헬리코박터 파일로리 감염과 위식도역류질환 (Helicobacter pylori and Gastroesophageal Reflux Disease)

  • Helicobacter pylori (H. pylori) infection is associated with gastroduodenal diseases such as gastritis, peptic ulcer diseases, gastric adenocarcinoma, and mucosa-associated lymphoid tissue lymphoma. Furthermore, various extragastroduodenal diseases have been suggested to be related with H. pylori infection. Although no single factor has been considered the cause of gastroesophageal reflux disease (GERD), a negative association was found between the prevalence of H. pylori and GERD severity. Additionally, the role of eradication therapy of H. pylori in reflux esophagitis is controversial, and a significant correlation was found between successful H. pylori eradication rate and reflux esophagitis development. H. pylori infection remains an inconclusive and important issue in GERD. Thus, more experimental studies are necessary to elucidate the potential mechanisms.

  • H. pylori가 공식적인 발암인자로 지정된 이후, 많은 연구가 H. pylori감염과 위암종 발생간의 연관관계를 지지해 왔다. 비록 몇몇 연구가 위암종 발생에 대한 H. pylori 감염의 부정적인 영향에 대한 증거를 제공하기는 했지만, 이들 연구가 H. pylori 감염률을 과소평가했다는 새로운 증거들이 제시되고 있다. 이 종설에서는 위암과 H. pylori의 역학적인 생태, H. pylori 감염과 위암종 발생의 연관관계를 지지하는 증거들, 제시된 위암종 발생의 기작, 그리고 H. pylori 감염의 위암종 발생에서의 잠재적인 병리학적 역할을 기존 연구들을 바탕으로 살펴보았다. 결론적으로 H. pylori 감염은 위암종 발생과 긍정적인 연관관계를 가진다는 것을 밝힐 수 있었다.

Polyclonal Antibody Against the Active Recombinant Helicobacter pylori Urease Expressed in Escherichia coli

  • Helicobacter pylori is the etiologic agent of human gastritis and peptic ulceration and produces urease as the major protein component on its surface. H. pylori urease is known to serve as a major virulence factor and in a potent immunogen. In order to express the recombinant urease at a higher level, a DNA fragment containing the minimal H. pylori urease gene cluster was subcloned into a high copy-number vector. The recombinant H. pylori urease expressed in an E. coli strain that was grown in a rich medium supplemented with added nickel was purified to near homogeneity by using DEAE-Sepharose, Superdex HR200, and Mono-Q (FPLC) columns and the purified enzyme possessed the specific activity of 1255 U/mg. Polyclonal antibodies raised against the purified recombinant H. pylori urease were shown to be very specific when subjected to Western blot analysis, in which crude extracts from the H. pylori ATCC strain and the recombinant E. coli strains expressing various bacterial ureases were exnmined for cross-reactivity.

Susceptibility of various Helicobacter pyloris to New Antiulcer Agents

  • Helicobacter pylori (H. pylori) is a microaerophilic spiral bacterium and infection by it in the human stomach causes gastritis, furthermore, it is considered to be involved in the pathogenesis of peptic ulcers and the development of gastric carcinoma. We assessed the inhibitory activity of new antiulcer drugs against Helicobacter pylori. The activities of new antiulcer agents against Helicobacter pylori strains were determined by the standard agar dilution method with blood agar base #2, supplemented with 5% sheep blood and 4 antibiotics to support growth of these organisms. They were inoculated by multipoint inoculator and incubated at 37$^{\circ}C$ for 3 days under microaerophilic atmosphere. The MIC of antiulcer agents was the lowest concentration that inhibited visible growth of these organisms. According to results of various biochemical tests, these bacteria were identified as Helicobacter pylori strains. And the MIC results showed that the strains were very susceptible to omeprazole and YJA20379s. Some of YJA20379s were more potent than omeprazole. These results suggest that our new antiulcer drugs have potent inhibitory activity against Helicobacter pylori, so that our new antiulcer drugs might be useful for the clinical eradication of gastrointestinal Helicobacter pylori.

위염 및 Helicobacter pylori에 미치는 Amomi Semen Butanol 추출물의 영향 (Effects of Amomi Semen Butanol Fraction on Gastritis and Helicobacter pylori)

  • In a preliminary screening of plant extracts for the antigastritic and anti- Helicobacter pylori (H. pylori) actions in rats, the ethanol extract of Amomi Semen (AS) showed positive activity in HCl Ethanol-induced gastric lesions and H. pylori. Among the systematic fractions of hexane, chloroform, butanol and water, the most potent butanol fraction significantly reduced HCl, Ethanol-induced gastric lesions at the oral dose of 350 mg/kg. Also butanol fraction has an inhibitory effect on the growth of H. pylori $(MIC=1.43\;{\mu}g/mL)$. In pylorus ligated rats, butanol fraction showed decrease in the volume of gastric secretion and acid output, of which effects were stronger in other fractions. We isolated 6 subfractions by column chromatography. The protective effects of 6 subfractions of Amomi Semen were also significant in the HCl, Ethanol induced gastric lesion model. These results might suggest that they had inhibitory action in gastric lesion through inhibition of gastric acid secretion. Butanol fraction of AS can be applied as treatment of H. pylori. Butano fractions and ethanol extract of AS was carried out or the development of a new gastroprotective supplementary product.

백김치 유래 유산균을 이용한 요구르트의 Anti-Helicobacter pylori 활성 (Anti-Helicobacter pylori Activity of Yogurt Fermented with Lactic Acid Bacteria from Baikkimchi)

  • 백김치로부터 분리된 유산균으로 제조된 요구르트를 냉장 보관하는 동안 미생물학적 및 물리화학적 특성 및 Helicobacter pylori ATCC 43504에 대한 항균 활성을 조사하였다. 요구르트의 유산균수, 적정산도, 점도 및 총 고형물 함량은 사용된 균주에 따라 유의적인 차이가 있었으나, 발효 직후부터 7일간 저장하는 동안 유의할만한 차이 없이 일정하게 유지되었다. Lactobacillus brevis BK11과 Lactobacillus paracasei BK57 균주로 발효시킨 요구르트는 인공 위액과 담즙액에 대해 다른 균주들 보다 강한 저항성을 보였다. 한편, 이들 유산균으로 제조한 요구르트 내에 존재하는 유산 생성량은 상대적으로 높았으므로 H. pylori와 혼합 배양한 결과 대조구에 비해 유의적인 항균 효과를 나타낸 것으로 추정되었다. 특히, L. brevis BK11에 의해 발효시킨 요구르트에 의해선 AGS 세포에 대한 H. pylori의 부착을 억제할 수 있었고, 이들이 생산하는 urease의 활성을 낮추는데도 효과적이라는 것을 확인하였다.
