Relationship of Socioeconomic Status to Self-Rated Oral Health (사회경제적 수준에 따른 주관적 구강건강 수준의 차이)

  • How to eliminate health disparity to ensure health equity is one of major issues that are handled across the world. The purpose of this study was to examine any possible differences in self-rated oral health state according to socioeconomic status and the relationship between the two based on the data of the 5th National Health & Nutrition Examination Survey of 2010~2012. As for differences in self-rated oral health state according to sociodemographic characteristics, the women considered themselves to be in poorer oral health than the men. The older respondents found themselves to be in poorer oral health, and there was a tendency that the respondents who were less educated and whose household income was smaller rated their own health as worse. When a logistic regression analysis was made to determine influential factors for self-rated oral health status, the women perceived they were in better oral health than the men did, and the better-educated respondents were more likely to consider themselves healthier. Concerning disparities in self-rated health state according to income level, there were broader differences in that regard according to an increase of income. The findings of the study illustrated that there was oral health inequity according to social stratum. It's required to make a nationwide effort to promote national oral health, and appropriate support should especially be provided for disadvantaged people at the same time in order to get rid of the gap in oral health among different social classes, as there is a yawning gap between them and the other classes.

An Association of Periodontitis and Diabetes (치주질환과 당뇨병의 관련성)

  • This study was conducted to evaluate the association between periodontal disease and diabetes mellitus. The final analysis population of this study was composed of 4,830 adults with diabetes mellitus or periodontal disease and aged 19 years or older, based on the third-edition data of the 4th Korea National Health and Nutrition Examination Survey (KNHANES) (in 2009). Diabetic status and potential confounders were used in questionnaire materials and physical examination materials, and the presence of periodontal disease was used in the materials for oral health examination by a dentist. For diabetic status, fasting plasma glucose (FPG) levels <100 mg/dl were subcategorized into normal group and FPG levels ${\geq}100mg/dl$ into impaired fasting glucose group; glycosylated hemoglobin (HbA1c) levels <7% into normal group and HbA1c ${\geq}7%$ into diabetes group, on the basis of the American Diabetes Association. According to the 2009 Korea Health Statistics, the case where the pocket depth is 3 mm or more was defined as periodontal disease. The association between the two diseases was evaluated through $x^2$-test and logistic regression analysis using R-commander 2.14. In impaired fasting glucose group, community periodontal index (CPI) groups 3 to 4 had higher risks for periodontal disease 1.23 times (95% confidence interval, 1.07~1.42) than those of CPI groups CPI 0~2, even after adjustment for several confounders. In addition, periodontal disease and diabetes mellitus showed statistically significant differences depending on age, sex, income level, educational background, smoking status, alcohol consumption, and snack intake. The analyses of the third-edition data of the 4th KNHANES showed that there was a statistically significant association between periodontal disease and diabetes mellitus as examined by means of CPI in this study.

Convergence factors among their physical state, function and activities influencing on the cognition of elderly residents in a community (지역사회 재가 노인의 인지수준에 영향을 미치는 신체상태와 기능 및 활동의 융합요인)

  • A descriptive research is studied to identify the physical factors affecting to cognitive function among elderly residents over 65 years old in a community. The data were collected from 352 participants as part of a health-related survey by home visits in Seoul in 2010-2012. Their cognitive function was measured using the Korean form of Hasegawa Dementia Scale(HDS-K) and the collected data were analyzed by using t-test, ANOVA, and multiple regression analysis. From the analysis, the prevalence rate of cognitive impairment was 13.6%. Elderly residents in a community showed different cognition levels by the status of age, change of weight, change of BMI, walking and flexibility exercise. Especially, the lowest cognition level was found in the normal BMI group with decreased BMI change by over 3. The factors influencing on cognition level of the elderly with normal BMI were age, change of weight, change of BMI, and walking exercise. The variance indicated 12.2% as their cognition level. Therefore, for preventing the cognitive impairment of the elderly that were rapidly decreased of BMI, we need the program to manage their nutrition and walking exercise.

