Effects of an intensive asthma education program on asthmatic children and their caregivers (천식 환아와 보호자를 대상으로 한 집중 교육 프로그램의 효과)

  • Purpose : Asthma is one of the most common chronic childhood disease. Education of asthmatic children and their families about asthma and its management may improve disease control, reduce symptoms, and improve school performance. The aim of the study was to evaluate the effects of an intensive asthma education program in asthmatic children and their families on outcome measure of asthma management behavior scale, knowledge about asthma, self efficacy scale and quality of life. Methods : Fifteen asthmatic children and their families were invited the intensive asthma education program which including allergen avoidance, management of asthma, correct use of the inhalation devices and control of exercise-induced asthma (study group). Fifteen asthmatic children and their families those who did not participate this program were served as control group. Participants were asked to complete a written questionnaire before and 3-month after the program. Results : After completing the intensive education program, significant improvement of the childrens asthma management behavior scale (27.1 vs. 32.2, P=0.011), belief and knowledge about asthma (14.2 vs. 17.9, P<0.001), self efficacy (47.9 vs. 49.7, P=0.091) and quality of life (79.6 vs. 88.6, P<0.001) was noted in the study group by measuring questionnaires. There are increasing tendencies in parental asthma management behavior scale and knowledge about asthma. Conclusion : This intensive asthma education program is effective in improving asthma control, self efficacy and quality of life of asthmatic children. This should serve as a national model for family-based programs for asthmatic children and their families.

  • This study was conducted to investigate what contents nurses want in the intensive major course of junior nursing college. Data were collected for four months from October 2002 to February 2003 by structured questionnaires. Subjects were 240 registered nurses in working at five hospitals and four public health centers and elementary or middle schools in the capital and its suburbs and country. The collected data were analyzed using SPSS 8.0 program. The results of this study were summarized as follows : 1. Nurses working at hospitals want the programs which involve the contents as follows : emergency nursing/CPR, infection control, critical care, caner pt. care, pain control, hospice, pulmonary disease pt. care, analysis of lab. exam, heart disease pt. care, chronic disease pt. care, DM pt. care, digestive-system disease pt. care, nervous-system disease pt. care, elderly pt. care, immune-system disease pt. care, endocrine-system disease pt. care. 2. Nurses working at public health centers want the programs which involve the contents as follows : home nursing, family nursing, management of DM, HT, degenerative disease, and chronic liver disease, elderly pt. care, health education, maternity nursing, rehabilitation nursing. 3. Nurses working at elementary or middle schools want the programs which involve the contents as follows : prevention of drug abuse or misuse, sex education and consultation, health education, consultation technique, emergency care, prevention of child obesity, prevention and management of adult disease in children, teaching method, method of school health room management.

Level of radiation dose in university hospital non-insured private health screening programs in Korea

  • Objectives The aim of this study is to evaluate radiation exposure resulting from the comprehensive health examinations of selected university hospital programs and to present basic data for research and management strategies on the health effects of medical radiation exposure. Methods Radiation-based diagnostic studies of the comprehensive health examination programs of ten university hospitals in Seoul, Korea, as introduced in their websites, were analyzed. The medical radiation studies of the programs were reviewed by radiologists. Only the effective doses of the basic studies were included in the analysis. The optional studies of the programs were excluded. Results Among the 190 comprehensive health examination programs, 132 programs (69.5%) included computed tomography studies, with an average of 1.4 scans. The average effective dose of radiation by program was 3.62 mSv for an intensive program for specific diseases; 11.12 mSv for an intensive program for cancer; 18.14 mSv for a premium program; and 24.08 mSv for an overnight program. A higher cost of a programs was linked to a higher effective dose (r=0.812). The effective doses of the examination programs for the same purposes differed by as much as 2.1 times by hospital. Inclusion of positron emission tomography-computed tomography was the most critical factor in determining the level of effective dose. Conclusions It was found that radiation exposure dose from comprehensive health exam programs targeted for an asymptomatic, healthy public reached between 3.6 and 24 times the annual dose limit for the general public. Relevant management policies at the national level should be provided to minimize medical radiation exposure.

  • The expanded spiral model in this paper consists of five processes of architecture design, architectural construction, architectural maintenance, operation, and architectural management. The former four processes are executed alternatively, while the latter architectural management process is executed continuously interacting with the other processes during the whole life cycle of the system. The expanded spiral model provides a conceptual framework to sort discussions of architectural degeneration into those of product-oriented processes and those of management processes, making it possible to incorporate the models and body of knowledge about project and program management especially those of Project Management Institute, into discussions of architectural degeneration. A good architecture decomposes the software-intensive system into components mutually interacting in a well-formed structure. The architecture design process and the architectural construction process together create the object system with well-designed architecture. The architectural maintenance process prevents the implemented architecture deviate from the designed architecture. The architectural management process monitors the changes of requirements including architecturally significant requirements, supports the other processes to be executed reflecting various perspectives of stake-holders, and creates and documents the reasons of architectural decisions, which is considered as a key element of the architecture.

  • This study aims to develop a convergence optimized LOS(Length of Stay) management system that can provide efficient by predicting LOS on outpatient information for inducing the LOS to manage their own activities. web program has been implemented to comput in real-time predicting LOS by using the predicted LOS model of outpatient information. The predict model was derived management targets of long term predicted patient group and intensive care patient group. The optimized LOS(Length of Stay) management system was confirmed efficient for optimizing management of LOS that can provide by the long-term predicting alarm and over LOS alarm service for long term predicted patient group and intensive care patient group. Therefore the trial operating policy alternative on extension of predicted LOS is needed to utilized convergence optimizing system on LOS.

