BACKGROUND/OBJECTIVES: This study aimed to test the association between APOA5 3'-UTR variants (rs662799) and cardiometabolic traits in Koreans. SUBJECTS/METHODS: For this study, epidemiological data, Apolipoprotein A5 (APOA5) genotype information, and lymphoblastoid cell line (LCL) biospecimens from a subset of the Ansung-Ansan cohort within the Korean Genome and Epidemiology study (KoGES-ASAS; n = 7,704) as well as epidemiological data along with genomic DNA biospecimens of participants from a subset of the Korea National Health and Nutrition Examination Survey (KNHANES 2011-12; n = 2,235) were obtained. APOA5 mRNA expression was also measured. RESULTS: APOA5 rs662799 genotype distributions in both the KoGES-ASAS and KNHANES groups were 50.6% for TT, 41.3% for TC, and 8.1% for CC, which are similar to those in previous reports. In both groups, minor C allele carriers, particularly subjects with CC homozygosity, had lower high-density lipoprotein (HDL) cholesterol and higher triglyceride levels than TT homozygotes. Linear regression analysis showed that the minor C allele significantly contributed to reduction of circulating HDL cholesterol levels [${\beta}=-2.048$, P < 0.001; ${\beta}=-2.199$, P < 0.001] as well as elevation of circulating triglyceride levels [${\beta}=0.053$, P < 0.001; ${\beta}=0.066$, P < 0.001] in both the KoGES-ASAS and KNHANES groups. In addition, higher expression levels of APOA5 in LCLs of 64 healthy individuals were negatively associated with body mass index (r = -0.277, P = 0.027) and circulating triglyceride level (r = -0.340, P = 0.006) but not significantly correlated with circulating HDL cholesterol level. On the other hand, we observed no significant difference in the mRNA level of APOA5 according to APOA5 rs662799 polymorphisms. CONCLUSIONS: The C allele of APOA5 rs662799 was found to be significantly associated with cardiometabolic traits in a large Korean population from the KoGES-ASAS and KNHANES. The effect of this genotype may be associated with post-transcriptional regulation, which deserves further experimental confirmation.
간 효소(ALT와 AST)는 주로 간기능을 검사하는데 활용되지만, 간 효소와 심혈관질환 사망과의 관련성에 대한 연구들이 보고되고 있다. 본 연구는 간 효소와 전체사망원인의 관련성을 확인하고자 하였다. 연구자료는 2005년 8월과 2006년 12월에 한국농촌코호트(5개 지역) 연구에 참여한 40세 이상의 성인 10,110명의 건강검진 및 설문조사 자료와 2012년 12월 통계청의 사망자료를 병합하여 사용하였다. 분석방법은 5개 범주로 구분한 간 효소에 의한 사망위험비는 Cox의 비례위험모델을 이용하여 다른 위험요인들을 보정하여 분석하였다. 연구결과 지금까지 사망의 중요 위험요인들의 사망위험비는 65세 이상이 3.5배, 남자가 3.8배, 흡연 3.2배, WHR이 높을수록, 운동 안함 1.6배, 지질성분 및 PP2가 높을수록 높았으며, 이런 변수들을 보정한 상태에서 AST가 50 IU/L 이상일 때 사망위험비가 2.198배(95% CI: 1.217-3.971)였고, ALT도 증가하는 경향을 보였다. 이런 결과는 높은 값의 AST는 사망 위험을 높이는데 영향을 미치며, ALT도 사망을 예측하는데 유용한 도구가 될 수 있다는 것을 제시한다.
This study investigated the potential role of dietary factors associated with obesity and metabolic syndrome (MetS) in Koreans. The scoping review method was used to evaluate the studies that utilized the secondary data sets comprising the Korean National Health and Nutrition Examination Survey (KNHANES) and the Korean Genome and Epidemiology Study (KoGES). Articles published between 2012 and 2022 were identified using RISS, KISS, DBpia, PubMed, and ScienceDirect databases. In all, there were 32 published articles on obesity and 119 on MetS. Obesity research included eight articles on nutrients, 12 on food items/food groups, two on dietary patterns, nine on dietary behavior/eating habits, and one on the dietary index. MetS studies comprised 34 articles on nutrients, 43 on food items/food groups, seven on dietary patterns, 25 on dietary behavior/eating habits, and 10 on the dietary index. Carbohydrates, alcohol, and coffee consumption were the most frequently studied dietary factors for obesity and MetS. The primary areas of study were largely focused on nutrients and food items/food groups. Thus, to overcome the paucity of information on the relationship of dietary patterns and dietary indexes with obesity and MetS, there is a need for further research using the KNHANES and KoGES data sets.
