일개 종합병원 종합(민간)검진 비용 영향요인 분석 (The analysis of Factors associated with the Health Examination expenditure in a General Hospital based on the cased)

  • Purpose: In this study, the general characteristics of subjects who spent more than a certain amount of cost for general medical examination at the general hospital health promotion center, and the characteristics of disease, family history, and lifestyle (smoking, alcohol, physical activity, oral care) significantly differed in cost expenditure. We intend to provide basic data for establishing an appropriate marketing strategy for comprehensive examination. Method: It was conducted for users who received comprehensive checkups at a health promotion center at a general hospital in Seoul. The research data collection period is for 979 people who performed comprehensive examinations from January 2019 to December 2020. In order to carry out a comprehensive examination, a questionnaire before the examination was distributed to the subjects who visited the hospital to prepare, and the investigation was conducted in a way that the subjects of the investigation directly filled in. Results: There was a significant influence on the difference in expenditure for comprehensive examination according to the gender, age, and type of health insurance of the subject. In addition, there were significant differences in expenditure according to the presence or absence of disease and the type of family history. Weight loss, smoking history, smoking period, smoking frequency, drinking history, and drinking frequency all had significant effects on cost expenditure. Also, strength training and oral treatment management showed a significant effect on the cost of comprehensive examination. The number of flossing and interdental brushing was also found to have a significant effect. According to the results of multiple regression analysis, disease history (t=2.683, p<.01) and mean smoking frequency (t=4.315, p<.001) appeared to have the most significant effect on expenditure statistically. In other words, when the subject has a history of disease and when the average number of smoking is large, it means that the comprehensive examination cost is remarkably large. Conclusion: By using these contents, hospitals can further refine the marketing of the examination center. In addition, a more convenient and specialized process should be used by patients by linking the general medical department and the examination center well. In terms of management of operating medical institutions, this can be expected to create patients and increase profits.

일반 방사선검사의 소요 시간 실태조사 (Investigation of the Time Required for General Radiography)

  • In this study, by analyzing the examination time for each procedure, the appropriate workload of radiologic technologist is analyzed based on the actual examination time in the current clinical setting by comparing with the examination time in the radiology field setting of the health insurance review and assessment service. In addition, this result is introduced into the calculation of relate value units; it was attempted to provide accurate and objective evidence in the field of radiology. From May 2020 to December 2021, the study retrospectively investigated the examination times recorded in the electronic medical record and picture archiving and communication system at 5 tertiary general hospitals and 1 general hospital. The total of 16 examination parts are applied in this study, including the head, sinuses, chest, ribs, abdomen, pelvis, cervical, thoracic, lumbar, shoulder, elbow, wrist, hip, femur, knee, and ankle. The minimum number of images that could be obtained per radiation generator was 3.6 images for one hour, and the maximum was 6.4 images. When 50% median of procedure time is calculated, the minimum number of images that could be obtained was 16.7 images and maximum was 35.3 images; in addition, minimum examination time is 1.7 minutes, and maximum time is 3.6 minutes. In conclusion, it is judged that there will be insufficient explanation time for basic infection instructions such as hand hygiene during the examinations in current clinical practice. It is believed that radiologic technologists will contribute to providing higher-quality of radiation examination services to the public by complying with guidelines for work and setting appropriate workload on their own.

Has Income-related Inequity in Health Care Utilization and Expenditures Been Improved? Evidence From the Korean National Health and Nutrition Examination Survey of 2005 and 2010

  • Objectives: The purpose of this study is to examine and explain the extent of income-related inequity in health care utilization and expenditures to compare the extent in 2005 and 2010 in Korea. Methods: We employed the concentration indices and the horizontal inequity index proposed by Wagstaff and van Doorslaer based on one- and two-part models. This study was conducted using data from the 2005 and 2010 Korean National Health and Nutrition Examination Survey. We examined health care utilization and expenditures for different types of health care providers, including health centers, physician clinics, hospitals, general hospitals, dental care, and licensed traditional medical practitioners. Results: The results show the equitable distribution of overall health care utilization with pro-poor tendencies and modest pro-rich inequity in the amount of medical expenditures in 2010. For the decomposition analysis, non-need variables such as income, education, private insurance, and occupational status have contributed considerably to pro-rich inequality in health care over the period between 2005 and 2010. Conclusions: We found that health care utilization in Korea in 2010 was fairly equitable, but the poor still have some barriers to accessing primary care and continuing to receive medical care.

의료기관 종별에 따른 전산화단층촬영장비 보유현황과 검사료간의 관계 (Relationship Between Type of Medical Institutions According to the Equipment List and Inspection Fee Computed Tomography)

