The study on outpatient-clinic practice by shift system at a hospital in Taegu (대구지역 한 중소병원의 교대제 근무에 의한 외래진료에 관한 연구)

  • Medical consumers(patients) want evening-clinic because of busy work. For patient's convenience and improving service, hospital should accept it. Considering payment system and patient's demand, personnel expenses, hospital can not accept. The practice of shift system to accept patient's demand and hospital's economic aspect was made. To analysis the effect of the system and probability to alternative to evening clinic this study was done. This study was composed of basal study, intervention, evaluation of effect. The basal study were composed of studying demand on evening clinic, the number of beds, doctors employee, the time table of practice and work, and the number of patients at arrival time. The intervention composed of changing of practice time, changing of working time by the number of patients at arrival time, increasing of employee. The evaluation of effect were composed of evaluating the number of patient at time, the effect of shift system, the comparison of the number of in and out patients and questionnairing the practice of shift system. In the practice time at 2 shift system First team works 7-15 hours and Second team 12-20 hours. there are no lunch and supper time. At 18-20 hours the number of patients were 25-30. The number of patient a depart were 6-7. The number of out-patient increase in 13% and inpatient increase in 10% before the system. Doctors(100%), employee(94.6%), and patients(86.4%) approved this system. The advantage of this system were utilization of surplus time, lengthen the practice time, even distribution of patients and shortening of waiting time, rapid treatment of emergent patients. The disadvantage of this system were shortage of manpower, not all depart practice, continuity of practice, no lunch and supper time, irregular rounding. At present because of small Demanding on evening clinic, this shift system was economical. To succeed this study more effectively all depart in hospital participate. But because of economical reason it is impossible for hospital to do it. If the government assist the economic loss that all depart participate in this system it is very helpful for hospital to succeed in implementing this system more early.

  • The growing concern for the protection of personal information has made it critical to implement effective technologies for privacy and data management. By observing the limitations of existing approaches, we found that there is an urgent need for a flexible, privacy-aware system that is able to meet the privacy preservation needs at both the role levels and the personal levels. We proposed a conceptual system that considered these two requirements: a graph-based, access control model to safeguard patient privacy. We present a case study of the healthcare field in this paper. While our model was tested in the field of healthcare, it is generic and can be adapted to use in other fields. The proof-of-concept demos were also provided with the aim of valuating the efficacy of our system. In the end, based on the hospital scenarios, we present the experimental results to demonstrate the performance of our system, and we also compared those results to existing privacy-aware systems. As a result, we ensured a high quality of medical care service by preserving patient privacy.

Usage of Library Collection, Services & Assistance Among Ophthalmologists - An Opinion Survey

  • Accurate and reliable Information improves decision making, reduces costs, and saves time. The ophthalmic Libraries supply quality ophthalmic information to the ophthalmologists to serve the patients in a better way and stimulate the growth of ophthalmic field. This paper aims to study the usage of library collection, services and assistance among the Ophthalmologists - Eye Doctors in India. A survey method is used to obtain the ophthalmologists' usage of library collection, services and assistance. Through convenient sampling method, a structured questionnaire is circulated to the ophthalmologists in India and 633 ophthalmologists are responded. Among the 633 ophthalmologist, 82.15% of the ophthalmologists use the library collection - book. 73.46% of the ophthalmologists use the online data / journals. 89.73% of the ophthalmologists seek assistance to locate books/articles/documents. There exists a significant difference between the ophthalmologists use of Library Service and gender (p value 0.001**). There exists a significant difference between the ophthalmologists use of Library Assistance / help and designation category (p value 0.000**). There exists a significant difference between the ophthalmologists use of Library Assistance / help and working experience (p value 0.017**). There exists a significant difference between Library Services (p value 0.009**), Library Assistance / help (p value 0.000**) and institution type. The study results will help the ophthalmic libraries to serve the user better.