Research on Farmers' Occupational Health Services in Finland (핀란드의 농업보건서비스 사례와 시사점)

  • This study aimed to explore the direction of development of agricultural safety and health system in Korea based on experience on Farmers' Occupational Health Services (FOHS) of Finland. In Finland additional and specialized Occupational Health Services (OHS) for self-employed farmers were developed and have been conducted by municipal primary health care centers with cooperation of National Centre for Agricultural Health (NCAH) of Finnish Institute of Occupational Health, Social Insurance Institute (SII) and Farmers' Social Insurance Institution (FSII). Based on the study, in order to activate the FOHS in Korea, the analysis results could suggest the development of four prerequisites. Firstly, center for farmers' safety and health like NCAH, which conducts surveillance on health and safety status, standardization of intervention contents and epidemiological research is needed in national institution. Secondly, development of evaluation tools with lower cost and difficulty like walk-through survey for checking status of systems of safety in the farms is needed. Thirdly, farmers' occupational health insurance which will be initiated on near future should be connected to preventive intervention programs like FOHS. Lastly, training and qualification of more local occupational health experts and units is needed.

Health Status and Medical Utilization of Women in Rural Area (농촌지역 여성의 건강수준과 의료이용에 대한 연구)

  • Objectives: This study was conducted to examine health inequality for gender and region in Korea. Especially it focused on health status such as disease prevalence and medical utilization of rural women. Methods: Data from the Korea national health and nutrition survey in 2001 were used. The final sample size was 37,108 individuals with age 20 and over. This study applied the logistic regression for nominal variables such as disease prevalence and unmet care needs and with the regression for continuos variables such as the length and costs of medical services. Results: Rates of disease prevalence and unmet care needs for chronic disease in rural area are higher than those in middle cities and big cities, and regional differences of those for women are more than those for mens with controlling ages. There could be interaction effect with region and sex. Conclusions: This study suggests that health policy maker should take consider of special status of rural women who are in health inequality.

Development of Health Service Weight for Resource Allocation and Performance Monitoring (자원 배분과 성과 모니터링을 위한 보건사업 가중치 개발)

  • Objectives: This study was conducted to estimate health service weight for resource allocation and performance monitoring using Basic Priority Rating System. Methods: The Health service would be classified according to New Health Plan 2010, and Burden of disease collected from preceding studies. The data of severity of health problem and effectiveness of intervention were collected through the survey of experts' suggestion. The health service weight was estimated in the formula which is Basic Priority Rating System. Results: In the result of analysis, the health service weight of Infectious disease was ranked highest at 58.97% followed by Anti-smoking campaign(14.07%), Hypertension(3.87%), Diabetes mellitus(3.40%), Cancer(2.90%), Cardiovascular-Cerebrovascular diseases(2.86%), Physical activity(2.10%), Moderate drinking(2.07%), Medical examination(1.92%), Mental health promotion(1.72%), Serious mental illnesses(1.62%), Nutrition(1.52%), Oral health promotion(1.15%), Oral diseases(1.10%), Addiction(0.73%). Conclusions: We think the result of this study provides a rational basis for resource allocation and performance monitoring of health service.

  • The purpose of this study was to investigate the actual ICT utilization and recognition, to identify the effective and relevant chapters of home economics which can be taught using ICT in teaching and learning method. There were 372 secondary school teachers being participated in the survey. The results of this study can be summarized as follows 1. The reason why they have ICT utilization Instruction is to help the learners improve the academic accomplishment in the development stage through providing data from web-surfing and presentational data such as developed CD, animation and presentations. 2. Regarding ICT utilization capability. teachers have the capability to send and receive E-mail, make a presentation data. practical use of Internet. In addition, the group with less than 10 years experience have a higher capability in information technology. 3. Concerning ICT utilization recognition, this result shows that ICT Instruction is the most effective to motivate learners, and teachers anticipate ICT Instruction would improve the quality of the teaching & learning. 4. The chapter such as 'how to make clothing and recycling'(database). 'the foundation of cooking and its application' (database). 'gender and acquaintance of the opposite sex', 'nutrition of during adolescence' (simulation program) should be the first priority in the development.

