IoT-based Architecture and Implementation for Automatic Shock Treatment

  • The Internet of Things (IoT) is being used in a wide variety of fields due to the recent 4th industrial revolution. In particular, research is being conducted that combines IoT with the medical field such as telemedicine. Among them, the field of shock detection is a big issue in the medical field because the causes of shock are diverse, treatments are very complex, and require a high level of medical knowledge and experience. The transmission of infectious diseases is common when treating critically ill patients, especially patients with shock. Thus, to effectively care for shock patients, we propose an architecture that continuously monitors the patient's condition, and automatically recommends a drug injection treatment according to the patient's shock condition. The patient's hemodynamic information is continuously monitored, and the patient's shock generation information is recorded periodically. With the recorded patient information, the patient's condition is determined and automatically injected with necessary medication. The medical team can find out whether the patient's condition has improved by checking the recorded information through web applications. The study can help relieve the shortage of medical personnel and help prevent transmission of infectious disease in medical staff. We look forward to playing a role in helping medical staff by making recommendations for the diagnosis and treatment of complex and difficult shocks.

Detection of Fever with Subcutaneously Implanted Thermo-Loggers in Cattle Administered with Lipopolysaccharide

  • The aim of this study is to determine whether subcutaneous temperature (ST) was correlated with rectal temperature (RT) in cattle with inducing artificial fever. In order to determine the correlation between their temperatures, the experiment was performed as follow: Among nine Holstein steers, lipopolysaccharide (LPS) was intravenously administered at a dose of $0.5{\mu}g/kg$ of body weight to six Holstein steer, then, 6 ml of saline was administrated to three steers as a control group. After LPS injection, ST was recorded using subcutaneously implanted thermo-logger sensors at 10-min intervals, and RT was measured using a digital thermometer at 0, 1, 2, 3, 4, 8 and 12 h. In steers with LPS injection, RT was highest at 3 to 4 h and recovered to a pre-challenge temperature at 8-22 h. A similar fluctuation was shown in ST except for an unexpected decrease at 1 h, and a positive correlation between RT and ST was observed in LPS-challenged steers (r = 0.497, P = 0.04). This result suggests that ST could be utilized as an index for early detection of infectious diseases or physiological events.

Serological Detection of Borrelia burgdorferi among Horses in Korea

  • Lyme disease is a tick-borne zoonotic infectious disease caused by Borrelia burgdorferi. The present study assessed the infection status of B. burgdorferi among horses reared in Korea using ELISA and PCR. Between 2009 and 2013, blood samples were collected from 727 horses throughout Korea. Data for each animal including age, gender, breed, and region of sample collection were used for epidemiological analysis. Overall, 38 (5.2%; true prevalence: 5.5%) of 727 horses were seropositive by ELISA. There were statistically significant differences according to breed and region (P<0.001) whose differences might be attributed to the ecology of vector ticks and climate conditions. Using 2 nested PCR, none of the samples tested positive for B. burgdorferi. Thus, a positive ELISA result can indicate only that the tested horse was previously exposed to B. burgdorferi, with no certainty over the time of exposure. Since global warming is likely to increase the abundance of ticks in Korea, continuous monitoring of tick-borne diseases in Korean horses is needed.

중소병원의 2015 MERS 위기 대응: G병원의 사례 (A Small-and-medium-sized Hospital's Crisis Management during 2015 MERS Outbreak: A Case of G Hospital)

  • Purpose: Crisis is inevitable to every organization and therefore, successful crisis management is critical to the organizations' survival and prosperity. With the understanding, this study aims to draw propositions for successful crisis management of hospitals when facing infectious disease outbreak. For the purpose, a case of a small and medium sized hospital's experience of crisis management during 2015 Middle East Respiratory Syndrome outbreak was analyzed. Methodology/Approach: The detailed internal circumstances and experiences of the hospital during the MERS outbreak were identified by in-depth interview as well as the extensive material review, and analyzed under the view of the theories of accident, error, and crisis in relation of organization management Findings: Overall, nine propositions are drawn by the phase of crisis. In pre-crisis phase, for example, 'the hospital preparedness has positive influence on the effective responding to the crisis'. In detection phase, 'the mindfulness of the hospital organizations' as well as the individuals' has positive influence on detecting the crisis signals'. In crisis phase, for example, 'improvising naturally occurs in crisis by the unknown disease, therefore, a component site supervisor coordinating such improvision is important'. Lastly, in post-crisis phase, 'successful crisis responding experience facilitates the positive hospital culture'. Practical implication: From the experience of a small and medium size hospital, it is suggested that proactive system approach oriented by safety is beneficial for effective crisis management.

