Obesity and Screening Compliance for Breast and Cervical Cancer in Korean Women

  • Purpose : This study was performed to assess whether the weight status is associated with screening rates of breast and cervical cancer in Korean women. Methods: Study participants included women aged between 30 and 80 years from the 4th Korea National Health and Nutrition Examination Survey from 2007 to 2009. Body mass index was classified into ${\sim}18.4kg/m^2$ (underweight), $18.5{\sim}22.9kg/m^2$ (normal), $23{\sim}24.9kg/m^2$ (overweight), $25.0{\sim}29.9kg/m^2$ (moderate obesity) and $30.0kg/m^2$~ (severe obesity) according to the Asia Pacific Standards of WHO recommended definition of obesity. Screening rates of breast and cervical cancer were estimated by the recommendation of the National Cancer Screening Program of the National Cancer Center, Korea. Results : The overall screening rates for breast and cervical cancer were 51.3% and 50.1%, respectively. After covariate adjustment, the screening rates for breast cancer (adjusted odds ratio, 0.70; 95% confidence interval, 0.51 to 0.97) and cervical cancer (adjusted odds ratio, 0.71; 95% confidence interval, 0.53 to 0.94) were significantly lower in the women with severe obesity. Conclusion: Obesity is associated with lower compliance with breast and cervical cancer screening guidelines in Korean women.

Effect of Season, Parity and Lactation on Reproductive Performance of Sows in a Tropical Humid Climate

  • The aim of this study was to analyze post-weaning reproductive performance of Large White sows in relation to season, parity and their lactation performance under tropical conditions in Guadeloupe (French West Indies, $16^{\circ}$ Lat. N, $61^{\circ}$ Long. W.). This work was based on data recorded in the experimental unit of INRA from January 1993 to December 2003. Two seasons were determined a posteriori from climatic parameters recorded continuously in a station close to the experimental unit. Mean ambient temperature was higher during the hot season than the warm season ($26^{\circ}C$ vs. $24^{\circ}C$) but relative humidity was comparable for both seasons (i.e. 87% on average). Season had a significant effect on all reproductive parameters analyzed. Primiparous sows weaned in the hot season had a higher probability of a prolonged weaning to estrus interval, WEI (odds ratio was 4.1; p<0.01) but multiparous sows were not affected. A higher probability of a prolonged weaning to conception interval, WCI (odds ratio >2.5, p<0.01) and a lower subsequent farrowing rate (-10%, p<0.01) were found for sows weaned in the hot season. A higher daily feed intake during lactation reduced the probability of a prolonged WEI (p<0.05). Body weight and average back-fat thickness at farrowing affected WEI and WCI (p<0.05), whereas body weight and average backfat thickness change in lactation did not. This study confirms the negative effects of the hot season on primiparous reproductive performance. It also indicates that lactation performance influences sow non-productive period.

Study of Physical Activities and Nutrient Intakes of Adults with metabolically healthy obesity and those with metabolically unhealthy obesity (대사적으로 건강한 비만 성인과 대사적으로 불건강한 비만 성인의 신체활동 및 영양소 섭취량에 관한 연구)

  • This study divides subjects into people with metabolically healthy obesity and those with metabolically unhealthy obesity to identify the physical activities and nutrients influencing adults with obesity. Participants in the sixth Korea National Health and Nutrition Examination Survey conducted from January through December 2013 were selected as the subjects, and an analysis of complex survey samples was conducted with them. It was found that the number of days of a flexibility exercise (Odds ratio (OR)=1.18, 95% confidence interval (95% CI)=1.01-1.38), water intake (OR=1.04, 95% CI=1.01-1.06) and protein intake (OR=1.07, 95% CI=1.02-1.13) for one week were higher in people with metabolically healthy obesity than in those with metabolically unhealthy obesity, while their fat intake (OR=0.99, 95% CI=0.98-1.00)and carbohydrate intake (OR=0.98, 95% CI=0.97-0.99) were lower. Based on the results of this study, it was found that nursing intervention for obesity would need an approach according to the metabolic condition of obesity.

