Primary Segmental Volvulus of the Small Bowel -Report of 2 Cases- (소장의 국한적 원발성염전 2례 보고)

  • Primary segmental volvulus of the small intestine is not associated with malrotation, malfixation of the midgut, nor other primary small bowel lesions such as small bowel tumors. This entity is known to be more prevalent in adult and in certain global areas associated with particular diet habits. There have been very few reports in neonates, but not in this country so far. The author reports two cases of primary segmental volvulus. Case 1 was a septic 4-day-old girl with hematochezia due to jejunal volvulus with partial necrosis and panperitonitis. Resection of the segment and Bishop-Koop enterostomy were successful. Case 2 was a 3-day-old boy, who had ileal volvulus with ultra-short length of ileal atresia, probably due to intrauterine segmental volvulus. Limited resection of the atresia and spreading of the mesenteric base were enough to recovery. The rarity of the pathognomonic findings and limitation of the diagnostic workup due to rapid progression limit early diagnosis and good survival rate in this particular condition.

Isolation of Enterobacteriaceae from bile, urine and intestine in slaughtered pigs and its susceptibility to antibiotics (도축돈의 담즙, 뇨 및 장관에서 장내세균의 분리 및 항생물질 감수성)

  • Seventy-nine strains of Enterobacteriaceae isolated from 117 slaughtered pigs (bile, urine, small intestine, cecum and rectum) in 1995 were examined for biotypes and susceptibility to 19 antibiotics with MicroScan WalkAway 40/96. The obtained results were as follows : 1. Among the twenty-two species isolated from the samples, Proteus mirabilis, E. coli and Enterobacter cloacae were commonly encountered. 2. The distribution frequency of isolates from cecum, small intestine, rectum, bile, and urine was 31(38.8%), 25(31.3%), 18(22.8%), 3(3.7% ), and 2(2.5% ), respectively. 3. A majority of isolates were sensitive to 16 antibiotics, singly or in combination. And these isolates were commonly susceptible to various antibiotics such as Cp, Ts, Azt, Caz, To, Gm, Cfz, Crm, Am and Cfx, in order. Whereas the Salmonella spp was susceptible to Cf, Ti and Pi, and Proteus mirabilis to Imp, Tim, Cft and Cz. Meanwhile, no effect was found to Cf, Ak and Cax. 4. Among the antibiotic resistant strains, a total of 17 reistant patterns was noted End of these Ak Tim 45(57.0%), Ak Am Cf Cfx Cfz Tim 8(10.1%) and Ak Ti Tim 6(7.6% ) were frequently encountered.

  • An experimental studies were done to investigate the spontaneous movements to the isolated ileum with the liquid extracts of Wahngkigunchunghapleechungtang. Then the effects of isolated ileum, pylorus-ligated and indomethacin-induced ulcer, gastric juice secretion, and transport abilities of the small and large intestine were studied. The following results have been obtained; 1. Wahngkigunchunghapleechungtang showed to have an inhibitory effects on the smooth muscle contraction of the isolated ileum by acetylcholine chloride and barium chloride. 2. Wahngkigunchunghapleechungtang showed to have an inhibitory effect on the contraction of the gastric fundus-strip by acetylcholine chloride and barium chloride. 3. The preventive effects were not recognized on the pylorus-ligated ulcer, by administration of Wahngkigunchunghapleechungtang. 4. The preventive effects were recognized on the indomethacin-induced ulcer by administration of Wahngkigunchunghapleechungtang. 5. The Inhibitory effects on the secretion gastric juice, free & total acidity, and pepsin output were recognized by administration of Wahngkigunchunghapleechungtang. 6. The transport abilities in the small intestine were not recognized by administration of Wahngkigunchunghapleechungtang. 7. The transport abilities in the large intestine were increased by administration of Wahngkigunchunghapleechungtang. According to the above results, it is considered that the experimental effects of Wahngkigunchunghapleechungtang has correspond to the oriental medical literatures.

Synthesis and Evaluation of 5-Aminosalicyl-glycine as a Potential Colon-specific Prodrug of 5-Aminosalicylic Acid

  • As a new colon-specific prodrug of 5-aminosalicylic acid (5-ASA), 5-aminosalicyl-glycine (5-ASA-Gly) was prepared by a simple synthetic route in good yield. Apparent partition coefficients of 5-ASA-Gly were lower than those of 5-ASA, which determined in$ CHCl_{3}$/pH 6.8 buffer or n-octanol/pH 6.8 buffer system. Stability of 5-ASA-Gly by peptidases was investigated by incubation of 5-ASA-Gly with the homogenates of tissue and contents of stomach, proximal small intestine or distal small intestine of rats at $37^{\circ}C$. 5-ASA was not detected, indicating that the prodrug was stable in the upper intestine. The amount of 5-ASA liberated from incubation of the prodrug in cecal or colonic contents of rats was about 65% or 27% in 8 hrs, respectively, which indicated that the prodrug activation took place more readily in the rat cecum whose bacterial counts are high like human colon. Results from in vitro experiments suggested 5-ASA-Gly as a promising candidate of a colon-specific prodrug of 5-ASA.

