The Study on The Property of the Phoria and Asthenopia Inducing Factor in Adults in Jeon-Buk Area (전북지역 성인의 사위특성과 안정피로 유발인자에 관한 연구)

  • We examined refractive error, the corrected visual acuity, phoria and fusional reserve of 114 healthy myopes and hyperopia who had no strabismus, no ocular and physical diseases. We evaluated the occurrence of asthenopia according to phoria and fusional reserve. 41 out of 114 subjects which exophoria was 69% and esophoria was 5.5%, orthophoria was 26% made a complaint against asthenopia. The subjects who had exophoria of $0-6{\Delta}$ in the range of normal state was 61.4%, while the subjects who had exophoria of $7{\Delta}$ or more in the range of abnormal state was 38.6%. Reducing fusional reserve was associated with increasing phoria. The fusional reserve twice or more than phoria were 27.2%, and twice or under were 72.8%. Futhermore, AC/A ratio for heterophoria was found from 1.0 to 12.6 and its relationship to asthenopia of phoria could not be determined.

Expected Values for the Analysis of Accommodation and Vergence Dysfunction of Young Adluts (젊은 성인의 조절 및 이향운동 기능장애 분석을 위한 표준 값)

  • Purpose: The purpose of this study was to determine the expected values of accommodation and vergence system for analysis of binocular vision. Methods: Seventy three healty subjects without past history of eye disease were selected for this study, Patients showing clinical binocular anomalies including manifest squint and intermittent heterotropia were excluded and data was collected on 54 patients(mean age, 25.3${\pm}$2.7 year). Near point of convergence, distance and near phoiras were measured based on objective deviation angle. And then AC/ A ratio, relative accommodation were measured and all of the data were compared with the Morgan's expected values. Results: The values of distance and near heterophoria were 1.24${\pm}$2.72${\Delta}$ and 2.70${\pm}$4.91${\Delta}$ respectively and the gradient AC/A ratio was 3.92${\pm}$2.17${\Delta}$/D which were all about the same compared with the Morgan's expected values. The values of distance negative fusional convergence was same, however, distance positive fusional convergence was 7${\Delta}$ greater than the Morgan's one. There were no significant differences in the range values of both near vergences while the range of distance vergence was relatively larger than the Morgan's one. Positive relative accommodation was +0.37D greater and negative relative accommodation was -0.63D greater than the Morgan's values. Conclusions: The findings in this study appear that the Morgan's expected values have a possibilities of including considerable proximal convergence value. In comparison with Morgan's values, Korean's normal expected values have much different values of especially BO limit, so if the Morgan's values are directly applied to binocular vision analysis for Korean people, it may lead to an inaccurate binocular vision diagnosis. This results can be used to diagnose the accommodation and vergence system of Korean normal young adults.

Relationship of Fixation Disparity and Heterophoria According to Fixation Distance (원, 근거리에서 주시시차와 사위와의 관계)

  • Purpose: This study measured Y-intercept that means a fixation disparity, X-intercept that means a associated phoria and slope of a fixation disparity curve (FDC), which are variables of dissociated phoria and the FDC. We searched for the result value and examined the direction, distribution pattern and a variable that give an most affect on dissociated phoria and the FDC at distance and near. Also this study examined that there were statistically significant differences between distance and near, comparing the result value. Methods: We measured the dissociated phoria and the fixation disparity for 51 subjects at distance and near in June, 2007. All subjects ranged from 20 to 25 years of age (average 21.72${\pm}$1.88 years old) and had no eye disease. At distance the dissociated phoria measured with the distance MIM card (muscle imbalance measure card, Bernell co., USA), and the fixation disparity measured with the modified Mallett Far Unit (Bernell co., USA). At near the dissociated phoria measured with the near MIM card (muscle imbalance measure card, Bernell co., USA), and the fixation disparity measured with the Wesson fixation disparity card (American Optical co., USA). Results: The percentage distribution of types of fixation disparity curves was that at distance prevalence of Type I (74.6%) was the highest, followed by Type IV (17.6%) and Type II (3.9%), Type III (3.9%) and that at near prevalence of Type I (53.0%) was the highest, followed by Type III (29.4%), Type IV (13.7%) and Type II (3.9%). 2. There were significantly correlation in dissociated phoria, fixation disparity (Y-intercept) and associated phoria (X-intercept). 3. The fixation disparity at distance was most affected by associated phoria (X-intercept) (p=0.000). The distance dissociated phoria was most affected by fixation disparity (Y-intercept) (p=0.342), but the influence was weak. 4. The fixation disparity at near was most affected by associated phoria (X-intercept) (p=0.000). The near also dissociated phoria was most affected by associated phoria (X-intercept) (p=0.009). The result that compared the each variables with the same variables at distance and near had statistically significant on paired t-test for among dissociated phoria (t=7.529, p=0.000), X-intercept (t=5.860, p=0.000), the Y-intercept (t=4.640, p=0.000) but slope of the FDC did not differ significant (t=1.336 p=0.188). Conclusions: Relationship of fixation disparity and Heterophoria had close correlation at distance and near.

