패혈증환자에서 혈청 TNF-$\alpha$ 및 IL-1$\beta$ (Clinical Implication of Serum TNF-$\alpha$ and IL-1$\beta$ Measurement in Patients with Sepsis)

  • 패혈증에서는 지속적인 내독소 자극에 의해 염증 반응이 계속 유발될 수 있으나, 염증반응에 주도적인 역할을 하는 말초혈액 단핵구는 반응도를 스스로 낮춤으로서 염증 반응을 감소시키는 경향을 보이는데, 이러한 현상을 내독소 내성(endotoxin tolerance)라고 한다. 내독소 내성은 과다한 염증반응에 대한 자체의 방어작용으로 이해되고 있다. 본 연구에서는 조직액에서 감염균이 증명된 패혈증 환자를 대상에서 추출한 말초 혈액 단핵구에서 내독소 내성의 현상을 관찰하고자 하였다. 방법 : American College of Chest Physician-Society의 Critical Care Medicine Consensus Conference에서 정한 sepsis의 기준을 만족하고, 감염균이 확인된 14명의 환자를 대상으로 하였으며 질환의 중증도는 APACHE II score를 이용하여 평가하였다. 대상환자의 혈청에서 TNT-$\alpha$와 IL-1$\beta$의 농도를 측정한 뒤, 말초혈액 단핵구를 추출, 배양하여, 내독소(100 ng/ml)로 자극하였다. 내독소 자극 후 24시간 뒤에 배양 상층액에서 ELISA 방법으로 TNT-$\alpha$와 IL-1$\beta$의 농도를 측정하였다. 결과 : 대상 환자는 총 14명(남자 : 8 명, 여자 6명)으로 APACHE II score는 12.2$\pm$5.7 이었다. 감염의 원발 부위는 뇨관감염, 폐렴, 아급성심내막염, 도관감염, 급성담관염 등이었다. 감염 원인균은 그람음성 간균 10례, 그람 양성균 6례였고, 이 중에는 복합감염이 2례가 있었다. 환자군의 혈청 TNF-$\alpha$는 4명에서 측정이 기능했고 평균치는 29.7$\pm$527.7 pg/ml 이었다. IL-1$\beta$는 1명에서 측정가능했고 측정치는 16.1 pg/ml 이었다. 내독소 자극 후 말초혈액 단핵구 배양 상층액의 TNF-$\alpha$농도는 패혈증 환자는 2,703$\pm$2,066 pg/ml, 대조군은 2,101$\pm$1914 pg/ml 이었고, IL-1$\beta$ 농도는 환자군은 884$\pm$1,050 pg/ml, 대조군은 575$\pm$558 pg/ml으로 두 군 사이에 의미있는 차이는 없었다. 결론 : 본 연구에서는 예상과는 달리 패혈증 환자의 말초혈액 단핵구에서 내독소내성을 관찰할 수 없었다. 향후 보다 중증의 환자를 대상으로 하고, 조기에 혈액을 채취하며, 말초혈액 상층액에 혈청을 추가하는 등의 보완을 통하여 재평가가 필요할 것으로 판단된다.

  • Objectives : To provide the information of efficacy for Sagunja-tang (Sijunzi-tang; SG), it was evaluated the anti-inflammatory effect. SG, a widely used herbal formula in tranditional Korean medicine, has been used to treat for the Boki-invigorating. In many studies, plant-derived anti-inflammatory efficacies have been investigated for their potential inhibitory effects on lipopolysaccharide (LPS)-stimulated macrophages. This study was performed to examine the anti-inflammatory effects of SG extract on LPS-stimulated RAW 264.7 cells. Methods : The productions of nitric oxide (NO), prostaglandin (PG)$E_2$, interleukin (IL)-6 and tumor necrosis factor (TNF)-$\alpha$ were examined in a macrophage cell line, RAW 264.7 cells, in the presence of the SG extract. RAW 264.7 cells were incubated with LPS $1\;{\mu}g/mL$ and SG extract for 18 hours. The anti-inflammatory activity of SG was investigated by carrageenin-induced paw edema in rats. The paw volume was measured at 0, 2 and 4 hours following carrageenin-induced paw edema in rats. Results : SG extract showed inhibitory effect on $PGE_2$, IL-6 and TNF-$\alpha$ by LPS-stimulated RAW 264.7 cells. But SG extract was not inhibitory effect on NO by LPS-stimulated RAW 264.7 cells. And administration of SG extract (1 g/kg) showed a reduction in carrageenin-induced paw edema on rats. Conclusions : These results suggest that SG extract has anti-inflammatory activities in vitro and in vivo models.

