APEC SEN Mental Health Training Curriculum for Seafarers across the APEC Region

  • In light of the unprecedented occupational health and safety concerns that have been raised in the wake of the COVID-19 pandemic, the importance of seafarers' mental health has been significantly underlined. Despite the fact that there has been a significant amount of research on the psychological threats of seafarers' mental health, mutual cooperative actions on this issue have not been properly coordinated among seafarers' relevant bodies and stakeholders in APEC region. This paper introduces the APEC SEN's initiatives that have been undertaken to improve the mental health of seafarers, in collaboration with medical professionals, seafarers' unions, maritime administrations, and other stakeholders, through the development of training programmes and materials for both seafarers and modulators, resulting in a more effective approach to mental healthcare promotion.

A Concept Analysis of the Quality of Life for the Children (아동의 삶의 질에 대한 개념 분석)

  • Purpose: The purpose of this study was to attempt an accurate understanding of the quality of life for the children and provide basic materials for application to nursing research, intervention and further theoretical development. Method: Data obtained from such research methods as the lexical and literature review were analysed according to Walker and Avant's(1995) process of concept analysis. Results: As a result of analysis, it was found that the attributes of the concept of the quality of life for the children included value/acceptance, family relationship, safety, good relationship with friends, right of children, school life, parent relationship, leisure, economical stability, not child abuse, and health. There was the coexistence between antecedent element and the outcomes of subjective judgement - health, family, economic strength, education, friends, welfare, social-cultural environment which indicates the coexistence of positive and negative emotions. Conclusion: nursing intervention should be provided for children by concentrating focus on the coordination of internal and external situations in order to prompt them to express their life with positive emotions. Experiential reference was used to suit the purpose of this study based on the use of objective and subjective indicators, and the development of the instrument considering the children were proposed because of the absence of the unified measurement instrument.

A sampling and estimation method for monitoring poultry red mite (Dermanyssus gallinae) infestation on caged-layer poultry farms

  • Background: The poultry red mite, Dermanyssus gallinae, is a serious problem in the laying hen industry worldwide. Currently, the foremost control method for D. gallinae is the implementation of integrated pest management, the effective application of which necessitates a precise monitoring method. Objectives: The aim of the study was to propose an accurate monitoring method with a reliable protocol for caged-layer poultry farms, and to suggest an objective classification for assessing D. gallinae infestation on caged-layer poultry farms according to the number of mites collected using the developed monitoring method. Methods: We compared the numbers of mites collected from corrugated cardboard traps, regarding with length of sampling periods, sampling sites on cage, and sampling positions in farm buildings. The study also compared the mean numbers of mites collected by the developed method with the infestation levels using by the conventional monitoring methods in 37 caged-layer farm buildings. Results: The statistical validation provided the suitable monitoring method that the traps were installed for 2 days on feed boxes at 27 sampling points which included three vertical levels across nine equally divided zones of farms. Using this monitoring method, the D. gallinae infestation level can be assessed objectively on caged-layer poultry farms. Moreover, the method is more sensitive than the conventional method in detecting very small populations of mites. Conclusions: This method can be used to identify the initial stages of D. gallinae infestation in the caged-layer poultry farms, and therefore, will contribute to establishment of effective control strategies for this mite.

Measures for a closer-to-real estimate of dietary exposure to total mercury and lead in total diet study for Koreans

  • Previous Korean total diet studies (KTDSs) have estimated dietary exposure to toxic chemicals based on 110-120 representative foods selected from over 500 foods appeared in the Korea National Health & Nutrition Examination Surveys (KNHANES), which would result in a possible underestimation. In order to find measures for a closer-to-real estimate of dietary exposure to heavy metals, this study examined the feasibility of mapping foods to the representative foods in the KTDS by comparing estimates. In mapping, those foods not analyzed in the 2009 KTDS (443 out of 559 foods appeared in the 2007 KNHANES) were mapped to the 114 representative foods used in the 2009 KTDS based on the closeness in regards to biological systematics and morphological similarity. Dietary exposures to total mercury and lead were re-estimated using the content of total mercury and lead in 114 foods analyzed in the 2009 KTDS, food intake, and individual's own body weight for respondents in the 2007 KNHANES instead of mean body weight of Koreans used in the 2009 KTDS. The re-estimates of exposure with mapping were approximately 50% higher than the original estimates reported in the 2009 KTDS. In addition, mapping enabled the comparison of percentile distribution of the exposure among populations of different age groups. In conclusion, estimates via mapping resulted in a more comprehensive estimation of dietary exposure to heavy metals present in foods that Koreans consume.

  • Background: The industrial revolution that took place in the United Kingdom (UK) between 1760 and 1830 led to profound social change. Occupational medicine was concerned with the diagnosis, treatment, and prevention of occupational diseases, that is, diseases directly caused by exposure to workplace hazards. A similar pattern of development has occurred globally. Methods: A review of relevant literature. Results: The international conceptualization and development of occupational health occurred during the 20th century. A new paradigm for occupational health has emerged that extends the classical focus on what might be termed "health risk management" that is, the focus on workplace hazards and risk to health to include the medical aspects of sickness absence and rehabilitation, the support and management of chronic noncommunicable diseases, and workplace health promotion. Conclusion: The future strategic direction for occupational health will be informed by a needs analysis and a consideration of where it should be positioned within future healthcare provision. What are the occupational health workforce implications of the vision for occupational health provision? New challenges and new ways of working will necessitate a review of the competence and capacity of the occupational health workforce, with implications for future workforce planning.

