Experiences of Life-Sustaining Treatment Decisions among Patients with Terminal Cancer

  • Purpose: The purpose of this study was to examine the life-sustaining treatment decisions of terminal cancer patients. Methods: Data on 10 terminal cancer patients who decided to withhold or withdraw from treatment were collected using in-depth interviews conducted from February 8 to October 30, 2019. Data were collected until saturation was reached and then analyzed using Colaizzi's phenomenological method. Results: In this study, six thematic clusters were identified: "having complicated feelings", "making choices to protect everyone", "accepting and preparing for death", "feeling distress", "pursuing spiritual wellbeing", and "evaluating the new system". Conclusion: When facing death, terminal cancer patients often made choices to protect their family and their dignity with uneasiness of mind when deciding to withdraw from life-sustaining treatments. Though many patients had accepted and prepared for death, they experienced distress about leaving children behind after death. They also pursued spiritual well-being to find peace after deciding to withdraw from life-sustaining treatment. In addition, participants evaluated the new system of policies pertaining to decisions on life-sustaining treatment. Thus, various approaches regarding acceptance and preparation for death, communication with family, hope, and spiritual comfort should be taken in educational interventions to assist terminal cancer patients as they decide whether to withdraw from life-sustaining treatment.

A new record of high temperature tolerance species, Pyropia kitoi com. nov.(Bangiaceae, Rhodophyta), from Korea

  • Neoporphyra kitoi Ma. Abe, N. Kikuchi, Tamaki, Tom. Sato, Murase, Fujiyoshi & Mas. Kobayashi has been known as an endemic species in Japan. Its high temperature tolerance suggests that it could be advantageous for cultivation. In this study, we collected it from the Ulleungdo island, Korea and transferred it into Pyropia for a new combination, identified as Pyropia kitoi(Ma. Abe, N. Kikuchi, Tamaki, Tom. Sato, Murase, Fujiyoshi & Mas. Kobayashi) D.J. Kim, T.O. Cho & B.Y. Won comb. nov. based on morphological and molecular analyses. Pyropia kitoi is also reported as a new record species in the list of Korean macroalgal flora. Although we didn't observe the emergence of new blades from the rhizoidal cells, which is a key character for this species, our molecular analysis of rbcL revealed that our samples from Korea were congruent with "Neoporphyra kitoi" from Japan and were nested within the clade of Pyropia. The gene sequence divergence between the Korean and Japanese samples was 0-0.2%.

경도인지장애 노인의 인지기능, 도구적 일상생활수행능력, 우울 및 삶의 질 관계 (Cognitive Functions, Instrumental Activities of Daily Living, Depression and Quality of Life in the Elderly with Mild Cognitive Impairment)

  • Purpose: This research study not only examine the relationship between cognitive function, IADL, depression and quality of life, but also clarifies factors impacting quality of life for the elderly. The purpose of this study is to administer health promotion programs and to improve quality of life for the elderly. Methods: Data were collected from May 20, to June 10, 2014. The participants were 157 elderly persons with mild cognitive injury (MCI), who had under 23 points on MoCa-K from three different senior welfare centers in Daejeon. The questionnaires were conducted by four university students after receiving instruction from the researcher and consisted of general characteristics, cognitive function, depression and quality of life. SPSS WIN 19.0 program was used for data analysis. Results: Cognitive function showed positive correlation with quality of life (r=175, p=.028). IADL did not show correlation with quality of life (r=-.005, p=.947). However cognitive function(r=-.344, p<.001) and depression (r=-.623, p<.001) showed negative correlation with quality of life. In addition, the factors impacting quality of life were cognitive function (${\beta}=.14$, p=.29) and depression (${\beta}=-.61$, p<.001) which accounted for 39% of quality of life. Conclusion: These findings showed that the participants had depression and their cognitive function was low. As a result, the more participants' cognitive function decreased, the more their quality of life decreased. In addition, the more their depression decreased, the more their quality of life improved.

End-of-Life Care Practice in Dying Patients after Enforcement of Act on Decisions on Life-Sustaining Treatment For Patients in Hospice and Palliative Care or at the End of Life : A Single Center Experience

  • Purpose: The Act on Hospice and Palliative Care and Decisions on Life-Sustaining Treatment for Patients at the End of Life came into force in February 2018 in Korea. This study reviews the practices of end-of-life care for patients who withdrew or withheld life-sustaining treatment at a tertiary care hospital, addresses the limitations of the law, and discusses necessary steps to promote patient-centered self-determination. Methods: We retrospectively analyzed the medical records of patients who died after agreeing to withhold life-sustaining treatment in 2018 at our university hospital. The cause of death, the intensity of end-of-life care, and other characteristics were reviewed and statistically analyzed. Results: Of a total of 334 patients, 231 (69%) died from cancer. The decision to stop life-sustaining treatment was made by family members for 178 patients overall (53.3%) and for 101 (43.7%) cancer patients, regardless of the patient's wishes. When the patient decided to stop life-sustaining treatment, the time from the authorization to withhold life-sustaining treatment to death was longer than when the decision was made by family members (28.7±41.3 vs 10.5±23.2 days, P<0.001). Conclusion: In many cases, the decision to discontinue life-sustaining treatment was made by the family, not by the patient. In order to protect human dignity based on the patients' self-determination, it is necessary for patients to understand their disease based on careful explanations from physicians. Ongoing survey-based research will be necessary in the future.