Suppressive Effects of Furonaphthoquinone NFD-37 on the Production of Lipopolysaccharide-Inducible Inflammatory Mediators in Macrophages RAW 264.7

  • 2-Methyl-2-(2-methylpropenyl)-2,3-dihydronaphthoquinone[2,3-b]furan-4,9-dione (N FD-37) is a synthetic furonaphthoquinone compound. In this study, we determined that NFD-37 could inhibit the lipopolysaccharide (LPS)-induced production of inflammatory mediators in macrophages RAW 264.7. This compound inhibited LPS-induced nitric oxide (NO) or prostaglandin (PG) $E_{2}$ production in dose-dependent manners, with $IC_{50}$ values of 7.2 ${\mu}M$ and 5.3 ${\mu}m$, respectively. As the positive controls, pyrrolidine dithiocarbamate (30 ${\mu}M$) exhibited a $57{\%}$ inhibition of NO production, and NS-398 ($1{\mu}M$) manifested a $48{\%}$ inhibition of $PGE_2$ production. The inhibitory effects of NFD-37 on NO and $PGE_2$ production were determined to occur in conjunction with the suppression of inducible NO synthase or cyclooxygenase-2 expression. NFD-37 also inhibited the production of LPS-inducible tumor necrosis factor-${\alpha}$, interleukin (IL)-$1{\beta}$ and IL-6, at $IC_{50}$ values of 4.8-8.9 ${\mu}M$. We also determined the anti-inflammatory efficacy of NFD-37 using carrageenin-induced paw edema in experimental mice.

Anti-neuroinflammatory Effects of 12-Dehydrogingerdione in LPS-Activated Microglia through Inhibiting Akt/IKK/NF-κB Pathway and Activating Nrf-2/HO-1 Pathway

  • Ginger, one of worldwide consumed dietary spice, is not only famous as food supplements, but also believed to exert a variety of remarkable pharmacological activity as herbal remedies. In this study, a ginger constituent, 12-dehydrogingerdione (DHGD) was proven that has comparable anti-inflammatory activity with positive control 6-shogaol in inhibiting LPS-induced interleukin (IL)-6, tumor necrosis factor $(TNF)-{\alpha}$, prostaglandin (PG) $E_2$, nitric oxide (NO), inducible NO synthase (iNOS) and cyclooxygenase (COX)-2, without interfering with COX-1 in cultured microglial cells. Subsequent mechanistic studies indicate that 12-DHGD may inhibit neuro-inflammation through suppressing the LPS-activated $Akt/IKK/NF-{\kappa}B$ pathway. Furthermore, 12-DHGD markedly promoted the activation of NF-E2-related factor (Nrf)-2 and heme oxygenase (HO)-1, and we demonstrated that the involvement of HO-1 on the production of pro-inflammatory mediators such as NO and $TNF-{\alpha}$ by using a HO-1 inhibitor, Zinc protoporphyrin (Znpp). These results indicate that 12-DHGD may protect against neuro-inflammation by inhibiting $Akt/IKK/I{\kappa}B/NF-{\kappa}B$ pathway and promoting Nrf-2/HO-1 pathway.

Effects of Pogonatherum paniceum (Lamk) Hack extract on anti-mitochondrial DNA mediated inflammation by attenuating Tlr9 expression in LPS-induced macrophages

  • BACKGROUND/OBJECTIVES: Mitochondrial DNA leakage leads to inflammatory responses via endosome activation. This study aims to evaluate whether the perennial grass water extract (Pogonatherum panicum) ameliorate mitochondrial DNA (mtDNA) leakage. MATERIALS/METHODS: The major bioactive constituents of P. paniceum (PPW) were investigated by high-performance liquid chromatography, after which their antioxidant activities were assessed. In addition, RAW 264.7 macrophages were stimulated with lipopolysaccharide, resulting in mitochondrial damage. Quantitative polymerase chain reaction and enzyme-linked immunosorbent assay were used to examine the gene expression and cytokines. RESULTS: Our results showed that PPW extract-treated activated cells significantly decrease reactive oxygen species and nitric oxide levels by reducing the p2phox and iNOS expression and lowering cytokine-encoding genes, including IL-6, TNF-α, IL-1β, PG-E2 and IFN-γ relative to the lipopolysaccharide (LPS)-activated macrophages. Furthermore, we observed that LPS enhanced the mtDNA leaked into the cytoplasm, increasing the transcription of Tlr9 and signaling both MyD88/Irf7-dependent interferon and MyD88/NF-κb p65-dependent inflammatory cytokine mRNA expression but which was alleviated in the presence of PPW extract. CONCLUSIONS: Our data show that PPW extract has antioxidant and anti-inflammatory activities by facilitating mtDNA leakage and lowering the Tlr9 expression and signaling activation.