Graphic Method of Visual Analysis and Norms Determination for the Far Distance Phoria (원거리사위별 그래프 분석과 표준값 결정)

  • We investigated the visual analysis of the far distance phoria for 83 subjects who have emmetropia or myopia eyes, using the graphic method, and determined new norms. We got the following results from these experiments. 1. The measured norm of far distance phoria is $1.12{\Delta}$, exophoria, the that of near distance phoria is $0.50{\Delta}$, exophoria, the that of negative relative convergence at far distance(N.R.C.) is ${\times}$/11/5, the that of positive relative convergence at far distance(P.R.C.) is 10/20/8, the that negative relative convergence at near distance(N.R.C.) is 11/21/5, the that of positive relative convergence at near(P.R.C.) is 10/20/8, the that of negative relative accommodation(N.R.A.) is +2.54D, and the that of positive relative accommodation(P.R.A.) is -2.60D, respectively. We know from theses results that the measured norms of far distancet phoria is higher than Morgan's norms, while those of near distance phoria are lower than Morgan's norms. 2. We also know from these experiments that Morgan's norms are closely related to A, B, and C groups and Morgan's criterion can more usefully apply to the visual analysis. 3. In Sheard's criterion and Percival's criterion, the prism prescription is occurred when the exophoria of the far-point phoria is over $7{\sim}9{\Delta}$, and the esophoria of that is over $4{\sim}6{\Delta}$.

Relationship Between Subjective Symptoms with Near Work and Binocular Function (근업시 자각 증상과 양안시 기능의 관계)

  • The aim of this work was to study the relationship between subjective symptoms and binocular function during near work in forty one(12-17yr) schoolchildren. CISS symptom survey, amplitude of accommodation, accommodative facility, neagative relative accommodation(NRA)/positive relative accommodation(PRA) near point of convergence(NPC), heterophoria, AC/A ratios were measured. The results of measurement were compared with normal expected value of each test and classified into normal and abnormal group. Accommodative insufficiency was more general than convergence insufficiency and esophoria was greater than exophoria. NRA/PRA showed very high and AC/A ratios were very low compare with normal expected value. The results of CISS was as follows; most frequent symptom was tired, mean score was $16.63{\pm}7.49$, mean number of symptoms were $10.07{\pm}3.04$, and mean number of serious symptoms were $1.98{\pm}2.13$. After classified into 4 groups according to score of symptoms, number of symptoms, and degree of symptoms and then analyze the correlation compare with binocular vision test results. NRA was most correlated with group comparison, accommodative facility was most correlated with score of symptoms, NRA was most correlated with number of symptoms, and near horizontal phorias were most correlated with serious symptoms. NRA showed most correlative relation with near symptoms.