Genetic Analysis and Serological Detection of Novel O-Antigen Gene Clusters of Plesiomonas shigelloides

  • Plesiomonas shigelloides, a member of the family Vibrionaceae, is a gram-negative, rod-shaped, facultative anaerobic bacterium with flagella. P. shigelloides has been isolated from such sources as freshwater, surface water, and many wild and domestic animals. P. shigelloides contains 102 O-antigens and 51 H-antigens. The diversity of O-antigen gene clusters is relatively poorly understood. In addition to O1 and O17 reported by other laboratories, and the 12 O serogroups (O2, O10, O12, O23, O25, O26, O32, O33, O34, O66, O75, and O76) reported previously by us, in the present study, nine new P. shigelloides serogroups (O8, O17, O18, O37, O38, O39, O44, O45, and O61) were sequenced and annotated. The genes for the O-antigens of these nine groups are clustered together in the chromosome between rep and aqpZ. Only O38 possesses the wzm and wzt genes for the synthesis and translocation of O-antigens via the ATP-binding cassette (ABC) transporter pathway; the other eight use the Wzx/Wzy pathway. Phylogenetic analysis using wzx and wzy showed that both genes are diversified. Among the nine new P. shigelloides serogroups, eight use wzx/wzy genes as targets. In addition, we developed an O-antigen-specific PCR assay to detect these nine distinct serogroups with no cross reactions among them.

뉴캣슬병 바이러스 검출 및 병원성 감별을 위한 Duplex RT-PCR법 개발 (Development of a Duplex RT-PCR Assay for the Simultaneous Detection and Discrimination of Avirulent and Virulent Newcastle Disease Virus (NDV))

  • 본 연구에서 NDV의 L유전자와 F유전자를 표적 부위로 각각 제작한 primer 세트를 사용함으로써 하나의 PCR 튜브에서 NDV 검출(386 bp의 증폭 크기)과 함께 병원성 NDV(229 bp의 증폭 크기)를 동시에 감별 증폭할 수 있는 dRT-PCR 검사법을 개발하였다. 개발된 dRT-PCR검사법은 NDV를 특이적으로 검출하고, 병원성을 감별하였다. 특히 국내 병성감정 실시기관에서 적용 중인 기존의 RT-PCR 상용키트에서는 검출하지 못하는 class I NDV과 PPMV(class II 유전형 VI형)을 NDV를 검출함과 동시에 병원성 NDV도 감별가능하였다. 개발된 dRT-PCR 검사법의 검출 민감도는 약 $10^{3.0}EID_{50}/0.1mL$로 평가되었다. 또한 ND발생국의 야외 시료에 적용했을 때, NDV 공통항원 검출율은 94.4%였으며, 병원성 NDV 검출율은 100%이었다. 그러므로 본 연구에서 개발한 dRT-PCR 검사법은 의심축 사례에서 ND를 신속 정확하게 진단하는 데 유용할 진단 방법을 제공할 수 있을 것으로 판단된다.

2005년부터 2007년 사이 양식 넙치, Paralichthys olivaceus 질병에 대한 통계 자료 (Statistical data on infectious diseases of cultured olive flounder Paralichthys olivaceus from 2005 to 2007)

  • 2005년부터 2007년 사이 매분기별로 포항, 울산, 기장, 거제, 완도 지역에서 채집한 양식넙치를 대상으로 기생충, 세균 및 바이러스에 대한 병원체 모니터링 결과를 분석하였다. 총 2,238마리 중 1,528마리(68.3%)에서 병원체가 검출되었다. 연도별 검출률을 비교한 결과, 2005년도에는 60.6%, 2006년도에는 66.7%, 2007년도에는 72.3%의 병원체 검출률을 나타내어 해마다 증가하는 것으로 나타났다. 조사시기별 총 병원체 검출률은 2월에 63.5%, 5월에 67.3%, 8월에 75.1%, 11월에 64.2%로 나타났다. 기생충, 세균 및 바이러스의 검출률은 각각 36.7%, 32.8%, 31.4%로 나타났다. 병원체의 단독감염률 및 혼합감염률은 각각 33.6%와 34.6%로 나타났다. 질병별 분포 조사에서 가장 높은 검출률을 나타낸 병원체는 Trichodina sp. (28.2%), viral nervous necrosis virus (24.3%), Vibrio spp. (11.6%), viral hemorrhagic septicaemia virus (10.5%) 순으로 나타났다.

