The angulation and the position change of the planned implant after tomographic imaging (단층촬영상에서 계획된 임플랜트 매식 각도 및 위치에 대한 스텐트 핀의 상대적 각도 및 위치에 대한 평가)

  • Purpose: To measure the differences of the splint pin angulation and the position of the planned implant site after conventional tomographic analysis. Materials and Methods: The angulation and the location of the metal splint pin retained in acrylic stent were compared with the corrected angulation and the location of the implant fixture on the 331 tomographic images. Results: The stent pins were located buccal in 40%, lingual in 10% to the corrected implant site after analysis of the conventional tomographic image. The angle and the location of the maxillary splint pin were mainly directed buccal on incisor and canine regions. The angle and the location of the splint pins in premolar and molar regions needed less corrections in both maxilla and mandible. Conclusions: This study demonstrated that the use of tomographs was essential for successful dental implant planning.

  • This study was performed to establish the Cephalometric standards and growths change of Korean Teen aged group by McNamara's Analysis. The data from Cephalometric films of 161 male and 205 female subject aged from 10 to 18 years of Normal Korean were used for this study. The following results were obtained: 1. Means, standard deviation from measured values in each age group and sexual values were obtained. 2. Greater measurement stowed in male than female in Condylion to point A, Condylion to Gnathion, MaxilloMandible differential and ANS to mouton in linear measurement of 18 age group. 3. Condylion to point A, Condylion to gnathion, MaxilloMandible differential and ANS to Menton are increased as the age increased. 4. Facial axis angle and Mandible plane angle of 15 and 18 aged group showed less value than that of 10,11,12 age group. 5. There was no significant value changes of Lower incisor to A-Po, SNA angle and Lower pharynx by the increasing of age.

THE STUDY OF COMPARISON BEFORE AND AFTER ORTHODONTIC TREATMENT IN CLASS I BIALVEOLAR PROTRUSION CASES WITH FOUR BICUSPID EXTRACTION (제 1 소구치 발치를 동반한 제 I 급 치조성 양악 전돌 환자의 치료 전${\cdot}$후 비교에 관한 연구)

  • The purposes of present study were to evaluate changes in models and lateral cephalometric head films during orthodontic treatment and to compare the amount of incisal retraction and anterior movement of molars with the two approaches of the retraction method of canine(sectional canine retractions vs sliding canine retractions) and the anchorage management(head gears vs no head gears, transpalatal arches vs no transpalatal arches and lingual arches vs no lingual arches) and to evaluate changes during orthodontic treatment in models with relation to lateral cephalometric head films. 67 Korean women with Angle's Class I bialveolar protrusion were selected, whose initial chronologic age was above 16 yrs. Models and lateral cephalometric head films were taken before and after orthodontic treatment with four bicuspid extraction. the results were obtained as follows. 1. Significant decreases were observed in intermolar width, arch length and arch perimeter of maxilla and mandible but significant difference was not observed in intercanine width of maxilla and mandible during treatment period. 2. The linear change of the upper incisor to upper lip was 2.84:1 and the linear change of the lower incisor to lower lip was 1.45:1 3. There were no significant differences between the two groups(sectional canine retractions vs sliding canine retractions), the !we groups(transpalatal arches vs no transpalatal arches) and the two groups(lingual arches vs no lingual arches) in the amount of incisal retraction and anterior movement of molars. There were a greater amount of maxillary incisal retraction and a lesser amount of anterior movement of maxillary molars with the use of head gears than no use of head gears. 4. Changes during orthodontic treatment in models with relation to lateral cephalometric head films were obtained as follows : 1) Maxilla Central incisors were moved 3.79mm backward, canines were moved 0.22mm laterally and 3.70mm backward, and molars were moved 0.535mm medially and 2.29mm forward. 2) Mandible Central incisors were moved 3.04mm backward, canines were moved 0.145mm laterally and 3.92mm backward, and molars were moved 0.755mm medially and 1.77mm forward.

  • Patients who want to reduce their lip protrusion usually estimate the severity of the lip protrusion on the frontal aspect. Most orthodontists have a perplexed experience of a reduced thin line of vermilion border on the frontal aspect as incisors we retracted, even though the lip protrusion is thought to be reduced favorably on the sagittal aspect. Some patients also look older after orthodontic treatment because of severe lip thinning. This unaesthetic reduction of vermilion border urges us to study the vertical lip change during orthodontic procedure. The purpose of this study was to evaluate the vertical lip and perioral soft tissue changes in respect to incisor retraction in an effort to analyze which factors might be responsible for their vertical changes, using the multiple regression analysis. The results were as follows. 1. Upper and lower lip philtrum length(SnLs, LiB') were increased after retraction of anterior teeth, where as upper and lower vermilion height(LsSuls, StmiLi), and vermilion length(LsLi) were decreased. 2. Upper and lower lip length(SnStms, StmiB'), and soft tissue lower anterior facial height(SnMe') did not show any significant difference after treatment. 3. The increase of the upper lip philtrum length was mainly influenced by the extrusion of upper anterior teeth(${\Delta}U1V$), and the increase of the lower lip philtrum length was mainly influenced by the initial overjet before treatment. 4. The decrease of the upper and lower lip vermilion height was mainly influenced by the decrease of upper lip thickness.

