Effects of Polyporus Herbal-acupuncture at KI10 on LPS-induced nephritis in rats (LPS로 유도된 흰쥐의 신장염에 대한 음곡(KI10) 저령약침의 효과)

  • Objectives: The purpose of this study was to evaluate the effects of Polyporus Herbal-acupuncture(PO-HA) at KI10 (Umgok) on nephritis induced by lipopolysaccharide(LPS) in rats. Methods: Rats were allocated into normal, control, and 2 experimental groups. The rats in the control group were intra-peritoneally injected with LPS for nephritis induction. The rats in the groups of experiment 1 and experiment 2 were treated with Saline injection, and PO-HA, respectively at KI10 three times for a week and then intra-peritoneally injected with LPS. To evaluate the effects of PO-HA at KI10, WBC count in blood, creatine, tumor necrosis factor-alpha (TNF-${\alpha}$), cytokine-induced neutrophil chemoattractant 1(CINC-1) in serum, urinary volume, creatinine, total protein in urine, myeloperoxidase(MPO) in kidney were measured. Results: PO-HA at KI10 significantly suppressed the increase of WBC in blood, TNF-${\alpha}$, CINC-1 in serum, MPO in kidney of LPS-stimulated rats. PO-HA at KI10 significantly suppressed the increase creatinine, total protein in urine of LPS-stimulated rats. Conclusions: According to these results, it is postulated that PO-HA at KI10 has an anti-inflammatory and renal-protective effects on LPS-induced nephritis in rats. Therefore, it is suggested that PO-HA at KI10 may be an useful therapeutics for nephritis in clinical field after further researches.

Pharmacopuncture for Acute peripheral facial paralysis: A systematic review and meta-analysis of randomized controlled trials

  • Objectives: This study aims to evaluate the efficacy of pharmacopuncture in the treatment of acute peripheral facial paralysis (APFP) through a meta-analysis of randomized controlled trials to assess the clinical evidence for pharmacopuncture as a treatment for PFP. Methods: Literature was searched using databases. The search terms used were "pharmacopuncture," "sweet BV," "apipuncture," "apitoxin," "injection acupuncture," and "herbal acupuncture" in English and "bongchim," "bongyakchim," and "yakchim" in Korean. The RevMan 5.2 program was used for meta-analysis. Results: In all, 1902 studies were screened and the full texts of 219 articles were reviewed. Of these, nine studies were included in the systematic review and nine studies were included in the meta-analysis. Treatments utilizing bee venom, sweet bee venom (SBV), Soyeom, Calculus Bovis Fel Ursi Moschus (BUM), Hominis Placenta, and Suseunghwagang (SSHG) were observed in nine RCTs. In the forest plot, the diamond favors the PA treatment against the control treatment. (95% CI: 0.34 [0.08, 0.60], Z = 2.57, P = 0.01), which Our meta-analysis in RevMan 5.2 confirmed the significant efficacy of PA in treating APFP. Conclusion: Our systematic review and meta-analysis suggest that treatments using Soyeom, bee venom, and BUM are effective in APFP and they reduce the duration of postauricular pain occurring at the acute stage of APFP. However, due to small effect size, further investigation is needed.

  • Objectives This study was conducted to investigate randomized controlled trial studies about clinical treatment of traditional Chinese medicine for chondromalacia patellae. Methods We searched randomized controlled trial studies about traditional Chinese medicine treatment for chondromalacia patella through China National Knowledge Infrastructure (CNKI). 27 articles published from 2015-2019 were finally chosen and analyzed by published year, number of samples, evaluation criteria, treatment duration, treatment method. Results Various treatments such as acupuncture therapy, herbal medicine, moxibustion therapy, manipulation, external treatment were practiced in China. Most of them were combined with western medicine or injection, rehabilitation therapy. Conclusions There were diverse studies about traditional Chinese medical treatment for chondromalacia patellae in China. In Korea, more clinical research about chondromalacia patellae is still needed. This study will be helpful for future research on Korean medicine for chondromalacia patellae.