The Effectiveness of Environmental Management through Environmental Surveillance (환경감시를 통한 환경관리의 효과)

  • This study aims to assess how effective environmental management can be accomplished through monitoring of environmental conditions in patient discharge rooms within healthcare facilities through direct observation and the use of fluorescent markers. From March to July 2013, this study evaluated 448 check-out beds in wards and intensive care units before and 494 after intensive environmental monitoring activities. The collected data were analyzed using the SPSS 21.0 program. According to the study's findings, direct observation increased from 95.2% prior to the implementation of intensive environmental monitoring activities to 98.9% following the implementation, which was statistically significant. The non-detection rate of fluorescent markers exhibited an increase from 96.1% prior to the commencement of intensive environmental monitoring activities to 98.0% following their implementation. However, it should be noted that this observed increase was not deemed statistically significant. In light of the results of this research, it is imperative to evaluate the effectiveness of environmental management by employing a variety of assessment methods, including direct observation and fluorescent markers.

Influence of Posttraumatic Stress Symptoms and Conflict Management Styles on Nursing Performance of Intensive Care Unit (중환자실 간호사의 외상후 스트레스와 갈등관리 유형이 간호업무수행에 미치는 영향)

  • Purpose : This study examined the factors influencing posttraumatic and conflict management styles for nursing performance in intensive care units (ICUs). Methods : In this study, 250 nurses from eight general hospitals in three cities participated. Structured self-report questionnaires were used to collect data on posttraumatic, conflict management styles, and nursing performance. Finally, the data were analyzed by SAS 9.3 program. Results : The mean of total sum scores was 31.29, and the high risk of posttraumatic symptoms was 61.2%. It was noted that nursing performance is significantly correlated with collaboration, compromise, accommodation styles, and intrusion. Collaboration styles (${\beta}=0.39$, p<.001) and hyperarousal (${\beta}=-0.22$, p=.050), ICU experience below 1 year (${\beta}=-0.21$, p=.027) and that of 5-10 years (${\beta}=-0.19$, p=.049), and compromise style (${\beta}=0.16$, p=.049) were found to be the factors influencing nursing performance with 35.9% explanatory power value of regression model. Conclusions : The results of the study reveal that conflict management styles, hyperarousal, and ICU experience are factors predicting the successful performance of ICUs. These findings emphasize the need of developing interventions to reduce stress symptoms and conflicts in ICUs.

Effect of the Management of Infection for Methicillin Resistant Staphylococcus aureus at an Neurosurgical Intensive Care Unit (신경외과중환자실 내에서 MRSA 감염관리 효과)

  • Purpose: The effectiveness of an infection control program is important to hospital quality improvement and decreases of mortality rate and prevalence. Methicillin resistant Staphylococcus aureus (MRSA) is the most common pathogen causing nosocomial infection. The aim of the study was to identify the most important risk factors for acquiring an MRSA, to evaluate the MRSA incidence rates after the nursing intervention in Neurosurgery intensive care unit (ICU). Methods: Clinical data were collected prospectively from December 2008 until July 2009 in Neurosurgery ICU. The patients were divided into preintervention and postintervention groups. An infection was defined as an MRSA if it occurred 48 hr after admission to the Unit. Infection control program including hand washing, education of health care workers about MRSA, standard precaution and contact isolation of patients were applied for three month. Results: A total of 85 patients were included in the study. Forty-five patients of S. aurerus were detected. Among 45 of S. aurerus, MRSA were isolated from 38 patients. The incidence MRSA rate of postintervention group was 26.9% while incidence MRSA rate of preintervention group was 66.7%. In total, The incidence MRSA rate was 44.7%. The incidence of MRSA have decreased in the postintervention as compared with the preintervention group. Conclusion: The infection control program for MRSA was effective to decrease the MRSA isolation rate. The health care workers regular hand washing, education of nosocomial infection control is important enough to be emphasized.

  • The aim of this study is providing basic data to design successful in-service training program for teacher librarians by analyzing their preference on subjects of customized intensive programs for indicators of teacher expertise development. According to the survey, teacher librarians seem to regard training subjects such as Reading Education and Information Literacy Instruction related educational information services as core jobs and prefer developing instructional contents and materials. Under the levels of school it seems that teacher librarians in the elementary school are interested in programs for library activation, management of volunteers, analyses of users' needs and curricula, evaluations of user instruction and information literacy instruction. Older teacher librarians favor an understanding of metadata, building and supporting information system and instruction. Therefore, training subjects for teacher librarians should be formed by linking strategies between school library instruction and subject curricula. And in terms of the method of training, case studies and practical training might be better than lectures based on the theory.

  • Objectives: The purpose of this study is to examine the effects of care burden among mothers of children with developmental disabilities on depression and the moderating effects of formal and informal social support. Method: The self-report questionnaires were administered to 200 mothers of children with developmental disabilities (aged 6~20 years) in pre-adulthood in Gyeongi-do. A hierarchical regression analysis was conducted. Result: First, There is a need to provide a mental health program that can relieve depression. Second, Individualized intervention strategies for mothers' psychological counseling and support programs for intensive case management and parenting stress management programs are needed. Third, Individual visit case management is required. In addition, it is necessary to expand the eligibility for care services and to maintain long-term programs. Conclusion: The findings suggested that there is a need to provide mothers with not only personal assistance program but also services such as mothers' psychological counseling, peer mentor program, and parenting stress management.
