Sparganosis in the Lumbar Spine : Report of Two Cases and Review of the Literature

  • Sparganosis is a rare parasitic infection affecting various organs, including the central nervous system, especially the lumbar epidural space. This report describes the identification of disease and different strategies of treatments with preoperative information. A 42-year-old man presented with a 2-year history of urinary incontinence and impotence. He had a history of ingesting raw frogs 40 years ago. Magnetic resonance (MR) imaging showed an intramedullary nodular mass at conus medullaris and severe inflammation in the cauda equina. A 51-year-old woman was admitted with acute pain in the left inguinal area. We observed a lesion which seemed to be a tumor of the lumbar epidural space on MR imaging. She also had a history of ingesting inadequately cooked snakes 10 years ago. In the first patient, mass removal was attempted through laminectomy and parasite infection was identified during intra-operative frozen biopsy. Total removal could not be performed because of severe arachnoiditis and adhesion. We therefore decided to terminate the operation and final histology confirmed dead sparganum infection. We also concluded further surgical trial for total removal of the dead worm and inflammatory grannulation totally. However, after seeing another physician at different hospital, he was operated again which resulted in worsening of pain and neurological deficit. In the second patient, we totally removed dorsal epidural mass. Final histology and enzyme-linked immunosorbent assay (ELISA) confirmed living sparganum infection and her pain disappeared. Although the treatment of choice is surgical resection of living sparganum with inflammation, the attempt to remove dead worm and adhesive granulation tissue may cause unwanted complications to the patients. Therefore, the result of preoperative ELISA, as well as the information from image and history, must be considered as important factors to decide whether a surgery is necessary or not.

  • The author presented several points of experienced materials obtained from the joint meetings of consultation-liaison Psychiatric division with oncologic department in Kyung Hee University Hospital. The joint meetings which have been held regularly every other week were very helpful not only for the psychiatrist but also for the cancer department stags to manage the cancer patients actively. The joint meetings have been progressed into more cohesive and active communication as time goes by. Most of the subjects discussed in the meeting was focused on the psychological and behavioral problems of the cancer patients. Besides, the difficulties arising from the chemotherapy were discussed. Probably the most difficult task for the treatment team was to provide complete information while respecting the patient's right to deny their situation The liaison psychiatrist usually gave comments to the charge doctor and/or nusing staff how to evaluate the patient's behavior and what would be the influential factors in developing the doctor-patient relationship. It was found that many cancer patients and their family members had their own peculiar illness behavior and disease concept which led patients to take non-medical or moreover, anti-therapeutic care. The family members were found to play an influential role in the choice of treatment method and progression of the disease. Another role of the liaison psychiatrist in the oncology ward was to encourage the treatment team members. In the practical point of view, it was not easy for the liaison psychiatrist to have time regularly for the cancer patients to encourage the liaison activities. And it seems to be fundamental that the consultation fee for the liaison psychiatrist should be set up at the resonable level.

Satisfaction towards clinical training institution according to clinical practice contents in the dental hygiene students (치위생과 학생들의 임상실습내용에 따른 실습기관별 만족도)

  • Objectives: The purpose of the study was to investigate the satisfaction towards clinical training institution according to clinical practice contents in the dental hygiene students. Methods: A self-reported questionnaire was completed by 382 dental hygiene students of five colleges in Busan, Ulsan, and Gyeongsangnam-do from August 20 to September 30, 2013. The data were analyzed using SPSS WIN 20.0 program for t-test, one-way ANOVA, and Pearson's correlation analysis. The questionnaire consisted of the general characteristics of the subjects and the satisfaction towards the clinical training institution. The satisfaction was composed of 44 questions including the general satisfaction, practice contents, practice guide, training time of practice, training environment, practice evaluation, and personal relationship by Likert 5 point scale. Results: The students were most satisfied with the practice contents of the dental university hospitals. The dental hospital and dental clinics were the most satisfied choice in the personal relationships. Public health center dental clinics bestowed the students with the most satisfied practice guidance, time, environment, and evaluation. Practice contents and practice time were the most influential factors to dental hygiene department satisfaction to the students. Conclusions: In order to enhance the clinical practice satisfaction, it is necessary to have the continuous relationship with the clinical training institution. The practice satisfaction is influenced by the contents and environment of the clinical training institution.

