연구목적: 임플란트 보철물의 유지형태 중 나사 및 시멘트 혼합 유지형의 경우 나사 풀림력 등에 영향을 주는 임플란트 구성 성분의 응력에 관한 연구가 부족하였다. 임플란트 상부 보철물의 유지형태, 즉, 시멘트 유지와 나사 유지, 그리고 이 두 가지 유지형태가 서로 연결된 혼합형의 임플란트 보철물의 응력분산의 특징들을 3차원 유한요소분석법을 이용하여 비교하고자 하였다. 연구재료 및 방법: 하악골에서 제1소구치 부위와 제1대구치 부위에 2개의 임플란트 (SS II, Osstem Co. Ltd., Seoul, Korea)를 식립한 가상의 3본 계속가공의치를 모델화하였다. 지대주 종류와 그 위치에 따라, 4가지 모형 군으로 나누어 실험하였다. 모형 1은제1대구치와 제1소구치 각각의 고정체에 모두 동일한 시멘트 유지형 지대주인 Comocta abutment (Osstem Co. Ltd) 를 장착하여 3본 계속가공의치를 합착시킨 경우이고, 모형 2는 제1대구치와 제1소구치 각각의 고정체에 모두 나사 유지형 지대주인 Octa abutment (Osstem Co. Ltd) 를 장착하여 3본 계속가공의치를 나사로 고정시킨 경우이며, 모형 3은 제1대구치의 고정체에는 시멘트 유지형 지대주인 Comocta abutment를 장착하고, 제1소구치에는 나사 유지형 지대주인 Octa abutment를 장착한 후 3본 계속가공의치를 각각 시멘트 합착 및 나사로 고정시킨 경우이다. 그리고 모형 4는 모형 3에서 각각 제1대구치 및 제1소구치의 지대주를 맞바꾼 후 3본 계속가공의치를 나사 및 시멘트로 고정시킨 경우로 나누었다. 평균저작압인 하중을 대구치 565 N과 소구치 288 N의 힘으로 설정하고 수직방향으로 중심와와 협측 교두정에, 그리고 $30^{\circ}$ 경사 하중을 협측 교두정 부위에 준 다음 골, 고정체, 지대주, 그리고 지대주 나사 등에 나타나는 von-Mises stress 양상을 평가하였다. 결과: 네 가지 모형 중 나사 유지형 지대주인 Octa abutment를 제1대구치와 제1소구치 부위에 사용한 모형 2가 전반적으로 가장 낮은 안정적인 응력 분포를 보였다. 네 가지 모형 모두 피질골 및 고정체에 미치는 응력 크기 및 분포는 거의 유사하며, 치조골에 작용하는 응력은 하중의 종류와 상관없이 주로 피질골에 집중되었다. 지대주, 지대주 나사, 그리고 보철물 나사 등에 미치는 응력 크기나 분포는 모형에 관계없이 나사 유지형인 경우가 시멘트 유지형인 경우에 비해 낮은 안정적인 값을 보였다. 제1대구치와 제1소구치의 상부 구조물의 차이에 의한 교호작용 (reciprocal action)은 상대적으로 약하였다. 모든 부위에서 중앙 수 직하중, 교두정 수직하중, 그리고 교두정 경사하중의 순으로 응력값이 증가하였다. 결론: 본 유한 요소실험의 한계내에서 나사 및 시멘트 혼합 유지형의 임플란트 보철물은 시멘트 유지형만 사용하는 경우와 비교하여 주위에 더 큰 응력을 나타내지는 않았다. 이상적인 passive fit의 가정하에서 나사 유지형의 임플란트 보철물이 본체와 주위에 가장 작은 응력을 나타내었다.
Background: Poly-L-lactide materials combined with hydroxyapatite (u-HA /PLLA) have been developed to overcome the drawbacks of absorbable materials, such as radiolucency and comparably less implant strength. This study was designed to evaluate the usefulness of u-HA/PLLA material in the repair of orbital medial wall defects. Methods: This study included 10 patients with pure medial wall blow-out fractures. The plain radiographs were taken preoperatively, immediately after, and 2 months after surgery. The computed tomography scans were performed preoperatively and 2 months after surgery. Patients were evaluated for ease of manipulation, implant immobility, rigidity and complications with radiologic studies. Results: None of the patients had postoperative complications, such as infection or enophthalmos. The u-HA/PLLA implants had adequate rigidity, durability, and stable position on follow-up radiographic studies. On average, implants were thawed 3.4 times and required 14 minutes of handling time. Conclusion: The u-HA/PLLA implants are safe and reliable for reconstruction of orbital medial wall in terms of rigidity, immobility, radiopacity, and cost-effectiveness. These thin yet rigid implants can be useful where wide periosteal dissection is difficult due to defect location or size. Since the u-HA/PLLA material is difficult to manipulate, these implants are not suitable for use in complex 3-dimensional defects.