Comparison of Magnetocardiogram Parameters Between a Ischemic Heart Disease Group and Control Group (정상군 및 허혈성 심질환 환자군에서의 심자도 파라미터 비교)

  • The electrical current generated by heart creates not only electric potential but also a magnetic field. We have observed electrophysiological phenomena of the heart by measuring components of magnetocardiogram(MCG) using 61 channel superconducting quantum interference device(SQUD) system. We have analyzed the possibility and characteristics of MCG parameters for diagnosis of ischemic heart disease. A technique for automatic analysis of MCG signals in time domain was developed. The methods for detecting the position, the interval, the amplitude ratio, and the direction of single current dipole were examined in the MCG wave. The position and interval parameters were obtained by calculating the gradients of a envelope curve which could be formed by the difference between the maximum and minimum envelope of multi-channel MCG signals. We show some differences of the frequency contour map between the normal MCG and the abnormal (ischemic heart disease) MCG. The direction of single current dipole can be defined by rotating the magnetic field according to Biot-Savart's law at each point of MCG signals. In this study, we have examined the direction of single current dipole from searching for the centroids of positive and negative magnetic fields. The amplitude ratio parameters for measuring 57 deviation consisted of A$_{T}$/A$_{R}$ and other ratios. and We developed a new analysis method, which is based on the frequency contour map of electromagnetic field. Using theses parameters, we founded significant differences between normal subjects and ischemic patients in some parameters.

Differences of Personality Characteristics According to the Suicide Symptoms in Patients with Depression (우울증 환자에서 자살 증상 유무에 따른 성격 특성의 차이에 관한 연구)

  • Objectives : The purpose of this study was to examine personality traits associated with suicidal symptoms (with history of suicide attempt or suicidal idea) in depressed patients. Methods : A sample 186 patients diagnosed with major depressive disorder was divided into two groups : suicidal group (with history of suicide attempt or suicidal idea, n=70) and non-suicidal group (without history of suicide attempt or suicidal idea, n=116). NEO Personality Assessment System were used to evaluate personality trait. Results : Neuroticism was high in suicidal group (p=0.041). In multivariate logistic regression, Neuroticism was also a significant influence on suicidal symptoms (Odds Ratio=1.04, 95% Confidence Interval 1.01-1.07, p=0.0145). Conclusions : High Neuroticism is a useful information to identify individuals at high risk of suicide in depressive patients.

Low Back Pain and Related Factors in Railroad Engineers (일부 철도기관사의 요통경험 및 관련요인)

  • Objectives: The purpose of present study was to investigate the experience and point prevalence rate and factors related with Low Back Pain (LBP) in train egineers. Methods: Questionnaires were completed by 324 train engineers in Daejeon railroad administration from May 2002. The information was used to estimate odds ratio (OR) and 95$\%$ confidence intervals (CI) for factors relation to LBP. A retrospective study design was used. Results: The experience rate for LBP was 67.9$\%$, 54$\%$ in one year interval prevalence, 53.4$\%$ in 6 months interval prevalence, and 47.8$\%$ in a point prevalence rate. Variables significantly associated with LBP experience were age (p=0.0327), train vibration(p=0.0015), labour hour(p=0.0034), and pay(p=0.0534). As subjects got older, the higher experience for LBP was (OR=1.1, 95$\%$ CI 1.0-1.2). LBP experience rate for people who had felt train vibration was a higher than those who did not (OR=2.5, 95$\%$ CI 1.4-4.4, OR=2.3 95$\%$ CI 1.3-4.0 in a point prevalence). The people who worked for long hour was a higher than those who did not (OR=2.8, 95$\%$ CI 1.4-5.6, OR=2.2, 95$\%$ CI 1.1-4.5 in a point prevalence). The people who were not satisfied with pay was a higher than those who were (OR=1.7, 95$\%$ CI 1.0-3.0). Factors related to a point prevalence rate were train vibration(p=0.0027), chair vibration (p=0.0444), and labour hour(p=0.0340). LBP a point prevalence rate for people who had felt the vibration of train chair was a higher than those who did not (OR=1.8 95$\%$ CI 0.7-2.0). Conclusions: Results from present study indicated that a statistically significant factors associated with LBP experience were age, train vibration, labour hour, and pay, Factors related to a point prevalence rate were also train vibration, the vibration of train chair, and labour hour.

A Study of the Relations between the Bacterial Concentration and the Environmental Factors in the Factories using Water Soluble Metal Working Fluids (수용성 금속가공유 취급사업장에서 세균농도와 환경인자의 관계에 대한 연구)

  • Objectives: The objective of this study was to investigate the relations between the bacterial concentration and the environmental factors in the water soluble metal working fluids at factories. Methods: The bacterial concentrations for airborne and fluid samples of 7 factories were quantified during the summer season. And we statistically analysed the relations between the bacterial concentrations and the factors such as temperature, relative humidity, usage quantity, mixing ratio and exchange interval. Results: The geometric mean levels of the airborne bacterial concentrations were 79.1(range : N.D.~686) $CFU/m^{3}$ and 68.1(range: N.D.~919) $CFU/m^{3}$ in the process and outdoor. The airborne bacterial concentrations showed no statistical difference by process, usage quantity, mixing ratio and exchange interval. The airborne bacterial concentrations had negatively weak correlations with air temperature and relative air humidity(p<0.05). The bacterial concentrations and pH showed significantly negative correlations in the fluids(p<0.05). And the airborne bacterial concentrations in factories and those in metal working fluids showed no statistical relationship. Conclusions: In the water soluble metal working fluids using factories, the airborne bacterial concentrations of the process were related to those of the outdoor and environmental factors, rather than the onsite contaminated metal working fluids.