Determinants of untreated experiences among persons with physical disability (일부 지체장애인의 미치료 경험에 영향을 주는 결정요인 분석)

  • Objective: This study is examined the factors affecting forms of untreated experiences in persons with physical disability. Method: The data collected from 461 persons with physical disability in community. Based on the Behavioral Model of Andersen, predisposing, enabling, and need factors are hypothesized to affect persons with physical disability's untreated experiences. The data were analyzed by statistical methods such as frequency and multiple logistic regression analysis. Result: Participation rates of untreated experiences were 26.7%. The significant factors of persons with physical disability's untreated experiences are predisposing factors (gender, partner, and religion), enabling factors (income, private insurance, information of assistive device, disability discrimination, and subjective discrimination), and need factors(subjective health status health screening and chronic disease). Also untreated experiences related to gender, subjective health status, health screening, and chronic disease factors using multiple logistic regression analysis. Conclusion: Implications of the findings were discussed and the recommendations for the improvement of health care utilization, subjective health statue. Especially, development of health education and program should be needed persons with physical disability.

Research trend of health life expectancy using oral health indicators (2010-2020) (구강건강지표를 활용한 건강수명 연구경향 분석: 최근 10년간의 논문분석(2010-2020))

  • Purpose: The purpose of this article is to clearly describe research trends on health life expectancy using oral health indicators that have been published from 2010 to 2020 then suggest the direction of future research. Methods: Online academic databases in English (PubMed, Web of Science and Embase) were used to find those articles by applying a variety of keywords, including terms (adjusted life year, adjusted life expectancy, dental and oral). We identified relevant articles based on the following classification method of Mathers: (1) health gaps, (2) health expectancies. Results: Among 1,728 articles from the online databases, the final 13 studies satisfied the inclusion criteria and were selected for analysis. Health life expectancy studies indicate that research growth was recently achieved overseas. Among the literature collected in this study, 10 studies using health gap indicators yielded seven Disability-Adjusted Life Year (DALY), and three calculated Quality-Adjusted Life Year (QALY), which differed in the nature of the survey data used in the study measuring DALY and QALY. There are only three health expectancies and the number of papers were smaller than the health gap study. Conclusion: Establishing a foundation to calculate health life expectancy indicators through the development and improvement of oral health level are needed. More studies in the area of health life expectancy estimation research is based on actual prevalence and oral health-related quality of life are also needed.

An Analysis of the Trends of Korean National Health Insurance Administrative Cost (건강보험 관리운영비 추이 분석)

  • Social solidarity, equity in financing, and efficiency in administration have been core issues in the development of Korean health insurance reformation since 1988. This study is to investigate the trend of administrative cost in Korean National Health Insurance from various aspects. For the analysis of administrative cost, the expenditures of each insurance society and the National Health Insurance Corporation are divided into 4 items of (1) insurance benefit, (2) administrative cost, (3) an agency provision accounts, (4) other expenses, and then they are reorganized. The analyses based on 5 types of the health insurance administrative cost showed that efficiency in administration has been improved generally. We, however, should consider qualitative aspects such as customer's satisfaction with health insurance administration, prompt service, control of unjust expenditure (unjust claims), and provision of medical service including health consultation in assessing efficiency of administration. And, in order to connect the administrative costs of health insurance with efficiency, we need to give a fundamentally new definition, which can contain elaborateness of expenditure in details including the structure and evaluation method of administrative costs. It may be necessary to develop new indicators or analyzing methods hereafter.

  • In recent years, nanotechnology has revolutionized global healthcare and has been predicted to exert a remarkable effect on clinical medicine. In this context, the clinical use of nanomaterials for cancer diagnosis, fertility preservation, and the management of infertility and other pathologies linked to pubertal development, menopause, sexually transmitted infections, and HIV (human immunodeficiency virus) has substantial promise to fill the existing lacunae in reproductive healthcare. Of late, a number of clinical trials involving the use of nanoparticles for the early detection of reproductive tract infections and cancers, targeted drug delivery, and cellular therapeutics have been conducted. However, most of these trials of nanoengineering are still at a nascent stage, and better synergy between pharmaceutics, chemistry, and cutting-edge molecular sciences is needed for effective translation of these interventions from bench to bedside. To bridge the gap between translational outcome and product development, strategic partnerships with the insight and ability to anticipate challenges, as well as an indepth understanding of the molecular pathways involved, are highly essential. Such amalgamations would overcome the regulatory gauntlet and technical hurdles, thereby facilitating the effective clinical translation of these nano-based tools and technologies. The present review comprehensively focuses on emerging applications of nanotechnology, which holds enormous promise for improved therapeutics and early diagnosis of various human reproductive tract diseases and conditions.

Isolation and Development of Quantitative Determination of Torilin from the Torilis Fructus (사상자 (Torilis Fructus)의 지표성분으로 Torilin의 분리 및 함량분석법 개발)

  • For the propose of the quality control of Torilis Fructus, one major compound was isolated by several column chromatograpies and identified by its spectral data as torilin (8,11-dihydroxy-8-angeloyl-11-acetyl-4-guaien-3-one). A quantitative analysis of torilin using HPLC method showed that the average contents at 230 and 254 nm were $7.167{\pm}0.168mg/g\;and\;7.052{\pm}0.160mg/g$, respectively, in 18 samples collected throughout the regions of Korea and China. This method provided a tool for standardization of the commercial Torilis Fructus.