Balloon-Occluded Retrograde Transvenous Obliteration versus Transjugular Intrahepatic Portosystemic Shunt for the Management of Gastric Variceal Bleeding

  • Background/Aims: Gastric varices (GVs) are a major cause of upper gastrointestinal bleeding in patients with liver cirrhosis. The current treatments of choice are balloon-occluded retrograde transvenous obliteration (BRTO) and the placement of a transjugular intrahepatic portosystemic shunt (TIPS). We aimed to compare the efficacy and outcomes of these two methods for the management of GV bleeding. Methods: This retrospective study included consecutive patients who received BRTO (n=157) or TIPS (n=19) to control GV bleeding from January 2005 to December 2014 at a single tertiary hospital in Korea. The overall survival (OS), immediate bleeding control rate, rebleeding rate and complication rate were compared between patients in the BRTO and TIPS groups. Results: Patients in the BRTO group showed higher immediate bleeding control rates (p=0.059, odds ratio [OR]=4.72) and lower cumulative rebleeding rates (logrank p=0.060) than those in the TIPS group, although the difference failed to reach statistical significance. There were no significant differences in the rates of complications, including pleural effusion, aggravation of esophageal varices, portal hypertensive gastropathy, and portosystemic encephalopathy, although the rate of the progression of ascites was significantly higher in the BRTO group (p=0.02, OR=7.93). After adjusting for several confounding factors using a multivariate Cox analysis, the BRTO group had a significantly longer OS (adjusted hazard ratio [aHR]=0.44, p=0.01) and a longer rebleeding-free survival (aHR=0.34, p=0.001) than the TIPS group. Conclusions: BRTO provides better bleeding control, rebleeding-free survival, and OS than TIPS for patients with GV bleeding.

Comparison of local flaps versus skin grafts as reconstruction methods for defects in the medial canthal region

  • Background: The medial canthal region features a complex three-dimensional and internal anatomical structure. When reconstructing a defect in this area, it is crucial to consider both functional and aesthetic aspects, which presents significant challenges. Generally, local flaps are preferred for reconstruction; however, skin grafts can be used when local flaps are not feasible. Therefore, we conducted a comparative analysis of surgical outcomes skin grafts when local flaps were not feasible, to determine which surgical method is more effective for medial canthal region reconstruction. Methods: Twenty-five patients who underwent medial canthal region reconstruction using skin grafts or local flaps from 2002 to 2021 were enrolled. Patient information was obtained from medical records. Five plastic surgeons evaluated the surgical outcomes based on general appearance, color, contour, and symmetry. Results: Skin grafts were used in eight patients and local flaps were used in 13. Combined reconstructions were employed in four cases. Minor complications arose in four cases but improved with conservative treatment. No major complications were reported. Recurrence of the skin cancer was noted in two cases. All categories showed higher scores for the local flap compared to both skin graft and combined reconstruction; however, the differences were not statistically significant respectively. Conclusion: The choice of appropriate surgical methods for reconstructing defects in the medial canthal region depends on various factors, including the patient's overall health, the size and depth of the defect, and the degree of involvement of surrounding structures. When a local flap is not feasible, a skin graft may provide favorable surgical outcomes. Therefore, a skin graft can serve as a viable alternative for reconstructing the medial canthal region.

