Purpose: This study was performed to suggest reference line-pair values of panoramic images with clinically desirable qualities using an arch-form phantom stand. Materials and Methods: The line-pair test phantom was chosen. A real skull model was selected for setting the arch-form model of the phantom stand. The phantom stand had slits in four regions (incisor, premolar, molar, TMJ). Four raw images of the test phantom in each region and one raw image of the real skull were converted into 50 test phantom images and 50 skull phantom images with various line-pair values. 50 post-processed real skull phantom images were divided into 4 groups and were randomly submitted to 14 evaluators. Image quality was graded on a 4 point scale (1. good, 2. normal, 3. poor but interpretable, and 4. not interpretable). The reference line pair was determined as the first line-pair value scored less than 2 points. Result: The mean scores tended to decrease as the line-pair values increased. The reference line-pair values were 3.19 LP/mm in the incisor, 2.32 LP/mm in the premolar and TMJ, and 1.88 LP/mm in the molar region. Conclusion: Image quality evaluation methods and criteria should be able to assess various regions considering the characteristics of panoramic systems. This study suggested overall and regional reference line-pair values and established a set of standard values for them.
The purpose of this study was to classify mandibular dental arch forms based on Raberin's method, and to compare Raberin's arch forms with that of the Korean's, and to designate arch form of bracket level according to distance between cusp tip and buccal surface of bracket level. The sample consisted of 159 mandibular dental casts showing normal occlusion which was taken from 62 males and 97 females of the Korean, aging from 13 to 25 years. The model was taken by X-ray. The landmarks were cusp points which expressed the mandibular dental arch line of cusp tips and buccal points which were measured from cusp tips to buccal surfaces of bracket level. The landmarks on the film were digitized, and measurements and statistics were performed. The results were as follows; 1. The models were classified as type 1, type 2, type 3, type 4 and type 5 by the author, and polynomial functions of the six degree and R-square values were calculated using statistical method, and each calculated equations explained each group with the least R-square value of 0.97, and each arch forms' were plotted. 2. The distribution of type 1 was $17.6\%$, type 2 $20.8\%$, type 3 $20.8\%$, type 4 $16.3\%$ and type 5 $24.5\%$. 3. The Korean arch form was characterized by larger width, smaller height compared to the French arch form. 4. The designated arch form of bracket level, viz the distance between cusp point and buccal point was calculated. The distance between cusp point and buccal point of incisor was 1mm, canine 1.9mm, first premolar 2.5mm, second premolar 2.6mm, first molar 2.7mm and second molar 2.7mm.
Background: Cleft lip and palate deformity have unknown patterns of maxillofacial growth and development. The maxillofacial growth can be affected either by congenital or environmental factors such as infection and trauma. Surgical repair of cleft lip and palate may interfere the subsequent growth and development of maxillofacial region. The purpose of this study is to evaluate the characteristics of maxillofacial growth patterns in adult cleft lip and palate patients. Materials and Methods: The material for this study consisted of 17 adult male patients with cleft lip and palate. Cephalometric tracing and measurements were done by one investigator. The relationship between 17 cleft lip and palate patients and Korean norms were evaluated statistically. Results: There were statistically differences in Na. perpendicular to point A, SNA angle, effective maxillary length, maxillofacial differencial. Wit's appraisal and upper incisor to point A(p < 0.01). Pogonion to Na. perpendicular also statistically differed(p < 0.05). Other measurements didn't statistically differ. Conclusion: It was evident that in adult cleft lip and palate patients, maxilla was retruded and short. Careful cleft lip and palate repair and treatment are recommended for facilitating normal growth of maxilla.
In orthodontic field, it is very important to understand the normal growth. Such an understanding can be derived from observation of normal growth in various samples from childhood to adulthood, and this builds a foundation from which growth abnormality or variation can be defined. Thus, a broad data collection of normal children, as well as basic study reviewing such data become necessary. The relationship between the mean values of cephalometric measurements in Growth and Development was studied among the groups(boys and girls) of Korean chidren from the ages 6-years to 16-years. 220 boys 170 girls were chosen as subjects : Cephalometric X-ray were taken for 3 years and hard tissue analysis on McNamara and Ricketts Analysis which was divided into measurements of 5 parts(Cranial base, Cranial base and Maxilla & Mandible, Maxilla and Mandible, Mandible, Dental measurements). The relationship of craniofacial growth was studied. The following conclusions were obtained: 1. There were statistically significant differences in anterior cranial base between the two sexes of 14 and 15-year grouips. 2. In comparison of growth amounts among different age groups, statistically singnificant difference in Posterior facial height exhisted among $10\~11,\;12\~13\;and\;14\~15$ year-old interval groups. This pattern increased with aging. 3. Na perpendicular to A showed earlier growth peak in females(11-12years) than males(12-13years). When horizontal measurements of point A and Pogonion are compared, mandibular growth appeared to be greater. 4. Maximum growth peak of cranial base and mandible was earlier in females (11-12years) than males(13-14 years). 5. Upper central incisor flared out with aging, and there were increases in lower incisor to A-Pg, and lower incisor inclination There was significant difference between the two sexes in 10-year-old group.