The Development of Nursing Standards for Coronary Artery Disease Patients Who Received Medical Treatment (내과적 치료를 받는 관상동맥질환자의 간호표준 개발)

  • Purpose: This study was conducted to develop nursing standards for medical patients with Coronary Artery Disease (CAD) patients. Methods: This study was a methodological study. The content of nursing standards for medical patients with CAD developed in this study was validated. The nursing standards of cardiovascular nursing developed by the American Nurses Association (2008), the nursing standards developed by the Korea Nurses Association (2003) and a clinical manual of patients with CAD developed by K teaching hospital were reviewed. Literature regarding CAD nursing standards was also reviewed. The basic contents of nursing standards for medical patients with CAD were selected by an expert group including two nursing faculties, a cardiovascular unit manager, and two cardiologists. A pilot study was conducted then in real clinical settings, in which includes cardiovascular outpatient clinic, cardiovascular inpatient units, and cardiac intensive care units to evaluate clinical suitability of the nursing standards. Results: The final version of the nursing standards for medical patients with CAD included 12 standards, 24 criteria, 38 indicators and 92 nursing activities. Conclusion: The nursing standards developed in this study can be used in evaluating quality of nursing service and in educating nurses who are involved in patients with CAD.

Validity and Reliability of a Measure of Patient Satisfaction with Physical Therapy (물리치료환자 만족도 도구의 타당도 및 신뢰도 분석)

  • Patient satisfaction is an important factor in evaluating the quality of care. Patient satisfaction may be used to evaluate provider services and facilities, and used to predict the patient returns to a facility. The patients decision whether the patient returns to a facility or whether the patient recommends the facility to other people may be affected by a variety of factors of patient satisfaction. This study is to develop and test of a translated and modified Goldstein's instrument that measures patient satisfaction among physical therapists patients and clients. A self-administered questionnaire survey was conducted in Seoul, Chung-Joo and Bu-Cheon cities. Survey data was obtained from 743 patients who visited the physical therapy practice at university hospitals, general hospitals and clinics. The instrument developed by Goldstein was used and translated into Korean. Several items were added to the instrument. Patient's opinions of service in each domain measured using 5-point Likert-type scales that ranged from strongly disagree to strongly agree. Principal components analysis with varimax rotation was used to indicate which questions examined similar components of patient satisfaction. Component analysis indicated eight scales (kindness, scheduling, recommendation, convenience of parking, privacy, and waiting time). Different types of validity were established well. The coefficient of reliability (Cronbach alpha =.97) obtained for the instrument was clearly within a desired range.

A Comparative Study on the Concepts of the Chuna(推拿) (추나(推拿)의 개념 비교연구)

  • The Chuna(推拿) in Korea has been developed since 1990's. Korean Chuna which is the only one manual therapy done by medical doctor is riched by absorbing other nation's manual therapy like Chinese Tuina(中國推拿) and Chiropractic therapy of U.S.A. So, We find that Korean Chuna's peculiarity and development by comparing Chinese Tuina, Chiropractic of U.S.A and Japanese manual therapy which influenced establishment of Korean Chuna. We compared each manual therapy's history, development, diagnosis, correction and treatment of human body, frequently contacting disease and medical service of the present state. Korean Chuna has absorbed other nation's manual therapy and advantage of preserving muscle-skeleton and spinal disorder disease. But, for more prosperity, it needs union of various conception and diagnosis and more research and application to more various disease, for example in internal medicine, gynecology or pediatry and founding system as regular academic work inside the college the possibility which objection and it will practice more completeness. And as one of specialty apprentice doctor subject inside the hospital, the possibility of raising a quality improvement of study and research environment.

  • Recently the development of medical modality like as MRI, 3D US, DR etc is very active. Therefore it is more required not only the enhancement of quality in medical service but the improvement of medical system based on quantization, minimization, and optimization of high speed. Especially, as the changing into the digital modality system, it gets to start using ASIC(Application Specific Integrated Circuit) to realize one board system. It requires the implementation of hardware debugging and effective speedy algorithm with more speed and accuracy in order to support and replace existing device. If objected image could be linked to high speed process board with special interface and pre-processed using FPGA, it can be used in real time image processing and protocol of HIS(Hospital Information System). This study can support the basic circuit design of medical image board which is able to realize image processing basically using digitalized medical image, and to interface between existing device and image board containing image processing algorithm.