Stereospecific Synthesis of the (2R,3S)- and (2R,3R)-3-Amino-2-hydroxy-4-phenylbutanoic Acids from D-Glucono-δ-lactone

  • The enantiomerically pure (2R,3S)- and (2R,3R)-3-amino-2-hydroxy-4-phenylbutanoic acids (AHPBA) 1 and 3 are readily obtained from D-glucono-a-lactone. Both AHPBAs are the structural key units of KMI derivatives which are the potent inhibitors of BACE 1 ($\beta$-secretase) and HIV protease. Additionally, the obtained AHPBAs 1 and 3 are converted to dipeptides of bestatin stereoisomers 2 and 4.

한국여성노인들의 노년에 적응과 노년기 삶의 모습에 대한 질적 연구 (How Do Korean Elderly Women Adapt to Old Age?:Themes and Sources of Meaning in Their Later Lives from Life Stories)

  • The purpose of this study is to explore what makes the elderly women resilient in old age and how they reconstruct their later lives. This study used life histories extracted from tape-recorded interviews of 13 elderly women and analyzed key issues focused on how to adapt in old age. Main results are following: First, emerging themes to maintain life satisfaction in old age are related to both informants' inner and outer life worlds; subjective reinterpretation of life, feeling rich, family, social activities, overcoming social stigma. With supporting and supplementing one another, these five dimensions are important contributors to better later life. Second, where my informants search for meaning of life in old age is the critical clue of understanding the later life styles. Some of them are still searching meaning of life in roles as a ‘mother’, while others are finding their sense of worth outside the family or in themselves. These results show that the elderly are interacting with themselves and their surroundings in adapting to old age and different tie styles according to different sources of life satisfaction manifest the changes among the elderly and Korean society.

임상간호사의 일과 삶의 균형과 간호서비스 질의 관계에서 일과 삶의 균형 조직문화가 미치는 매개효과 (Mediating Effect of Organizational Culture Supporting Work-Life Balance on the Relationship between Work-Life Balance and Quality of Nursing Service in Clinical Nurses)

  • Purpose: This study aimed to confirm the mediating effect of work-life balance organizational culture in the relationship between work-life balance and quality of nursing service for clinical nurses. Methods: Participants included 224 nurses from four hospitals located in D city. Data was collected from 3 to 21 August 2020 using a self-report questionnaires. Descriptive statistics, t-test, one-way ANOVA, Scheffé test, and Pearson's correlation coefficient were used for analysis. The mediating effect was assessed by Baron and Kenny's three-step hierarchical regression analysis and bootstrapping methods. Results: There were significant relationships between work-life balance and work-life balance organizational culture (r=.45, p<.001), work-life balance and quality of nursing service (r=.18, p=.005), and work-life balance organizational culture and quality of nursing service (r=.34, p<.001). Also, work-life balance organizational culture (β=.32, p<.001) showed a total mediating effect on the relationship between work-life balance and quality of nursing service. Conclusion: The development and provision of programs that can improve the work-life balance and establish an organizational culture that supports it will help improve the quality of nursing services.

Contribution analysis of Hanwoo carcass traits on unit price in national slaughter house

  • The aim of this study was to analyze the contribution factors (backfat thickness, eye muscle area, carcass weight, marbling score, and feeding period) affecting meat unit price (South-Korean Won / Kg of meat). The best slaughtering age to maximize unit price was also assumed. All data used in this study were acquired from the Korea Institute for Animal Products Quality Evaluation from 2010 to 2014. Contributions to the estimated unit price of cows by the following factors, backfat thickness, eye muscle area, carcass weights, feeding period, and marbling score were 2.65%, 0.04%, 1.58%, 1.58%, and 95.72%, respectively. Contribution to estimated unit price of steers by the same factors (backfat thickness, eye muscle area, carcass weights, feeding period, and marbling score) were 7.88%, 1.24%, 0.07%, 90.81%, and 95.72%, respectively. Slaughtering ages ranged from 26 to 36 months and the data were separated into each month for an 11 month period. The unit price of meat from Hanwoo slaughtered at 30 months was highest among groups. The lowest unit price was observed in the group belonging to the Hanwoo slaughtered at 36 months. In conclusion, of all contributing factors, marbling score affected unit price the most. Based on our results, it is recommended that the optimal slaughtering age be set at 30 months to maximize unit price. Moreover, the feeding of beef cattle past 30 months of age is not recommended because of the increase in feeding costs.

SSR Marker Linked to f Locus in Soybean

  • Soybean has a morphological type with a broadened and flattened stem. Fasciation has been suggested as a new gene for soybean research. SSR marker linked to the $\Large f$ locus that controls fasciation phenotype has not identified within 10 cM. A mapping population consisting of 94 $F_2$ progenies was derived from a cross between wild type Clark (FF) and fasciation mutant C32 (${\Large f}{\Large f}$). The phenotype of $F_2$ individual plants was recorded at R2 and R3 growth stage from field. One-thousand 10-mer oligonucleotide RAPD primers and 29 SSR primers selected from the D1b+W of the soybean molecular linkage map were used. A genetic map was constructed from the segregating 35 RAPD, four SSR markers and one phenotypic(wild type/fasciation) marker. The segregation ratios of 3 : 1 observed in the $F_2$ population and the Chi-square values strongly suggest that the fasciation trait is controlled by a single recessive gene. Satt537 marker was linked to $\Large f$ locus at a distance of 9.6 cM. Assignment of the $\Large f$ locus to linkage group D1b+W and identification of markers can be used as an initial step for fine mapping of the $\Large f$ gene.
