A Case of the Minimal Change Nephropathy Associated with Small Cell Lung Cancer (소세포폐암에 동반된 미세변화 신증후군 1예)

  • Nephrotic syndrome is a relatively rare complication of malignancy. A few cases of nephrotic syndrome accompanying Hodgkin's disease, non-Hodgkin lymphoma, leukemia and other malignancies have been reported since the first case of the nephrotic syndrome associated with extrarenal malignancy was reported in 1922. Hodgkin's disease and solid tumors are known to be the most common malignancies accompanying nephrotic syndrome. The pathologic findings of kidney in patients with Hodgkin's disease commonly show minimal change nephropathy. Membranous glomerulonephropathy is the most common pathologic feature in patients with solid tumors. Although membranous glomerulonephropathy related to small cell lung cancer has rarely been reported in Korea, minimal change nephropathy accompanying small cell lung cancer has never been reported. We present here a case of a 70 year-old male with minimal change nephropathy that was related to small cell lung cancer. We detected small cell lung cancer during the diagnosis work-up of nephrotic syndrome. We suggest that nephrotic syndrome can be a manifestation of underlying malignancy.

Clinicopathologic Assessment of Ocular Adnexal Lymphoproliferative Lesions at a Tertiary Eye Hospital in Iran

  • Background: The most common type of ocular lymphoma is non-Hodgkin lymphoma (NHL), categorized into two groups: indolent (slow growing) and aggressive (rapid growing). Differentiating benign reactive lymphoid hyperplasia (RLH) from malignant ocular adnexal lymphoma (OAL) is challenging. Histopathology, immunohistochemistry (IHC) and flow cytometry have been used as diagnostic tools in such cases. Materials and Methods: In this retrospective case series, from 2002 to 2013 at Farabi Eye Center, 110 patients with ocular lymphoproliferative disease were enrolled. Prevalence, anatomical locations, mean age at diagnosis and the final diagnosis of the disease with IHC were assessed. Comparison between previous pathologic diagnoses and results of IHC was made. Immunoglobulin light chains and B-cell and T-cell markers and other immuno-phenotyping markers including CD20, CD3, CD5, CD23, CD10, CYCLIND1 and BCL2 were evaluated to determine the most accurate diagnosis. The lymphomas were categorized based on revised European-American lymphoma (REAL) classification. Results: Mean age ${\pm}$ SD (years) of the patients was $55.6{\pm}19.3$ and 61% were male. Patients with follicular lymphoma, large B-cell lymphoma or chronic lymphocytic leukemia/small cell lymphoma (CLL/SLL) tended to be older. Nine patients with previous diagnoses of low grade B-cell lymphoma were re-evaluated by IHC and the new diagnoses were as follows: extranodal marginal zone lymphoma(EMZL) (n=1), SLL(n=1), mantle cell lymphoma (MCL) (n=3), reactive lymphoid hyperplasia RLH (n=2). Two cases were excluded due to poor blocks. Flow cytometry reports in these seven patients revealed SLL with positive CD5 and CD23, MCLwith positive CD5 and CyclinD1 and negative CD23, EMZL with negative CD5,CD23 and CD10. One RLH patient was negative for Kappa/Lambda and positive for CD3 and CD20 and the other was positive for all of the light chains, CD3 and CD20. Orbit (49.1%), conjunctiva (16.1%) and lacrimal glands (16.1%) were the most common sites of involvement. Conclusions: Accurate pathological classification of lesions is crucial to determine proper therapeutic approaches. This can be achieved through precise histologic and IHC analyses by expert pathologists.

  • A randomized, double blind, placebo-controlled human intervention study involving 62 healthy volunteers was performed to investigate the effects of Keumsa sangwhang (Phellinus linteus) mushroom extracts (KPLE) on natural killer (NK) cell activity in peripheral blood. The volunteers were randomly distributed into two groups, one receiving KPLE (3.3 g/day) and the other a placebo by oral administration for 8 weeks. In this study, the number of NK cells did not increase with KPLE administration, however the cytotoxic activity of NK cells against the Jurkat leukemia cell line increased significantly. This result suggests that administration of KPLE induces cell-mediated immunity by increasing NK cell activity in humans.

