여러 연구에서 임플란트 지지 보철물의 수직 침하에 대해 보고되고 있지만, 구강내 임플란트 지지보철물의 기능 시 수직침하 양상에 대해서는 알려진 바 없다. 본 증례에서는 2명의 환자에서 구치부 단일 치아 임플란트 지지 보철물의 3차원적인 변화를 평가하였다. 내부연결형 임플란트를 식립하였으며 통상적인 방법으로 Screw-Cement Retained Prosthesis(SCRP) 형태의 보철물을 제작해 장착하였다. 구강스캐너(Cerec Omnicam, Dentsply Sirona, USA)를 이용해 보철물 장착 시, 장착 1주, 1개월, 1년 후 구강 스캔을 시행하였으며 디지털 분석 프로그램(Geomagic Control X, 3D systems, USA)을 이용해 기간에 따른 위치 변화를 3차원적으로 평가하였다. 인접 치아를 기준으로 보철물 장착 후 임상적인 위치 변화가 나타났으며 변화량은 시간이 지남에 따라 증가하였다.
목적: 이 연구의 목적은 타이타늄 구성요소를 가진 지르코니아 지대주가 내부연결 임플란트에 사용되었을 때 나사 풀림에 대해 알아보는 것이다. 열순환 후 타이타늄 소켓을 가진 지르코니아 지대주와 타이타늄 지대주의 나사 풀림과 제거력을 비교 분석하였다. 연구 재료 및 방법: 내측연결 임플란트와 내측연결 타이타늄 지대주, 타이타늄 소켓을 가진 외측연결 지르코니아 지대주를 준비하였다. 내부연결형 임플란트에 5개의 타이타늄 지대주를(대조군), 타이타늄 소켓을 가진 5개의 지르코니아 지대주(실험군)를 각각 연결하고 30 Ncm의 토크로 조인 후 열순환 처리 전후의 지대주 나사 제거력을 측정하였다. 각 시편들은 열순환 장치의 수조에서 $5^{\circ}C$와 $55^{\circ}C$의 물에 60초씩 교대로 2,000회의 열순환을 시행하였다. 각 시편의 나사 풀림을 조사하고 열순환 전후에 나사 제거력을 통계학적으로 분석하였다. 결과: 두 그룹 모두에서 열순환 후 지대주 나사 풀림을 보이지 않았다. 열순환 전후의 나사 제거력 차이는 대조군에서 $-1.34{\pm}2.53Ncm$, 실험군에서 $-1.26{\pm}2.06Ncm$ 로 나타났다. 독립표본 T 통계분석 결과 두 군간에 유의한 차이를 보이지 않았다(P > 0.05). 결론: 이 실험의 결과 내에서 내부연결 임플란트를 위한 지르코니아 지대주 내 타이타늄 소켓이 열순환 후 지대주 나사풀림에 타이타늄 지대주와 비교하여 유의한 영향을 나타내지 않았다.
연구목적: 임플란트 경부 디자인의 차이가 경부 치밀골의 응력분포에 미치는 영향을 유한요소법을 이용하여 분석하고자 한다. 연구재료 및 방법: 몸체 형상은 유사 (직경 4.3 mm, 길이 10 mm)하나 경부 디자인이 internal형, external형, 및 submerged형으로 다른 3종의 임플란트 시스템 ((주)덴티스, 대구, 대한민국)에 대하여 축대칭 유한요소모델을 사용하여 해석하였다. 악골 폭경은 7 mm로 동일하게 모델링하였고 하중 조건으로는 임플란트 장축에 평행한 수직하중 50 N, 임플란트 장축에 $45^{\circ}$방향으로 작용하는 경사력 50N을 설정하였다. 결과: 해석한 3종의 임플란트 모두 경부 피질골에 응력집중을 발생시켰으며 디자인 차이에 따른 응력 수준의 차이가 관찰되었다. 경부 피질골의 최대 응력은 수직력 조건에서 internal형, submerged형, external형의 순으로 컸으며, 그 값은 각각 2.71, 2.60, 2.48 MPa였다. 경사력 조건에서는 internal형, external형, submerged형의 순으로 컸으며 그 값은 각각 9.30, 9.14, 8.97 MPa였다. 결론: 임플란트 경부 디자인이 주위 치밀골의 응력 분포에 영향을 미칠 수 있지만 그 차이는 크지 않은 것으로 분석되었다.
