Adrenal Insufficiency in Patients with Rotator Cuff Tear: Prevalence, Risk Factors and Influence on Outcome

  • Background: This study measured the rate of adrenal insufficiency among rotator cuff tear patients, and determined the roles of steroid treatment and herbal medicine as specific risk factors of adrenal insufficiency. Methods: We prospectively evaluated the 53 patients who agreed to participate in the study and underwent arthroscopic rotator cuff repair due to a chronic full-thickness tear from March 2012 to September 2013. The diagnosis of adrenal insufficieny was made by rapid adrenocortical hormone test before the operation. We collected information regarding their history of steroid and herbal medicine treatment within the last 12 months. Results: The rate of adrenal insufficiency among the rotator cuff tear patients in our study was 18.9% (10 of 53 patients). Steroid treatment (p=0.034), frequency of local steroid injection (p=0.008), and herbal medicine treatment (p=0.02) showed significant association with the risk of adrenal insufficiency. Conclusions: In this study, careful taking of a patient's medical history, such as the use of steroids and herbal medicine, is important when investigating adrenal function before surgery.

  • In the 21st century, the era of artificial intelligence, it is demanded to make a change of the paradigm of education by the recent impact of the 4th industrial revolution. The education up to now has emphasized knowledge, meanwhile the human resources for the future are required to be armed with four C's: critical thinking, creativity, communication and collaboration capability, rather than being equipped with just knowledge. That is because the future society demands such abilities, especially the creativity of each individual. At this point, we are asked to consider the aim of education and teaching methods. In school education, students are to be respected and considered able to develop their potentials by themselves. They shouldn't be estimated by tests in the process of learning as they are now. We reconsider the aim of education here by taking a look at Whitehead's opinions and the present educational situations.

  • Ethylene glycol poisoning is treated mainly by alcohol dehydrogenase inhibition therapy and hemodialysis. Early recognition and initiation of treatment is important because toxic metabolites increase over time by hepatic metabolism; however, there is no confirmative diagnostic tool in our clinical setting. Therefore, diagnosis is dependent on history, high anion gap acidosis, high osmolal gap, etc.. Diagnosis and treatment are delayed in cases where history taking is not possible, such as a mental changed patient. Authors report on two cases of ethylene glycol poisoning by contrasting clinical outcomes, demonstrating the importance of early diagnosis and treatment for achievement of a good outcome.

A Study on the Framework Schema of Jusimpo-Style Buddhist Halls of Goryeo Period (고려시대 주심포 불전의 가구형식에 관한 연구)

  • The purpose of this study is to find framework schema of early J usimpo-style Buddhist halls (Geungnakjeon Hall of Bongjeongsa Temple, Muryangsujeon Hall of Buseoksa Temple, and Daeungjeon Hall of Sudeoksa Temple). Though the halls are known as built in the late Goryeo Period, they show the influence of the architectural style of the early Unified Silla Period. To find the adopted modules and proportions of these halls, this study conceived a schematic diagram based on the whole frame structure taking reference from the Cai-Fen system in Yingzao Fashi. In these three halls, the heights of each cross-beam (Dori) are made up by the layers of member and member units. This study computes the values of Cai, Zhi, and Fen which can apply to both the section and the plan. The vertical section structure is determined by combining the standard member heights (Cai) and the standard unit heights (CaiZhi). The bays of columns are made by multiples of the standard member width (Fen).

Investigation of seismic performance of super long-span cable-stayed bridges

  • With the further increase of span length, the cable-stayed bridge tends to be more slender, and becomes more susceptible to the seismic action. By taking a super long-span cable-stayed bridge with main span of 1400m as example, structural response of the bridge under the E1 horizontal and vertical seismic excitations is investigated numerically by the multimode seismic response spectrum and time-history analysis respectively, the seismic behavior and also the effect of structural nonlinearity on the seismic response of super long-span cable-stayed bridge are revealed. Furthermore, the effect of structural parameters including the girder depth and width, the tower structural style, the tower height-to-span ratio, the side-tomain span ratio, the auxiliary piers in side spans and the anchorage system of stay cables etc on the seismic performance of super long-span cable-stayed bridge is investigated numerically by the multimode seismic response spectrum analysis, and the favorable earthquake-resistant structural system of super long-span cable-stayed bridge is proposed.

A case of dystrophic calcification in the masseter muscle

  • Background: Dystrophic calcification can occur in any soft tissue with the absence of a systemic mineral imbalance and is often associated with trauma, infection, or inflammation. It is easily found in the site of the heart and skeletal muscles and rarely appears in the head and neck area. Case report: We present a rare case of multiple calcified masses in the left masseter muscle of a 26-year-old female with a history of trauma in the area. In computed tomography, multiple radiopaque masses were observed inside the left masseter muscle and blood test results were normal. The calcified masses were diagnosed as dystrophic calcification and removed by surgery without any complications. Conclusion: Different types of calcifications may occur in the cheek area, and they need to be distinguished from dystrophic calcification. Thorough clinical examination and history taking is required together with blood testing and radiographic examinations.