  • Bee Venom (BV) has been treated in inflammatory diseases such as rheumatoid arthritis (RA). Bee venom contains several biologically active non-peptide substances as well as two major known peptides; the hemolytic peptide melittin (50%) and the neurotoxic peptide apamin, and a number of minor peptides. Previous our study showed that BV blocked LPS and SNP-induced production of NO and PG through inactivation of NF-kB which regulates expression of COX-2 and iNOS. (omitted)

Comparative Study of the Endotoxemia and Endotoxin Tolerance on the Production of Th Cytokines and Macrophage Interleukin-6: Differential Regulation of Indomethacin

  • Endotoxin tolerance reduces the capacity of monocytes to produce proinflammatory cytokines, results in cellular immune paralysis, and down-regulates the production of helper T (Th)1 type cytokines with a shift toward a Th2 cytokine response. Prostaglandin (PG)E$_2$ in the immune system also results in macrophage inactivation and the suppression of Th1 activation and the enhancement of Th2 activation. However, the inhibitory effects of PGE$_2$ on the altered polarization of the Th cell and macrophage interleukin (IL)-6 production characterized in part by cellular immune paralysis in a state of endotoxin tolerance is unclear. This study was undertaken, using indomethacin, to investigate the role of endogenous PGE$_2$ on the Th cytokines and macrophage IL-6 production in a state of endotoxin tolerance compared to those with endotoxemia mice, wherein, in this latter case, the increased production of proinflammatory cytokines and PGE$_2$ is exhibited. Endotoxemia was induced by injection of lipopolysaccharide (LPS; 10 mg/kg in saline) i.p. once in BALB/c mice, and endotoxin tolerance was induced by pretreatment with LPS (1 mg/kg in saline) injected i.p. daily for two consecutive days and then with LPS 10 mg/kg on day 4. Splenocytes or macrophages were obtained from endotoxemia and endotoxin tolerance models pretreated with indomethacin, and then cytokine production was induced by Con A-stimulated splenocytes for the Th cytokine assays and LPS-stimulated macrophages for the IL-6 assay. Our results showed that endotoxemia led to significantly reduced IL-2 and IL-4 production, to significantly increased IL-6 production, whereas interferon $(IFN)-{\gamma}$ production was not affected. Indomethacin in the case of endotoxemia markedly attenuated $IFN-{\gamma}$ and IL-6 production and didnt reverse IL-2 and IL-4 production. Endotoxin tolerance resulted in the significantly reduced production of IL-2 and $IFN-{\gamma}$ and the significantly increased production of IL-4 and IL-6. Indomethacin in endotoxin tolerance greatly augmented IL-2 production, significantly decreased IL-4 production, and slightly attenuated IL-6 production. These findings indicate that endogenous PGE$_2$ may mediate the suppressed Th1 type immune response, with a shift toward a Th2 cytokine response in a state of endotoxin tolerance, whereas endotoxemia may be regulated differentially. Also, endogenous PGE$_2$ may mediate macrophage IL-6 production in the case of endotoxemia to a greater extent than in the case of endotoxin tolerance.

