Cognitive Function and Activity of Daily Living of Older Adults Using Long-term Care Service (장기요양 이용 재가노인의 인지기능과 일상생활 능력)

  • The purpose of this study was to analyze the level of the cognitive function and activities of daily living of the beneficiary older adults at home based on Korean Long-term Care Insurance System. A cross-sectional descriptive survey was conducted from November 2010 to May 2011, the final respondents were 1,026 beneficiary older adults taking home visit care covered in Korean long-term care insurance system. The questionnaire included general characteristics of subjects, cognitive function, ADL(Activity of daily living). The data was analyzed using the SPSS 20.0 version. There was significant difference in cognitive function and ADL between 1st Grade, 2nd Grade and 3rd Grade of long-term care classification. The correlated factors of cognitive function were ADL, long-term care grade, disability of arm and leg, limitation of joint, bed sore and tube feeding. The correlated factors of ADL were cognitive function, long-term care grade, disability of arm and leg, bed sore and tube feeding. This study suggests that cognitive functions have to be mainly considered in long-term care grade. It is necessary to make an effort to develop long-term care grade in Korean long-term care insurance system an cognitive function improvement program for the beneficiary older adults. Above all things government will be seriously contemplating of revise contents for long-term care grade to provide quality of care for the older adults.

Ownership of Long-Term Care Facility and Incidence of Pressure Ulcers among Republic of Korea

  • Background: In 2008, Korea implemented a new type of social insurance known as "long-term care insurance". We examined the association between ownership of long-term care facilities and the incidence of pressure ulcers after the implementation of "long-term care insurance". This study is a population-based retrospective cohort study from 2006 to 2013. Methods: We used medical claims data from the Korean National Health Insurance Corporate Elderly Cohort Database from 2006 to 2013. These data comprise a nationally representative sample. To avoid confounders, only patients admitted to one long-term care facility and who stayed for >70% of the follow-up time were included; as a result, 3,107 individuals were enrolled. The main independent variable was the operating entity of the long-term care facility (local government, corporate bodies, and private for-profit owners), and the dependent variable was the 1-year incidence of pressure-ulcers. Survival analysis (Cox proportional hazard model) was used as an analysis method. Results: Compared to patients admitted to local government long-term care facilities, patients admitted to private long-term care facilities had a significantly higher 1-year risk of pressure ulcers (hazard ratio [HR], 1.94; 95% confidence interval [CI], 1.29-2.91); the risk was especially high among patients who were cognitively dependent (HR, 2.34; 95% CI, 1.25-4.37). Conclusion: Patients admitted to private for-profit long-term care facilities were more likely to have pressure ulcers compared to those in local government and corporate body long-term care facilities. Appropriate assessment tools and publicly available information, as well as more restricted legal requirements, are needed to improve the care quality and outcomes of patients in long-term care facilities.

The Want, its Determinants and the Willingness to Pay of the Long Term Care Service (장기요양 서비스를 누가, 얼마나, 얼마에 원하고 있는가? - 장기요양 서비스의 욕구와 결정요인 및 지불의사금액 -)

  • Before introducing the national long-term care insurance in 2008, the want for long term care service has to be estimated and analysed. This study estimates the demand and analyses what determines the want of long term care service. This study investigated data of 3f6 elderlies, that was collected by age stratified random sampling. The elderies resided in Onyang 4 - dong (urban area) and Dogo-myun (rural area) In the city of Asan. The researchers visited the elderlies and their care giver, and assessed their demand for the long term care service and examined physical, mental, socio-economic status by the assessment tools for Korean Long-Term Care System. $64\%$ of the those who are entitled to be served refuse the long term care service. $26.7\%$ of them wants for home care service and $7.9\%$ want facility care service. It is estimated that the want of home care service are three or four times as much as that of facility care service. The demand for long term care service is 5.155 times higher for those who live in rural area (p=0.000), 3.040 times higher for those who do not have spouse(p=0.057), and 3.356 times higher for the people who is in medicaid than medical insurance(p=0.029). However, income(p=0.782), means(p=0.614), living alone(p=0.223), number of family to live with (p=0.341) and age of the elderly(p=0.420) are not related with the demand of long term care service. The assessment tools for Korean Long-Term Care System for need evaluation of the long term care service can reflect the demand well.(p=0.024) If medical care will cover $80\%$ of total cost, the willingness to pay of the out of pocket money of the people with medical insurance is 67,400 Korean Won(66.77 US$) for the home care service and 182,500 Korean Won(180.78 US$) for the facility care service. There is possibility that long term care demand is still small after Introducing the long term care Insurance due to the care given by family members. When developing service delivery system of long term care insurance, rural area has to be given more consideration than urban area because of the higher demand. The people who do not have spouse or are in medicaid have to be given special consideration as well.