Immunomodulatory Effects of an Extract from Pinus koraiensis Cone Peel in LP-BM5 Murine Leukemia Viruses-Induced Murine Acquired Immune Deficiency Syndrome (면역 결핍 동물모델에서 잣피 추출물의 면역조절 효과)

  • The immune system is a complex process within the body that protects against disease. Recently, many studies have attempted to discover immunomodulative compounds from natural sources. Pinus koraiensis (PK) cone shell is a by-product of PK. One of the major compounds of PK cone shell is dehydroabietic acid, which has bioactivity, including antiulcer and anti-inflammatory activities. Therefore, this study was performed to examine the immunomodulative effects of PK cone shell. The immunomodulatory effects of PK cone shell extracted with 20% ethanol (EtOH) in vivo were examined initially by measuring the natural killer (NK) cell activity, phagocytic activity, Th1/Th2 cytokines release, serum immunoglobulin, and T/B cell proliferation. The NK cell activity and phagocytic activity were increased significantly by a treatment with a 20% EtOH extract of PK cone shell. Th1 type cytokine and T cell proliferation increased and Th2 type cytokine, B cell proliferation and serum immunoglobulin A, G, and E decreased after a treatment with PK cone shell extract. The 20% EtOH extract of the PK cone shell normalized the unbalanced production of Th1/Th2 type cytokine. This suggests that a 20% EtOH extract of PK cone shell has great potential as a health food.

Inhibitory Effects of Syzygium aromaticum Ethanol Extracts on IgE Mediated RBL-2H3 cell Activation (IgE 매개 RBL-2H3 세포 활성화에 대한 정향 에탄올 추출물의 억제 효과)

  • Objectives : In this report, we investigated the effect of ethanol extract of Syzygium aromaticum(L.) Merr. & Perry.(SAE) on the RBL-2H3 cell-mediated allergic response and studied its possible mechanisms of action. Methods : Cytotoxicity on RBL-2H3 cell was evaluated by MTT assay. Anti-allergic activity of SAE was assessed by ${\beta}$-Hexosaminidase and Histamine secretion, ${\beta}$- Hexosaminidase and Histamine secretion were measured by ELISA assay. Evaluate the mechanisms of effect of SAE on the secretion of degranulate mediators, we examined the effect of SAE on the activation of mitogen-activated protein kinases using western blot analysis. Results : SAE had no cytotoxicity on rat basophilic leukemia cell(RBL-2H3). Moreover SAE dose-dependently inhibited RBL-2H3 cell degranulation and histamine release. SAE specifically blocked the IgE-induced p38 mitogen-activated protein kinase activation. Conclusions : Our findings provide evidence that Syzygium aromaticum ethanol extract inhibits mast cell derived allergic reaction, and also demonstrate the involvement of p38 MAPK phosphorylation.

The Effects of Bujeong hangamtang on antitumor Immune Response (부정항암탕(扶正抗癌湯)이 항종양(抗腫瘍) 면역반응(免疫反應)에 미치는 영향(影響))

  • Bujeonghangamtang(扶正抗癌湯) has been used for cure of tumor as a traditional medicine without any experimental evidence to support the rational basis for its clinical use. This study was carroed out to evaluate the possible therapeutic or antitumoral effects of Bujeonghangamtang extract against tumor, and to carry out some mechanisms responsible for its effect. Some kinds of tumor were induced by .the typical application of 3-methylcholanthrene (MCA) or by the implantation(s.c) of malignant tumor cells such as leukemia cells(3LL cells) or sarcoma cells(Sl80 cells). Treatment of the Bujeonghangamtang water-extract (dailly 1mg/mouse, i. p.) was continued for 7 days prior to tumor induction and after that the treatment was lasted for 20 days. Against squamous cell carcinoma induced by MCA, Bujeonghangamtang decreased not only the frequency of tumor production but also the number and the weight of tumors per tumor bearing mice (TBM). Bujeongmngamtang also significantly suppressed the development of 3LL cell and S180 cell-implanted tumors in occurence-frequency and their size, and some developed tumors were regressed by the continuous treatment of Bujeonghangamtang extract into TBM. In vitro, treatment of Bujeonghangamtang extract had no effect on the growth of some kinds of cell line such as FsaII, A431 strain but significantly inhibited the proliferation of 3LL, S180 cells and augmented the DNA synthesis of mitogen-activated lymphocytes. Bujeonghangamtang also stimulated the migrative ability of leukocyte, the MIF and IL-2 production of T lymphocytes, but not IL 6 production of B cells. Bujeonghangamtang-administration to mice enhanced NK cells attivities. These results demonstrated that Bujeonghangamtang extract exhibited a significant prophylactic benefits against tumors and its antitumor activity was manifested depending on the type of tumor cells. And these results also suggested that effect of Bujeonghangamtang might be chiefly due to nonspecific enhancement of NK cell activities and cell-mediated immune responses.

