Biomechanical Analysis of Lower Limb Joint Motions and Lumbar Lordosis during Squat and Stoop Lifting (쪼그려 들기와 허리 굽혀 들기 시 하지관절 움직임과 요추 전만에 관한 생체역학적 분석)

  • In this study, lower extremity joint kinematics and kinetics and lumbar lordosis were investigated for two different symmetrical lifting techniques(squat and stoop) using the three-dimensional motion analysis. Twenty-six male volunteers lifted boxes weighing 5, 10 and 15kg by both squat and stoop lifting techniques. There were not significant differences in maximum lumbar joint moments between the two techniques. The hip and ankle contributed the most part of the support moments during squat lifting, and the knee flexion moment played an important role in stoop lifting. The hip, ankle and lumbar joints generated power and only the khee joint absorbed power in the squat lifting. The knee and ankle joints absorbed power, the hip and lumbar joints generated power in the stoop lifting. The bi-articular antagonist muscles' co-contraction around the knee joint during the squat lifting and the eccentric co-contraction of the gastrocnemius and semitendinosus were found to be important for straightening up during the stoop lifting. At the time of lordotic curvature appearance in the squat lifting, there were significant correlations in all three lower extremity joint moments with the lumbar joint. Differently, only the hip moment had significant correlation with the lumbar joint in the stoop lifting. In conclusion, the knee extension which is prominent kinematics during the squat tilling was produced by the contributions of the kinetic factors from the hip and ankle joints(extensor moment and power generation) and the lumbar extension which is prominent kinematics during the stoop lifting could be produced by the contributions of the knee joint kinetic factors(flexor moment, power absorption, bi-articular muscle function).

  • Painful periarticular calcification most commonly occurs within the rotator cuff of the shoulder and rarely around the elbow, hip, foot, and neck. As acute inflammatory reaction develops, severe pain, exquisite tenderness, local swelling, and limitation of motion with pain occur. In case of calcific tendinitis of the shoulder, it can be easily diagnosed according to the symptoms and with x-ray. However, in lesions of the hip, as it is a rare location and usually involves pain in the posterolateral aspect of the thigh, which can simulate radicular pain from a lumbar intervertebral disc, it could be difficult to diagnose. Hence, physicians usually focus on lumbar lesions; therefore, misdiagnosis is common and leads to a delayed management. Here, we report the case of a 30-year-old female patient with calcific tendinitis of the rectus femoris that was successfully managed with ultrasound-guided steroid injection. This study offers knowledge about the rectus femoris calcification.

Two Case Reports of Patients Diagnosed with Avascular Necrosis of the Femoral Head Treated with Korean Medicine Treatment Including Chuna Manual Therapy, Pharmacopuncture and Acupuncture (추나 요법 및 약침, 침치료를 비롯한 한방치료로 증상이 호전된 대퇴골두 무혈성 괴사 환자 2예 보고)

  • Objectives The purpose of this study is to report the effects of Korean medicine treatments including Chuna manual therapy, pharmacopuncture and acupuncture for two patients who have pain in hip joint diagnosed with avascular necrosis of the femoral head. Methods The treatment effect was evaluated with a numeric rating scale (NRS), range of motion (ROM), physical examination (Patrick test), and score of Harris hip score (HHS). Results After treatment, symptoms of the two patients such as hip joint pain and gait dysfunction were improved. The NRS decreased from 8 (right [Rt.]), 7 (left [Lt.]) to 4 (Rt.), 3 (Lt.) in case 1 and from 9 to 2 in case 2. Also the patients showed improvement in ROM and score of HHS increased from 55.5 to 88.5 in case 1 and from 63.5 to 85.5 in case 2. In addition, positive Patrick test improved to negative. Conclusions Korean medicine treatments including Chuna manual therapy, pharmacopuncture and acupuncture can be effective in relieving pain and recovering the fucnction of hip joint associated with avascular necrosis of the femoral head, even when surgery is recommended. Although further study such as controlled studies is needed.

Effects of Hand Acupressure on Low Back Pain, Range of Joint Motion, and Postpartum Depression in Women in Postpartum Period (수지요법이 산욕기 여성의 요통, 관절가동범위, 산후우울감에 미치는 효과)

  • The purpose of this study is to investigate the effects of Hand Acupressure Therapy on lower back pain, hip joint range of motion, and depression in postpartum women. Methods: Data was collected from March 21 to May 1, 2022 from 40 postpartum women. 𝛘2-test, independent t-test, repeated measures analysis of variance (ANOVA), and Scheffe's test were performed using SPSS/WIN 21.0. Results: Compared with the control group, the lower back pain score of the experimental group that performed Hand Acupressure therapy was lower, and the angle of joint range of motion was increased. Postpartum depression scores were also shown to decrease, and all of these results were statistically significant. Conclusions: Based on this study, it was confirmed that Hand Acupressure therapy can be used as an effective non-invasive intervention for back pain, joint range of motion, and postpartum depression, and suggested the implications and direction of further research.

