구미강활탕 및 발효 구미강활탕 추출물의 급성독성 연구 (Acute Toxicity Study on Gumiganghwal-tang and Fermented Gumiganghwal-tang Extracts)

  • Objectives : Traditional medicine Gumiganghwal-tang (GT) has been used in Asia to treat inflammatory diseases including common cold, pain, fever, and algor. In this study we investigated the acute toxicity and safety of GT and fermented GT (FGT). Methods : Acute toxicity and safety were evaluated in male and female ICR mice orally administered 0 (control) and 2,000 mg/kg of GT and FGT. After the administration of GT and FGT, we observed mortality, body weight, clinical symptoms. After necropsy, organ weights were measured and blood analysis was performed. Results : There was no mortality and clinical symptoms according to the administration of GT and FGT. Comparing with control group, there were no significant alterations on the organ weight, complete blood cell count and biochemical parameters. Conclusions : Median lethal dose of GT and FGT considered to be over 2,000 mg/kg in both male and female mice, and recognized as safe with no toxicity.

Drug-herb interactions: Mechanisms involved and clinical implications of five commonly and traditionally used herbs

  • Herbal remedies are commonly used by patients worldwide. Because these herbal preparations share the same metabolic and transport proteins with prescribed medicines, the potential for a drug-herb interaction is substantial and is an issue of significant concern. This review paper summarizes drug-herb interactions involving inhibition or induction of cytochrome P450 enzymes, drug transporters as well as modulation of drug pharmacodynamics. An increasing number of in vitro and animal studies, case reports and clinical trials evaluating such interactions have been reported, and implications of these studies are discussed in this review. The most commonly implicated drugs in the interaction include anticoagulants, antiplatelets, immunosuppressants, anti-neoplastics, protease inhibitors, and some antidepressants. Pharmacokinetic and/or pharmacodynamic interactions of five commonly used herbal remedies (danshen, garlic, Ginkgo biloba, ginseng, and St John's wort) with these drugs are presented, with focus of discussion being the potentials for interaction, their mechanisms and clinical implications. There is a necessity for adequate pharmacovigilance to be carried out in minimizing unanticipated but often preventable drug-herb interactions.

생약 중 Fenpyroximate의 분석법 연구 (Analytical Methods of Fenpyroximate in Herbal Medicines)

  • Fenpyroximate는 생약 재배시 살응애제로 사용되는 피라졸계 살충제이다. 본 실험은 HPLC-PDA와 HPLC-MS/MS를 이용하여 생약 중 fenpyroximate의 잔류분석법 확립을 위해 수행하였다. 생약(감초와 홍화) 중 잔류하는 fenpyroximate를 아세톤으로 추출하고, 추출액을 다시 포화식염수와 dichloromethane을 이용하여 액-액 분배한 후 아미노프로필 카트리지와 florisil로 정제한 것을 HPLC-PDA와 HPLC-MS/MS로 정량하였다. 생약별로 fenpyroximate를 2 또는 3 수준으로 첨가하여 수행한 회수율 범위는 72.0~106.4%이었으며 이때의 변이계수는 0.2~4.4였다. 따라서 본 분석법은 생약 중의 fenpyroximate 잔류 분석법으로서의 충분한 재현성과 감도를 나타내었다.

기허가 한약제제를 대상으로 한 인지장애 치료 유효 약물 탐색 - 인지장애 임상연구를 위한 예비연구 (Exploring the Effective Herbal Prescription for Cognitive Disorder Treatment among Licensed Herbal Medicines in Korea - A Preliminary Study for Clinical Trial of Cognitive Disorders)

  • It is necessary to investigate the efficacy of commercial Korean herbal medicine for cognitive disorder. The purpose of this study is to select candidates among licensed herbal medicines that are expected to be effective in the treatment of cognitive disorders and can be used in future clinical trial. From laboratory experiments, we first found individual single herbal drugs that could be effective for cognitive disorder, and then through experts recommendation, selected five priorities of single herbal drug and obtained the information of the best herb as a combination of each herbal drug. To derive the final herbal prescriptions, we searched the KFDA drug information system for licensed herbal medicines containing each drug and its combination. As a result of laboratory experiments and experts recommendation, we found that the five effective single herbal drugs for cognitive disorders. They are Ginseng Radix, Acori Graminei Rhizoma, Cyperi Rhizoma, Coptidis Rhizoma, Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata(Pinelliae Rhizoma, Hoelen cum Pini Radix, and Rehmanniae Radix Preparata were tied for $5^{th}$). And licensed herbal prescriptions derived from the planned search are Palbohoichoon-tang, Taehwa-hwan, Bosim-hwan, and so on. Among these, in consideration of the feasibility of research and possibility of success in development, Yukgunja-tang, Samhwangjichul-hwan can be selected as future study subjects. Through experimental studies and expert recommendations, we have derived herbal prescriptions that can be effective in treating cognitive disorders from licensed herbal medicines.

건조방법에 따른 한약재 중 이산화황 잔류량 및 지표성분 변화 (The Variation of Residual Sulfur Dioxide and Marker Components of Herbal Medicines during Drying Process)

  • The residual sulfur dioxide in the five herbal medicines dried with burning bituminous coal for 9 hours showed from 339 (Glycyrrhizae Radix) to 1,138 mg/kg (Zingiberis Rhizoma) measured right after drying. The residual sulfur dioxide, from 70.1 to 95.5%, was remained in the above five matrices even after 30 days passed. The marker components concentration in above five herbal medicines like glycyrrhizic acid, betaine et al., were almost same after drying in oven and with burning bituminous coal.