Importance and Influence in Factors of Selecting Dental Clinics in Some Regions (일부지역의 치과병·의원 선택요인의 중요도와 영향)

  • The purpose of this study was to offer the importance on factors of selecting dental institution according to general characteristics targeting general dental patients, and the information of having important influence upon choosing dental clinic and dental hospital. The survey was carried out from August 2009 to September. As a result of analyzing SAS(ver 9.1), the following conclusions were obtained. In the importance on factors of selecting dental institution according to general characteristics, it was thought to be important in convenience of movement or mobility such as distance, transportation, and parking convenience according to age level. According to final academic background, the importance in a factor of rumor in dental clinic was high(p < 0.05). According to job, the significant difference was shown in rumor of dentistry and parking convenience(p < 0.05). According to income, the factor in the appearance of training a medical specialist was indicated to be statistically higher in the group with over 5 million won than the group with under 2 million won(p < 0.05). Given seeing the distribution of opinion about dental hospitals (comparison with dental clinics), the excellence in the medical staff's ability was indicated to be the highest in all of the factor of influencing the use satisfaction(${\beta}=0.18$), the factor of influencing the recommendation level(${\beta}=0.21$), and the factor of influencing the re-use intention(OR=2.09).

Treatment of Unruptured Intracranial Aneurysms in South Korea in 2006 : A Nationwide Multicenter Survey from the Korean Society of Cerebrovascular Surgery

  • Objective : There have been no clinical studies regarding the epidemiology and treatment outcome for unruptured intracranial aneurysm (UIA) in South Korea yet. Thus, The Korean Society of Cerebrovascular Surgery (KSCVS) decided to evaluate the clinical and epidemiological characteristics, and outcome of the treatment of UIA in 2006, using the nationwide multicenter survey in South Korea. Methods : A total of 1,696 cases were enrolled retrospectively over one year at 48 hospitals. The following data were obtained from all patients : age, sex, presence of symptoms, location and size of the aneurysm, treatment modality, presence of risk factors for stroke, and the postoperative 3D-day morbidity and mortality. Results : The demographic data showed female predominance and peak age of seventh and sixth decades. Supraclinoid internal carotid artery was the most common site of aneurysms with a mean size of 5.6 mm. Eight-hundred-forty-six patients (49.9%) were treated with clipping, 824 (48.6%) with coiling, and 26 with combined method. The choice of the treatment modalities was related to hospital (p=0.000), age (p=0.000), presence of symptom (p=0.003), and location of aneurysm (p=0.000). The overall 30-day morbidity and mortality were 7.4% and 0.3%, respectively. The 30-day mortality was 0.4% for clipping and 0.2% for coiling, and morbidity was 8.4% for clipping and 6.3% for coiling. Age (p=0.010), presence of symptoms (p=0.034), size (p=0.000) of aneurysm, and diabetes mellitus (p=0.000) were significant prognostic factors, while treatment modality was not. Conclusion : This first nation-wide multicenter survey on UIAs demonstrates the epidemiological and clinical characteristics, outcome and the prognostic factors of the treatment of UIAs in South Korea. The 30-day postoperative outcome for UIAs seems to be reasonable morbidity and mortality in South Korea.

A Study on the Level of Perception of the Effect on Women's Health of the Artificial Abortion - Centered on Female and Male Adults - (인공유산이 여성건강에 미치는 영향에 관한 인식정도연구 - 성인 남녀 중심으로 -)

  • This study sought to find out the level of perception of the effect on women's physical, mental, socio-culture health of the artificial abortion. The research design was a descriptive correlational study and selected by convenient sampling. The subjects were 510 female and male adults who live in Seoul, Korea. Data were collected from 20th September, 1999 to 28th October, 1999. The data were analyzed through the SPSS program by use of frequency, percentage, t-test, ANOVA and Scheffe test as a post hoc contrast, $x^2$-test, Pearson correlation coefficient and stepwise multiple regression. The results of the study are as follows. 1. The subject who experienced artificial abortion was 24.3% and 468% of these had experienced repeated artificial abortion. 54.4% was using the contraceptives and there was a significant difference in the contraceptives between that women wish to use and that women are really using. 2. For the opinion on the artificial abortion, 77.3% of respondents was pro-abortionists and the most acceptable reasons of pro-choice were" In case of unmarried" and "For the women's health". 3. On the whole, the subjects perceived that artificial abortion had a negative effect on the women's health, especially, on the women's mental health. Among the items, 'guilty feeling about the baby' was the highest. 4. The related factors to the level of perception of the effect on women's health of the artificial abortion were in sex, age, occupational status, marital status, educational level, the number of children, abortion history, contraceptives women use, the frequency of the consideration on the problem of artificial abortion and the position toward abortion such as pro or anti. The main influencing factors on the level o0f perception were artificial abortion history, occupational status and the frequency of the consideration on the problem of artificial abortion. Theses factors explained 35% of total variance of the level of perception 5. The variables related to the experience of artificial abortion were age, occupational status, marital status, religion, educational level, the number of children and whether women practice contraceptives or not.