  • We were trying to study the validity of Puffer fish's poison(Tetrodotoxin- TTX) to make a traditional Korean Medical treatment. The following conclusions were made after literary studies. 1. The first record of the puffer fish dates back 2000 years ago in the Chinese text Book of Mountain and Sea and other texts from the similar period. 2. Puffer fish's poison IS known as tetrodotoxin which is an amino perhydroquinazoline compound. It has a chemical formula of $C_{11}H_{17}N_3O_8$ in the hemiacetal structure and has the molecular weight of 319. 3. Tetrodotoxin (TTX) plays a role as potent neurotransmitter blocker by blocking the $Na^+$ -gate channel which hinders the influx of $Na^+$ ion into the cell. 4. Symptoms of the puffer fish poisoning ranges from blunted sense in the lips and tongue, occasional vomiting in the first degree to sudden descending of the blood pressure, apnea, and other critical conditions in the fourth degree. Intoxication of the puffer fish poison progresses at a rapid pace as death may occur after an hour and half up to eight hours in maximum. Typical death occurs after four to six hours. 5. Ways to treat the puffer fish poisoning include gastric irrigation, induce vomiting, purgation, intravenous fluid injection, and correcting electrolytic imbalance and acidosis. In cases of dyspnea, apply oxygen inhalation and conduct artificial respiration. 6. Tetrodotoxin (TTX) may be applied in treating brain disorders, ocular pain, excess pain in the large intestine and ileum, and relieving tension of the skeletal museles, neuralgia, rheumatism, arthritis, and etc. 7. In terms of Oriental medicine, the puffer fish poison has characteristics of sweet, warm, and poisonous. It's known efficacies are to tonify weakness, dispel damp, benefit the lower back, relieve hemorrhoid, kills parasites, remove edema, and so forth. And the puffer fish eggs processed with ginger are said to be effective against tuberculosis and lung cancer, thus, it's validity must be investigated and further research should be followed.

  • Objectives : Through the literatures on the effets of Bufonis Venenum, we are finding out the clinical possibility and revealing the more effective to intractable diseases. Methods : We investigated the literatures of Oriental Medicine and experimental reports about Bufonis Venenum. Results : 1. Bufonis Venenum is made of bufonidae bufo bufo gargarizans cantor or bufo melanostictus schneider of white serum which secreted from parotid gland or dermato gland, and it is dried for using. 2. In oriental medicine, Bufonis Venenum has been mainly used on the tumors, cacanthrax and dermatic disease, and then it has been clinically used on infantile athrepsia, tetanus, sore throat, toothache, and so on. 3. The pharmacological effects of Bufonis Venenum are cardiotonic, respiration stimulation, depressor or vasopressor, topical anesthcsia, hallucination, striped muscle stimulation, antiasthmatic, antibacterial, antiinflammatory, anticancer, diuretic, immuno effects, etc. 4. Bufonis Venenum is largely divided in ether binding steroid compound, hydroxyl steroid compound, carboxyl or aldehyde steroid compound, indole compound, and adrenaline, cholesterole, etc. 5. Symptoms of Bufonis Venenum poisoning in digestive system are vommitig, abdominal pain, diarrhea, dehydration, in circulatory system are palpitation, shock, bradycardia, in nervous system are vertigo, somnolentia, muscle-tendon reflex weakness, and critical conditions to tissue necrosis and heart attack. 6. Ways to treat Bufonis Venenum poisoning include gastric irrigation with $0.2~0.5\%$ potassium permanganate fluid and atropine $0.5{\sim}1.0mg$ subcutaneous injection. From the chinese book of Bon Cho Gang Moke(本草綱目), if white serum of Bufonis Venenum enter the eyes, it happens the edema and pain. And then washed the eyes by juice of Lithospermi Radix(紫草) that the edema is removed. Conclusions and Discussion : The results from above literary studies show that prescriptions and Aqua-acupuncture of Bufonis Venenum could be clinically used to sedative, anticonvulsant, antibacterial, antiinflammatory, anticancer and topical ataralgesia. However it is expected that pharmacological and side effects of Bufonis Venenum are further studied.

Effects of the Pharmacopuncture with MOK on Immune Regulation by Th1/Th2 Cytokines in L-Thyroxine-Induced Hyperthyroid Rats (L-thyroxine으로 유발된 갑상선기능항진증 랫트에서 MOK 약침의 Th1/Th2 사이토카인 조절에 의한 면역조절 효과)