본 연구는 우리나라 성인 남녀에서 관상동맥질환의 주요 예측 지표인 AIP와 식품군 섭취의 관련성을 분석하였다. 본 연구에서는 40-79세의 성인 남녀 133,381명을 대상으로 AIP 5분위 그룹에 따른 식품군 섭취량을 제시하였으며, 그 결과 AIP가 높은 그룹에서 낮은 그룹에 비해 곡류, 김치/피클류의 섭취가 높았고, 콩류, 종실류, 육류, 난류, 생선류, 우유 및 유제품 등의 섭취가 낮은 경향을 보였다. 본 연구는 한국인의 식사와 관상동맥질환 위험도 간의 관계에 대한 중요한 정보를 제공하며, 관상동맥질환 예방을 위해 다양한 식품군이 포함된 균형 잡힌 식사가 중요함을 시사한다. 본 연구 결과는 관상동맥질환을 예방할 수 있는 식사지침 제시를 위한 기초자료로 활용될 수 있을 것이다. 추후 연구를 통해 한국인의 식사패턴을 분석하여 심혈관질환 발생 및 사망과의 관련성을 조사하고, 이를 통해 한국인의 심혈관질환 예방에 기여할 수 있을 것으로 기대한다.
Objectives: This study aimed to analyze the associations of income, marital status, and health behaviors with hypertension in male and female over 40 years of age in the Korea. Methods: The data were derived from the Korean Genome and Epidemiology Study (KoGES; 4851-302) which included 211 576 participants. To analyze the relationships of income, marital status, and health behaviors with hypertension in male and female over 40 years of age, multiple logistic regression was conducted with adjustments for these variables. Results: The prevalence of hypertension increased linearly as income decreased. The odds ratio for developing hypertension in people with an income of <0.5 million Korean won (KRW) compared to ≥6.0 million KRW was 1.55 (95% confidence interval [CI], 1.25 to 1.93) in the total population, 1.58 (95% CI, 1.27 to 1.98) in male, and 1.07 (95% CI, 0.35 to 3.28) in female. The combined effect of income level and marital status on hypertension was significant. According to income level and marital status, in male, low income and divorce were most associated with hypertension (1.76 times; 95% CI, 1.01 to 3.08). However, in female, the low-income, married group was most associated with hypertension (1.83 times; 95% CI, 1.71 to 1.97). Conclusions: The results of this study show that it is necessary to approach male and female marital status separately according to income in health policies to address inequalities in the prevalence of hypertension.

Purpose: This study investigated the incidence and risk factors of cataract in people with diabetes mellitus (DM) using data from Ansan cohort of the Korean Genome and Epidemiology Study (KoGES). Methods: Data from a total of 329 patients with type 2 DM without cataract who participated in Ansan cohort of the KoGES from baseline survey (2001-2002) to fifth follow-up visit (2011-2012) were examined. The characteristics of the subjects were analyzed with frequency and percentage, and mean and standard deviation. Cataract incidence was measured as incidence proportion (%). For risk factors of cataract, hazard ratio (HR) and 95% confidence interval (CI) were obtained using the Cox proportional hazard model. Results: The cataract incidence over a 10-year follow-up period was 19.1% (15.1 in males and 25.8 in females), and mean age at the incidence of cataract was 63.48 years (61.58 years in males and 65.31 years in females). Age (HR=1.09, 95% CI=1.05-1.13) and HbA1c (HR=1.21, 95% CI=1.07-1.37) or the duration of DM (HR=1.05, 95% CI=1.00-1.09) were found to be independently associated with cataract development. Conclusion: Cataract development in people with DM is common, and its likelihood increases with age, HbA1c, and the duration of DM. Considering negative effect of cataract on their quality of life and economic burden, nurses should identify people with DM at a higher risk of cataract development, and plan individual eye examination programs to detect cataract development as early as possible.