Purpose: Surface modification is important techniques in modern dental and orthopedic implants. This study was performed to try embedding of bioactive materials in porous Ti implants. Methods: Porous Ti implant samples were fabricated by sintering of spherical Ti powders in a high vacuum furnace. It's diameter and height were 4mm and 20mm. Embedding process was used to suction and vacuum chamber. Loading properties of porous Ti implants were evaluated by scanning electron microscope(SEM), confocal laser scanning microscope(CLSM), and UV-Vis-NIR spectrophotometer. Results: Internal pore structure was formed fully open pore. Average pore size and porosity were $10.253{\mu}m$ and 17.506%. Conclusion: Porous Ti implant was fabricated successfully by sintering method. Particles are necking strongly each other and others portions were vacancy. This porous structure can be embedded to bioactive materials. Therefore bioactive materials will be able to embedding to porous Ti implants. Bioactive materials embedding in the porous Ti implant will induced new bone faster.
목적: 임플란트 보철 수복에서 심미적 요구도가 증진되면서 최근에는 고강도, 고인성을 지니면서 생체적합성 우수하고 치아의 색과 조화로운 지르코니아 지대주의 사용이 증가하는 추세이다. 몇몇 임상보고를 통해 지르코니아 지대주의 우수한 주위조직 반응과 단기간의 성공적인 결과가 보고 되었으나, 장기간의 안정성을 평가하는 기계적 강도에 관한 연구는 미흡한 실정이다. 이에 본 연구에서는 직경, 각도, 연결방식이 상이한 임플란트 고정체 - 지르코니아 지대주의 단순 파절강도와 반복하중에 의한 피로한계 측정을 통해 지르코니아 지대주의 기계적 안정성을 평가해 보고자 하였다. 재료와 방법: ISO규정에 따라 지르코니아 지대주-고정체를 변연골이 3mm 흡수된 조건에서 $30^{\circ}$경사하중으로 압축굽힘강도를 측정하고, 그 값의 80%를 최대하중으로 10%를 최소하중으로 하여 10Hz의 sine형 반복하중을 $5{\times}10^6$ 주기를 초과하는 조건의 내구성한계를 측정하였다. 지르코니아 지대주로는 regular diameter를 가지면서 external butt joint의 straight 지대주와 $17^{\circ}$ angled 지대주, 그리고 narrow diameter 이면서 external butt joint의 straight 지대주와 internal conical joint의 straight 지대주를 사용하였다. 압축굽힘강도 값을 일원분산분석과 사후검정으로 검증하였으며, 피로파절면을 전자주사현미경을 통해 관찰했다. 결과: 압축굽힘강도는 최소 927N 이상으로, 직경의 차이에 따른 유의한 차이를 보였으며( P<.05), 내구성 한계는 503N에서 868N까지의 번위를 보였다. 결론: 지르코니아 지대주는 구강내 기능하중을 견딜수 있는 적절한 기계적 강도를 가지는 것으로 판단된다.
목적: 본 연구는 토크 게이지를 이용하여, 치과 기공사들이 임플란트 기공을 진행할 때 가하는 토크의 양을 측정해 분석하고, 이러한 결과가 보철물에 어떠한 결과를 미치는 알아보기 위함이다. 연구 재료 및 방법: 치과 기공사들이 임플란트 기공시 임플란트 나사에 가하는 토크를 디지털 토크 게이지를 이용하여 측정한다. 이후 기성지대주와 analog를 이용하여, 제조사의 권장 토크로 조였을 때의 길이를 통계학적으로 비교 분석하였다. 결과: 치과 기공사들의 임플란트 기공시 평균 토크는 $1.563{\pm}0.332Ncm$로 나타났으며, 제조사의 권장토크 값으로 조였을 때와 비교하면, 외부연결형의 침하량 차이는 없으나, 내부연결형의 침하량의 차이는 통계적으로 유의한 차이를 보였다(P < 0.05). 결론: 치과기공실에서도 제조사의 권장토크를 이용하여 보철물 기공작업을 시행하고, 정확한 토크 게이지의 사용법을 익히면, 기공오차에 따른 임플란트의 부적합을 어느 정도 해소할 수 있을 것이라 사료된다.
Background: The purpose of this retrospective study was to evaluate the clinical utility of an implant with a sandblasted, large-grit, acid-etched (SLA) surface and internal connection. Methods: Six patients who received dental implants in the Department of Oral and Maxillofacial Surgery, Chonnam National University Dental Hospital, were analyzed by factors influencing the success rate and marginal bone loss. Factors included patient's age, sex, implant installation site, whether bone graft was done, type of bone graft materials, approaching method if sinus lift was done, and the size of the fixture. In addition, the marginal bone loss was analyzed by using a radiograph. Results: All implants were successful, and the cumulative survival rate was 100 %. Average marginal bone loss of 6 months after the installation was 0.52 mm and 20 months after the functional loading was 1.06 mm. Total marginal bone resorption was 1.58 mm on average. There was no statistically significant difference in mesial and distal marginal bone loss. Conclusions: The short-term clinical success rate of the implant with an SLA surface and internal connection was satisfactory. Moreover, the marginal bone loss was also consistent with the implant success criteria.