LPS로 자극된 미세아교세포에서 풀무치 에탄올 추출물의 신경염증 억제 효능 (Anti-neuroinflammatory Effects of a Locusta migratoria Ethanol Extract in LPS-stimulated BV-2 Microglia)

  • 뇌신경 질환의 주요 원인이 되는 것으로 알려진 미세아교세포의 과도한 활성화에 의한 신경염증반응에서 풀무치 에탄올 추출물이 미세아교세포의 염증 반응에 미치는 영향을 검토하였다. 미세아교세포의 활성화를 유도하기 위해 LPS를 사용하였으며, LPS 처리에 의해 신경염증반응의 지표인 NO의 생성량과 이들을 조절하는 iNOS, COX-2의 발현이 증가됨을 확인 할 수 있었다. 그러나 풀무치 에탄올 추출물을 1시간 전처리 한 후 LPS를 처리한 경우 추출물의 농도에 의존적으로 이들의 발현량이 현저히 감소되는 것을 확인 하였다. 또한 LPS 처리로 인해 분비되는 염증성 cytokine들의 생성량도 풀무치 에탄올 추출물에 의해 현저히 억제 됨을 확인 할 수 있었다. 따라서 본 연구의 결과는 미세아교세포의 과도한 활성화로 인해 발생되는 뇌 신경질환의 치료 소재로서 풀무치 에탄올 추출물의 활성 가능성을 제시하였다.

황금작약탕(黃芩芍藥湯)의 RAW 264.7 대식 세포에서의 항염증 효과에 관한 연구 (The Study of Anti-inflammatory Effect of Hwanggeumjakyak-tang Extract in RAW 264.7 Macrophage)

  • Objectives : Hwanggeumjakyak-tang (huangqin shaoyao tang, HJT) has been used to treat acute enteritis in traditional oriental medicine. However, there has been a lack of studies regarding the effects of HJT on the inflammatory activities and effector inflammatory disease mechanism about macrophage before is not known. So we examined the effect of HJT water extract on pro-inflammatory mediators in lipopolysaccharide (LPS) - stimulated mouse macrophage, RAW 264.7 cells. Methods : Cells were treated with 2 ug/mL of LPS 1 h prior to the addition of HJT. Cell viability was measured by MTS assay. The production of nitric oxide (NO) was determined by reacting cultured medium with Griess reagent. The expression of cyclooxygenase-2 (COX-2), inducible NO synthase (iNOS) and mitogen-activated protein kinases (MAPKs) was investigated by Western blot, RT-PCR. The content of level of cytokines (prostaglandin (PG) $E_2$, interleukin (IL)-6, IL-12, Tumor necrosis factor-alpha (TNF-${\alpha}$) and monocyte chemoattractant protein-1 (MCP-1)) in media from LPS-stimulated Raw 264.7 cells was analyed by ELISA kit. Results : HJT inhibited the production of NO, $PGE_2$, IL-6 as well as the expressions of iNOS, COX-2 but did not inhibit the production of IL-12, TNF-${\alpha}$, MCP-1 in the murine macrophage, RAW 264.7 cells. HJT also had suppression effects of LPS-induced MAPKs activation Conclusion : These results suggest that HJT has an anti-inflammatory therapeutic potential, which may result from inhibition of MAPK phosphorylation, thereby decreasing the expression of pro-inflammatory genes.

Mild encephalopathy with a reversible splenial lesion in a girl with acute pyelonephritis

  • We report the case of a 12-year-old girl who had mild encephalopathy with a reversible splenial lesion (MERS) associated with acutepyelonephritis caused by Escherichia coli. The patient was admitted with a high fever, and she was diagnosed with acute pyelonephritis based on pyuria and the results of urine culture, which detected cefotaxime-sensitive E. coli. Although intravenous cefotaxime and tobramycin were administered, her fever persisted and her C-reactive protein level increased to 307 mg/L. On day 3 of admission, she demonstrated abnormal neuropsychiatric symptoms, such as delirium, ataxia, and word salad. Magnetic resonance imaging (MRI) of the brain performed on day 4 showed marked hyperintensities in the bilateral corpus callosum and deep white matter on diffusion-weighted images, with corresponding diffusion restriction on apparent diffusion coefficient mapping. No abnormalities or pathogens were detected in the cerebrospinal fluid; however, lipopolysaccharides (LPS, endotoxin) were detected in plasma (41.6 pg/mL), associated with acute neurological deterioration. Her clinical condition gradually improved, and no neurological abnormalities were observed on day 6. Follow-up brain MRI performed 2 weeks later showed near-disappearance of the previously noted hyperintense lesions. In this patient, we first proved endotoxemia in a setting of MERS. The release of LPS following antibiotic administration might be related to the development of MERS in this patient. The possibility of MERS should be considered in patients who present with acute pyelonephritis and demonstrate delirious behavior.