  • Objectives : This study was performed to investigate immune regulatory effects of the pharmacopuncture with MOK on hyperthyroid rats. Methods : The experimental hyperthyroidism was prepared by the intraperitoneal injection of L-thyroxine(LT4, 0.5 mg/kg) once daily for 2 weeks in Sprague-Dawley(SD) rats. The pharmacopuncture with MOK extract(MOK pharmacopuncture) at doses of 0.3 or 3 mg/kg was injected on acupuncture points in the thyroid glands of hyperthyroid rats once a day for 2 weeks. Propylthiouracil(PTU, 10 mg/kg) as a reference group was subcutaneously injected into the dorsal neck. We measured the levels of $IFN-{\gamma}$ and IL-4 in the sera of rats using enzyme-linked immunosorbant assay(ELISA) and determined the expression of $IFN-{\gamma}$, IL-4, IL-10, and Foxp3 in spleen tissues by reverse transcriptase-polymerase chain reaction(RT-PCR). Results : The treatment of MOK pharmacopuncture in hyperthyroid rats significantly decreased the serum levels of Th1 cytokine, $IFN-{\gamma}$(p<0.01 for MOK 0.3 mg/kg, p<0.05 for MOK 3 mg/kg, and p<0.05 for PTU) and significantly increased the levels of Th2 cytokine, IL-4(p<0.05 for MOK 0.3 mg/kg, p<0.001 for MOK 3 mg/kg, and p<0.05 for PTU) compared to control group. Also, the MOK pharmacopuncture significantly increased IL-4 expression(p<0.05 for MOK 3 mg/kg, and p<0.05 for PTU), IL-10(p<0.05 for MOK 3 mg/kg, and p<0.01 for PTU), and Foxp3(p<0.01 for MOK 0.3 mg/kg, p<0.05 for MOK 3 mg/kg and p<0.01 for PTU) in spleen tissues of hyperthyroid rats compared to control group. Conclusions : Our results suggest that MOK pharmacopuncture can help to ameliorate the pathological progression of hyperthyroidism by regulation of the Th1/Th2 imbalance.

A Review of Clinical Studies for Chinese Medicine Treatment of Post-concussion Syndrome Using the CNKI Database (뇌진탕 후 증후군의 중의치료에 대한 임상연구 동향 - CNKI검색을 중심으로)

  • Objectives: The aim of this study was to investigate the latest clinical studies on Korean medicine treatment of post-concussion syndrome (PCS) in the Chinese National Knowledge Infrastructure Database (CNKI). Methods: We searched the last 10 years of clinical studies discussing Oriental medicine-based treatments for PCS in the CNKI database. The search focused on the authors, publication year, type of study, purpose of the study, method and duration of treatment, evaluation criteria, and results of each article. Results: Of the 22 selected studies, 1 was a non-randomized controlled trial (nRCT), 2 were case series, and 19 were randomized controlled trials (RCTs). Treatments included oral herbal medicine in 16 studies, oriental medicine through external use in 2 studies, acupuncture in 3 studies, and traditional Chinese medicine injection in 2 studies. The most frequently used herb was Cnidii Rhizoma (川芎). All 22 studies confirmed the efficacy of Oriental medicine treatments. Conclusion: More varied and scientifically designed clinical studies are required to develop treatments for PCS. The results of this study could be used as basic data for further PCS studies.

Research Trend of Oriental Medical Treatments for Burning Mouth Syndrome (구강작열감증후군의 한의학적 치료에 대한 최근 국내외 임상 연구 동향)

  • Objectives : The purpose of this study is to analyze the oriental medical treatments of burning mouth syndrome, understand the tendency of treatment, and apply it to clinical settings. Methods : We collected case reports, retrospective cohort studies and RCT studies related to oriental medical treatments of burning mouth syndrome using domestic and Chinese databases(CNKI, KISS, RISS, OASIS, KCI). Search terms include 'Burning Mouth Syndrome', 'BMS', 'Burning Tongue', '灼口综合征', and 'oriental medicine', 'oriental medical treatment', 'Korean medicine' and '中医'. A search was conducted by appropriately combining keywords. Results : A total of 27 papers were included in the analysis. Among them, 9 are case studies, 1 is a retrospective cohort study, 1 is a before and after study and 16 are RCT studies. Treatments for burning mouth syndrome included herbal medicine, acupuncture, electro-acupuncture, acupoint injection, auricular acupuncture treatment, external use herbal medicine and gargling. In all studies, symptoms of burning mouth syndrome were alleviated after oriental medical treatments. In 14 RCT studies comparing western medical treatments, the results of the treatment group that included oriental medical treatments were found to be more significant, except for one. Conclusions : As a result of the study, oriental medical treatments are effective in treating burning mouth syndrome. In the future, we hope that clinical research related to oriental medical treatments of burning mouth syndrome will be actively conducted so that evidence-based treatment can be implemented.

