Lipid soluble ginseng extract was administered by oral route in doses of 600 mg/patient daily in cancer patients over 2 months and 6 months. The administration of ginseng extract in cancer patients maintained the ratio of CD4/CD8 and number of the natural killer cell in the normal range during the administration period. Also its administration showed a positive effect on tumor values in 87.5% of patients in 2 month-group and in 50% of patients in 6 month-group, as determined by various cancer markers. Liver and kidney functions maintained normal condition during administration period of 6 months. Although there was no statistical significance, these data suggest that lipid soluble ginseng extract may be useful as an adjuvant therapeutic agent and nutritional supplement for the improvement of immune function and health in cancer patients. This study would provide the basis for the research in which the antitumor and immunopotential activity of lipid soluble ginseng extract for cancer patients are evaluated in formal clinical trial with statistically significant patient number.

This study investigates the characteristics of patients hospitalized at a convalescent hospital, by considering patient groups based on their medical record information. It further analyzes and correlates the factors, thus providing basic data required to improve the patient classification system at convalescent hospitals. The data includes total information of 213 medical records of patients discharged from a convalescent hospital in the Jeonbuk region during a period of one year, from January to December 2016. The study examines and correlates the days of hospitalization and the number of diseases, revealing a positive correlation having a correlation coefficient, thereby indicating that infliction with a greater number of diseases results in longer hospitalization. Based on these findings, the study raises the need to revise and supplement items on the patient assessment report to help determine the patient groups and identify medical efforts to be actually provided to patients. In addition, a proper care service system for each patient group based on their respective problems that are intensively managed according to the patient groups will be an essential element in the efficient management of convalescent hospitals. Furthermore, an important task addressed will be in managing the health of the elderly population at the national level.

This study examined the improving effect of DFA IV obtained from bacterial fermentation on the gut health. The effects of the dietary DFA IV on the intestinal mass, short chain fatty acids production and pH were evaluated in vivo. Sprague-Dawley rats were fed the 0% (control) or 1% DFA IV supplemented diets for 3 weeks. Supplementation of DFA IV resulted in a significant increase in cecal tissue and wall weights. Together with the lowering of the cecal and colonic pH, the amount of acetate and butyrate increased by 1.6 and 3.2 fold of the control group in the cecum, respectively, in the rats fed DFA IV diets. The DFA IV diet also significantly increased the cecal lactate 1.5 fold compared to the control diet, indicating that dietary DFA IV stimulated the growth of lactic acid bacteria and bifidobacteria in the intestine. Based on the above results, it is concluded that the dietary DFA IV may be used as a putative prebiotic supplement.

Background : The Clostridium difficile is the most important identifiable cause of nosocomial infectious diarrhea and colitis, which lengthens hospital stay. Recently incidence of C. difficile has been increasing in an university hospital, and an intervention for prevention and control of C. difficile associated diarrhea (CDAD) was in prompt need. Methods : Subjects were the patients in the neurosurgical intensive care unit(NCU) where C. difficile was most frequently isolated. To increase participation of various departments, we used the CQI method, because management of CDAD requires a wholistic approach including control of antibiotics, barrier precaution and environmental cleaning and disinfection. Duration of the CQI activities was 9 months from April to December 1999. Results : The identified problems were misuse and overuse of antibiotics, lack of consciousness of medical personnels and the possibility of transmission from the contaminated environment and tube feeding. Education for proper use of antibiotics and management of C. difficile infection, use of precaution stickers, supplement of handwashing equipments, emphasis on environmental disinfection, and the change of the process of tube feeding were done. The CDAD rate in NCU was significantly decreased after the CQI program (8.6 case per 1,000 patient days from January to April 1999 vs 4.8 from May to December 1999). The distribution of neurosurgical wards including NCU among the total number of isolated C. difficile from the clinical specimens dropped from 49.4% in January to April to 33,7% in May to December. The average hospital stay of the neurosurgical department changed from 19.6 days to 15.2 days. Also, the effect of the CQI activities for C. difficile may have affected the incidence of vancomycin resistant enterococci (VRE). Duration and dosage of certain antibiotics used in the NS department were decreased. The distribution of neurosurgical department in the number of VRE isolated patients declined from 18.4% to 11.1%. Conclusion : Infection control of resistant organisms such as C. difficile is likely to be successful when management of environmental contamination an collaborative efforts of decreasing the patients' risk factors such as antibiotics management and decreasing the length of hospital stay come simultaneously. For this work, related departments need to actively participate in the entire process under a common target through discussions for identifying problems and bringing up solutions. In this respect, making use of a CQI team is an efficient method of infection control for gathering participation and cooperation of related departments.