  • The main purpose of this study is to reveal the properties and specialities of architectural space after Modernism. Space was the main theme of Modernity in architecture and they insist Modern architectural space had its own characteristics compared with the earlier period. With view that another paradigm of space is rising as a criticism of and departure from Modernity, this study try to show what is the contents of difference and how it express in reality. We can find a new trends in architectural spaces, that are the production of totally different circumstantial background as like information society, digital media environment, post structuralism, new science and heterotopian situation in urbanism. We can call it Post Modernity in architectural space that would find the 'Otheness' and expand the territory of architecture. It means not only the change of architectural space itself but also different cognition and perception of space is taking place. Contemporaru architectural space has lost its materiality and conventional socio-cultural functions. Dematerialization and media or image -like being is the new characteristics instead. It demands a new way of being in a different life-world as well as the new expenence of architecture.

Accelerated Durability Analysis of Suspension System (Suspension System의 가속내구해석)

  • The durability test, along with the crashworthiness test, requires the most time and expense in the vehicle development process. The durability design using CAE tools reduces the time required for both the durability test and actual vehicle production. Existing dynamic stress analyses designed fir the analysis of vehicle fatigue mainly calculate the dynamic stress history and fatigue after performing dynamic analysis and stress analysis with relevant software applications and then superpositioning the dynamic load history and stress influence coefficient at each joint. This approach is a complex process, taking into account the flexibility of the parts. It is, however, incapable of giving accurate consideration to the contacts between components, the non-linearity of materials, and tire-road surface interactions. This approach also requires that the analysts have an expertise in software applications of various kinds or an expert in each area must perform the analysis. This requires as a great deal of manpower and time. In order to complement the existing approaches for dynamic stress analysis, this study aims at the following: (1) to suggest the simple and accurate analysis technique which is capable of producing all the possible necessary results; (2) to reduce dramatically the time and manpower needed to construct a model designed to analyze dynamics, quasi-static stress, and fatigue; and (3) to enable an accurate analysis of fatigue by improving the accuracy of dynamic stress. we verify the presented analysis method through durability evaluation of the knuckle of passenger car.

  • "Saengsanbibang" is Obsterics technical books published in 1915. After the publishing of first edition, it was published 15 times so I supposed that this book was very popular in that times. And this book is also valuable because Obsterics books written in Korea are uncommon. But there had not been enough detailed study on this book, so I tried to study on the formation of this book. For that, I compared the formation between this book and 4 kinds of typical Obsterics technical books in Qing dynasty, "Taesansimbub", "Taesanbiseo", "Daesaengyoji", and "Dalsaengpyeon". As the result, "Saengsanbibang" refered to "Dalsaengpyeon" for the contents that the reason and prevention of distocia, the basic principle for antenatal care, and the formulae to tonify blood. The contents that the taking methods of Saenhwatang (生化湯) accroding to the diseases, are mainly affected by "Taesanbiseo". Some contents which had been refered in "Taesansimbub", "Daesaengyoji" weren't refered to "Saengsanbibang". So "Saengsanbibang" is valuable because this book easily summarized the medical knowledge for spreading it to ordinary people. I look forward to more specific study for "Saengsanbibang".

Hamlet's (Un)manly Grief: the Cult of the Past in the Age of Theatrical Power

  • The mourning and grief practice richly registered in Shakespeare's Hamlet is one of the abiding themes that critics have been fascinated with. This paper attempts to take a fresh look at the issue by building its arguments on Benjamin's insight that the modern art (mechanically) reproducing the exhibition value brings about the destruction of the ritual value and favors the conditions of melancholy. Instead of taking for granted that Hamlet's performance of grief is fundamentally different from those of other characters such as Gertrude, Ophelia, and Laertes, this paper argues that Hamlet's performance comes to be recognized masculine and different from others, only because he presents himself to be so through his theatrical performance as well as his princely power that the subjects (others in the story) ought to ascribe to. To prove this point, this paper closely analyzes Hamlet's rhetorics and the ways he constructs his mourning self, which is emblematic of the shift in art history that Benjamin has characterized with the terms of "ritual value" and "exhibition value." In conclusion, this paper suggests that Shakespeare's Hamlet marks the change of the historical horizon, a permanent removal from the past in which the ritual value had been once protected, pushing us to a new age to live with melancholy and the disconnection from things and their muted language.
