Direct Multiplex Reverse Transcription-Nested PCR Detection of Influenza Viruses Without RNA Purification

  • This paper describes the development a of direct multiplex reverse transcription-nested polymerase chain reaction (PCR) method, devised for simultaneous detection and typing of influenza viruses. This method combines the direct reverse transcription reaction without RNA purification with the enhancement of sensitivity and specificity of nested PCR. The method successfully detected three major human influenza viruses: influenza virus A subtype 1 (H1N1) and subtype 3 (H3N2), and influenza B virus (B). The minimum number of virus particles (pfu/ml) necessary for detection in spiked saliva samples was 200 (H1N1), 140 (H3N2), and 4.5 (B). The method's sensitivity and simplicity will be convenient for use in clinical laboratories for the detection and subtyping of influenza and possibly other RNA viruses.

신종인플루엔자 유행시 대학생의 건강행위에 영향을 미치는 요인 (Affecting Factors on Health Behavior of University Students during Pandemic Influenza A (H1N1))

  • Purpose: This study was performed to identify the affecting factors on health behavior of university students during pandemic Influenza A (H1N1). Method: The participants in this study were 283 students in H university. Data were collected through self-reported questionnaires in September, 2009. The collected data were analyzed by ANOVA, Pearson correlation and stepwise multiple regression using SPSS/WIN 18.0. Result: The percentage of correct answers regarding knowledge of Influenza A (H1N1) was 34.6%. Risk perception and compliance of health behavior were very poor, $17.92{\pm}6.26/30$ and $2.00{\pm}1.68/9$, respectively. There was a positive correlation among knowledge, risk perception and health behavior. Knowledge and risk perception of Influenza A (H1N1) and intention of getting an influenza vaccination for this year explained 16.3% of variance in health behavior. Conclusion: An educational program focusing on strategy to enhance university students' knowledge and risk perception of Influenza A (H1N1) would be effective in improving their health behavior against it.

Genetic and Antigenic Characterization of Swine H1N2 Influenza Viruses Isolated from Korean Pigs

  • H1N2 influenza viruses are circulating in pigs worldwide and cause considerable economic losses to the pig industry. We genetically analyzed the genes of our isolates from Korean pigs, and compared the antigenicity of our isolates with swine H1N2 viruses isolated from pigs in the U.S.A. In addition, we serologically surveyed the infection rate of swine H1N2 viruses in pigs. We found that H1N2 isolates from Korean pigs are genetically more related to swine H1N2 viruses isolated from pigs in the U.S.A. than those in European countries. When antigenicity was compared, our isolates were weakly reacted to antibodies against swine H1N2 viruses isolated from pigs in the U.S.A. The serological surveillance using sera from pigs in Korea showed that about 46% was positive for H1N2 viruses. Our results suggest that swine H1N2 viruses are widespread in Korean pigs, and the development of a vaccine against H1N2 viruses may help to control their infection in pigs.

Oseltamivir를 투약한 양성대조군과 비교한 인플루엔자의 한약 치료 효과 - RCT 연구를 중심으로 - (Review of Clinical Research for Herbal Medicine in the Treatment of Influenza Compared with Oseltamivir)

  • Objectives The purpose of this study is to analyze clinical studies on effectiveness of herbal medicine in influenza compared with Oseltamivir. Methods We searched the randomized controlled trials (RCTs) with herbal medicine treatment on influenza compared to Oseltamivir from the Pubmed, CNKI, OASIS, NDSL, J-stage, and CiNii. Results 14 out of 717 studies were selected and analyzed. The herbal medicine treatment had a significant effects on the alleviation of fever, cough, sore throat than the Oseltamivir control treatment. Herbal medicine had same therapeutic effectiveness like Oseltamivir on duration of influenza-like symptoms and viral shedding. No serious adverse reactions were reported from the herbal medicine treatment. Conclusions The results of these trials showed that the herbal medicine may be more effective than the Oseltamivir in the treatment of influenza. Well-designed RCTs for domestic herbal medicine treatment on influenza are needed to prove its efficacy clearly.