A Study on the Long-Term Relationship Intention Process According to Consumer Characteristics (소비자 특성별 장기적 관계지향성 형성과정 연구)

  • The purpose of this study is to examine the differences of long-term relationship intention process according to consumer characteristics. Data for this research was collected from 540 women older than 20 years old in Seoul, Gwangju, and Gyeonggi-do. The results are as follows. First, relationship consumers having relationship with stores were different from customers having relationship with a brand in specific store in the long-term relationship intention process. Second, consumers having relationship for a long period and consumers having relationship for a short period showed differences in long-term relationship intention process. Third, strong relationship consumers and weak relationship consumers showed differences in long-term relationship intention process. Forth, involvement groups showed differences in long-term relationship intention process. These results imply that fashion retailers need set up the relationship strategy for subdivision groups along consumer characteristics.

  • In this study, I analysed the influence of the Korean Officially Supported Export Credit System upon Improvement upon Korean Export, investigated Korean Officially Supported Export Credit System focussing on the Short-term Export Loan System & Medium and Long-term Export Loan System in Korea Eximbank and sought for the measures overcoming problems therewith. In my research I started from a basic model analyzing Short-term Export Loan & Medium and Long-term Export Loan using added several variables that have effect on export such export relative price, world income and the amount of post-shipment credit bank guarantee. As a result, amount of Short-term Export Loan & Medium and Long-term Export Loan, world income and export relative price showed expected responses and Short-term Export Loan was significant. In case of amount of Short-term Export Loan System was statistical significance.

A Study on the Relationship Between on-site Training During the School Year and Job Satisfaction after Employment: A Comparison between Short-term and Long-term on-site Training (재학 중 현장실습과 취업 후 직무만족 간의 관계에 관한 연구: 장·단기현장실습의 비교)

  • The purpose of this study is to investigate the relationship between post - employment job satisfaction and both short-term (4-12 weeks) and long-term (12+weeks) on-site training. For this purpose, 405 graduates who had completed on-site training (205 short-term, 200 long term) during the school year were surveyed. The results of the comparative analysis of both short-term and long-term on-site training participants are as follows: In both short-term and long-term on-site training, it was found that on-site performance during the school year did not directly affect post-employment job satisfaction. In the case of short-term on-site training participants, job match and organizational commitment were found to have no mediating effect on the correlation between on-site training performance and job satisfaction. On the other hand, in the case of long-term on-site training participants, the analysis showed that job match and organizational commitment had mediating effects on the correlation between on-site training performance and job satisfaction. These effects are not solely attributable to differences in duration of training; the differences in operating systems and the degree of preparation derived from these systems also affect the level of on-site training for students, businesses, and schools. This paper summarizes these findings and suggests the following improvement plans for on-site training in the future: First, short-term on-site training is required to establish a systematic basis in order to enhance students' preparedness level. Second, both short-term and long-term on-site training should improve the skills and field understanding for students' majors through systematic quality management during the training period. Third, it is necessary for universities to increase expectations and quality of short-term on-site training for all involved while simultaneously reducing the gap between educational goals and practice in this field.

