  • The purpose of this study is to attempt a Analysis of a pointed-end Equipment Arm Safety-Accident for Fault Tree Analysis. Three major techniques were used first problem is Z-Model by which accident Analysis & prevention of a pointed-end Industry can be made, Fault Tree Analysis(FTA) bywhich quantification of a pointed-end Equipment accident Analysis can be made it 5 years in past and the third, manual-written by which minimal cut set to accident can be Identified. A example has been made of issue point a pointed-end Equipment that the Arm in loader happen to Injuries. According to the Analysis lack of safety knowledge, unsafety-behavior seem to be the primal cause of accident. Comparision of the accident cause to actual report demonstratesthat the FTA a efficient tool for Industrial Accident prevention.

  • We reviewed the scientific literature on psychological effects of Chemical, Biological, Radiological, Nuclear, Explosive (CBRNE) disaster, which is one of the biggest threat to modern society. CBRNE disaster has the potential to cause specific physical symptoms and psychological distress in victims ; moreover, various toxic symptoms and carcinogenesis/mutation would be an important issue. Bioterrorism can cause localized outbreaks of infectious disease or pandemic disaster. Somatization as well as posttraumatic stress symptoms and depression are the characteristic psychological symptoms in CBRNE disaster's victims. CBRNE disasters could lead to large-scale public fear and social chaos due to the difficulties involved in verifying the extent of exposure and unfamiliar area to the common people. In the evacuation process, problems associated with adjustment and conflict between victims and residents should be considered.

  • Building a stable knowledge base is an important issue in the application of knowledge engineering. In this paper, we present an algorithm for detecting and locating discrepancies in the line of the reasoning process especially when discrepancies occur on belief values. This includes backtracking the rule firing from a goal node of the rule network. Retracting a belief function allows the current belief state to move back to another belief state without the rule firing. It also gives an estimate, called contribution measure, of how much the rule has an impact on the current belief state. Examining the measure leads the expert to locate the possible cause of problem in the rule. For non-monotonic reasoning, the belief retraction method moves the belief state back to the previous state. A tracing algorithm is presented to identify and locate the cause of problem. This also gives repair suggestions for rule refinement.

  • Silent Data Corruptions (SDCs) is a serious reliability issue in many domains of computer system. The identification and protection of the program instructions that cause SDCs is one of the research hotspots in computer reliability field at present. A lot of solutions have already been proposed to solve this problem. However, many of them are hard to be applied widely due to time-consuming and expensive costs. This paper proposes an intelligent approach named SDCPredictor to identify the instructions that cause SDCs. SDCPredictor identifies SDC-causing Instructions depending on analyzing the static and dynamic features of instructions rather than fault injections. The experimental results demonstrate that SDCPredictor is highly accurate in predicting the SDCs proneness. It can achieve higher fault coverage than previous similar techniques in a moderate time cost.

  • Metallic impurities, such as Fe, Cu, and Au, become generation and recombination centers for minority carriers when combined with oxide precipitates or silicon self-interstitial clusters. As these centers may cause leakage and discharge in silicon devices, their prevention through gettering of the metallic impurities is an important issue. In this article, key aspects of intrinsic gettering, such as oxygen control, wafer cleaning, device area denudation, and bulk oxygen precipitation are discussed, and a practical method of implementing intrinsic gettering is outlined.

  • In recent booming up of the ship building market, the supplying of large scale casting is difficult to keep the delivery schedule for ship yard because of the restrict manufacturer in Korea. And also, it is main cause to rise-up the cost of castings. This paper describes the outline of guidance of optimizing design for Rudder Horn Casting to reduce the risk of the delivery problem to ship yard.

  • There is no assurance that malware could only cause virtual damage to computer programs and data as its potential is endless. However, legal provisions were earlier developed to cater to either a physical damage caused by a physical action or a virtual damage caused by a virtual action. When crossovers occur, it becomes quite uncertain as to how viable the current laws are in handling this matter. The author seeks to address the issue from the perspective of the laws of Malaysia.

  • Root Cause Analysis (RCA) has been widely used as a structured approach to investigate patient safety incidents. RCA helps identify what, how, and why something happened, therefore preventing recurrence of incidents. Since many quality tools can be used during RCA, various formats of RCA exist. If RCAs are performed incorrectly or incompletely, they are likely to produce unusable results. To address this issue, RCA software has been developed. The use of RCA software in investigating patient safety incidents may offer several advantages, such as potential reduction in learning time, shortening of the analytic process, facilitation of collection, analysis, and presentation of data and production of meaningful RCA reports. We introduced six healthcare RCA software and compared characteristics. Results from this study will enable the RCA team to choose proper RCA software.