면역저하환자에서 인플루엔자백신의 면역원성 평가 (Assessment of Influenza Vaccine Immunogenicity in Immunocompromized Host During 2009 Influenza Season: A Single Institution Experience)

  • 목 적 : 인플루엔자는 소아암 환자에게 이환율과 사망률이 높은 질환이나 소아암 환자에 대한 예방 접종률은 낮은 상태이다. 본 연구에서는 소아암 환자를 대상으로 인플루엔자 예방접종의 면역원성과 부작용에 대한 평가를 시행하였다. 방 법 : 2009년 10월부터 12월까지 원자력의학원에서 인플루엔자 예방접종(SK influenza IX vaccine$^{(R)}$)을 받은 25명의 소아암 환자를 대상으로 연구를 시행했다. 예방접종일과 접종 후 30일 뒤 2회에 걸쳐 채혈하였고 혈구응집억제 항체가를 측정하였다. 백신의 면역원성은 접종 전과 접종 후 30일의 혈구응집억제 항체가 1:40 이상인 피험자 비율, 접종 후 30일의 항체 양전율, 접종 전과 접종 후 30일 사이의 GMT 증가 배수로 평가하였다. 결 과 : 본 연구대상자 중에서 심각한 예방접종관련 부작용을 경험한 대상자는 없었다. 접종 후 혈구응집억제 항체가 1:40 이상을 보인 피험자의 비율은 H1N1 항원에 대해 68%, H3N2 항원에 대해 40%, B 항원에 대해 36%였다. 항체양전율은 H1N1 항원에 대해 12%, H3N2 항원에 대해 16%, B 항원에 대해 20%였다. GMT 증가 배수는 H1N1 항원에 대해 0.9, H3N2 항원에 대해 1.2, B 항원에 대해 1.8이었다. 결 론 : 본 연구의 대상자들은 인플루엔자 백신에 대해 제한적인 면역반응을 보였으나 일부 대상자들에게서 항체 양전이 나타났고 심각한 예방접종관련 부작용이 없었던 점을 고려할 때 소아암환자를 대상으로 매년 정기적인 인플루엔자 예방접종이 추천된다.

계절 인플루엔자: 단일기관 연구 (Clinical and Laboratory Findings of the 2012 Winter Seasonal Influenza A and B Outbreak at a Single Institution)

  • 목적: 인플루엔자 바이러스는 전세계적으로 소아 급성호흡기 감염의 주된 원인 바이러스로, 5세 미만 환아에서 매년 높은 이환율과 사망률이 보고되고 있다. 본 연구에서는 2012년 원주지역에 발생한 A형 인플루엔자 및 B형 인플루엔자의 임상양상의 차이, oseltamivir 투약군과 비투약군의 차이를 비교, 분석하여 향후 치료 및 예후 판단에 도움을 얻고자 하였다. 방법: 2012년 1월부터 3월까지 원주 세브란스 기독병원에 내원한 18세 이하의 환아 중, 인플루엔자 신속 항원검사 양성으로 보고된 환아를 대상으로 후향적으로 조사하였다. 결과: 연구기간 동안 A형 인플루엔자 환아는 374명(83.7%), B형 인플루엔자 환아는 72명(16.6%)이 보고되었다. 시기적으로 A형 인플루엔자는 2월에 가장 많았으며(n=186), B형 인플루엔자는 3월에 가장 많은 수가 보고되었다(n=36). 발열이 가장 흔한 증상이였고(97.1%), 그 외 기침, 콧물, 가래 순으로 증상이 관찰되었으며 증상 및 검사실 결과에서 인플루엔자 A, B형 간에 통계학적으로 의미 있는 차이는 없었다. oseltamivir 치료 시, 특히 발열 후 2일 이내에 투여한 경우에 비치료군에 비하여 입원율 및 입원 기간, 폐렴의 발생 빈도가 통계적으로 낮게 나타났다. 결론: 본 연구에서는 인플루엔자 A와 B 군간의 혈액검사 및 증상, 경과 등에는 통계학적으로 유의한 차이가 관찰되지 않았으나, oseltamivir 처방을 받은 환아나, 발열 2일 내에 치료를 시행한 경우 더 양호한 경과를 보였다.

소아 입원환자에서의 A형 및 B형 인플루엔자 임상 비교 (Clinical Comparison of Influenza A and B Virus Infection in Hospitalized Children)

  • 목적: 소아 입원환자에서 A/H1N1, A/H3N2형 및 B형 인플루엔자 감염을 비교하고 항바이러스제의 효용성을 분석하고자 하였다. 방법: 2014년 1월부터 4월까지 창원파티마병원에 인플루엔자 감염으로 입원한 소아 환자들을 후향적으로 분석하였다. 결과: 총 302명 중 인플루엔자 A/H1N1형 15명(5.0%), A/H3N2형 100명(33.1%), B형 187명(61.9%)이었다. A는 24개월 미만, B는 24개월-6세 사이 감염자에서 높은 분포를 보였고(P=0.005). B형 인플루엔자 감염군에서 발열 기간이 유의하게 길었다(P=0.001). 총 161명(53.3%)가 백신 접종자였으며, 감염 환자군 모두에서 oseltamivir를 복용한 환자들의 발열 기간이 유의하게 더 짧은 것으로 나타났다. 결론: A형과 B형 인플루엔자 환자는 연령 분포 및 임상 경과에 유의한 차이를 보였으며, oseltamivir는 효과의 차이는 있었으나 두 군 모두에서 효용성이 있다.