The aim of this study is to propose suggestions for establishing Korean regulatory standards of infant formula. Accordingly, the regulatory standards for food contaminants and additives in Korean infant formulas were compared and analyzed with those in CODEX, EU, Australia and New Zealand. Several suggestions for regulations were found from different countries. Firstly, it is advisable that additives for nutrient supplement of infant formula be classified as types of nutrients. Secondly, it is proposed that guidelines should be set on the maximum amount of additives in infant formula. Thirdly, pathogens such as Staphylococci and Salmonella of infant formula should be regulated. Finally, present regulations need to establish the maximum permissible levels of some pesticides, Pb and Al, that other countries are already regulating. These proposed recommendations would broaden the scope of infant formula regulatory standards needed for infants' health.

Sounds and music have been used in various ways in the East and the West, as it is well known that they influence the human mind and health a lot. Recently, there have been many studies regarding the remedy through sounds and music in the West and they have made good effects in the actual therapy. In the East, people knew that sounds and music would make great effects on human mind long time ago and they placed importance on sounds and music. Although there is a Sound Therapy in Oriental Medicine, it is not studied sufficiently in modern Oriental Medicine. It does not have clear standards for the clinical application and has not been frequently used. Accordingly, Yukjagyeol which has long been used in Korean Medicine and Younggamudo which was made by Kim Il-bu in late Chosun were compared from the perspective of acoustics and demonstrative treatment theory of Korean Medicine. Younggamudo by Kim il-bu allot the five sounds such as 'Eum, Ah, Eo, Ih and Uh' to 'spleen, lung, liver, heart and kidney' respectively. As the five sounds are all vowels and vibrate the vocal band as much as possible, they vibrate and resonant the respective organs to help them to function and promote circulation. In oriental medicine, there is 'Singing and Dancing Treatment'. They say songs and dances nurtures personality, supplement blood stream and comfort the mind. As the five sounds of Younggamudo take the melody while vibrating the vocal band sufficiently, when it passes from Youngto Ga(Songs) we may dance moving our bodies according to the rhythm. Therefore Gongbeop of Younggamudo helps the functionality of human organs and promotes blood circulation. As Yukjagyeol is Sabeop(discharging method), it should be used for those who have surplus Sagi and should not use for Heojeung. As Younggamudo is Bobeop(supplementing method), it is not for Shiljeung but for Heojeung which lacks of Jeonggi. It is considered that healthy people without specific disease can use it as a preventive method of 'Chimijeong' to maintain their physical and mental health. It is considered that we can have useful effects if we apply Yukjagyeol and Younggamudo dialectically with the concept of Bosa. It is considered that concrete application to Sound Gigong can be possible by searching for the methodology for empirical further study and clinical experiments and clinical application in the future.
"Act on Remedies for Injuries from Medical Malpractive and Mediation of Medical Disputes)" has been enacted to solve medical dispute. In addition, mediation and arbitration procedures have started since April 8th, 2012 from the Establishment of Korea Medical Dispute Mediation and Arbitration Agency. The average initiation rate of mediation for the past three years turned out to be 43%. Hereupon, Establishment of Korea Medical Dispute Mediation and Arbitration Agency has created a solution for automatic initiation if relevant to particular conditions to improve initiation rate of mediation procedures and passed it through the Assembly plenary session in May, 2016 and promulgated on the 30th of the same month. However, even if mediation procedure initiation rate is increased, there is no guarantee for mediation establishment rate to be improved according to current law. If Establishment of Korea Medical Dispute Mediation and Arbitration Agency intends to increase aforementioned value, automatic initiation is not the only solution. Instead, it seems to be a major assignment to identify fundamental reasons for why major health care facilities have not participated in it and to restore reliability on them. In addition, among crimes specified