라포에 대한 개념 분석 (Concept Analysis of Rapport)

  • Purpose: The purpose of the study was to identify the attributes of rapport, their antecedents and consequences. Methods: Walker and Avant's method for concept analysis was used to analyze the concept. Articles published from January 2007 to May 2017 were searched through PubMed, CINAHL, and RISS databases using "rapport" as a keyword. Results: The attributes of rapport were as follows: 1) understanding; 2) sharing information; 3) empathy; 4) harmonious relationship. The antecedents of rapport were as follows: 1) effective communication skills; 2) relation-oriented attitude. The consequences of rapport were forming trust, therapeutic relationship, improvement of quality of care, increased patients satisfaction and increased nurses satisfaction. Conclusion: Rapport can be defined as a harmonious relationship that understand each other and share ideas and information.

중환자실 간호사의 요통 영향요인 (Factors Affecting Low Back Pain in Nurses in Intensive Care Unit)

  • Purpose: This study was conducted to identify the factors that influence low back pain in nurses in Intensive Care Units (ICUs). Methods: A descriptive-correlational design was used, with a convenience sample of 116 nurses from one university hospital in Seoul. The data collected were processed using SPSS Window 17.0 Program for actual numbers and percentages, differences in the dependent variable according to general characteristics, and mean, standard deviations, correlation coefficients and multiple regression analysis. Results: The factor influencing low back pain in nurses in ICUs was functional ability (${\beta}$=.652). This one factor explained 41.9% of low back pain in nurses in ICUs. Conclusion: These results show that functional ability was related to nurses' low back pain in nurses in ICUs. These results further suggest that more research is needed to develop program involving functional ability to prevent and management of low back pain in nurses in ICUs.

  • Critical illness neuromyopathy (CINM) is a common but frequently underdiagnosed condition in critically ill patients that contributes to ventilator weaning failure and limb weakness in intensive care unit (ICU). CINM is subdivided into critical illness polyneuropathy and critical illness myopathy, and the occurrence of these conditions in the ICU is associated with multiple organ failure due to sepsis or certain medications. CINM survivors might have persistent functional disabilities and a poor quality of life. This situation demonstrates the need for efforts to minimize or prevent CINM in critically ill patients. This article provides a current overview of CINM and the associated clinical strategies.

  • Purpose: This paper aimis to analyze the characteristics of ward plan of Korean hospital architecture constructed in each periods and identify the change since 1945. Methods: Literature review was conducted while ward plans for analysis were sellected. And then the important points of ward design were analyzed in each periods. Results: The important points of ward design - type of corridor in ward plan, number of beds per nursing unit, location of nurse station, organization of multi-bed rooms, location of patient toilet, number of day room, concentration and separation of elevator halls for patient and general- were analyzed and the characteristics of change were identified in each periods. Implications: This paper could be a basic data for arranging the history of hospital architecture in Korea.

IgG Western Blot for Confirmatory Diagnosis of Equivocal Cases of Toxoplasmosis by EIA-IgG and Fluorescent Antibody Test

  • The performance values of available techniques used in serodiagnosis of toxoplasmosis are satisfactory but they raise problems of equivocal and discordant results for very low IgG titers. Recently marketed, LDBio-Toxo II IgG Western blot (IB) showed an excellent correlation with the dye test. We estimated the proportion of equivocal and discordant results between the enzyme immunoassay Platelia Toxo IgG (EIA-IgG) and fluorescent antibody test (FAT) and assessed the usefulness of the IB as a confirmatory test. Out of 2,136 sera collected from pregnant women, 1,644 (77.0%) tested unequivocally positive and 407 (19.0%) were negative in both EIA-IgG and FAT. The remaining 85 (4%) sera showed equivocal or discordant results. Among them, 73 (85.9%) were positive and 12 (14.1%) were negative in IB. Forty-one (89.1%) equivocal sera in EIA-IgG and 46 (86.8%) equivocal sera in FAT were positive in IB. Reducing the cut-off values of both screening techniques improved significantly their sensitivity in detecting very low IgG titers at the expense of their specificity. In conclusion, equivocal results in routine-used techniques and their discordance in determination of the immune status in pregnancy women were not uncommon. IB test appeard to be highly useful in these situations as a confirmatory technique.

병원조직 구성원의 직장애착에 영향을 미치는 요인 (Factors Affecting on Organizational Commitment of Hospital Employees)

  • This study was carried out to find out the factors affecting on organizational commitment of hospital employees. For the purpose, the questionnaire-based research was done with 1280 hospital employees who were working in 2 university hospitals and 4 general hospitals located in Taegu City and Kyungpook Province from November 20, 1997 to December 22, 1997. The results of the study were summarized as follows. There were significant correlations among environmental variables, psychological variables, structural variables, vertical conflict, horizontal conflict, job satisfaction, and organizational commitment. By path analysis, variables such as pay, work involvement, work definiteness, met expectations, positive affectivity, distributive justice, job security, expectations before entering a hospital had significant positive effect on job satisfaction in order of size, while, vertical conflict, job opportunity, and horizontal conflict had significant positive effect on job satisfaction in order of size, while, vertical conflict, job opportunity, and horizontal conflict had signifecant negative effect in order of size. Variables such as job satisfaction, job security, work involvement, pay, vertical conflict, met expectations, distributive justice, positive affectivity, work unit control, job autonomy, and job routinization had significant positive direct effect on organizational commitment in order of size, while, job opportunity, job hazards and resource inadequacy had significant negative direct effects in order of size. It was found that the following variables, listed in order of size, had significant total effects on organizational commitment : job satisfaction, job security, job opportunity, work involvement, pay, met expectations, distributive justice, positive affectivity, job hazards, and work unit control. In considering abovc findings, it is recommended that programs for job satisfaction promotion, job security, decrease of intent to leave, work involvement promotion, adequate pay, met expectations, distributive justice, positive affectivity promotion, job hazards decrease, and work unit control grant should be implemented to increase organizational commitment.

임상간호현장에서의 실행연구 여정 (A Journey to Action Research in a Clinical Nursing Context)

  • Purpose: The purpose of this study was to examine the effectiveness of Action Research (AR) approach in nursing. Methods: Participants were 64 perioperative nurses recruited from C hospital in Gwangju, Korea. The nurses were engaged in the project through 2 cycles of planning, acting, observing, and reflecting. A mixed-methods design was used to examine changes in participants and their knowledge management practice. Quantitative data were analyzed using SPSS 20.0 program and qualitative reflection data underwent content analysis. Results: During the project, participants developed standardized pre-operative checklists and opened an Internet Cafe to better manage their perioperative nursing information. At the end of the project, there was a significant increase in nurses' knowledge management (p=.015) and the rate of surgical material prescription errors decreased from 8.0% to 2.9%. Core AR project team members' teamwork skills and organizational commitment increased significantly (p=.040, p=.301, respectively). The main themes that emerged from the qualitative data were learning how to solve problems in practice, facilitating team activities through motivation, barriers of large participation, and rewarded efforts and inflated expectations. Conclusion: The AR project contributed to empowering participants to solve local problems. AR is a useful methodology to promote changes in practices and research participants.