Volume difference in upper central incisor preparation according to the changes of restorative design and marginal location (상악 중절치 삭제 시 수복 디자인과 변연부 위치에 따른 부피 변화)

  • Purpose: The aim of this study was to evaluate the volumetric change of teeth after preparation for various designs and margin locations through Micro CT analysis (Skyscan 1076: SKYSCAN, Konitch, Belgium). Materials and methods: The 36 artificial teeth were used to determine reduction volume of upper central incisor. According to the restorative design these 36 teeth were divided into 4 groups and according to the marginal location each group was divided into 3 subgroups. The volume of unprepared teeth was obtained by using Micro CT and the volume of prepared teeth was obtained in the same method. The CT scanned images before and after preparation were superimposed. Results: The volume difference was significantly increased as follows: traditional laminate veneer < full laminate veneer < all ceramic crown < metal ceramic crown. One-way ANOVA and Tukey multiple comparison analyses were used to analyze the data in this study. In each group the volume difference was significantly increased as follows: 1 mm above CEJ < CEJ < 1 mm below CEJ (P<.05). The % volume difference of all ceramic crown and metal ceramic crown was 31 - 48% and that of laminate veneer was 14 - 30%. The volume difference of the traditional laminate veneer was 1/3 of that of metal ceramic crown. The full laminate (1 mm below CEJ) and all ceramic crown (1 mm above CEJ) showed a similar volume difference. Metal ceramic crown showed 13.7% more volume difference than all ceramic crown. Conclusion: There exists the difference in volumetric change according to designs of restoration and margin locations of preparation.

A CLINICAL STUDY ON THE CHANGE OF TMJ SYMPTOMS FOLLOWING IVRO IN THE MANDIBULAR PROGNATHISM (IVRO를 이용한 하악골 전돌증 환자의 수술전후 악관절 증상 변화에 대한 임상적 평가)

  • Preoperative and postoperative TMJ symptoms were observed in mandibular prognathism of 30 patients operated on by Le Fort I osteotomy and intraoral vertical ramus osteotomy. The alterations of TMJ symptoms were investigated and the relationship between changes of TMJ symptoms and some cephalometric values including occlusal plane angle, mandibular plane angle, posteior ramal height and a degree of deviation of mandibular incisor midline to facial midline were observed. The results are as follows. The incidence of patient with TMJ symptoms before orthognathic surgery was 40% and after orthognathic surgery was 20%. The most frequetly encountered symptoms in orthognathic TMJ dysfunction patients were TMJ pain and/or clicking. After surgery 66% of the preoperative symptomatic patients reported improvement TMJ symptoms. On the orther hand 2 patient (6%) of the preoperative asymptomatic patients developed TMJ symptoms after surgery. Preoperatively, 60% of the facial asymmetric patients with mandibular prognathism had TMJ symptomas. The more severe facial asymmetry was, the higher incidence of TMJ symptoms was. The alteration of occlusal plane angle and mandibular plane angle seems to be one of the contributing factors which make to change TMJ symptoms in orthognathic patients. But its amount seems to be low significance. Increase or decrease of posterior ramal height have influence on the change of TMJ symptoms.

Correlation analysis of gingival recession after orthodontic treatment in the anterior region: an evaluation of soft and hard tissues

  • Purpose: The aim of this study was to investigate and identify the main causes of periodontal tissue change associated with labial gingival recession by examining the anterior region of patients who underwent orthodontic treatment. Methods: In total, 45 patients who had undergone orthodontic treatment from January 2010 to December 2015 were included. Before and after the orthodontic treatment, sectioned images from 3-dimensional digital model scanning and cone-beam computed tomography images in the same region were superimposed to measure periodontal parameters. The initial labial gingival thickness (IGT) and the initial labial alveolar bone thickness (IBT) were measured at 4 mm below the cementoenamel junction (CEJ), and the change of the labial gingival margin was defined as the change of the distance from the CEJ to the gingival margin. Additionally, the jaw, tooth position, tooth inclination, tooth rotation, and history of orthognathic surgery were investigated to determine the various factors that could have affected anterior periodontal tissue changes. Results: The mean IGT and IBT were 0.77±0.29 mm and 0.77±0.32 mm, respectively. The mean gingival recession was 0.14±0.57 mm. Tooth inclination had a significant association with gingival recession, and as tooth inclination increased labially, gingival recession increased by approximately 0.2 mm per 1°. Conclusions: In conclusion, the IGT, IBT, tooth position, tooth rotation, and history of orthognathic surgery did not affect labial gingival recession. However, tooth inclination showed a significant association with labial gingival recession of the anterior teeth after orthodontic treatment.