Immediate Effect of Hip Hinge Exercise Stretching on Flexibility of Lower Limb, Pelvic Tilting Angle, Proprioception and Dynamic Balance in Individual with Hamstring Tightness

  • Objective: The purpose of this study was to measure the immediate effect of hip hinge exercise stretching on hamstring flexibility, pelvic tilting angle, proprioception, and dynamic balance in individual with tightness of the hamstring. Design: A randomized controlled trial. Methods: A total of 35 healthy young adults (27 males, 8 females) volunteered for this study and randomly divided into three groups (Hip hinge exercise stretching group, passive stretching group, and PNF stretching group). The hamstring flexibility, pelvic tilting angle, knee joint proprioception, dynamic balance was conducted for 3 times. In order to evaluate the hamstring flexibility, the active knee extension test was performed. Forward bending test was performed to examine pelvic tilting angle.The proprioception was tested by the joint position sense test and dynamic balance was evaluated by Y balance test. Results: The hamstring flexibility, pelvic tilting angle and dynamic balance were significantly improved between three groups before and after intervention (p<0.05). Dynamic balance was significantly difference between the three groups in the posterolateral direction (p<0.05). Conclusions: This study result showed that hip hinge exercise stretching was the most effective method for increasing hamstring flexibility, pelvic tilting angle and dynamic balance. In addition, it is necessary to study whether hamstring stretching is effective in low back pain patient with hamstrings tightness.

The Relationship between Hip Abductor Muscle Strength and Lumbar Instability in Patients with Chronic Low Back Pain (만성 요통환자의 요추부 불안정성과 고관절 외전근 근력수준간의 상관관계)

  • Purpose: The purpose of this study was to investigate the relationship between hip abductor muscle strength and lumbar instability in patients with chronic low back pain. Methods: Fifty-two female patients were recruited for this study. The patients' history was recorded and was used to determine the general characteristics of the female complaints. The women were additionally examined to determine whether the level of pain was characteristic of patients with chronic lumbar instability. The following tests were also carried out in the subjects during the examination: 1) the prone instability test. 2) the test for aberrant movement patterns during lumbar flexion test. 3) the straight leg raising test. 4) posterior-to-anterior mobility test, and 5) the test for age and strength of the hip abductor muscle following assessment of the dominant side. In particular, hip abductor muscle strength was evaluated using a dynamometer. Results: The test results showed that the number of positive responses for the five types of lumbar instability tests performed, was significantly related to the strength of the hip abductor muscle. The average hip abductor muscle strength in total subjects was $72.89{\pm}7.66N$, whereas the average hip abductor muscle strength in subjects who showed positive responses to more than four out of the five tests, was $44.70{\pm}5.79N$. Conclusion: The results demonstrated that the hip abductor muscle strength and lumbar instability were negatively correlated. The lower was the strength of the hip abductor muscle, the higher was the possibility of lumbar instability.

A Rehabilitation for Total Hip Replacement in Korean Medicine: A Report of Three Cases (고관절 전치환술 후 한방 재활치료에 대한 증례 보고)

  • The objective of this study was to propose a rehabilitation protocol for total hip replacement in Korean medicine and to report its effectiveness. Three patients who had undergone total hip replacement were treated using the rehabilitation protocol in Korean medicine. We evaluated the outcome evaluating physical examination findings of the hip joint, numeric rating scale, pain disability index, Harris hip score, and walking state. After the treatment, we observed that hip movement and muscular strength improved. Moreover numeric rating scale, pain disability index, Harris hip score, and walking state of patients improved. A rehabilitation in Korean medicine can be effectively used for total hip replacement patients. The limitation of this study was the insufficient number of cases. Further studies are needed to design a rehabilitation protocol in Korean medicine.

Ultrasonographic Findings of Normal Hip Joint and Painful Hip due to Soft Tissue Problem (고관절 초음파의 정상 소견 및 고관절 주위 질환의 초음파 소견)

  • The lesions around hip joint including bone, joint and soft tissue can cause the pain. For diagnosis of these lesions, physical examination and simple X-ray were accomplished primarily. Some special cases, CT or MRI was a useful tool. However ultrasonography could provide non-invasive and dynamic images for the lesions of tendon and bursa, and it could be a useful tool for follow up after hip surgery. Due to the deep location of hip joint, ultrasonography is not easy for physician to examine and its application was impossible in some obese patients. This article deals with the normal and pathologic ultrasonographic findings of the hip joint and various applications using ultrasonography.

Management of Irq-Un-Nasa (Sciatica) by Fasd (Venesection): A Case Report

  • Objectives: Sciatica is a musculoskeletal pain sensed in the leg along with the distribution of the sciatic nerve, which is sometimes accompanied by low back pain and is most commonly caused by a disc herniation. In the Unani system of Medicine, Irq-un-Nasa (Sciatica) is defined as the pain which starts from the hip joint and descends towards the foot. It is a type of Waja-ul-Mafasil which is developed due to the accumulation of Khilt-e-Dam or Khilt-e-Balgham ghaleez in the hip joint. The conventional system of Medicine offers several medications and surgeries to manage sciatica with limited clinical evidence of effectiveness. These cases aim to provide insight into the effects of Fasd (Venesection) in Irq-un-Nasa. Case Presentation: Fasd was performed in the saphena minor vein of two clinically diagnosed patients with sciatica after the initial assessment. Two sittings of Fasd, once a week in each patient, were performed, followed by telephonic assessments for two months. The intensity of sciatic pain reduced with subsequent sittings of Fasd, and there was no recurrence of any symptoms and signs again during complete follow-up. Conclusion: Based on the results of the present case report, it appears that such cases of Irq-un-Nasa can be managed with Fasd, and the quality of life of such patients can also be improved.

  • According to development of medical science and engineering, people has made efforts to solve tile discomfort of body accident as well as the life span. Many cases that reduce pain of a hip joint or limitation of motion using Revision Total Hip Replacement are getting increased. However it has not proved which material is the most appropriate. Scientists are still studying about materials of the artificial joint. In accordance with advance of science, the materials are Betting better. Recently, when Revision Total Hip Replacement is performed, typical materials are Ti-alloys Ceramics. We have studied materials of artificial joints and analyzed which one is better for human's body.