Pathologic tooth migration (PTM) caused by periodontal disease is a relatively common disease with a prevalance rate of 30~55%. It is one of the reasons for periodontal treatment of patients. PTM occurs when various factors related to the position of the tooth are altered and clinical features of PTM include facial flaring, diastema, proximal tilting, rotation, and extrusion of teeth. The purpose of this study was to assess the treatment of teeth of hopeless prognosis through the practice of modified intentional replantation on pathologic migrated tooth. This study presents cases using modified intentional replantation for the treatment of pathologic tooth migration caused by severe periodontitis. Modified intentional replantation is a technique that prepares extraction socket by using an implant drill. Based on 3 cases performed with modified intentional replantation, an improvement of clinical periodontal indicators such as probing pocket depth, bleeding on probing, tooth mobility, and pathologic teeth migration was observed after surgery. Also, the patients were satisfied with the functional and esthetic improvement.
This study was performed to demonstrate the method of image reformation for dental implants, using a personal computer with inexpensive softwares and to compare the images reformatted using the above method with those using Dentascan software. CT axial slices of 4 mandibles of 4 volunteers from GE Highspeed Advantage(GE Medical systems, U.S.A.) were used. personal computer used for image reformation was PowerWave 604/120 (Power computing Co, U.S.A. ) and softwares used were Osiris (Univ. Hospital of Geneva, Switzerland) and ImportACLESS Vl.1 (Designed Access Co., U.S.A.) for importing CT images and NIH image 1.58 (NIH, U.S.A.) for image processing. Seven image were selected among the serial reconstructed cross-sectional images produced by Dentascan. Seven resliced cross-sectional images at the same position were obtained at the personal computer. Regression analysis of the measurements of PC group was done against those of DS group. Measurements of the bone height and width at the reformer cross-sectional images using Mac-compatible computer was highly correlated with those using workstation with Dentascan software(height : r2= 0.999, p<0.001, width : r2= 0.993, p <0.001). So, it is considered that we can use a personal computer with inexpensive software for the dental implant planning, instead of the expensive software and workstation.
Purpose: The capsular contracture has been the most common complication of augmentation with breast implant, a side effect quite difficult to treat. The latest trends in the correction of capsular contracture include total capsulectomy or conversion of implant pocket. In this study, in an attempt to correct capsular contracture, the authors performed reoperation which involved capsulectomy through peri-areolar approach and dual-plane conversion. The authors hereby report the clinical results of such correction of capsular contracture and examine the efficacy. Methods: The authors selected 46 patients who were admitted to the clinic from January 2004 to January 2007 (37 months), and performed dual-plane conversion through solely peri-areolar approach. Two types of operation were done: dual-plane conversion from subglandular plane or from submuscular plane. Results: The average follow-up time after conversion to the dual-plane position was 10 months. During the follow-up period, 83.1% of patients recovered from capsular contracture and were Baker class I, and in 10.9% the condition had relapsed into Baker class II or III contracture. Conclusion: This study has proven the effectiveness of the dual-plane conversion operation for correcting established capsular contracture after previous augmentation mammaplasty. In this study, all cases of dual-plane conversion operation was performed through peri-areolar approach, which can prevent the occurrence of visible scar on inframammary fold.
The objective of this paper is to propose a new technology of the orthopaedic surgery using the combination of reverse engineering (RE) based on CT data and rapid prototyping (RP). The proposed technology utilizes symmetrical characteristics of the human body and capability of the combination of RE and RP, which rapidly manufactures three-dimensional parts from CT data. The original .stl data of injured extents are generated from the mirror transformation of .stl file fur uninjured extents. The physical shape before injuring is manufactured from RP using the original .stl data. Subsequently, pre-operative planning, such as a selection of proper implants, preforming of the implant, a decision of fixation locations and an insert position for the implant, an estimation of the invasive size, and pre-education of operators are performed using the physical shape. In order to examine the applicability and the efficiency of the proposed surgical technology, various case studies, such as a distal tibia commented fracture, a proximal tibia plateau fracture and an iliac wing fracture of pelvis, are carried out. From the results of case studies, it has been shown that the proposed technology is an effective surgical tool of the orthopaedic surgery reducing the operational time, the operational cost, the radiation exposure of the patient and operators, and morbidity. In addition, the proposed technology could improve the accuracy of operation and the speed of rehabilitation.