Pharmacopuncture for Cervicogenic Dizziness

  • Objectives: Pharmacopuncture is a treatment that medicinal fluid including herbal extract is injected in body under qi/flavor theory and meridian theories. There are a number of studies investigating the efficacy of pharmacopuncture for cervicogenic dizziness but its usage differs in each study. This study aimed to review previous studies of pharmacopuncture treatment for cervicogenic dizziness to navigate the direction of improvement. Methods: Literature review was conducted on studies aimed at pharmacopuncture for cervicogenic dizziness. The randomized controlled studies which assessed the efficacy of pharmacopuncture on cervicogenic dizziness were selected. The studies were searched in Pubmed, RISS, OASIS and CNKI. After selecting eligible studies, the authors read the articles and summarized the points those are necessary in pharmacopuncture treatment for cervicogenic dizziness. Results: Twenty seven studies and 2,709 participants were included. The diverse solutions were used and the most popular were Salvia miltiorrhiza, Angelica gigas, a compound in Gastrodia elata, Panax notoginseng saponins. Some studies investigated the efficacy of conventional medical compounds (lidocaine, vitamin B, dexamethasone) injected in acupoints. Adopting acupoints and combining with body acupuncture have shown the tendency to enhance the cure rate. Pharmacopuncture could improve the efficacy of conventional treatment for cervicogenic dizziness. Conclusion: As the solution and injection method varies, principles of pharmacopuncture for dizziness should be investigated.

Experimental Studies on the Efficiency of the Injection of Mokhyangsunkisan extract, Mokhyangsunkisan plus Rheum undulatum L. extract and Palmisunkisan extract (목향순기산(木香順氣散) 수침액(水鍼液)의 효능(效能)에 관(關)한 실험적(實驗的) 연구(硏究))

  • This thesis is on the effects of the Injections of Mokhyangsunkisan extract, Mokhyangsunkisan plus Rheum undulatum L. extract and Palmisunkisan extract clinical experiments wre done with medicines to research their analgesic and anti-convulsive action and their efficiency on $O_3$ and Xylene-poisoned lung damage and the pulmonary thromboembolism of rats and mice. The results are as follows; 1. As to the analgesic action, each Injection of Mokhyangsunkisan extract, Mokhyangsunkisan plus Rheum undulatum L. extract and Palmisunkisan extract had significant effects. Among them the Injection of Mokhyangsunkisan plus Rheum undulatum L. extract had the highest effect. 2. As to the anti-convulsive action, each Injections of Mokhyangsunkisan extract and Palmisunkisan extract had significant effects whereas that of the Injection of Mokhvangsunkisan plus Rheum undulatum L. extract had no effect. 3. In the frsearch of the Lung TBA values of $O_3$-poisoned rats. each Injection of Mokhyangsunkisan extract and Palmisunkisan extract had significant effects whereas that of the Injection of Mokhyangsunkisan plus Rheum undulatum L. extract had no effect. 4. None of the Injection of Mokhyangsunkisan extract, Mokhyangsunkisan plus Rheum undulatum L, extract and Palmisunkisan extract had significant effects in the experiments about the variation of the $Na^+$ contents and $K^+$ contents in the serum electrolytes of the $O_3$-poisoned rats. 5. In the research of the Lung TBA values of Xylene-poisoned rats all the three Injections had prominent effects. 6. In the research of the Lung weight in Xylene-poisoned rats each Injections of Mokhyangsunkisan extract and Palmisunkisan extract had significant effects whereas that of the Injection of Mokhyangsunkisan plus Rheum undulatum L. extract had no effect. 7. In the experiments about the variation of the $Na^+$ contents in the serum electrolytes of the Xylene-poisoned rats the Injection of Mokhyangsunkisan extract had a significant effect whereas those of the Injections of Mokhyangsunkisan plus Rheum undulatum L. extract and Palmisunkisan extract had no significant effects. 8. None of the Injection of Mokhyangsunkisan extract, Mokhyangsunkisan plus Rheum undulatum L. extract and Palmisunkisan extract had significant effects in the experiments about the variation of the $K^+$ contents in the serum electrolytes of the Xylene-poisoned rats. 9. All the three Injections had prominent death-repressive effects on the pulmonary thromboembolism induced by Sodium Arachidonate. 10. Each Injection of Mokhyangsunkisan extract and Palmisunkisan extract had death-repressive effects on the pulmonary thromboembolism induced by ADP, whereas the Injection of Mokhyangsunkisan plus Rheum undulatum L. extract had a feeble death-repressive effect By all results of the clinical experiments. the following conclusions are drown; Each Injection of Mokhyangsunkisan extract and Palmisunkisan extract has analgesic and anti-convulsive effects and is also effective for the $O_3$ and Xylene-poisoned Lung damage and pulmonary thromboembolism of rats and mice. The Injection of Mokhyangsunkisan plus Rheum undulatum L. extract has on analgesic effect and is also effective for the pulmonary thromboembolism. Especially its analgesic effect is prominent. Therefore, each Injection of Mokhyangsunkisan extract and Palmisunkisan extract can be used for injection to improve the pulmonary functions whereas the Injection of Mokhyangsunkisan plus Rheum undulatum L. extract is partly effective for the improvement of the pulmonary function. In view of the results so far achieved, fluid acupuncture therapy can be applied as well as traditional way of oral administration of the decoction.