PURPOSE. The success of an implant-prosthetic rehabilitation is influenced by good implant health and an excellent implant-prosthetic coupling. The stability of implant-prosthetic connection is influenced by the rotational tolerance between anti-rotational features on the implant and those on the prosthetic component. The aim of this study is to investigate the rotational tolerance of a conical connection implant system and its titanium abutment counterpart, in various conditions. MATERIAL AND METHODS. 10 preparable titanium abutments, having zero-degree angulation (MegaGen, Daegu, Korea) with an internal 5-degree conical connection, and 10 implants (MegaGen, Daegu, Korea) were used. Rotational tolerance between the connection of implant and titanium abutments was measured through the use of a tridimensional optics measuring system (Quick Scope QS250Z, Mitutoyo, Kawasaki, Japan) in the as-received condition (Time 0), after securing with a titanium screw tightening at 35 Ncm (Time 1), after tightening 4 times at 35 Ncm (Time 2), after tightening one more time at 45 Ncm (Time 3), and after tightening another 4 times at 45 Ncm (Time 4). RESULTS. The group "Time 0" had the lowest values of rotational freedom (0.22 ± 0.76 degrees), followed by the group Time 1 (0.46 ± 0.83 degrees), the group Time 2 (1.01 ± 0.20 degrees), the group Time 3 (1.30 ± 0.85 degrees), and the group Time 4 (1.49 ± 0.17 degrees). CONCLUSION. The rotational tolerance of a conical connection is low in the "as received" condition but increases with repetitive tightening and with application of a torque greater than 35 Ncm.

The biocompatibility and durability of implant fixtures are major concerns for dentists and patients. Mechanical complications of the implant include abutment screw loosening, screw fracture, loss of implant prostheses, and implant fracture. This case report aims to describe management of a case of fixture damage that occurred after screw fracture in a tissue level, internal connection implant and microscopic evaluation of the fractured fixture. A trephine bur was used to remove the fixture, and the socket was grafted using allogeneic bone material. The failed implant was examined by scanning electron microscopy (SEM) and energy dispersive X-ray spectroscopy (EDS), which revealed a fractured fixture with both normal and irregular bone patterns. The SEM and EDS results give an enlightenment of the failed fixture surface micromorphology with microfracture and contaminated chemical compositions. Noticeably, the significantly high level of gold (Au) on the implant surface and the trace amounts of Au and titanium (Ti) in the bone tissue were recorded, which might have resulted from instability and micro-movement of the implant-abutment connection over an extended period of time. Further study with larger number of patient and different types of implants is needed for further conclusion.
Purpose: The purpose of this retrospective study with 4-12 years of follow-up was to compare the marginal bone loss (MBL) between external-connection (EC) and internal-connection (IC) dental implants in posterior areas without periodontal or peri-implant disease on the adjacent teeth or implants. Additional factors influencing MBL were also evaluated. Methods: This retrospective study was performed using dental records and radiographic data obtained from patients who had undergone dental implant treatment in the posterior area from March 2006 to March 2007. All the implants that were included had follow-up periods of more than 4 years after loading and satisfied the implant success criteria, without any peri-implant or periodontal disease on the adjacent implants or teeth. They were divided into 2 groups: EC and IC. Subgroup comparisons were conducted according to splinting and the use of cement in the restorations. A statistical analysis was performed using the Mann-Whitney U test for comparisons between 2 groups and the Kruskal-Wallis test for comparisons among more than 2 groups. Results: A total of 355 implants in 170 patients (206 EC and 149 IC) fulfilled the inclusion criteria and were analyzed in this study. The mean MBL was 0.47 mm and 0.15 mm in the EC and IC implants, respectively, which was a statistically significant difference (P<0.001). Comparisons according to splinting (MBL of single implants: 0.34 mm, MBL of splinted implants: 0.31 mm, P=0.676) and cement use (MBL of cemented implants: 0.27 mm, MBL of non-cemented implants: 0.35 mm, P=0.178) showed no statistically significant differences in MBL, regardless of the implant connection type. Conclusions: IC implants showed a more favorable bone response regarding MBL in posterior areas without peri-implantitis or periodontal disease.
