PURPOSE. To investigate the influence of crown material (lithium-disilicate, 3Y-TZP zirconia) and abutment type (rigid implant, resin tooth with artificial periodontium) on wear performance of their antagonist teeth and adjacent teeth. MATERIALS AND METHODS. A mandibular left first molar (#36) with adjacent human teeth (mandibular left second premolar: #35, mandibular left second molar: #37) and antagonistic human teeth (maxillary left second premolar: #25, maxillary left first molar: #26, maxillary left second molar: #27) was prepared simulating a section of the jaw. Samples were made with extracted human molars (Reference), crowned implants (Implant), or crowned resin tooth analogues (Tooth). Crowns (tooth #36; n = 16/material) were milled from lithium-disilicate (Li, IPS e.max CAD) or 3Y-TZP zirconia (Zr, IPS e.max ZirCAD, both Ivoclar Vivadent). Thermal cycling and mechanical loading (TCML) in the chewing simulator were applied simulating 15 years of clinical service. Wear traces were analyzed (frequency [n], depth [㎛]) and evaluated using scanning electron pictures. Wear results were compared by one-way-ANOVA and post-hoc-Bonferroni (α = 0.05). RESULTS. After TCML, no visible wear traces were found on Zr. Li showed more wear traces (n = 30-31) than the reference (n = 21). Antagonistic teeth #26 showed more wear traces in contact to both ceramics (n = 27-29) than to the reference (n = 21). Strong wear traces (> 350 ㎛) on antagonists and their adjacent teeth were found only in crowned groups. Abutment type influenced number and depth of wear facets on the antagonistic and adjacent teeth. CONCLUSION. The clinically relevant model with human antagonistic and adjacent teeth allowed for a limited comparison of the wear situation. The total number of wear traces and strong wear on crowns, antagonistic and adjacent teeth were influenced by crown material.
A titanium based screw shaped dental implant was first introduced by Branemark and a treatment protocol where the restoration of edentulous area by connecting abutment after the osseointegration of the titanium surface of the implant and surrounding bone structure has been proposed. Although this protocol is widely accepted as a standard up to date, the healing duration of 3-6 months as well as the need for provisional prostheses during this period present as a major drawback. Immediate loading has been accomplished through the advent of various implant designs, enforced surface treatments, diverse forms of abutment, and delicate surgical techniques together with the increase in demand from the patients. The success rate of the immediate loading technique has been first reported as 85.7% by Dr. Schnitman in 1990 which recently has been reported up to 100% in the case of immediate loading in single tooth by Dr. Kan. To ameliorate the success rate of immediate loading technique, selection of patients presenting a sound bone quality and quantity, acquiring primary stability through delicate surgical techniques and fabrication of prostheses which accounts for biological stabilities should all be taken into consideration. This presentation introduces the understanding of biological stability of immediate loading, various methods for measurement of stability and clinical cases regarding immediate loading technique.
Traditional removable partial denture is a successful treatment for partial edentulous patients. When the abutment is formed with unilateral minority teeth, satisfaction with the use of removable partial denture may be lowered due to patient's discomfort and damage of the abutment. Recently, it has been reported that the unilateral posterior extension partial denture is used as a bilateral posterior extension partial denture using implant fixed prostheses as abutments. In this case, by using implant surveyed crown prostheses, bilateral posterior extension partial denture is fabricated in site that is predicted to have a poor prognosis. This resulted in improved support, maintenance, and stabilization of the removable partial denture, which were economically beneficial to patient and satisfied with use.
PURPOSE. Zirconia has exceptional biocompatibility and good mechanical properties in clinical situations. However, finite element analysis (FEA) studies on the biomechanical stability of two-piece zirconia implant systems are limited. Therefore, the aim of this study was to compare the biomechanical properties of the two-piece zirconia and titanium implants using FEA. MATERIALS AND METHODS. Two groups of finite element (FE) models, the zirconia (Zircon) and titanium (Titan) models, were generated for the exam. Oblique (175 N) and vertical (175 N) loads were applied to the FE model generated for FEA simulation, and the stress levels and distributions were investigated. RESULTS. In oblique loading, von Mises stress values were the highest in the abutment of the Zircon model. The von Mises stress values of the Titan model for the abutment screw and implant fixture were slightly higher than those of the Zircon model. Minimum principal stress in the cortical bone was higher in the Titan model than Zircon model under oblique and vertical loading. Under both vertical and oblique loads, stress concentrations in the implant components and bone occurred in the same area. Because the material itself has high stiffness and elastic modulus, the Zircon model exhibited a higher von Mises stress value in the abutments than the Titan model, but at a level lower than the fracture strength of the material. CONCLUSION. Owing to the good esthetics and stress controllability of the Zircon model, it can be considered for clinical use.
PURPOSE. The aim of this study was to assess the effect of hemispherical dimple structures on the retention of cobalt-chromium (Co-Cr) crowns cemented to titanium abutments, with different heights and numbers of dimples on the axial walls. MATERIALS AND METHODS. 3.0-mm and 6.0-mm abutments (N = 180) and Co-Cr crowns were prepared. The experimental groups were divided into two and four dimple groups. The crowns were cemented by TempBond and PANAVIA F 2.0 cements. The retention forces were measured after thermal treatments. A two-way Analysis of Variance (ANOVA) and post-hoc Tukey HSD test were conducted to analyze change in retention forces by use of dimples between groups, as well as t test for the effect of abutment height change (α = .05). RESULTS. Results of the two-way ANOVA showed a statistically significant difference in retention force due to the use of dimples, regardless of the types of cements used (P < .001). A significantly higher mean retention forces were observed in the groups with dimples than in the control group, using the post hoc Tukey HSD test (P < .001). Results of t test displayed a statistically significant increase in the retention force with 6.0-mm abutments compared with 3.0-mm abutments (P < .001). The groups without dimples revealed adhesive failure of cements, while the groups with dimples showed mixed failure of cements. CONCLUSION. Use of hemispherical dimples was effective for increasing retention forces of cemented crowns.