Midterm Results of Aortic Valve Replacement Using Tissue Valve (조직판막을 이용한 대동맥판막치환술의 중기성적)

  • Background: The durability of the tissue valve is important in choice between a mechanical valve and a tissue valve in cardiac surgery. We studied the mid-term results of tissue valve in the aortic position. Material and Method: The subjects were 380 patients who had undergone aortic prosthesis replacement between May 1990 and March 2009. We retrospectively analyzed hospital and outpatient records: the mean age was $69{\pm}9$ years; the male to female ratio was 227 : 162; and the mean follow-up duration was $46.7{\pm}40.8$ months (range 0~196 months). Result: 389 surgical cases in total had been taken with 380 patients. Early death occurred in 15 patients (3.9%). Overall survival rate at 1, 5 and 10 years were 92.3%, 78.1% and 54.2% respectively. Freedom from reoperation at 1, 5 and 10 years were 98.4%, 97.1% and 91.7% respectively. Freedom from structural valvular deterioration at 1, 5 and 10 years were 96.1%, 92.3% and 88.0% respectively. In the multivariate analysis of preoperative risk factors, young age (p<0.001) was significant risk factor for reoperation. High peak velocity in the postoperative period (p=0.034) and young age (p=0.029) were significant risk factors for structural valvular deterioration. Old age (p=0.001), long bypass time (p=0.035), concomitant coronary artery bypass graft surgery (p=0.003) and preoperative low left ventricular ejection fraction (p=0.003) were significant factors for early mortality. Preoperative estimated glomerular filtration rate (< 60 mL/min) (p=0.025) and persistent left ventricular hypertrophy (p=0.032) were the risk factors for late mortality. Conclusion: This study showed that the freedom from reoperation and the freedom from structural valvular deterioration in aortic tissue valve replacement were acceptable. It will be necessary to conduct further studies with long-term follow-up and more patients.

A Study on the Using Patterns and Using Promotion Strategy for Special Transport Service : the case of Suwon City, korea (특별교통서비스의 이용실태 및 활성화 방안 연구 : 수원시를 중심으로)

  • The purpose of this study is to contribute the use of special taxi for the transportation vulnerable such as elderly and pregnant woman in terms of improving transportation mobility and extending social participation. We analyzed the DB users of STS system operating in Suwon, and surveyed the preference statement. Specifically, we have built transportation mode choice models for the elderly and pregnant woman, and then we drew the activating STS plan by comparing influence factors for each model. Based on the results of the study, 23% of STS user was the elderly people and pregnant women, 31% of user was for hospital passage purpose, and short-haul passage within 2 ~ 5km was 70% level of the passage. According to preference survey results between STS and Bus, fare was the highest impact factor in the mode choices for the elderly, and the participants who are older and car owners have been found to prefer the special taxi. For the pregnant woman, travel time and fare were the main influence factors of choices, and it was discovered that they preferred the special taxi when they are car owners and trips was frequented. Also, if the fare of special taxi is about 70% of the regular taxi fare, The share rates of Special Taxi comparing to bus for the elderly and pregnant woman are analyzed to be 51.1% and 63.6% each. Therefore, the adjustment of the fare would be effective to encourage the use of the special taxi. The results of this study can be used as a basis for establishing the rational fare structure of the special taxi.