The Effect of Acute Coagulopathy in Profoundly Traumatic Patients on Acute and Early Deaths (고도 중증외상 환자에서 급성 혈액응고장애가 초기 및 조기 사망에 미치는 영향)

  • Purpose: Numerous studies have investigated the pattern of traumatic death with a focus on the injury mechanism, the severity of the injury and the presence of hemorrhage. Acute coagulopathy has been treated as only one of many complications. The purpose of this study was to investigate the influence of acute coagulopathy on acute and early death due to trauma. Methods: A retrospective analysis of trauma patients with injury severity score (ISS)${\geq}25$ who had been treated between January 2011 and December 2012 was conducted. Based on the time of injury, traumatic death was categorized into acute (within 48 hours) and early (from 3 to 7 days). The correlations between various parameters within 24 hours after injury and time of death were analyzed. Results: A total of 124 patients were enrolled. Of them, 8.1% (n=10) of the patients experienced acute mortality. For those patients, significant differences in initial systolic blood pressure, coagulopathy score, amount of transfusion, abbreviated injury scale of the head and neck, the abdomen and the extremities were noted. Early mortality was experienced by 7.0% (n=8) of the patients, only coagulopathy score was found to be a significant independent risk factor for acute (odds ratio: 3.127; 95% confidence interval: 1.185-8.252; p=0.021) and early mortality (odds ratio: 2.470; 95% confidence interval: 1.029-5.929; p=0.043). Conclusion: Acute traumatic coagulopathy has an important role in the mortality, even after the acute phase. Early management and prevention of acute coagulopathy may improve survival of trauma patients.

Laboratory Model Tests on the Load Transfer in Geosynthetic-Reinforced and Pile-Supported Embankment System (토목섬유보강 성토지지말뚝시스템에서의 하중전이 효과에 관한 모형실험)

  • A series of model tests were performed to investigate the load transfer by soil arching in geosynthetic-reinforced and pile-supported(GRPS) embankment systems. In the model tests, model piles with isolated cap were inserted in the model container and geosynthetics was laid on the pile caps below sand fills. The settlement of soft ground was simulated by rubber form. The loads acting on pile caps and the tensile strain of geosynthetics were monitored by data logging system. At the given interval ratio of pile caps, the efficiency in GRPS embankment systems increased with increasing the height of embankment fills, then gradually converged at constant value. Also, at the given height of embankment fills, the efficiency decreased with increasing the pile spacing. The embankment loads transferred on pile cap by soil arching increased when the geosynthetics installed with piles. This illustrated that reinforcing with the geosynthetics have a good effect to restraint the movement of surrounding soft grounds. The load transfer in GRPS embankment systems was affected by the interval ratio, height of fills, properties of grounds and tensile stiffness and so on.

Assessment of Noise Exposure and Hearing Loss Among Workers in Textile Mill (Thamine), Myanmar: A Cross-Sectional Study

  • Background: In a wide range of industries, noise-induced hearing loss remains one of the most prevalent occupational problems. This study aimed to assess the noise exposure level and associated factors of hearing loss among textile workers in Yangon Region, Myanmar. Methods: A cross-sectional study was conducted at a Textile mill (Thamine), Yangon Region, from April to December 2018. In total, 226 workers who were randomly selected from 3 weaving sections participated in face-to-face interviews using a structured questionnaire. A digital sound level meter and pure-tone audiometer were used for the assessment of noise exposure level and hearing loss, respectively. Logistic regression analysis was performed to assess the associated factors of hearing loss. Results: In total workers, 66.4% were exposed to ≥85 dB(A) of noise exposure, and the prevalence of hearing loss was 25.7%. Age ≥35 years, below high school education, hearing difficulty, tinnitus, hypertension, > 9 years of service duration in a textile mill were positively associated with hearing loss. After adjusting confounding factors, age ≥35 years (adjusted odds ratio = 6.90, 95% confidence interval = 3.45-13.82) and tinnitus (adjusted odds ratio = 2.88, 95% confidence interval = 1.13-7.37) were persistently associated with hearing loss. Conclusion: Providing occupational hazard education and enforcement of occupational safety regulations should be taken to decrease the noise exposure level. The regular audiometry test should be conducted for assessment of hearing threshold shift. The employer needs to implement a hearing conservation program in workplace when noise exposure reaches or exceeds 85 dB(A) for 8 hours.
