Attitudes of Medical Professions towards the Cooperated Medical Treatments of Korean Oriental Medicine and Western Medicine (한양방협진에 대한 의료전문직의 태도)

  • A survey was practiced to the medical doctors of Je-Han Medical Center and Taegu Catholic Medical Center from November to December 2004. The collected data was analysed by the SPSS10+. The analysis of frequency, cross table, the difference of mean was practiced. The focus of the analysis was to find the difference of attitudes between Korean Oriental medical doctors and Western medical doctors. The results of the analysis showed that Korean Oriental medical doctors are more active to the cooperated medical treatments than Western medical doctors. They also have more experience. And both of Korean Oriental medical doctors and Western medical doctors agree to the necessity of the cooperated medical treatments. But the preferred combination pattern of Korean Oriental medicine and Western medicine was different between two medical groups. Korean Oriental medical doctors prefer the one to one combination pattern. But Western medical doctors prefer to Western medicine centered combination pattern. This study has limitations in sampling prcesss and sample size. But considering that the study of the cooperated medical treatments is few, this study has considerable medical sociological meaning.

Legal Standings of the Patient and the Doctor within the National Health Insurance - With its focus on the issue of arbitrary medical charge cover - (건강보험에 있어서 의사와 환자간의 법률관계 - 임의비급여 문제를 중심으로 -)

  • In providing general medical treatments, the medical service contract between the patient and the doctor is the mutually responsible onerous contract. However, the nature of the mutually assumed contract standings of the patient and the doctor has been changing since the implementation of the national health insurance program. For instance, besides the cases of beyond excessive medical charges and medical negligence, if the doctor charged for his/her medical treatments violating the post-treatment/nursing cover criteria, the overpaid medical charge, regardless of being collected with the patient's consent, has to be refunded back to the patient. Medically needed aspects, treatment results, and unfair benefits favoring the patient are not at all taken into consideration in the health insurance scheme. This makes it easier for patients to get refunds for their share of the medical payments by involving the Health Insurance Review & Assessment Service or the National Health Insurance Corporation, without engaging in civil law suits (for reimbursement claim) against doctors. In other words, the doctor's responsibility to provide medical treatments and the patient's responsibility to pay for the medical treatment provided within the contractual realm are being demolished by the administrational arbitration of the National Health Insurance system. The basic rights of medical service providers, and the patient's right to choose are as important constitutional rights, as the National Health Insurance program, which is essential in the social welfare system. Furthermore, the development of the medical fields should not be prevented by the National Health Insurance system. If the medical treatment services can be divided into necessary treatments, general treatments, and high quality treatments, the National Health Insurance is supposed to guarantee the necessary and general treatments to provide medical treatments equally to all the insured with limited financial resources. However, for the high quality treatments, it is recommended that they should not be interfered by the National Health Insurance system, and that they should be left to the private contract between the patient and the doctor.

Research Trend of Oriental Medical Treatments for Burning Mouth Syndrome (구강작열감증후군의 한의학적 치료에 대한 최근 국내외 임상 연구 동향)

  • Objectives : The purpose of this study is to analyze the oriental medical treatments of burning mouth syndrome, understand the tendency of treatment, and apply it to clinical settings. Methods : We collected case reports, retrospective cohort studies and RCT studies related to oriental medical treatments of burning mouth syndrome using domestic and Chinese databases(CNKI, KISS, RISS, OASIS, KCI). Search terms include 'Burning Mouth Syndrome', 'BMS', 'Burning Tongue', '灼口综合征', and 'oriental medicine', 'oriental medical treatment', 'Korean medicine' and '中医'. A search was conducted by appropriately combining keywords. Results : A total of 27 papers were included in the analysis. Among them, 9 are case studies, 1 is a retrospective cohort study, 1 is a before and after study and 16 are RCT studies. Treatments for burning mouth syndrome included herbal medicine, acupuncture, electro-acupuncture, acupoint injection, auricular acupuncture treatment, external use herbal medicine and gargling. In all studies, symptoms of burning mouth syndrome were alleviated after oriental medical treatments. In 14 RCT studies comparing western medical treatments, the results of the treatment group that included oriental medical treatments were found to be more significant, except for one. Conclusions : As a result of the study, oriental medical treatments are effective in treating burning mouth syndrome. In the future, we hope that clinical research related to oriental medical treatments of burning mouth syndrome will be actively conducted so that evidence-based treatment can be implemented.