  • This study was done to investigate the clinical effects of Daegunchungtang on the peptic ulcer and the function of gastointestinal tract. The liquid extracts of Daegunchungtang was administerd orally to the rats and mice. Then the effects of isolated ileum, pylorus-ligated and indomethacine-induced ulcer, gastric juice secretion, and transport abilities of the small and large intestine, were studied. The following results have been obtained : 1.   • The literatural study on the effect of acupuncture for tinnitus was studied from the viewpoint of acupuncture effect. The conclusions are as follows. 1. The ear manages kidney, it relates with the heart meridian of hand soeum, kidney meridian of foot soeum, lung meridian of hand taeeum, spleen meridian of foot taeeum, stomach meridian of foot yangmyeong, gallbladder meridian of foot soyang, triple energizer meridian of hand soyang, small intestine meridian of hand taeyang. 2. According to classification of meridian in acupuncture treatment of tinnitus triple energizer meridian of hand soyang 18.0%, gallbladder meridian of foot soyang 16.6%, bladder meridian of foot taeyang 16.6%, small intestine meridian of hand taeyang 9.7%, large intestine meridian of hand yangmyeong 8.3%, stomach meridian of foot yangmyeong 5.5%, spleen meridian of foot taeeum 4.1%, pericardium meridian of hand gworeum 4.1%, lung meridian of hand taeeum 2.8%, heart meridian of hand soeum 2.8%, kidney meridian of foot soeum 2.8%, liver meridian of foot gworeum 2.8%, conception channel 2.8%, governor channel 2.8% have been used much in turn. 3. In the general points GB2 24times, TE3 22times, TE17 22times, SI19 20times, TE21 20times, KI 3 19times, BL23 17times, LI4 15times have been used much in turn.

Immunohistochemistry of Gastrointestinal Endocrine Cells in the Meckel′s Diverticulum of the Bean Goose, Anser fabalis Latham

  • The appearance of some gastrointestinal endocrine cells in the Meckel's diverticulum (MD) of the bean goose, Anser fabalis Latham was observed using specific antisera against serotonin, gastrin, cholecystokinin (CCK)-8, glucagon, secretin, somatostatin and human pancreatic polypeptide (HPP) with the peroxidase antiperoxidase (PAP) method. Among these specific antisera, serotonin-, gastrin-, CCK-8-, somatostatin- and HPP-immunoreactive cells were demonstrated in this study. Serotonin-, gastrin- and somatostatin-immunoreactive cells were detected at moderate frequency and CCK-8- and HPP-immunoreactive cells was rare and low frequencies, respectively. These immunoreactive cells were located in the superficial epithelium, intestinal crvpt and intestinal glands with spherical or spindle shaped cells having long cytoplasmic processes (open typed-cell). Mucosal layer of MD was composed of simple columnar epithelium and numerous intestinal glands. In addition, numerous lymphatic tissues were also demonstrated. In conclusion, histological profiles of MD were similar to any parts of the large intestine, especially the cecum, but the appearance, distribution and relative frequency of gastrointestinal endocrine cells were similar to those of upper parts of the small intestine. Although the exact digestive functions were unknown, the finding that the appearance, distribution and relative frequency of gastrointestinal endocrine cells in MD is similar to small intestine may be considered as distinct evidence that this organ may have some digestive functions.

  • A partial obstruction of small intestine was diagnosed by physical and radiographic examination in a dog which had signs of anorexia, vomiting, diarrhea and dehydration The dog was treated by surgical method. String-like foreign body was located from stomach to upper jejunum. Diffuse laceration, inflammation and perforation caused by foreign body were observed in intestine. Intestinal resection was carried out to remove the foreign body from upper duodenum to upper jejunum. The dog was convalesced successfully after operation.

  • Carcinoid tumors can be ubiquitous, but most will originate in four sites: appendix [38%], small intestine[24%], rectum[13%] or bronchus[11.6%]. And bronchial carcinoids are rare, accounting for only 1% to 6% of all primary lung tumors. Familial adenomatous polyposis cali, the most common form of the polyposis syndromes attributable to a genetic defect, is defined by demonstration of at least 100 adenomatous polyps in the large intestine. We experienced a case of typical bronchial carcinoid with familial adenomatous polyposis cali in 23 year old female patient, which was surgically treated by left lower lobectomy.

Effects of Immune Stress on Performance Parameters, Intestinal Enzyme Activity and mRNA Expression of Intestinal Transporters in Broiler Chickens

  • Immune stress is the loss of immune homeostasis caused by external forces. The purpose of this experiment was to investigate the effects of immune stress on the growth performance, small intestinal enzymes and peristalsis rate, and mRNA expression of nutrient transporters in broiler chickens. Four hundred and thirty-two 1-d-old broilers (Cobb500) were randomly assigned to four groups for treatment; each group included nine cages with 12 birds per cage. Group 1 = no vaccine (NV); Group 2 = conventional vaccine (CV); group 3 = lipopolysaccharide (LPS)+conventional vaccine (LPS); group 4 = cyclophosphamide (CYP)+conventional vaccine (CYP). The results demonstrated that immune stress by LPS and CYP reduced body weight gain (BWG), feed intake (FI), small intestine peristalsis rate and sIgA content in small intestinal digesta (p<0.05). However, the feed conversion ratio (FCR) remained unchanged during the feeding period. LPS and CYP increased intestinal enzyme activity, relative expression of SGLT-1, CaBP-D28k and L-FABP mRNAs (p<0.05). LPS and CYP injection had a negative effect on the growth performance of healthy broiler chickens. The present study demonstrated that NV and CV could improve growth performance while enzyme activity in small intestine and relative expression of nutrient transporter mRNA of NV and CV were decreased in the conditions of a controlled rational feeding environment. It is generally recommended that broilers only need to be vaccinated for the diseases to which they might be exposed.