A note on the distance distribution paradigm for Mosaab-metric to process segmented genomes of influenza virus

  • In this paper, we present few technical notes about the distance distribution paradigm for Mosaab-metric using 1, 2, and 3 grams feature extraction techniques to analyze composite data points in high dimensional feature spaces. This technical analysis will help the specialist in bioinformatics and biotechnology to deeply explore the biodiversity of influenza virus genome as a composite data point. Various technical examples are presented in this paper, in addition, the integrated statistical learning pipeline to process segmented genomes of influenza virus is illustrated as sequential-parallel computational pipeline.

Rapid Molecular Diagnosis using Real-time Nucleic Acid Sequence Based Amplification (NASBA) for Detection of Influenza A Virus Subtypes

  • Influenza A virus of the Orthomyxoviridae family is a contagious respiratory pathogen that continues to evolve and burden in the human public health. It is able to spread efficiently from human to human and have the potential to cause pandemics with significant morbidity and mortality. It has been estimated that every year about 500 million people are infected with this virus, causing about approximately 0.25 to 0.5 million people deaths worldwide. Influenza A viruses are classified into different subtypes by antigenicity based on their hemagglutinin (HA) and neuraminidase (NA) proteins. The sudden emergence of influenza A virus subtypes and access for epidemiological analysis of this subtypes demanded a rapid development of specific diagnostic tools. Also, rapid identification of the subtypes can help to determine the antiviral treatment, because the different subtypes have a different antiviral drug resistance patterns. In this study, our aim is to detect influenza A virus subtypes by using real-time nucleic acid sequence based amplification (NASBA) which has high sensitivity and specificity through molecular beacon. Real-time NASBA is a method that able to shorten the time compare to other molecular diagnostic tools and is performed by isothermal condition. We selected major pandemic influenza A virus subtypes, H3N2 and H5N1. Three influenza A virus gene fragments such as HA, NA and matrix protein (M) gene were targeted. M gene is distinguished influenza A virus from other influenza virus. We designed specific primers and molecular beacons for HA, NA and M gene, respectively. In brief, the results showed that the specificity of the real-time NASBA was higher than reverse transcription polymerase chain reaction (RT-PCR). In addition, time to positivity (TTP) of this method was shorter than real-time PCR. This study suggests that the rapid detection of neo-appearance pandemic influenza A virus using real-time NASBA has the potential to determine the subtypes.

2009년 유행 신종인플루엔자 A(H1N1)의 한약 및 기타 비전형적 치료에 대한 고찰 (An Overview of the Herbal Remedies and other Non-conventional Therapies for 2009 Novel Influenza A(H1N1))

  • Background : Since March 2009, when the first patient of novel influenza A (H1N1) was reported, many deaths have occurred in North and Central America. The start of the 2009 influenza pandemic was declared by WHO Director-General Dr. Margaret Chan on 11 June 2009, and the level of influenza pandemic alert raised from phase 5 to phase 6. There was no vaccine yet developed, and many experts worried that the novel H1N1 virus could kill as many or more as did the influenza pandemic in 1918-1919. Objective : To evaluate the possibility of treatment for 2009 novel influenza A (H1N1) using herbal remedies and other non-conventional therapies. Methods : We researched the clinical studies for novel H1N1 influenza virus-related herbal medicine or non-conventional medicine treatment using internet search engines including PubMed and CNKI. In addition, we reviewed many reports and clinical practice guidelines (CPG) for influenza A (H1N1). Results : Two case series were selected after reviewing 701 papers, and two CPG published by the Chinese government and Jilin province identified. They reported that the clinical symptoms were no more significant than seasonal influenza, and the condition of patients more than 45 years old was milder than those less than 45 years old. There are no patients with gastric problems, and oseltamivir has been used at the same time in all patients. Conclusion : The efficacy and effectiveness of herbal medicine and other non-conventional treatments for the novel influenza A (H1N1) is questionable, and more studies are needed to draw a firm conclusion. However, in the severe acute respiratory syndrome (SARS) experience in 2002/2003, it was demonstrated that herbal medicine can relieve all symptoms of SARS patients, promote absorption of lung inflammation, improve the degree of blood oxygen saturation, regulate immunological functions, reduce the required dosage of glucocorticoid and other medicines, and reduce case fatality rate. In light of the current situation that there is no vaccine or conventional treatment yet available, the study of herbal medicine and other non-conventional therapies are also necessary for appropriate evaluation.