Two Cases of Korean Medical Treatments of Sudden Sensorineural Hearing Loss without Corticosteroid Treatments (부신피질호르몬제제 치료를 받지 않은 돌발성 난청 환자 한방치험 2례)

  • Objective : The purpose of this study is to know the effect of Korean medical treatments on sudden sensorineural hearing loss(SSNHL) without systemic or intratympanic steroid treatments.Method : We conducted retrospective review who was diagnosed as SSNHL and treated with Korean medicine in Dept. of Otolaryngology Kyung Hee University Hospital at Gangdong. There were two patients with SSNHL who refused to be treated with western medicine and were treated with Korean medicine including acupuncture, moxibustion, and herbal medicine.Result : One patient recovered completely after eight days of admission, and another patient partial recovered during 30 days through Korean medical treatments.Conclusion : Through these studies, SSNHL is expected to treated more effectively by conventional Korean medical treatments than corticosteroid treatments alone. But the additional study is needed to reveal the exact effects of Korean medical treatments.

A Case Report of Tinnitus Occurred in the Sudden Sensory Neural Hearing Loss Treated with Korean Medical Treatments (돌발성 난청 환자에서 발생한 이명의 한방 치험 1예)

  • Objectives : The purpose of this study is to show the clinical effect of Korean Medical Treatments for Tinnitus with Sudden Sensory Neural Hearing Loss. Methods : The patient was treated by Korean Medical Treatments for 10weeks. The effect of treatments on tinnitus was measured with Visual Analogue Scale(VAS) and Korean Tinnitus Handicap Inventory(K-THI). And the effect of treatments on sudden hearing loss was measured by VAS. Results : VAS of Tinnitus was decreased from 10 to 3 points and K-THI score was decreased from 70 to 26 points. VAS of hearing loss was decreased from 9 to 8 points. Conclusions : Korean Medical Treatments are effective on Tinnitus.

The Effectiveness of Oriental Medical Treatments to Symptoms and Quality of Life in the Patients with Herniated Cervical Disc being Suggested Operation (경추 추간판 탈출증으로 수술 권유를 받은 환자에게 한의학적 치료가 임상증상 및 삶의 질 개선에 미치는 영향)

  • Objective : The purpose of this study is evaluated the effects of Oriental treatments to symptoms and quality of life in the patients with herniated cervical disc being suggested operation. Methods : 20 patients diagnosed with herniated cervical disc by MRI in hospital were treated by oriental medical treatments. Symptoms of the patients were assessed by analogue scale(VAS) and SF-36. Results : After 12 weeks treatments, the VAS of symptoms of herniated cervical disc and the score of SF-36 were significantly improved. Conclusions : It could be suggested that oriental medical treatments were effective for the patients with herniated cervical disc being suggested operation. Further study of the effect of oriental medical treatments on herniated cervical disc is needed.

  • External treatments have various curative effects. So it had been used to cure various patients. Especially, Zhao-Xue-Min(趙學敏) had thought that there are useful treatments of wander doctors. So, the "Chuan-Ya(串雅)" had been written by Zhao-Xue-min in 1759. The book had included treatments of wander doctors. it had included various external treatments. Therefore we would like to bring out it's external treatments in the part of ophthalmic, otolaryngologic and surgical diseases. We have researched external treatments of "Chuan-Ya", and then we have arrived at following conclusions. "Chuan-Ya" has 56 sentences that have related to external treatments in the part of ophthalmic, otolaryngologic and surgical diseases. And then, the treatments were, for the most of the part, bathing therapy, application therapy.

Therefore we would like to bring out it's external treatments theory 2) Conclusions We have researched external treatments of "Chuan-Ya", and then we have arrived at following conclusions. "Chuan-Ya" has 48 sentences that have related to external treatments in internal medicine. And it had recorded fire conducting method(引火法) to explain process of external teratment principle.

  • Objectives : AMH (Anti-Müllerian Hormone) is considered a sensitive marker of ovarian reserve, and it tends to be included among basic fertility tests. This paper is to report four pregnancies which were achieved by subfertile women with low AMH level after Korean medical treatments. Methods : Four cases of pregnancy by subfertile women, aged 34 to 37, with low AMH level (0.04 to 0.76 ng/mL), after Korean medical treatments between July 2012 and May 2015, were analysed. The patients were diagnosed as Kidney deficiency (腎虛), Blood extravasation (瘀血) and/or Liver Qi stagnation (肝氣 鬱結). Herbal medicine, acupuncture, pharmacopuncture and moxibustion treatments were applied. Results : The four subfertile women with low AMH level achieved pregnancy after Korean medical treatments. Conclusions : The case report suggests that Korean medical treatment can be an effective option for subfertile women with low AMH level before Assisted Reproductive Technology.

  • Objectives : The purpose of this study is to evaluate clinical effect of Korean medical treatments for the patient who were diagnosed as the Cauda Equina Syndrome. Methods : The patient was treated by Korean medical treatments including acupuncture, pharmacopuncture, herbal medication. The Improvement of clinical symptoms was assessed by Visual Analog Scale(VAS), Oswestry Disability Index(ODI) and Muscle Manual Test(MMT). Results : After treatments, VAS and ODI were generally decreased, MMT was improved in case. Conclusions : The Oriental medical treatments might be effective methods to treat the patient who were diagnosed as the Cauda Equina Syndrome.
