Elective tracheostomy scoring system for severe oral disease patients

  • Objectives: The purpose of this research was to create a scoring system that provides comprehensive assessment of patients with oromaxillofacial cancer or odontogenic infection, and to statistically reevaluate the results in order to provide specific criteria for elective tracheostomy. Materials and Methods: All patients that had oral cancer surgery (group A) or odontogenic infection surgery (group B) during a period of 10 years (2003 to 2013) were subgrouped according to whether or not the patient received a tracheostomy. After a random sampling (group A: total of 56, group B: total of 60), evaulation procedures were observed based on the group classifications. For group A, four factors were evaluated: TNM stage, reconstruction methods, presence of pathologic findings on chest posterior-anterior (PA), and the number of systemic diseases. Scores were given to each item based on the scoring system suggested in this research and the scores were added together. Similarly, the sum score of group B was counted using 5 categories, including infection site, C-reactive protein level on first visit, age, presence of pathologic findings on chest PA, and number of systemic diseases. Results: The scoring system rendered from this research shows that there is a high correlation between the scores and TNM stage in oral cancer patients, or infection sites in odontogenic infection patients. However, no correlation between pathologic findings on chest PA could be found in either group. The results also indicated that for both groups, the hospital day increased with the tracheostomy score. The tracheostomy score cutoff value was 5 in oral cancer patients and 6 in odontogenic infection patients which was used for elective tracheostomy indication. Conclusion: The elective tracheostomy score system suggested by this research is a method that considers both the surgical and general conditions of the patient, and can be very useful for managing patients with severe oral disease.

Water Content and Dry Density Measurement of Soil Using Flat TDR System (Flat TDR 시스템을 이용한 흙의 함수비와 건조단위중량 측정)

  • This study has been conducted to improve the conventional compaction management method by measuring the water content and dry unit weight of soil using the Time Domain Reflectometry (TDR) method. In order to verify the measured value of the developed flat TDR system, laboratory tests were conducted on six soils. Also, based on laboratory experiments, field tests were conducted to evaluate the applicability of the developed flat TDR system. Also, a comparison experiment was conducted with the Purdue TDR system. In addition, FE analysis was done to confirm the influence range of the Flat probe. As a result, it was confirmed that the influence range was about 10 cm. As a result of laboratory experiment, the water content ratio showed an error of about 0.4% on the average, and in the case of dry unit weight, it showed an error of about 1.6%. For the field test, the water content ratio and unit weight showed an error of 0.8% and 2.5%, respectively. Through the experimental results, it was confirmed that the measured value of the Flat TDR system is more accurate than that of the conventional TDR system.

  • A five-level VSI(Voltage Source Inverter) is introduced as a SVC(Static Var Compensator) like a large scale power source. The problems in using SVC are that the power device can easily be destroyed by voltage unbalance and accurate reactive power control is difficult because of voltage variation. A asymmetrical PAM(Pulse Amplitude Modulation) switching pattern is proposed to solve this problem and analyze both fundamental component and harmonic current in the system. Through experimental results of 3.5 kVA experimental test system. It is confirmed that DC capacitor voltage can be controlled by asymmetrical PAM switching pattern control.

  • In every large hospital, nurses must perform simple repetitive tasks such as measuring body temperature. Such tedious work reduces nurses' motivation to provide quality medical care, which is an important element of the medical services provided by a hospital. If a device were available to measure body temperature, nurses could focus on the more important aspects of providing quality medical care to the patients. However, body temperature is generally measured from the throat, anus, tympanum or armpit, where it is difficult to affix a patch type device. In addition, general body temperature measuring points shows moving artifact error; therefore, it is not good point to continually measure the temperature. In this paper, a patch type skin temperature measuring system was developed. To appropriately measure the skin temperature, a thermal transducer was implemented with a thin (0.5 mm) temperature sensor. The system is small and thin ($H6.6{\sim}5.3{\times}L35{\times}W24\;mm$), and weighs only 5 g including a battery, case and circuit; therefore, it is small and light enough to function as a patch type device. Moreover, the system worked for 5 days. To investigate differences between the experimental and conventional thermometer, simple clinical experiments were performed with 17 volunteers, and the result showed some correlation between the implemented system and conventional thermometer (Correlation coefficient = 0.647, P<0.1).

On Power Allocation Schemes for Bi-directional Communication in a Spectrum Sharing-based Cognitive Radio System

  • This paper presents the results of an investigation into bi-directional communication in spectrum sharing-based cognitive radio (Bi-CR) systems. A Bi-CR system can increase the spectral efficiency significantly by sharing the spectrum and through the bi-directional use of spatial resources for two-way communication. On the other hand, the primary user experiences more interference from the secondary users in a Bi-CR system. Satisfying the interference constraint by simply reducing the transmission power results in performance degradation for secondary users. In addition, secondary users also experience self-interference from echo channels due to full duplexing. These imperfections may weaken the potential benefits of the Bi-CR system. Therefore, a new way to overcome these defects in the Bi-CR system is needed. To address this need, this paper proposes some novel power allocation schemes for the Bi-CR system. This contribution is based on two major analytic environments, i.e., noise-limited and interference-limited environments, for providing useful analysis. This paper first proposes an optimal power allocation (OPA) scheme in a noise-limited environment and then analyzes the achievable sum rates. This OPA scheme has an effect in the noise-limited environment. In addition, a power allocation scheme for the Bi-CR system in an interference-limited environment was also investigated. The numerical results showed that the proposed schemes can achieve the full duplexing gain available from the bi-directional use of spatial resources.

  • Retail stores are mostly relying their product management on handwriting. This handwriting management is limited and poor at managing the status of stocking and releasing. Also, to confirm real-time product status, additional statistic system is required. POS system developed to overcome foresaid problems is operating only in big and special stores because of expensive price and limited space. Therefore, new automated management system which has low installation price needs to be developed and adopted instead of handwriting management system. In this paper, we developed real-time product management system which can manage the products in retail stores by utilizing NFC function in mobile device.

The Economics Evaluation of Grid-connected Photovoltaic Systems in Residential Houses

  • Purpose: To evaluate the economic performance of grid-connected photovoltaic system in residential house, household electricity bill policy of Korea Electric Power Corporation (KEPCO) must be applied precisely, and market tendency and uncertainty of system also need to be considered. In this study, to evaluate the economic feasibility of PV system, we measured PV power generation and electricity consumption of six of Green home in Daejeon through web based remote monitoring system. Method: We applied Monte-Carlo simulation based on life cycle cost analysis, to reflect an uncertainty of main factor in economic feasibility evaluation of photovoltaic system. Result: First, with deterministic analysis, the difference of NPV of cumulative financial savings among households varied from -3,310 ~ 24,170 thousand won, portraying notably big range. Also the possibility of getting the same result was 50% when applying uncertainty. Second, the higher electricity consumption is, the more economic feasibility of photovoltaic system increases because KEPCO uses progressive taxation in household electricity bill policy. Third, The contribution to variance of electricity price increases in NPV varied from 98.5% to 99.9%. While the inflation rate and annual degradation contributed very little to none.