Repeatability of New Phoria Test Using Color Chart and Color Filter (색 시표와 색 필터를 이용한 새로운 사위검사의 신뢰도)

  • Purpose: The repeatability of a new Red-Blue phoria chart test (Red-Blue phoria chart; RBP) was appreciated. Methods: Distance (5 m) and near (40 cm) heterophoria was measured in 38 visually normal subjects. Phoria tests using RBP, Howell phoria card (HP), and MIM card (MIM) were done and the repeatability of each phoria test was compared with one another. Results: The mean of horizontal deviations was that RBP was $-0.602{\pm}0.727{\Delta}$, HP was $-0.865{\pm}1.051{\Delta}$, and MIM was $-1.501{\pm}1.346{\Delta}$, at distance, and that RBP was $-2.566{\pm}2.352{\Delta}$, HP was $-2.804{\pm}2.411{\Delta}$, MIM was $-3.838{\pm}2.603{\Delta}$, at near. The repeatability was RBP>MIM>HP in distance tests and MIM>RBP>HP in near tests. Conclusions:RBP test is identified as a reliable phoria test having high repeatability.

Comparison of Binocular Function in Normal Subjects and Convergence Insufficiency (정상안과 폭주부족안의 양안시기능 검사값 비교)

  • Purpose: The aim of this study was to compare binocular functions such as near point of convergence (NPC), AC/A ratios, heterophoria, accommodation, accommodation lag, positive relative accommodation (PRA), negative relative accommodation (NRA), positive relative convergence (PRC), negative relative convergence (NRC) in normal subjects and convergence insufficiency. Methods: 86 subject (male n=45, female n=41, mean $age{\pm}SD=23.27{\pm}2.85$ years) without amblyopia, strabismus, and ocular pathology were studied. Forty three patients each group were classified as normal subjects and convergence insufficiency group based on AC/A ratio and far and near phoria. Binocular function of the two groups was measured using phoropter. r. Results: The values between normal subjects and convergence insufficiency were 5.71 cm and 7.07 cm for NPC, 5.28 and 2.81 for Heterophria AC/A, 0.92 exo/3.36 exo and 2.17 exo/10.84 exo for far and near phoria, 15.49/23.30/13.30 and 13.50/20.02/9.09 for PRC, respectively. However, there were no significant differences between normal subjects and convergence insufficiency for accommodative amplitude, accommodative lag, PRA, NRA, BI vergence test, BO vergence test, NRC. Conclusions: Both groups were a significant difference for cover test, NPC, Heterophria AC/A, far and near phoria, break point of PRC, and recovery point of PRC.

Evaluation of Applicability of Webtoon Content for Visual Function Education and Consultation for the General Public (일반인을 위한 시각기능 교육 및 상담용 웹툰 콘텐츠의 적용 가능성 평가)

  • The author thought about how to explain visual functions in an easy-to-understand way to customers visiting the optical shops and provide easy customer consultation, and attempted to evaluate the usability and value of the educational webtoon for patient consultation by applying it to the optical shops's visitors. Survey study was conducted targeting 152 customers who visited optical shops A in Incheon from January to December 2023. In this study, consultation was conducted using educational webtoons to help the general public understand concepts that are difficult to understand, such as binocular vision function, the concept of heterophoria, prism lenses, and binocular balance. The areas of 'understanding', 'interest', 'usability', 'necessity', and 'expression of expertise' felt by the visitors who received counseling were scored and evaluated, and differences according to counseling method were evaluated using an independent sample t-test. Statistical verification was performed. When the optometrist only explained verbally, the score distribution was relatively low: 1.96 points for understanding, 2.06 points for interest, 2.96 points for usability, 4.20 points for necessity, and 3.19 points for expertise. When the optometrist provided explanations along with the book, understanding was 4.14 points, interest was 3.28 points, usability was 3.60 points, necessity was 4.17 points, and expertise was 3.55 points. When the optometrist explained with a webtoon, understanding was 4.57 points, interest was 4.96 points, usability was 4.18 points, necessity was 4.65 points, and expertise was 3.15 points. When explanations were given only verbally, all items showed the lowest scores, whereas consultations using webtoons showed 4 points or more (excellent) in all items except the 'expression of expertise' item. It is evaluated that actively using webtoons in various consultation situations that occur in optical shops will help increase customer interest and concentration.