This study was performed to evaluate the effects of number and alignment of implant fixture and various bar designs on the retention of denture and the stress distribution. Six kinds of photoelastic mandibular models and nine kinds of overdenture specimens were designed. A unilateral vertical load was gradually applied on the right first molar to calculate the maximal dislodgement load of each specimen. A unilateral vertical load of 17 Kgf was applied on the right first molar and a vertical load of 10 Kgf was applied on the interincisal edge region. The stress pattern which developed in each photoelastic model was analyzed by the reflection polariscope. The results obtained were as follows: 1. The maximal dislodgement load reversely increased with the distance from the loading point to the implant fixture, while it linearly increased with that from the most posterior implant fixture to the mesial clip. The maximal dislodgement load also increased with the use of a cantilever bar. 2. Under the posterior vertical load, the stress to the supporting tissue of the denture base increased with the distance from the loading point to the implant future. The stress concentration on the apical area of the implant future reversely increased with the distance from the loading point to the implant future. 3. In the overdentures supported by two implant fixtures under the posterior vertical load. the specimen implanted on lateral incisor areas with a cantilever bar exhibited more favorable stress distribution than that without a cantilever bar. The specimen implanted on the canine areas without a cantilever bar, however, exhibited more favorable stress distribution. 4. In the overdentures supported by three implant fixtures. the specimen implanted ell the midline and canine areas exhibited more favorable stress distribution than that implanted oil the midline and the first premolar areas. 5. In the overdentures supported by four implant fixtures. the specimen implanted with two adjacent implant fixtures exhibited more favorable stress distribution than that implanted at equal distance under the posterior vertical load. 6. Under the anterior vertical load, the overdentures supported by three implant fixtures exhibited stress concentration on the supporting structure of the middle implant future. In overdentures supported by two or four implant futures, no significant difference was noted in stress distribution between the types of bars. These results indicate that the greater the number of implant fixtures, the better the stress distribution is. A favorable stress distribution may be obtained in the overdentures supported by two or three implant fixtures, if the location and the design of the bar are appropriate.

The purpose of this investigation was to know correlation of mean values between centric occlusion and centric relation by the cephalogram in Angle's Class Ⅰ and Ⅲ malocclusion subjects. 22 adults with Angle's Class Ⅰ malocclusion (17 men and 5 women, 21 to 27 years of age) and 14 adults with Angle's Class Ⅲ malocclusion (10 men and 4 women, 21 to 27 years of age) were selected from the dental students in Yonsei University. Each subject was given two lateral cephalometric radiographies and cephalometric analysis was performed. All data from these analyses was recorded and statistically processed with CYBER computer system. 1. The results were obtained as follows: There was a strong positive correlation between centric occlusion and centric relation in all subjects with Angle's Class Ⅰ and Ⅲ malocclusion. 2. In Angle's Class Ⅰ malocclusion, measurements in lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). In Angle's Class Ⅲmalocclusion, measurements in facial axis angle, mandibular plane angle, convexity of A point, lower incisor protrusion, lower facial height revealed significant difference between centric occlusion and centric relation (P<0.05). 3. When the mandible was moved from centric occlusion to centric relation, the mean distance of mandibular movement was 1.27㎜ (0.2-2.8㎜) in Angle's Class Ⅰ malocclusion, 1.70㎜ (0.55-4.15㎜) in Angle's Class Ⅲ malocclusion, and 1.44㎜ (0.2-4.15㎜) in all subjects.
Purpose: The purpose of the present study was to evaluate the postoperative skeletal stability of two-jaw surgery (Le Fort I osteotomy and bilateral sagittal split ramus osteotomy) via surgery first orthodontic treatment (SFOT) in class III malocclusion. Methods: Thirty-two patients who had two-jaw surgery via SFOT were included in this study. Serial lateral cephalograms were obtained before (T0), immediately after (T1), and six months after (T2) surgery. Twelve variables were measured for horizontal and vertical skeletal stability as well as for dental change. All measurements were evaluated statistically by a paired t-test ($P$ <0.05). Results: The mean skeletal changes were $0.1{\pm}2.5$ mm at point A and $-12.0{\pm}7.4$ mm at the pogonion. The mean horizontal relapse was 11.6% at the pogonion, and the mean vertical surgical changes included an upward displacement of $2.1{\pm}7.1$ mm and a forward displacement of $1.4{\pm}4.6$ mm at the pogonion. Upper incisor inclination decreased after surgery and was maintained at T2, and lower incisors were proclined from T1 to T2 by postsurgical orthodontic treatment. Conclusion: Postoperative skeletal stability of two-jaw surgery via surgery first orthodontic treatment in class III malocclusion was clinically acceptable.
