House dust mite-specific immunoglobulin E and longitudinal exhaled nitric oxide measurements in children with atopic asthma

  • Purpose: House dust mite (HDM) has been suggested to be the most important aeroallergen responsible for atopic asthma in Korea. We aimed to investigate that specific IgE antibodies to HDM and other common indoor aeroallergens contribute differently to total serum IgE and show different relationships with longitudinal fractional exhaled nitric oxide (FeNO) measurements in Korean atopic asthmatic patients. Methods: A total of 193 children aged 8 to 16 years with intermittent or mild persistent atopic asthma were recruited. Sera were assayed for total IgE and specific IgE antibodies to HDM and other common indoor allergens. FeNO was serially measured 10 times or more over 2 years when subjects were not receiving controller medications. Results: In 152 children who completed the study, IgE antibodies to specific HDM were more prevalent than those to other common indoor aeroallergens. In addition, IgE antibody titers to HDM were the strongest contributor to total IgE increases. Furthermore, only HDM-specific IgE antibody titer significantly correlated with maximum FeNO (r=0.21, P=0.029) and the rate of FeNO higher than 21 parts per billion (ppb) (r=0.30, P=0.002). Eight patients (5%) were found to have maximum FeNO of 21 ppb or less, suggesting the presence of a low FeNO phenotype among atopic asthmatic patients. Conclusion: The quantity of HDM-specific IgE antibody provides a possible explanation for increases of total IgE and significantly correlates with the amount and frequency of FeNO increases in Korean atopic asthmatic patients.

Overlapping Guillain-Barr$\acute{e}$ syndrome and Bickerstaff's brainstem encephalitis associated with Epstein Barr virus

  • A flaccid tetraparesis in Bickerstaff's brainstem encephalitis (BBE) is presumed to be a sign of overlapping Guillain-Barr$\acute{e}$ syndrome (GBS). In addition, BBE and Fisher syndrome, which are clinically similar and are both associated with the presence of the immunoglobulin G anti-GQ1b antibody, represent a specific autoimmune disease with a wide spectrum of symptoms that include ophthalmoplegia and ataxia. A 2-year-old boy presented with rapidly progressive ophthalmoplegia, ataxia, hyporeflexia, weakness of the lower extremities, and, subsequently, disturbance of consciousness. He experienced bronchitis with watery diarrhea and had laboratory evidence of recent infection with Epstein-Barr virus (EBV). He was diagnosed as having overlapping GBS and BBE associated with EBV and received treatment with a combination of immunoglobulin and methylprednisolone, as well as acyclovir, and had recovered completely after 3 months. In addition, he has not experienced any relapse over the past year. We suggest that combinations of symptoms and signs of central lesions (disturbance of consciousness) and peripheral lesions (ophthalmoplegia, facial weakness, limb weakness, and areflexia) are supportive of a diagnosis of overlapping GBS and BBE and can be helpful in achieving an early diagnosis, as well as for the administration of appropriate treatments.

  • Food allergy is defined as adverse reactions toward food mediated by aberrant immune mechanisms. Therefore, an allergic response to a food antigen can be thought of as an aberrant mucosal immune response. Food allergy most often begins in the first 1~2 years of life with the process of sensitization by which the immune system responds to specific food proteins, most often with the development of allergen-specific immunoglobulin E (IgE). Over time, most food allergeies are lost, although allergy to some foods is often long lived. The most important allergen sources involved in early food allergy are milk, eggs, peanut, soybean, meat, fish and cereals. Milk allergy seem to be associated with casein and whey protein. Important features of proteins as allergenicity are size, abundance and stability. Strategies for the prevention of milk allergy is breast-feeding, partially hydrolysised infant formula, using of probiotics, immune components in milk, preparation of low allergenicity milk protein and allergy therapy (immune therapy).

Efficacy of Egg Yolk Immunoglobulin (IgY) against Edwardsiella tarda Infection (계란난황항체의 Edwardsiella tarda에 대한 효능)

  • The present study evaluated effect of egg yolk immunoglobulin (IgY) from the hen immunized with Edwardsiella tarda. The purification of anti-E. tarda IgY was performed by polyethylene glycol (PEG). Purified IgY had heavy chain of 64 kDa and light chain of 27 kDa size. The IgY was instable against olive flounders digestive factors and artificially modulated pH 2 and 3. Nevertheless, some activity of IgY appeared in intestine. IgY was orally administered with viable E. tarda to the olive flounders and the efficacy of protection against E. tarda infection was evaluated. Orally administered IgY at a dose of 20 mg/fish delayed infection period of E. tarda cannulated at $10^{6{\sim}8}CFU/fish$ to small size (30~40 g) and middle size (110~120 g) flounder. Moreover, orally administered IgY at dose of 20 mg/fish inhibited the penetration of E. tarda cannulated at $10^8CFU/fish$ into the liver, kidney, spleen and gill via intestine. The fish orally administered with IgY showed increased survival rate. These results suggest that egg yolk containing anti-E. tarda IgY is effective in preventing edwardsiellosis.