A Study on Eating-out Behaviors Depending on Female Customers' Lifestyle in Ulsan.Gyeongnam Province (울산.경남지역 여성고객의 라이프스타일에 따른 외식행동에 관한 연구)

  • The current study aimed to analyze data conducting a survey with 211 females in Ulsan Gyeongnam province in order to understand what influence lifestyle types of female customers exert on eating-out behaviors from the perspective of eating-out motivation. So as to achieve research purpose, we conducted factor analysis, reliability analysis, t-test, ANOVA analysis, multiple regression analysis, correlation analysis and frequency analysis through SPSS 12.0 program. Findings of analyses indicated that items of lifestyle involve d 4 factors, and the item of eating out behaviors was extracted to be 1 factor. As a result of testing differences in lifestyle depending on residential areas, both type of chasing fashion and type of extroverted dependence have significant differences in variables, and both factors indicated that females living in Ulsan have higher mean values than those of females living in Gyeongnam. As a result of analyzing lifestyle depending on age, which was adopted in part, females at the ages of 31~40 had higher mean values than those in other ages in the type of chasing fashion, and females above the age of 51 had higher mean values than those in other ages in type of chasing health. As a result of lifestyle depending on academic backgrounds, university students or graduates had higher mean values than those in other academic backgrounds. For the influence of lifestyle types on eating-out behaviors depending on eating-out motivation, the type of chasing fashion, the type of purchasing the quality of life, and the type of chasing health were p<0.001, having significant effects.

A Study on a School Milk Program for Elementary School Students in Seoul (서울지역 일부 초등학생의 학교우유급식 실태 조사)

  • This study was performed to investigate the consumption pattern of school milk programs for elementary school students. The subjects were 518 students residing in Seoul area. The survey was conducted by using a self-administered questionnaire, and the collected data were analyzed by SAS package. Of the subjects, 88.2% of the students were provided with a school milk program, and the number of the students was higher in male students than in female ones(p<0.05). 81.8% of them drink milk every day from the school milk program. For the degree of satisfaction for milk served in the school milk program, 47.9% of the students were satisfied. An upset stomach after drinking milk and poor taste of milk were the main reasons why milk consumption decreased. Only 30.7% of the subjects perceived that they should drink milk in the school milk program. And the subjects anticipated that flavored milk and yogurt should be served to promote milk consumption in a school milk program. Therefore, a proper food guide and a dairy education program should be developed for elementary school students.

Effect of Health status and Health Behavior on the Diabetes Mellitus Prevalence (성인의 건강상태, 건강행위가 당뇨병 유병률에 미치는 영향)

  • Diabetes Mellitus (DM) is well known for increasing morbidity and mortality, especially related to their complications. The purpose of this study was to investigate the factors affecting the prevalence rate of DM and provide a fundamental material to develop an intervention strategy to reduce the prevalence rate of DM. The study subjects were adults aged over 19 on the basis of the primitive data of "The Fifth Korean National Health and Nutrition Examination Survey, 2012". Therefore, the data of 5995 participants were analyzed. For data process, the complex sample analysis module of SPSS 18.0 program was employed to add weighting before analysis. According to the analysis, the prevalence rate of DM of the study subjects was 10.5%. Regarding the odds ratio of DM prevalence, the subjects who graduated from middle school had the odds ratio 2.51 times higher than those who graduated from college and more; those in subjective bad health condition 4.77 times higher than those in subjective good health condition; those in obesity 1.44 times higher; those with high blood pressure 2.57 times higher; those with hyperlipidemia 2.63 times higher; those who fail to control their weight 1.31 times higher; those going on a diet 2.75 times higher. This study revealed that a level of education, perceived health status, obesity, high blood pressure, hyperlipidemia, weight control, and dietary therapy were the predictable variables of the prevalence rate of DM, and thereby suggested the nursing direction and research direction to reduce the prevalence rate of DM.
