Purpose: This study was conducted to evaluate the effect of a social support program on family caregivers' role strain in elderly long-term home care. Methods: The research adopted a non-equivalent control group pretest-posttest design. The number of participants was 25 in the experimental group and 25 in the control group sampled among family caregivers in elderly long-term home care. The experimental group participated in a 10-session social support program, which consisted of physical, emotional, informational, and material support. The effect of the program was evaluated by measuring family caregivers' role strain. Results: The experimental group showed a significant decrease in family caregivers' role strain in elderly long-term home care. Conclusion: The result suggests that the social support program was effective in decreasing family caregivers' role strain in elderly long-term home care. There is a need to develop more effective and systematicsocial support programsfor family caregivers of elderly long-term home care.
The purpose of this study was to develop strategy direction for visiting home care through a Strengths, Weaknesses, Opportunities, and Threats(SWOT) analysis. The methodological research-based study employed an analysis model based on a literature review and expert interviews. The outcomes of the SWOT analysis formed the main components of the strategic planning approach, whose application to the hospital-based visiting home care program of the Seoul Veterans Hospital was expected to be effective in increasing the turnover rate and hospital incomes.
Purpose: The purpose of this study was to identify the factors associated with the health-related quality of life of family caregivers. Methods: A cross-sectional study was conducted. This study included 191 primary family caregivers of elders who used home care services (home-visit nursing, home-visit care, daycare) covered by the public long-term care insurance. Data were collected using self-report questionnaires from December 2010 to June 2011. These data were analyzed by using hierarchical multiple regression. Results: The majority of the family caregivers were female (79.6%) and daughters-in-law (28.8%). The mean depression score was $6.33{\pm}6.49$ and the mean health-related quality of life score was $0.69{\pm}0.39$. It was found that the factors affecting the health-related quality of life of family caregivers included depression (${\beta}$=-.406, p<.001), home-visit nursing use (${\beta}$=.296, p<.001), and daycare use (${\beta}$=.178, p=.015), which accounted for 36.6% of their health-related quality of life. Conclusion: Using home-visit nursing and daycare services has a positive effect on the health-related quality of life of family caregivers. To improve health-related quality of life of family caregivers, South Korea needs to fully activate the home-visit nursing and daycare services, and to strengthen family support programs.

Purpose: Hospice care represents all meaningful aspects of physical, mental and economical status of the end stage patients. The purpose of this study was to estimate the cost of home-based hospice care. Method: Fifteen nurses participated in counting an hour for requirement and home visit data of 50 end stage patients were analyzed. The method of to estimate the cost of home-based hospice care was three ways. Result: In case, including traffic expense, Singles fixed fee per visit via direct inquiry was 112,970 won but in case, excluding traffic expenses, was 86,036 won and traffic expenses per visit was 26,934 won. Final cost of home-based hospice care integrated the fixed fee per the needed time for visit and fee-for services. The fixed fee per 30 minutes was 35,251 won and 60 minutes was 46,595 won and 90 minutes was 57,939 won. We included pain management and the management of emergency and bereavement care among fee-for services. Conclusion: The cost of hospice care should be establish for not only patient but the living spouse, families, and children of the dying and for anyone else affected by any patient's death.

The Japanese elderly welfare policy has focused on facility policy for the aged and preventive care service for healthy elderly people. This paper has conducted a comparative analysis on Geriatric Gealth Services Facility and Special Nursing Home for the Elderly. For this, each service function has been divided into six categories; daily life / nursing and caring / medical service / management / supply / miscellaneous. Then the change in real structure by category has been analyzed through a plan analysis on case facilities. In the Geriatric Health Services Facility, the biggest change was observed in 'livelihood' among six categories. In the Special Nursing Home for the Elderly, 'the nursing and care parts' and 'medical service part' are decreased since 1999. At that time, the facilities started to be individualized and divided into a unit. To pursue home-like care instead of unit care, there was a change in construction planning to help the aged with dementia live a self-sufficient life.
Purpose: This study is to investigate the recognition regarding Hospital - Based Home Care Service among physicians and nurses in a G University Hospital. Method: Data were gathered from 92 physicians and 257 nurses. working at G University Hospital. from January 15 to January 22. 2003 by questionnaire. The data were analyzed by using the SPSS/PC +. Result: 1) As to the previous information about a hospital based home care service. those who have been familiar to it were 85.9% of the physicians. and 98.8% of the nurses. 55.4% of the physicians and 55.1% of the nurses responded that they are willing to refer their patients to the home health care only if their patients and families want to. 47.8% of the physicians and 44.4% of the nurses perceived the present cost of a hospital based home care service to be moderate. 2) Most of the physicians and nurses reported that the most available service was 'wound dressing' (98.4%, 92.6%) and the least available service was 'incision and drainage'(1l7.4%, 42.8%). 3) As to the necessity of home nursing business and the anticipated effects. nurses perceived higher than physicians($3.46\pm.74$). and both showed a significant difference according to age of subjects. Conclusion: For the stabilization and successful implementation of a hospital based home care service. it should be accompanied with education programs about home care for physicians. in particular junior staff.