Purpose: This study focused on identifying the interaction effects of genetic and lifestyle-environmental factors on the development of type 2 diabetes mellitus (T2D). Methods: Study subjects were selected from the Korean Genome and Epidemiology Study (KoGES) from 2001 to 2014. Data on genetic variations, anthropometric measurements, biochemical data, and seven lifestyle factors (diet, physical activity, alcohol drinking, smoking, sleep, depression, and stress) were obtained from 4,836 Koreans aged between 40 and 59 years, including those with T2D at baseline (n = 1,209), newly developed T2D (n= 1,298) and verified controls (n = 3,538). The genetic risk score (GRS) was calculated by using 11 single-nucleotide polymorphisms (SNPs) related to T2D development and the second quartile was used as the reference category. A Cox proportional hazards regression model was used to evaluate the associations of GRS and lifestyle factors with T2D risk, controlling for covariates. Results: Multivariate regression analysis revealed that GRS was the strongest risk factor for T2D, and body mass index (BMI), smoking, drinking, and spicy food preference also increased the risk. Lifestyle/environmental factors that showed significant interactions with GRS were BMI, current smoking, current drinking, fatty food preference, and spicy food preference. Conclusions: Interactions between genetic factors and lifestyle/environmental factors were associated with an increased risk of T2D. The results will be useful to provide a new perspective on genetic profiling for the earlier detection of T2D risk and clues for personalized interventions, which might be more effective prevention strategies or therapies in individuals with a genetic predisposition to T2D.
Objectives: The Korean Genome and Epidemiology Study (KoGES), a multicenter-based multi-cohort study, has collected information on body composition using two different bioelectrical impedence analysis (BIA) machines. The aim of the study was to evaluate the possibility of whether the test values measured from different BIA machines can be integrated through statistical adjustment algorithm under excellent inter-rater reliability. Methods: We selected two centers to measure inter-rater reliability of the two BIA machines. We set up the two machines side by side and measured subjects' body compositions between October and December 2007. Duplicated test values of 848 subjects were collected. Pearson and intra-class correlation coefficients for inter-rater reliability were estimated using results from the two machines. To detect the feasibility for data integration, we constructed statistical compensation models using linear regression models with residual analysis and R-square values. Results: All correlation coefficients indicated excellent reliability except mineral mass. However, models using only duplicated body composition values for data integration were not feasible due to relatively low $R^2$ values of 0.8 for mineral mass and target weight. To integrate body composition data, models adjusted for four empirical variables that were age, sex, weight and height were most ideal (all $R^2$ > 0.9). Conclusions: The test values measured with the two BIA machines in the KoGES have excellent reliability for the nine body composition values. Based on reliability, values can be integrated through algorithmic statistical adjustment using regression equations that includes age, sex, weight, and height.
The purpose of this study is to investigate the proportion and associated risk factors of hypertriglyceridemia in rural Vietnamese women. Research data were collected as part of the Korean Genome and Epidemiology Study (KoGES). A cross-sectional study of 957 Vietnamese women in their 20 to 30s was conducted in rural areas of Bavi, Vietnam. Subjects were classified as hypertriglyceridemic (serum TG ${\geq}$ 150 mg/dL). Demographic, socio-economic details, anthro-pometric measurements, and blood profiles were recorded. The proportion of hypertriglyceridemic subjects was 22.0%, and the mean age of hypertriglyceridemics subjects was older than that of normo-triglyceridemic subjects (p < 0.05). In hypertriglyceridemic subjects, height, HDL-cholesterol, and LDL-cholesterol were significantly lower, compared to subjects with normo-triglyceridemia, while weight, body mass index, waist hip ratio, body fat %, blood pressure, fasting blood sugar, total cholesterol, and atherogenic index were higher, compared to those with normo-triglyceridemia. Intake of cereal and cereal products, total plant food, and cereal/potato fiber in subjects with hypertriglyceridemia was significantly higher, compared to normo-triglyceridemic subjects. Hypertriglyceridemic subjects had a significantly lower intake of animal calcium and retinol than normo-triglyceridemic subjects. Significant positive relationships were observed between the prevalence of hypertriglyceridemia and consumption of total plant food [OR (95% CI) for the highest tertile, compared to the lowest: 1.764 (1.131-2.750); p for trend = 0.008] and crude fiber [OR (95% CI) for the highest tertile compared to the lowest: 1.651 (1.092-2.497); p for trend = 0.027]. In addition, a significant inverse relationship was observed between the prevalence of hypertriglyceridemia and cholesterol intake [OR (95% CI) for the highest tertile, compared to the lowest: 0.601 (0.400-0.901); p for trend = 0.012]. These findings may provide basic data for use by policymakers and dieticians in future development of nutrition and health programs to encourage healthier eating habits, and to prevent hypertriglyceridemia advancing cardiovascular disease in rural Vietnamese women.