Anti-Allergic Effect of Ponciri fructus

  • The immature fruits of Poncirus trifoliata L. or Ponciri fructus (PF), well known as 'Jisil' in Korea, have been used against allergic diseases for generations, and still occupy an important place in traditional Oriental medicine. Anti-allergic effects of this fruit have been investigated in a few experimental models. Immunoglobulin E (IgE) is the principal immunoglobulin involved in immediate hypersensitivities and chronic allergic diseases. The effect of an aqueous extract of PF on in vivo and in vitro IgE production was investigated. PF dose-dependently inhibited the active systemic anaphylaxis and serum IgE production induced by immunization with ovalbumin, Bordetelia pertussis toxin and aluminum hydroxide gel. PF strongly inhibited interleukin 4 (IL-4)-dependent IgE production by lipopolysaccharide-stimulated murine whole spleen cells. In the case of U266 human IgE-bearing B cells, Ponciri fructus also showed an inhibitory effect on the IgE production. On the other hand, mast cell hyperplasia can be causally related with chronic inflammation. Stem cell factor (SCF), the ligand of the c-kit protooncogene product, is a major regulator and ohernoattractant of mast cells. Ponciri fiuctus (1 mg/mL) significantly inhibited the SCF-induced migration of rat peritoneal mast cells (RPMCs). RPMCs exposed to SCF (50 ng/mL) resulted in a drastic shape change with a polarized morphology while the cells exposed to Ponciri fructus (1 mg/mL) remained resting, with little or no shape alteration. The drastic morphological alteration and distribution of polymerized actin were blocked by pretreatment with Ponciri fructus. In addition, Ponciri fructus inhibited both TNF-alpha and IL-6 secretion from RPMCs stimulated with SCF. These results suggest that Ponciri fructus has an anti-allergic activity by inhibition of IgE production from B cells. These findings also provide evidence that Ponciri fructu inhibits chemotactic response and inflammatory cytokines secretion to SCF in mast cells.

  • Hyperimmunoglobulin E syndrome is a relatively rare primary immunodeficiency syndrome characterized by recurrent infection, abscess formation and marked elevation of serum IgE level. The common infectious organism is Staphylococcus aureus and recurrent infection indicates some defects in the immunologic system. Although the infection can affect various organs, gastrointestinal tract involvement is rare and only one case of colon perforation has been previously reproted. Herein we report another one case of colon perforation which ocurred in an 8-year-old girl with hyper immunoglobulin E syndrome. The patient was admitted to the hospital due to an abscess on right neck. The diagnosis of hyper immunoglobulin E syndrome was made because she had eczematoid dermatitis on the face, pneumatocele on left upper lung field and markedly elevated serum IgE level(>15,000 IU/ml) with a past histories of frequent scalp abscesses and otitis media. Abdominal pain developed on the 13th day of admission and abdominal plain X-ray revealed free air. An exploratory laparatomy was performed and two free perforations of the transverse colon were noted. Segmental resection and double barrel colostomy were performed. Colostomy closure was done 4 month later and she had no gastrointestinal problem during a follow up period of 15 months.

Isolation and purification of chicken egg yolk immunoglobulin against Edwardsiella tarda (Edwardsiella tarda에 대한 계란난황항체의 분리와 정제)

  • The present study compared purification methods of hen egg yolk immunoglobulin (IgY) from the hen immunized with Edwardsiella tarda. The purification of anti-E. tarda IgY was performed by four different methods, polyethylene glycol (PEG), chloroform polyethylene glycol (Chloroform-PEG), ammonium sulfate and purification kit. Purified IgY had heavy chain of 64 kDa and light chain of 27 kDa size. IgY purified from the hen immunized with E. tarda showed higher ELISA values and agglutination titers than those with IgY purified from the non-immunized hen as a negative control. In addition, purified IgY recognized similar E. tarda proteins to those with anti-E. tarda rabbit serum by western blotting. Purified IgY had an agglutination titer of 1:512 by PEG method and ammonium sulfate method, and 1:128 by chloroform-PEG method and purification kit. Moreover, PEG method was the most rapid method among the four different IgY purification methods. These results indicate that PEG method is effective purification method maintaining biological activity of the IgY.