In the present study, anatomical assessment of zygomaticofacial foramina (ZFFs) and zygomatic canals communicating with ZFFs were performed using cadaver micro-computed tomography images. It was suggested that all ZFFs were located above the jugale (Ju)-zygomaxillare (Zm) line, which is the reference line connecting the Ju and Zm, and most were located in the zygomatic body area (ZBA). The anteroposterior position of the ZFF in the ZBA was within a middle to posterior region and was most often located slightly posteriorly in males and closer to the middle of the region in females. The mean distance from the Ju-Zm line to the ZFF in the ZBA was 12.36 mm (standard deviation [SD] 1.52 mm) in males and 11.48 mm (SD 1.61 mm) in females. In zygomatic canals communicating with ZFFs, most zygomatic canals were type I canals, communicating from the zygomaticoorbital foramen and harboring the zygomaticofacial nerve, and the others were type II canals, communicating from the zygomaticotemporal foramen and located near the posterior margin of the frontal process. These results provide useful anatomical information for preventing nerve injury during surgical procedures for zygomatic implant treatment.
Purpose: Stress distribution and mandible distortion during lateral movements are known to be closely linked to bruxism, dental implant placement, and temporomandibular joint disorder. The present study was performed to determine stress distribution and distortion patterns of the mandible during lateral movements in Class I, II, and III relationships. Methods: Five Korean volunteers (one normal, two Class II, and two Class III occlusion cases) were selected. Finite element (FE) modeling was performed using information from cone-beam computed tomographic (CBCT) scans of the subjects' skulls, scanned images of dental casts, and incisor movement captured by an optical motion-capture system. Results: In the Class I and II cases, maximum stress load occurred at the condyle of the balancing side, but, in the Class III cases, the maximum stress was loaded on the condyle of the working side. Maximum distortion was observed on the menton at the midline in every case, regardless of loading force. The distortion was greatest in Class III cases and smallest in Class II cases. Conclusions: The stress distribution along and accompanying distortion of a mandible seems to be affected by the anteroposterior position of the mandible. Additionally, 3-D modeling of the craniofacial skeleton using CBCT and an optical laser scanner and reproduction of mandibular movement by way of the optical motion-capture technique used in this study are reliable techniques for investigating the masticatory system.
적절하지 않은 위치에 식립된 임플란트의 보철물은 심미적, 기능적 문제를 나타낼 수 있으나 이를 해결하기는 어려운 때가 많다. 이러한 경우에 재식립 혹은 연조직 및 경조직 이식을 포함하는 외과적 방법을 고려해 볼 수 있지만 결과 예측이 힘들며 긴 치유 기간 및 술 후 불편감과 추가적인 수술 비용이 동반된다. 반면, 보철적 방법으로는 치은 색상의 세라믹이 포함된 고정성 보철물을 제작하거나 가철성 보철물인 gingival mask 제작 등을 생각해볼 수있다. 본 증례에서는 80세의 남환이 과도하게 구개측으로 식립된 임플란트로 인한 상순과 보철물 사이의 음식물 저류에 대한 불편감 및 심미적인 부분에 대한 불만족을 주소로 내원하였고, gingival mask를 제작하여 단 기간 내에 만족스러운 결과를 얻었기에 이를 보고하고자 한다.

Purpose: The aim of this retrospective study was to determine the association between dental implants in the posterior region and traumatic occlusion in the adjacent premolars, using data collected during from 2002 to 2015. Methods: Traumatic occlusion in the adjacent premolars was assessed by examining clinical parameters (bleeding on probing, probing pocket depth, fremitus, and tooth mobility) and radiographic parameters (loss of supporting bone and widening of the periodontal ligament space) over a mean follow-up of 5 years. Clinical factors (gender, age, implant type, maxillary or mandibular position, opposing teeth, and duration of functional loading) were evaluated statistically in order to characterize the relationship between implants in the posterior region and traumatic occlusion in the adjacent premolars. Results: The study inclusion criteria were met by 283 patients, who had received 347 implants in the posterior region. The incidence of traumatic occlusion in the adjacent premolars was significantly higher for splinted implants (P=0.004), implants in the maxillary region (P<0.001), and when implants were present in the opposing teeth (P<0.001). The other clinical factors of gender, age, and duration of functional loading were not significantly associated with traumatic occlusion. Conclusions: This study found that the risk of traumatic occlusion in the adjacent premolars increased when splinted implants were placed in the maxillary molar region and when the teeth opposing an implant also contained implants.