  • In this study, the inhibitory effect on advanced glycation end products (AGEs) formation of 43 Korean herbal medicines has been evaluated. Among them, 16 Korean herbal medicines were showed to have significant effect ($IC_{50}$; <50 ${\mu}g/ml$) compared to positive reference, aminoguandine ($IC_{50}$: $76.47{\pm}4.81{\mu}g/ml$). Especially, five herbal medicines, Rubus coreanus (leaves, $IC_{50}$: $4.49{\pm}0.03{\mu}g/ml$), Rubus coreanus (twigs, $IC_{50}$: $3.80{\pm}0.34{\mu}g/ml$), Ampleopsis brevipedunculata (stems, $IC_{50}$: $7.43{\pm}0.09{\mu}g/ml$), Lindera erythrocarpa (leave, $IC_{50}$: $8.14{\pm}0.20{\mu}g/ml$), and Lindera erythrocarpa (stems, $IC_{50}$: $3.69{\pm}0.14{\mu}g/ml$) showed more potent inhibitory activity (approximately 9-20 fold) than the positive control aminoguanidine.

  • Advanced glycation end products (AGEs) have been postulated to play a central role in the development of diabetic complications. A variety of different agents that inhibit AGEs have been under investigation. In this study, 62 herbal medicines from China and Vietnam have been investigated with an in vitro evaluation system using AGEs formation inhibitory activity. Of these, 5 herbal medicines ($IC_{50}$ < $5{\mu}g/ml$) were found to have significant AGEs formation inhibitory activity. Particularly, herbal medicines Albizia odoratissima (twigs and leaves), Rhododendron spinuliferum (twigs and leaves), Dioscorea cirrhosa (stems and leaves), Illicium verum (stems and leaves) and Aglaia perviridis (stems and leaves), showed more potent inhibitory activity (approximately 16-26 fold) than the positive control aminoguanidine ($IC_{50}=76.47{\mu}g/ml$).

  • It is important to establish the safety of herbal medicine because of its frequent and widespread use in Korea. Several studies on the safety of herbal medicine have been performed and there have been rare serious adverse drug reactions from those reports in Korea. However, the results are not strongly supported because of not adopting appropriate enough research methodology as to make the safety issue clear. For improving the quality of the safety research on herbal medicine. including investigations of drug induced liver injury (DILl). the aim of this study was to suggest herbal medicine-induced liver injury investigation forms for performing reasonable safety research. After a systematic review of the preceding studies regarding herbal safety in Korea was performed in 2008, we assessed the quality and the limitations of the primary studies. Two investigation forms for herbal safety research were made as a following step. one a basic investigation form for herbal safety research and the other an advanced investigation form for suspected DILl cases, Those forms include the essential informations and data needed to make an appropriate assessment of whether DILl occurred during or after the use of herbal medicine. Guidelines for using those forms and other recommendations were also suggested. More rigorous studies are required for answering the safety issue of herbal medicine as well as the efficacy issue. We hope for wide use and improvement of those investigation forms in the study of herbal safety by many researchers for establishing better evidences in Korea.

중국 약용식물 추출물의 알도즈 환원 효소 억제 효능 검색 (VII) (Screening of Chinese Herbal Medicines with Inhibitory Effect on Aldose Reductase (VII))

  • Aldose reductase (AR) has been shown to play an important role in the development of the diabetic complications. To discover novel treatments for diabetic complications from natural sources, 59 Chinese herbal medicines have been investigated for inhibitory activities on AR. Among them, 10 herbal medicines, Catalpa fargesii (stem and leaf), Saussurea Laniceps(whole plant), Alnus nepalensis(stem and leaf), Swertia macrosperma (whole plant), Woodfordia fruticosa (stem and leaf), Elsholtzia bodinieri (whole plant), Elsholtzia fruticosa (whole plant), Rosa multiflora (fruit), Nardostachys chinensis (whole plant), Eurya groffii (stem and leaf) exhibited a significant inhibitory activity compared with 3,3-tetramethyleneglutaric acid (TMG) as positive control. Particularly, 4 herbal medicines, C. fargesii (stem and leaf), S. Laniceps (whole plant), A. nepalensis (stem and leaf), S. macrosperma (whole plant) showed two times more potent inhibitory activity than TMG ($5.37{\mu}g/ml$).

  • In this study, 64 Korean herbal medicines have been investigated with an in vitro evaluation systems using glycation end products (AGEs) formation inhibitory activity. Of these, 30 herbal medicines ($IC_{50}&lt;50{\mu}g/ml$) were found to have significant AGEs formation inhibitory activity. Of these, four herbal medicines ($IC_{50}&lt;5{\mu}g/ml$) were found to have significant AGEs formation inhibitory activity. Particularly, Cornus controversa (branches and leaves), Acer ginnala (stems and leaves), Platycarya strobilacea (flowers) and Picrasma quassioides (stems), showed more potent inhibitory activity (approximately 17-27 fold) than the positive control aminoguanidine ($IC_{50}=77.04{\mu}g/ml$).