PURPOSE. To evaluate the effects of different abutment geometries in combination with varying luting agents and the effectiveness of different cleaning methods (prior to re-cementation) regarding the retentiveness of zirconia copings on implants. MATERIALS AND METHODS. Implants were embedded in resin blocks. Three groups of titanium abutments (pre-fabricated, height: 7.5 mm, taper: $5.7^{\circ}$; customized-long, height: 6.79 mm, taper: $4.8^{\circ}$; customized-short, height: 4.31 mm, taper: $4.8^{\circ}$) were used for luting of CAD/CAM-fabricated zirconia copings with a semi-permanent (Telio CS) and a provisional cement (TempBond NE). Retention forces were evaluated using a universal testing machine. Furthermore, the influence of cleaning methods (manually, manually in combination with ultrasonic bath or sandblasting) prior to re-cementation with a provisional cement (TempBond NE) was investigated with the pre-fabricated titanium abutments (height: 7.5 mm, taper: $5.7^{\circ}$) and SEM-analysis of inner surfaces of the copings was performed. Significant differences were determined via two-way ANOVA. RESULTS. Significant interactions between abutment geometry and luting agent were observed. TempBond NE showed the highest level of retentiveness on customized-long abutments, but was negatively affected by other abutment geometries. In contrast, luting with Telio CS demonstrated consistent results irrespective of the varying abutment geometries. Manual cleaning in combination with an ultrasonic bath was the only cleaning method tested prior to re-cementation that revealed retentiveness levels not inferior to primary cementation. CONCLUSION. No superiority for one of the two cements could be demonstrated because their influences on retentive strength are also depending on abutment geometry. Only manual cleaning in combination with an ultrasonic bath offers retentiveness levels after re-cementation comparable to those of primary luting.

Purpose : This study was designed to evaluate the influence of implant abutment materials on detorque value and screw joint stability before and after dynamic fatigue test. Materials & Methods : The external hexagonal fixture and three different groups of abutment (titanium abutments, zirconia abutments, and UCLA abutments) were used. The detorque value before loading and after loading (cyclic loading up to $10^5$ cycles) of the abutment screw were measured. Result : 1. There was no significant difference in detorque value before loading among the each group. 2. There was no significant difference in detorque value after loading among the each group. 3. Detorque values before and after cyclic loading in each group were not significantly different. 4. There was no significant difference in loss percentage of removal torque before loading among the each group. 5. There was no significant difference in loss percentage of removal torque after loading among the each group. 6. There was no significant difference in loss percentage of removal torque according to loading among the each group. Conclusion : Short term screw loosening of three types of abutment was not significantly different. When bite force was applied, there was no significant difference in screw loosening between before loading and after loading.

Statement of problem: Implant inclination and cantilever loading increse loads distributed to implants, potentially causing biomechanical complications. Controversy exists regarding the effect of the intentionally distal-inclined implant for the reduction of the cantilever length. Purpose: This study investigated the stress distribution at the bone/implant interface and prostheses with 3D finite element stress analysis by using four different cantilever lengths and implant inclinations in a mandibular implant-supported bar overdenture. Material and methods: Four 3-D finite element models were created in which 4 implants were placed in the interforaminal area and had four different cantilver lengths(10, 6.9, 4 and 1.5mm) and distal implant inclinations$(0^{\circ},\;15^{\circ},\;30^{\circ}\;and\;45^{\circ})$ respectively. Vortical forces of 120N and oblique forces of 45N were applied to the molar area. Stress distribution in the bone around the implant was analysed under different distal implant inclinations. Results: Analysis of the von Mises stresses for the bone/implant interfaces and prostheses revealed that the maximum stresses occurred at the most distal bone/implant interface and the joint of bar and abutment, located on the loaded side and significantly incresed with the implant inclinations, especially over $45^{\circ}$. Conclusion: Within the limitations of this study, it was suggested that too much distal inclination over 45 degrees can put the implant at risk of overload and within the dimension of the constant sum of a anterior-posterior spread and cantilever length, a distal implant inclination compared to cantilever length had the much larger effect on the stress distribution at the bone/implant interface.
Purpose: This study is aimed to assess changes of stress distribution dependent on different connection lengths and placement of the fixture top relative to the ridge crest. Materials and methods: The internal-conical connection implant which has a hexagonal anti-rotation index was used for FEM analysis on stress distribution in accordance with connection length of fixture-abutment. Different connection lengths of 2.5 mm, 3.5 mm, and 4.5 mm were designed respectively with the top of the fixture flush with residual ridge crest level, or 2 mm above. Therefore, a total of 6 models were made for the FEM analysis. The load was 170 N and 30-degree tilted. Results: In all cases, the maximum von Mises stress was located adjacent to the top portion of the fixture and ridge crest in the bone. The longer the connection length was, the lower the maximum von Mises stress was in the fixture, abutment, screw and bone. The reduction rate of the maximum von Mises stress depending on increased connection length was greater in the case of the fixture top at 2 mm above the ridge crest versus flush with the ridge crest. Conclusion: It was found that the longer the connection length, the lower the maximum von Mises stress appears. Furthermore, it will help prevent mechanical or biological complications of implants.