on the Article 268 of Criminal Act in the Article $51^*$ of "Act on Remedies for Injuries from Medical Malpractive and Mediation of Medical Disputes)", revision must be made so that the clause of clue and death by occupational or gross negligence is applied. Furthremore, it is suggested to supplement previously insufficient policies with the operation so that mediation procedures created by Establishment of Korea Medical Dispute Mediation and Arbitration Agency are stably settled in the perspective of medical institutions including the establishment of new conditions for medical institutions founders or health and medical service personnel to claim the proxy payment for damage.
Determining the origin of the components in commercially available gelatin, a purified protein derived mostly from pig skin and bovine tissue, is a challenge, leading to concerns on the grounds of religious beliefs and health. Therefore, regular monitoring of labeling compliance by food control authorities is also necessary. In this study, we monitored the origin of gelatin capsules from 181 commercial dietary supplements that were available for purchase on the internet, using species-specific PCR assays. Fifty five products were labeled correctly, declaring that they used bovine-, fish- and plant-derived gelatin, whereas the other 126 capsules were labeled "gelatin" without specifying the origin. Gelatin in these capsules was obtained from cattle (n = 51), pigs (n = 31), or both (n = 44). Therefore, it is important to declare all of the raw materials used to produce gelatin capsules on the labels to best protect consumers' rights, religious beliefs, and health.
Today's rapidly developing health technology is accumulating vast amounts of data through medical devices based on the Internet of Things in addition to data generated in hospitals. The collected data is a raw material that can create a variety of values, but our society lacks legal and institutional mechanisms to support medical Big Data. Therefore, in this study, we looked at four major factors that hinder the use of medical Big Data to find ways to enhance use of the Big Data based healthcare industry, and also derived implications for expanding domestic medical Big Data by identifying foreign policies and technological trends. As a result of the study, it was concluded that it is necessary to improve the regulatory system that satisfies the security and usability of healthcare Big Data as well as establish Big Data governance. For this, it is proposed to refer to the Big Data De-identification Guidelines adopted by the United States and the United Kingdom to reorganize the regulatory system. In the future, it is expected that it will be necessary to have a study that has measures of the conclusions and implications of this study and to supplement the institutional needs to play a positive role in the use of medical Big Data.
Purpose: The skin pH is maintained by epidermal lactate, free fatty acids (FFAs), and free amino acids (FAAs). As a significant determinant of skin health, the skin pH is increased (less acidic) under abnormal and aged skin conditions. In a search for dietary alternatives that would promote an acidic skin pH, this study investigated the dietary effects of Lactobacillus plantarum CJLP55 isolated from Korean kimchi on the skin pH, and epidermal levels of lactate, FFAs, and FAAs in adult subjects. Methods: Seventy eight subjects (mean age $24.9{\pm}0.5years$, range 19 ~ 37 years) were assigned randomly to ingest CJLP55, Lactobacillus strain from kimchi, (n = 39, CJLP group) or placebo supplements (n = 39, placebo group) for 12 weeks in a double-blind, placebo-controlled trial. Skin pH and epidermal levels of lactate, FFAs and FFAs were assessed at 0, 6 and 12 weeks. Results: Although significant decreases in skin pH were observed in both the CJLP and placebo groups at 6 weeks, the skin pH was decreased significantly only in the CJLP group at 12 weeks. In parallel, the epidermal level of lactate in the CJLP group was also increased by 25.6% at 12 weeks. On the other hand, the epidermal level of FAAs were not altered in the CJLP and placebo groups, but the epidermal level of total FFAs, including palmitic acid and stearic acid, was lower in the CJLP group than in the placebo group over 12 weeks. The changes in the other FFAs, such as palmitoleic acid and oleic acid, were similar in the CJLP and placebo groups over 12 weeks. Conclusion: Overall, a dietary supplement of CJLP55 promotes acidic skin pH with a selective increase in epidermal lactate in adult subjects.