Induction of Apoptosis by Ethanol Extract of Lythrum anceps (Koehne) Makino in Human Leukemia U937 Cells (인체백혈병 U937 세포에서 부처꽃 에탄올추출물에 의한 apoptosis 유도)

  • Purple loosestrife-Lythrum anceps (Koehne) Makino is a herbaceous perennial plant belonging to the Lythraceae family. It has been used for centuries in Korea and other Asian traditional medicine. It has been showed pharmacological effects, including anti-oxidant and anti-microbial effects. However, the mechanisms underlying its anti-cancer effect are not yet understood. In this study, we investigated the mechanism of apoptosis signaling pathways by ethanol extract of Lythrum anceps (Koehne) Makino (ELM) in human leukemia U937 cells. Treatment with ELM significantly inhibited cell growth in a dose-dependent manner by inducing apoptosis, as evidenced by the formation of apoptotic bodies (ApoBDs), DNA fragmentation and increased populations of sub-G1 ratio. Induction of apoptosis by ELM was connected with up-regulation of death receptor (DR) 4 and DR5, pro-apoptotic Bax protein expression and down-regulation of anti-apoptotic Bcl-2 protein, and inhibitor of apoptosis protein (IAP) family proteins, depending on dosage. This induction was associated with Bid truncation, mitochondrial dysfunction, proteolytic activation of caspases (-3, -8 and -9) and cleavage of poly(ADP-ribose) polymerase protein. Therefore, our data indicate that ELM suppresses U937 cell growth by activating the intrinsic and extrinsic apoptosis pathways, and thus may have applications as a potential source for an anti-leukemic chemotherapeutic agent.

Gene Expression Profiling of 6-MP (6-mercaptopurine) in Liver

  • The KFDA (Korea Food & Drug Administration) has performed a collaborative toxicogenomics project since 2003. Its aim is to construct a toxicology database of 12 compounds administered to mice at initial phase. We chose 6-MP (6-mercaptopurine) which has been used in the treatment of childhood leukemia. It was administered at low (0.224 mg/kg) and at high (2.24 mg/kg) dose (5 mice per group) intraperitonealy to the postnatal 6 weeks mice, then the serum and liver were collected at the indicated time (6, 24 and 72 h) after scarification. Serum biochemical markers for liver toxicity were measured and histopathologic studies also were carried out. The gene expression profiling was carried out by using Applied Biosystems 1700 Full Genome Expression Mouse. By self-organization maps (SOM), we identified groups with unique gene expression patterns, some of them are supposed to be related to 6-MP induced toxicity, including lipid metabolism abnormality, inflammatory response, oxidative stress, ATP depletion and cell death. The potential toxic effects appearing as gene expression changes are dependent of the time of 6-MP but independent of the dosage of it. This study would contribute to establishment of international database as well as national one about hepatotoxicity.

  • Radiation-induced life shortening, carcinogenesis and other pathological changes were investigated in NIH(GP) mice after $^{60}Co-{\gamma}$ ray irradiation(100~700rads). The results were summarized as follows: 1. There were little difference in body weights, hematological examination and other clinical findings between normal and irradiated groups. 2. Mean survival time of the mice after irradiation were decreased dose-dependently. 3. Main gross findings of the mice irradiated were appeared as enlargement of spleen, thymus and lymph nodes, tumorous nodules of lung and cyst of ovary. Especially, enlargement of thymus was promineort in high dose group. 4. Microscopically, there were various findings including myeloid leukemia, thymic lymphoma, lung adenoma, adenosquamous cell carcinoma of pancreas, pneumonia and other pathological changes. Especially thymic lymphoma was highly frequent in the 700 rads group.

Inhibitory Effect of Benzyl Isothiocyanate on Proliferation in vitro of Human Glioma Cells

  • Malignant glioma, also known as brain cancer, is the most common intracranial tumor, having an extremely high mortality and recurrence rate. The survival rate of the affected patients is very low and treatment is difficult. Hence, growth inhibition of glioma has become a hot topic in the study of brain cancer treatment. Among the various isothiocyanate compounds, it has been confirmed that benzyl isothiocyanate (BITC) can inhibit the growth of a variety of tumors, including leukemia, glioma and lung cancer, both inside and outside the body. This study explored inhibitory effects of BITC on human glioma U87MG cells, as well as potential mechanisms. It was found that BITC could inhibit proliferation, induce apoptosis and arrest cell cycling of U87MG cells. In addition, it inhibited the expression of SOD and GSH, and caused oxidative stress to tumor cells. Therefore, it is believed that BITC can inhibit the growth of U87MG cells outside the body. Its mechanism may be related to the fact that BITC can cause oxidative stress to tumor cells.