한국의 건강검진 현황 (Current Status of Health Screening in Korea)

  • Along with a development of medical technology, a variety of tests, such as laboratory tests, x-ray and endoscopies are being used in health screening tests. As the tests determine the quality of health screening, test items and methods should be carefully selected. This study was to get hold of the test items of major health screening programs in Korea Most of the health screening programmes focused upon detection of risk factors and diagnosis of life - style related diseases (diabetes, hypertension, cardiovascular diseases, hypercholesterolemia, overweight, drinking, smoking, cerebrovascular diseases, osteoporosis) ,cancers (stomach, cervix, lung, breast, liver, colon, prostate, ovary, pancreas, thyroid, esophagus) , infectious diseases (hepatitis, tuberculosis, sexually-transmitted diseases, parasites) , chronic obstructive respiratory diseases, chronic renal diseases (bacteriuria hematuria, proteinuria) , anemia, glaucoma, hearing loss, Alzheimer disease, stress, early psychiatric diseases. The health screening tests were basic physical examination, basic laboratory tests (CBC, urinalysis, liver function tests, lipid tests, glucose, HbAlc, uric acid, electrolytes, serological tests (HBsAg, HBs-Ab, HCV-Ab, HIV-Ab, VDRL) EKG, x-ray (chest PA, CT) , endoscopy (gastroscopy, colonoscopy) , sonography(abdomen, thyroid, pelvis, breast) ,cytology (cervix) , bone density, tumor markets (NMP22, alpha-FP, CEA, CA-19-9, CA12S, PSA) and eye tests. Advanced technologies, like CT, PET, MRI, MRT/Angio, molecular testings) were widely used in hospital health screening programmes .In summary, a variety of tests were utilized in health screening in Korea. Those tests were utilized by stages or according to sex and age in most of health screening programmes, however a few program used tests to excess disregarding health screening subjects.

A New Multiplex-PCR for Urinary Tract Pathogen Detection Using Primer Design Based on an Evolutionary Computation Method

  • This work describes a new strategy for optimal design of Multiplex-PCR primer sequences. The process is based on the Particle Swarm Optimization-Simplex algorithm (Mult-PSOS). Diverging from previous solutions centered on heuristic tools, the Mult-PSOS is selfconfigured because it does not require the definition of the algorithm's initial search parameters. The successful performance of this method was validated in vitro using Multiplex-PCR assays. For this validation, seven gene sequences of the most prevalent bacteria implicated in urinary tract infections were taken as DNA targets. The in vitro tests confirmed the good performance of the Mult-PSOS, with respect to infectious disease diagnosis, in the rapid and efficient selection of the optimal oligonucleotide sequences for Multiplex-PCRs. The predicted sequences allowed the adequate amplification of all amplicons in a single step (with the correct amount of DNA template and primers), reducing significantly the need for trial and error experiments. In addition, owing to its independence from the initial selection of the heuristic constants, the Mult-PSOS can be employed by non-expert users in computational techniques or in primer design problems.

HRM 분석법을 이용한 패류 내 Megalocytiviruses의 검출과 유전적 분석 (Detection and Genetic Differentiation of Megalocytiviruses in Shellfish, via High-Resolution Melting (HRM) Analysis)

  • Viruses in the genus Megalocytivirus have been subdivided into four subgroups. Among these subgroups 2 and 4, represented by the red sea bream iridovirus (RBIV) and the olive flounder iridovirus (FLIV), respectively, are non-exotic. subgroups 1 and 3, represented by the red sea bream iridovirus (RSIV) and the infectious spleen and kidney necrosis virus (ISKNV), respectively, have not been detected in Korea and are known as exotic. Shellfish are filter-feeders, and can thus filter and accumulate Megalocytivirus in their digestive glands, allowing us to track viral contamination in surrounding aquatic environment. In this study, we developed a high-resolution melting (HRM) analysis to differentiate among subgroups of Megalocytivirus accumulated in shellfish, and confirmed the convenience and efficiency of this method. More than two subgroups of Megalocytivirus were found in the digestive gland of a single shellfish. We classified all Megalocytivirus viruses from shellfish in Korea into subgroups 2 and 4, although proportions of subgroups were different among regions. Compared to nucleotide sequencing analysis, HRM analysis is a simple and rapid method for differentiating of Megalocytivirus subgroups.