This study was undertaken to compare the craniofacial morphology of Class II, Division 1 malocclusion with that of normal occlusion in children, and to investigate the incidence of various Class II, Division 1 craniofacial skeletal patterns. The subjects consist of thirty seven boys and fifty three girls with Class II, Division 1 malocclusion, and forty six boys and eighty one girls 10-15 years with normal occlusion. Measurements were recorded, tabulated and analyzed on the lateral cephalograms by the degree of SNA, SNB and ANB. The following characteristics of the Class II, Division 1 skeletal pattern were observed. 1. The anteroposterior relationship of the maxilla to the cranium in the Class II, Division 1 was very similar to that of normal occlusion. 2, Mandible of the Class II, Division 1 malocclusion was in the posterior position in relation to the cranial anatomy when compared to normal. 3. The chin point as measured by SN Pog and NS Gn showed distal positioning in relation to normal occlusion. 4. SN to mandibular plane angle was large in Class II, Division 1 malocclusion. 5. Mandibular incisor inclination was not significantly different between Class II, Division 1 malocclusion and normal occlusion, but maxillary incisors inclined and positioned labially and consequently overjet was large in Class II, Division 1 malocclusion. 6. Class II, Division 1 malocclusion was divided into four types of craniofacial skeletal pattern. The most common Class II, Division 1 pattern was found to be type C in which SN-Mand. Pl. was above mean range of normal occlusion. The next frequent pattern was found to be type A in which maxilla and mandible were within normal range of protrusion while upper incisors were severly labially inclined.

Purpose: The aim this study was to compare the accuracy of orthodontists and dentomaxillofacial radiologists in identifying 17 commonly used cephalometric landmarks, and to determine the extent of variability associated with each of those landmarks. Materials and Methods: Twenty digital lateral cephalometric radiographs were evaluated by two groups of dental specialists, and 17 cephalometric landmarks were identified. The x and y coordinates of each landmark were recorded. The mean value for each landmark was considered the best estimate and used as the standard. Variation in measurements of the distance between landmarks and measurements of the angles associated with certain landmarks was also assessed by a subset of two observers, and intraobserver and interobserver agreement were evaluated. Results: Intraclass correlation coefficients were excellent for intraobserver agreement, but only good for interobserver agreement. The least reliable landmark for orthodontists was the gnathion (Gn) point (standard deviation [SD], 5.92 mm), while the orbitale (Or) was the least reliable landmark (SD, 4.41 mm) for dentomaxillofacial radiologists. Furthermore, the condylion (Co)-Gn plane was the least consistent (SD, 4.43 mm). Conclusion: We established that some landmarks were not as reproducible as others, both horizontally and vertically. The most consistently identified landmark in both groups was the lower incisor border, while the least reliable points were Co, Gn, Or, and the anterior nasal spine. Overall, a lower level of reproducibility in the identification of cephalometric landmarks was observed among orthodontists.

Objective: Sliding mechanics using orthodontic miniscrews is widely used to stabilize the anchorage during extraction space closure. However, previous studies have reported that both posterior segment displacement and anterior segment displacement are possible, depending on the mechanical properties of the archwire. The present study aimed to investigate the effect of archwire stiffness and friction change on the displacement pattern of the maxillary posterior segment during anterior segment retraction with orthodontic miniscrews in sliding mechanics. Methods: A three-dimensional finite element model was constructed. The retraction point was set at the archwire level between the lateral incisor and canine, and the orthodontic miniscrew was located at a height of 8 mm from the archwire between the second premolar and first molar. Archwire stiffness was simulated with rectangular stainless steel wires and a rigid body was used as a control. Various friction levels were set for the surface contact model. Displacement patterns for the posterior and anterior segments were compared between the conditions. Results: Both the anterior and posterior segments exhibited backward rotation, regardless of archwire stiffness or friction. Among the conditions tested in this study, the least undesirable rotation was found with low archwire stiffness and low friction. Conclusions: Posterior segment displacement may be unavoidable but reducing the stiffness and friction of the main archwire may minimize unwanted rotations during extraction space closure.