A Study of Comparison of Moral Distress and Burnout between Long-term Care Hospital Nurses and General Hospital Nurses (요양병원과 종합병원 간호사의 도덕적 고뇌와 소진 비교연구)

  • Purpose: The purpose of this study was to compare the moral distress and burnout between long-term care hospital nurses and general nurses. Methods: This study was a cross-sectional survey. Participants were 193 nurses (long-term care hospital 95, general hospital 98) working in a hospital in Seoul, Daegu and Busan city. Data were collected from October 02 to November 30, 2018 using a structured questionnaires and analyzed with SPSS/PC ver 21.0 programs. Results: The scores of moral distress by nurses working in long-term care hospitals were higher than those of nurses in general hospitals. The score of burnout by nurses working in general hospitals was significantly higher than that of nurses in long-term care hospitals. There was a significant positive relationship between moral distress and burnout in both long-term care hospital nurses and general hospital nurses. Conclusion: This research shows that long-term care hospital nurses have higher moral distress and burnout than those of general hospital nurses. Therefore, nursing managers and organization should strive to raise the perception of moral distress and burnout in long-term care hospital nurses.

Development of Education & Training Programs for Senile Long-Term Care Managers (the 2nd. grade) (2급 노인장기요양보호사 교육훈련 프로그램 개발)

  • Purpose: This study aimed to develop the first standardized education & training program for education of senile long-term care managers to improve the quality of senile long-term care service. Methods: The study developed programs through literature review, specialists' conference and interview with the linear approach from February, 2007 to May, 2007. Results: Senile long-term care managers should have qualification of 51 items in knowledge, 35 items in technology and 17 items in attitude. Total 8 subjects were education & training subjects of senile long-term care managers and the study proposed targets of each subject and education & training level divided into top, middle and bottom. Total education & training hours were composed of 70 hours in theory, 50 hours in practical technique and 40 hours in social welfare practice in agencies. Conclusion: The study will contributed development of the first education & training programs for education of senile long-term care managers. Therefore, the study proposes that the study will be applied to a systematical education & training course through program verification after applying real programs to senile long-term care managers and that it is necessary to develop the standardized education & training program for higher senile long-term care managers.

A Study on the Affecting Factors to Utilization of Long Term Care Hospitals According to the Elderly Long Term Care Insurance System in Korea (노인장기요양보험 도입 후 요양병원 이용에 영향을 미치는 요인)

  • The major purpose of this study is to find out relevant factors affecting utilization of Long Term Care Hospitals since the Elderly Long Term Care Insurance System was adopted in Korea. The sample hospitals of this study are 5 long term care hospitals located in 4 big cities and 1 local area. The research data were collected with structured questionnaire from 247 patients and patients' protectors in 5 sample hospitals. Analyzing methods are descriptive statistics, factor analysis and multiple regression with SPSS(version 12.0). Major results of this study are as follows. 1) Utilization and recommendation of patients is affected significantly by the level of hospital facilities (0.043), fee level(0.026), level of staff (0.000), and discomfort of services(0.001). 2) Level of staff is very positively correlated with utilization and recommendation of patients. 3) Discomport of services is very negatively correlated with utilization and recommendation of patients. On the basis of results this study conclude that the management of Long Term Care Hospitals is required conclude to improve the level of staff and facilities and to solve discomport problems of services for patients' marketing. And also more in-depth study on the utilization factors of long term care hospital in Korea is required.

Corporate Governance and Long-term Corporate Survival in an Emerging Economy (신흥국 기업의 지배구조와 기업의 장기 생존)

  • This paper investigates how corporate governance characteristics are related to long-term corporate survival in an emerging economy. We used the data of 311 companies listed on the Korean Stock Exchange (KSE) in 1979 and examined the survival chances of those companies through the IMF crisis in 1998, upon governance characteristics that are expected to increase long-term strategic orientations. We utilized Cox regression model for the analysis. The results indicate that firms with particular governance characteristics that may be tied to CEO's long-term orientations show higher long-term survivability. Specifically, the probability of a firm's long-term survival is increased when founding family ownership is sustained, the company ownership is concentrated, and the CEO is the largest shareholder. This study has significance in that it is one of initial tries to examine the impact of corporate governance on long-term corporate survival with large scale statistical analysis. Also, the study findings provide some clues as to why the portion of family firms in emerging economies is continuously increased, thus providing meaningful insights to corporate governance literature.
