminal care among care providers in long-term care facilities. Therefore, strategies to decrease compassion fatigue and burnout and improve compassion satisfaction are needed to improve the attitude towards terminal care among care providers in long-term care facilities.

장기현장실습 프로그램의 만족도 영향요인 탐색: K대학 IPP 프로그램을 중심으로 (An exploration of factors affecting the Satisfaction of long-term on-site training: Focusing on IPP of K University)

  • The purpose of this study was to investigate the factors affecting the satisfaction of the program for the quality improvement of the long-term on-site training operated by the university. The survey questionnaire data of 393 students who participated in the IPP program of K university were used to analyze the data and the results were as follows: First, factors affecting the satisfaction of long-term on-site training were corporation factors and university factors. Specifically, job contents, coaching, job scope, center support were found to have a significant effect in that order. Second, the factors affecting the satisfaction of long-term on-site training were different according to group characteristics(IPP participation types, major types, corporation types). It is necessary to intensively manage the common and important corporation factors, and it is necessary to closely examine each factor derived from group characteristics, and to support and strive at university level according to group characteristics in student counseling and matching.