Purpose: This study aimed to investigate the influencing factors on self care, respiratory difficulty, sleep impediment, anxiety and depression among patients with neuromuscular disease who are dependent on Home Mechanical Ventilator (HMV). Methods: 157 patients were recruited through hospital based home nursing care departments and HMV rental centers. Data were collected by questionnaires. The analytic methods were n(%)/$M{\pm}SD$, $x^2$/t-test or ANOVA and multiple linear regression. Results: Patients with Amyotrophic lateral sclerosis were more likely to utilize HMV, had shorter periods of morbidity and the most extended daily use of HMV. As for medical equipment, they had ambu-bag (87.2%) and oxygenator (15.4%). Reports of respiratory difficulty and sleep impediment were low. There were no significant difference. In contrast, reported anxiety and depression were high and showed significant difference between non invasive HMV and invasive HMV. Anxiety contributes to respiratory difficulty (t=3.62, p=.002), sleep impediment (t=2.06, p=.042), and depression (t=7.24, p<.001). However, home nursing care exerts a positive influence in reducing anxiety (t=-2.73, p=.008). Conclusion: Anxiety contributes to respiratory difficulty, sleep impediment, and depression. However, those who use home nursing care reported less anxiety. Home nursing care positively impacts patients dependent on HMV as a practical service and available resource.
This paper provides the guidelines from which to develop a visiting health care program at the Public Health Center in Korea and involves an expanded payment compensation system of preventive services based on the new long-term health insurance system in Japan. The function and management methods to achieve the goals practiced in a community contact center for elderly support which have recently been established will guide the specific directions and strategies that the Public Health Center should pursue. That is to say, comprehensive and continuous efforts will be put forth in preventive home visiting care targeting the elderly in certain jurisdictions. At this point in time in which the visiting care nursing program has not yet started, visiting health care provided by the Public Health Center oversees chronic diseases of a vulnerable population. But after it has been developed nationwide, the visiting health care system at the Public Health Center will be distinctive and focused on health promotion and prevention.
Since patients with strokes occupy a high priority among patients for home nursing, the development of guidelines for such nursing is required and the needs of these patients should be reflected in the guidelines. Therefore, this study was done to identify the content and levels of home nursing for patients with strokes and to utilize the data in developing the most effective home nursing guidelines for these patients. The level of A, D. L. and the ability to control upper extremities were measured, and through a questionnaire. the needs for home nursing and related variables were also evaluated. The subjects for the study were 121 patients, 58 inpatients and 63 home care patients who had had a Stroke. Data collection was done from September 1996 to January 1997. The collected data were analyzed utilizing SPSS /PC, and the results are as follows : 1. Home nursing need of inpatients The priority order of home nursing needs for inpatients was : "Training in emergency treatments and how to cope with a stroke"(2.28+1.06), next, "Explanation of diets as limited or recommended", and last, "Nursing care for sleeping Problems". 2. Home nursing need home of patients The priority order of home nursing needs for home patients was : "Care for the paralyzed side" (2, 89+.34), next, "Maintenance of right posture and how to change position" (2.87+.34), and last, "Counseling on sex". 3. Comparison of the levels of home nursing needs between in patients and home patients The results of analyses of home nursing needs according to causes were grouped into seven categories : and t-tests of the seven categories showed significant differences between the two groups in all categories, that is, the level of home nursing needs were significantly higher for home patients than for inpatients in all categories of home nursing. 4. Level of home nursing needs by characteristics The variables that have affected the level of home nursing needs for these patients were sex, profession, level of education, accompanying diseases, paralyzed position. A.D.L. levels and ability levels in coordinating upper extremities. There variables, displayed a reverse correlation with the level of home nursing needs, and the degree of correlation was high. In conclusion, the above results, show there were differences in the priority order of home nursing needs between inpatients and home patients : but the content of home nursing needs wanted by these patients was similar. Meanwhile, the levels of demand for home nursing was exceptionally higher on the part of home patients than inpatients. Purpose: The study analyzed continuing educational needs with the aim of developing continuing education program for home healthcare nurse specialists. Methods: A convenience sample of 101 home healthcare nurse specialists affiliated with 89 home health care agencies in Seoul, Busan, Dae-gu, Inchon, Jeonnam, Keongnam was used. Data was collected with a self-report questionnaire from May 10-31, 2010, and analyzed with descriptive statistics using SAS 9.13 program. Results: Of the participants, 88.1% reported that continuing education was necessary and 58.2% reported that the education was needed for peak job performance. The participants also reported that it would be proper if continuing education is given on a Saturday(56.4%) by home healthcare institute(49.5%) or homecare nurses association (38.6%) for 1-3 months(56.4%). The participants scored more than 3.0 in the current performance at six of the seven home healthcare nurse specialist roles, and scored more than 3.5 in the necessity of continuing education for seven roles in 47 jobs. Conclusion: Various programs for continuing education have to be developed and need to focused on the jobs needed for home healthcare nurse specialist and the most frequent disease. For this purpose, every home healthcare organization has to select their unique area and develop their own educational program. Furthemore, home healthcare nurses association has to build an education operating system that incorporates all program aspects.