Changes of lip morphology following mandibular setback surgery using 3D cone-beam computed tomography images

  • Background: The aims of this study are to evaluate the lip morphology and change of lip commissure after mandibular setback surgery (MSS) for class III patients and analyze association between the amount of mandibular setback and change of lip morphology. Methods: The samples consisted of 14 class III patients treated with MSS using bilateral sagittal split ramus osteotomy. Lateral cephalogram and cone-beam CT were taken before and about 6 months after MSS. Changes in landmarks and variables were measured with 3D software program $Ondemand^{TM}$. Paired and independent t tests were performed for statistical analysis. Results: Landmarks in the mouth corner (cheilion, Ch) moved backward and downward (p < .005, p < .01). However, cheilion width was not statistically significantly changed. Landmark in labrale superius (Ls) was not altered significantly. Upper lip prominence angle (ChRt-Ls-$ChLt^{\circ}$) became acute. Landmarks in stomion (Stm), labrale inferius (Li) moved backward (p < .005, p < .001). Lower lip prominence angle (ChRt-Li-$ChLt^{\circ}$) became obtuse (p < .001). Height of the upper and lower lips was not altered significantly. Length of the upper lip vermilion was increased (p =< 0.01), and length of the lower lip vermilion was decreased (p < .05). Lip area on frontal view was not statistically significantly changed, but the upper lip area on lateral view was increased and change of the lower lip area decreased (p > .05, p < .005). On lateral view, upper lip prominent point (UP) moved downward and stomion moved backward and upward and the angle of Ls-UP-Stm ($^{\circ}$) was decreased. Lower lip prominent point (LP) moved backward and downward, and the angle of Stm-LP-Li ($^{\circ}$) was increased. Li moved backward. Finally, landmarks in the lower incisor tip (L1) moved backward and upward, but stomion moved downward. After surgery, lower incisor tip (L1) was positioned more superiorly than stomion (p < .05). There were significant associations between horizontal soft tissue and corresponding hard tissue. The posterior movement of L1 was related to statistically significantly about backward and downward movement of cheilion. Conclusions: The lip morphology of patients with dento-skeletal class III malocclusion shows a significant improvement after orthognathic surgery. Three-dimensional lip morphology changes in class III patients after MSS exhibited that cheilion moved backward and downward, upper lip projection angle became acute, lower lip projection angle became obtuse, change of upper lip area on lateral view was increased, change of lower lip area decreased, and morphology of lower lip was protruding. L1 was concerned with the lip tissue change in statistically significant way.

THE STATISTICAL STUDY OF MAXILLARY MEDIAN DIASTEMA (상악(上顎) 중절치간(中切齒間) 정중리개(正中離開)에 관(關)한 통계학적(統計學的) 연구(硏究))

  • The author surveyed 2,082 schoolchildren (Male: 1,078, Female: 1,004) from 7 to 12 years old in "N" primary school in kwang Ju City, and studied on 304 schoolchildren (Male: 176, Female: 128) having maxillary median diastema. The results were as follows: 1. The prevalence of maxillary median diastema was 14.6%, and it showed the greater rate at the boys than at the girls and the highest rate at 8 years old in the both sexes. 2. The maxillary median diastema showed decreasing tendency with ageing. 3. The frenum distance in children without local factors showed no change and the divergent degree of maxillary median diastema showed decreasing tendency after the eruption of maxillary lateral incisors. 4. The rate of maxillary median diastema associated with local factors was 37.8% and it showed increasing tendency with ageing. 5. The prevalence of median diastema according to etiology was as follows; rotated tooth; 12.2%, supernumerary teeth; 10.9%, abnormal frenum; 9.5%, peg lateralis; 7.2%, open bite; 2.6%, abnormal pattern of interseptal alveolar bone; 2.6%, missing lateral incisor; 0.7%, tooth size discrepancy; 0.3%.

Diagnostic imaging of nasal malignant melanoma in a dog (개 비강에서 발생한 악성 흑색종의 영상 진단 증례)

  • A neutered male, 8-year-old, Pekingese, weighing 4.3 kg with a history of anorexia, sneezing, nasal discharge, and epistaxis for one month was referred. Soft tissue swelling around the nasal bone and small defects of the hard palate with a tiny round dark red mass were found on physical examination. The laboratory tests represented mild leukocytosis. On skull radiographs, soft tissue swelling and osteolytic change of the incisor bone, nasal bone, and maxilla were found. On computed tomography scan images, there was soft tissue attenuating opacity with calcified spots in the bilateral nasal cavities and frontal sinuses. Loss of nasal turbinate pattern and nasal septum was found. And destruction of the insicor bone, nasal bone, maxilla, hard palate, perpendicular palatine bone, and cribriform plate were identified. Nasal malignant melanoma was confirmed by nasal biopsy.