Increasing Prevalence of the Sensitization to Cat/Dog Allergens in Korea

  • Background: Recently, the number of domestic pets has increased. As a consequence, sensitization to animal allergens, such as cat or dog allergens, has become a problem. Objective: We studied the annual trends of sensitization to cats or dogs, and the characteristics of the patients. Methods: We retrospectively analyzed the medical records of 7,469 patients who visited a dermatology clinic and underwent an allergic profile test, from January 2011 to December 2015. Specific immunoglobulin E (IgE) levels to cat or dog antibody greater than 3.50 IU/ml were regarded as positive results. Results: In all, 274 patients showed significant increase in levels of specific IgE antibody to dog, and 307 revealed increase in levels of of specific IgE antibody to cat. The prevalence of these specific IgEs increased from 2011 to 2015. Independent risks for sensitization to cat allergens were sensitization to dog, but not to house dust, Dermatophagoides pteronyssinus, and D. farinae. Independent risks for sensitization to dog allergens were sensitization to cat, but not to house dust, D. pteronyssinus, and D. farinae. Total IgE level was not related to specific IgE level against either cats or dogs. Conclusion: In conclusion, the prevalence of sensitization to cat or dog has increased. Sensitization to cat or dog is related to each other, but is irrelevant to the total IgE level.

Antioxidant and Immunomodulatory Effects of Ulva pertusa kjellman on Broiler Chickens (육계에서 구멍갈파래의 항산화 및 면역조절 효과)

  • This study was conducted to test the efficacy of Ulva pertusa kjellman as immunomodulators under lipopolysaccharide (LPS) challenge in broilers by investigating their effects on serum superoxide dismutase (SOD) like ability, immunoglobulin concentration, and splenic cytokine mRNA expression. A total of ninety six1-d-old male broiler chicks (Ross 308) were divided into 4 treatment groups with 8 replicates of 3 birds in each group. NC (negative control, no immune substances), PC (positive control, ${\beta}$-glucan 25 ppm), Ulva P (Ulva pertusa kjellman Powder 3%), and Ulva E (Extract form Ulva pertusa kjellman 0.3%) were added in feed with respective substance. Heparinized venous blood and spleens were collected from five birds per dietary treatment at 5 wk of age. The SOD like activities in Ulva P and Ulva E were significantly increased in comparison with other groups (P<0.05). The immunoglobulin M content was lower in the Ulva E than other groups (P<0.05). Expression patterns of splenic cytokine mRNA in the IL-$1{\beeta}$, IL-2 and IL-6 were similar. Expression rate of IL-$1{\beta}$, IL-2 and IL-6 in Ulva pertusa kjellman (Ulva P, Ulva E) were decreased (P<0.05) in comparison to other groups. Expression rate of IL-$1{\beta}$, IL-2 and IL-6 were significantly lower in groups treated with Ulva E than Ulva P. In conclusion, dietary supplementation of Ulva pertusa kjellman improved SOD like activity and affect immune system by inhibiting inflammatory response in broiler chicks. In addition, it is more effective to use Ulva pertusa kjellman extract than powder for immunomodulatory function. These results suggest the possibility that Ulva pertusa kjellman could be used as the immunomodulator for inflammatory response in broiler chicks.

Anti-allergic Effect of Graviola Leaf Extract in Human Mast Cells (인간 비만세포에서 그라비올라 잎 추출물의 항알러지 효과)

  • This study is aimed at determining whether graviola (Annona muricata) beneficially influences immunoglobulin (Ig) E-mediated allergic reactions in human mast cells (LUVA cells). To examine the anti-allergic effect of graviola leaf extract (GLE), we treated antigen/IgE sensitized mast cells with extracts of various concentrations. In this study, the results of the in vitro model of IgE-mediated mast cell degranulation showed that GLE significantly inhibited the release of histamine, β-hexosaminidase, TNF-α, IL-4 and IL-6 in LUVA cells. Pretreatment with GLE suppressed the phosphorylation of antigen-induced Lyn and Syk, thus suppressing the downstream MAPKs pathways. The above results indicate GLE could suppress mast cell activation and allergic responses. Accordingly, it can be supported that GLE has the potential to be used as functional cosmetic material.