Determinant Factors' Impact on Managerial Performance through Management Accounting Systems in Indonesia

  • This study aims to examine the effects of factors related to management accounting systems and managerial performance. The factors include budgetary participation, decentralization, and management style. Furthermore, this research investigates the consequences of the management accounting systems on managerial performance and the direct effect between management style and budgetary participation on managerial performance. Survey questionnaires were distributed to both public and private hospitals in Palembang, the South Sumatera region, Indonesia. The sample consisted of 62 respondents from 15 hospitals Target respondents were all managers in hospitals, including financial managers, service managers, human resource managers, quality managers, and other managers. The questionnaire was distributed online to each hospital, and approximately five or more questionnaires were hardcopies. This research was conducted over less than six months. The data obtained were processed via excel files, then checked for conformity, and analyzed by partial least square (PLS) software with descriptive statistics. This study uses path analysis, which that is structural equation modeling (SEM). The findings show that management style, decentralization, and budgetary participation significantly affect management accounting systems. However, management style and budgetary participation did not influence managerial performance. Finally, the findings indicate that management accounting systems are positively affected by managerial performance.

Role Identification of Home Health Nursing Specialist (가정전문간호사의 역할 규명)

  • Purpose : The purpose of this study is to present the desirable level of home health care services by identifying the roles and activities by task of home health nursing specialists as well as to raise the level of professionalism in home health care services. Method : This is a methodological study. The roles and activities by task were identified through a review of literature and a state-of-the-practice survey, and were structured into a questionnaire after being reviewed and modified through a consensus of experts. The field survey was conducted on 136 home health nursing specialists at medical institutions, public health centers. public medical institutions, non-governmental organizations, and religious institutions in Seoul, Gyeonggi Province. Incheon, Busan, Daegu, and Gwanggju from June 4 to August 4. 2004. Seven roles. 34 tasks and 130 activities were identified in association with home health nursing specialists. Result : The roles of home health nursing specialists were identified as professional nursing service provider, advisor, educator, administrator, case manager, researcher and leader. Under these roles. 34 tasks and 134 task-specific activities were identified. Conclusion : The the roles and activities of home health nursing specialist identified in this study can be used in various home health care settings. These the roles and activities should provide the evaluation criteria of home health care services for institutions with existing home health care programs. An evaluation tool should be developed in order to ensure the hish quality of home health care services.

Effects of job insecurity and job engagement on turnover intention of paramedics in emergency medical institutions (응급의료기관에 근무하는 1급 응급구조사의 직업 불안정성 및 직무열의가 이직의도에 미치는 영향)

  • Purpose: This study aimed to identify the effects of job insecurity and job engagement on turnover intention of paramedics who work in emergency medical institutions. Methods: From October 14 to 28, 2014, data were collected by structured questionnaires from 171 paramedics who were working in emergency medical institutions. The data were analyzed by using SPSS/WIN 21.0. Results: Of the 171 subjects, 57.3% were temporary employees, of whom 87.5% were working in regional emergency medical centers. The mean scores were 3.19 for job insecurity, 4.58 for job engagement, and 3.28 for turnover intention. The correlation between the variables showed that the higher the job insecurity of the participants, the higher their turnover intention (r = .397, p <.001). Moreover, the higher their job engagement, the lower their turnover intention (r = -.354, p <.001). The variable that most significantly affected turnover intention was job insecurity. The coefficient of determination ($R^2$) of job insecurity and job engagement was 24.3%. Conclusion: A law should be enacted to involve paramedics as required personnel for emergency medical institutions in order to enhance the quality of emergency medical services and provide prompt and professional emergency medical services to emergency patients.