  • 본 연구에서는 CT 장비의 국내에서의 보급률과 이용률이 증가되고 있는 흐름과 증가의 변화와 장치의 변화가 검사부위에 나타나는 빈도 등을 정량적으로 파악하기 위하여 현재 우리나라의 의료기관 종별 및 시기별의 의료영상 진단 장비인 CT장치의 보유현황, 검사료 등을 조사하였다. 최근 10년간의 우리나라 의료기관 종별(상급종합병원, 종합병원, 병원, 의원, 치과병원, 보건의료원, 한방병원) 및 연도별(2003~2012)에 따른 CT장치 보유현황, 연도별 CT검사료를 의료기관 종별의 분포를 조사하였다. 우리나라 CT장치는 상급종합병원과 종합병원급 의료기관에서 보유한 총 비율은 2003년에 25.6%, 2011년에 32.5%정도인 반면, 병원과 의원급 의료기관에서 보유한 비율은 2003년에 73.5%와 2009년에 67.5%로 약 70%를 차지하고 있는 것으로 나타났다. 2011년도를 기준으로 하였을 때 상급종합병원과 종합병원에서 총 검사료의 82.4% 정도인 반면, 병원과 의원에서는 총 검사료의 17.6%로 나타났다. CT장비는 병원급은 증가하고 있는 추세이나 의원급은 감소하고 있는 것으로 나타났다. CT검사료 대부분은 80% 이상이 상급종합병원과 종합병원에서 청구하는 것으로 나타났다. 의료기관 종별 CT장비 보유현황과 검사료간의 상관관계가 유의미함을 알 수 있다. 그 중에서도 상급종합병원의 CT보유대수와 검사료간의 상관관계가 높았으며(p<.001), CT보유대수가 많아질수록 검사료 청구액도 높아진 것으로 나타났다.

종합건강검진서비스에 대한 고객만족도가 재이용의도와 건강신념, 자기효능감 및 건강증진행위 변화에 미치는 영향 (The Effects of the Customer Satisfaction of General Health Examination Service on Their Revisiting Intention and Change of Health Belief, Self-Efficacy and Health Promoting Behavior)

  • Purpose: This study was conducted to evaluate customer satisfaction of general health examination service and to determine how the customer satisfaction affected to revisiting intention, their change of health belief, self-efficacy, and health promoting behavior. Method: Data were collected with questionnaires from 92 customers, who took general health examination at G University Medical Hospital having symptoms of hypertension, diabetes, high blood cholesterol, and obesity. Data were analyzed by the SPSS for windows 10.0 program. Result: Level of customer satisfaction were closely related to one of revisiting intention (r=.791, p=.000). 2) Through multiple regression analysis, factors that affect the customer' revisiting intention were found to be satisfaction in consultation of results (F=126.166, p=.000), examination environment (F=77.490, p=.001), and examination process (F=55.932, p=.024). It could explain 64.4% of customers' revisiting intention. Group displaying higher level of satisfaction showed highly increase in health belief following the examination, and the difference was statistically significant (t=-2.035, p=.045). They also showed a more improved health promoting behavior following the examination, and the difference was statistically significant (t=-2.316, p=.023). Conclusion: Health belief and health promoting behavior was improved following the general health examination. In addition, these changes were more significant in customers who displayed in a higher level of customer satisfaction.

공공병원 건강검진센터의 공간과 면적구성에 관한 건축계획 연구 (A Study on the Architectural Planning of the Space and Area Composition of Health Examination Center in Regional Public Hospitals)

  • Purpose: Health examination centers of regional public hospitals are essential facilities for strengthening preventive medical services in local communities. This study is to organize architectural planning data related to function, space, and area composition of health examination center by the size of the regional public hospitals. Methods: The literature review on the function and spatial composition of the health examination center and the drawings of the regional public hospitals were analyzed. Results: The result of this study can be summarized into several points. 1) The function of the health examination center consists of a diagnostic area, patient area, and staff area based on general examination and comprehensive examination. 2) The type of spatial composition of the 300-bed public hospital examination center is a spatial linkage type with examination rooms of the other departments where general examination and comprehensive examination are undifferentiated. 3) The examination center of public hospitals with 500 beds or more is composed of an independent space-separated type with self-installed examination rooms, and general examination and comprehensive examination are divided. 4) The examination center of public hospitals with 300 beds around, the diagnostic area occupies most of the total area, around 80%, but in public hospitals with 500 beds or more, the proportion of diagnostic area drops to 50-60%, and patient area increases to 30-40%. Implications: The examination center planning of public hospitals requires an architectural planning approach to the function and spatial composition according to the size of the hospital.

당일 검사에 대한 원스톱 서비스 전과 후의 고객만족도 비교 (Comparison of Customer Satisfaction Before and After One-stop Service)

  • Purpose:Hospitals provide top medical service using exceptional manpower, medical technology, and state-of-the-art equipment, thus raising the standard of customer satisfaction. In addition, their medical service is becoming higher than before. One-stop service is a good way to improve the quality of customer-centered service as a qualitative marketing strategy. This study thus aims to facilitate subsequent research and compare customer satisfaction before and after one-stop service. Methods: The study included 72 patients who received the reserved examination and one-stop service for 20 days from April 23 to May 12, 2014. The surveyed questionnaire data were analyzed using SPSS 18.00. Results: The comparison results of customer satisfaction showed that the satisfaction score was generally high in the areas of kindness of examination staff, the speedy/accuracy of work processing of examination staff, and the kindness of reservation staff. The group before one-stop service showed their dissatisfaction with repeated visits and difficulty of booking a desired day. The group after one-stop service showed dissatisfaction with the long waiting time for examination or same-day treatment. Conclusion: The one-stop service showed good results, but new uncomfortable issues for the customer were revealed as well, which may result in more work of employees. Considering the characteristics of various clinical departments, the author hopes to find an efficient operation plan through the development and improvement of an appropriate one-stop service method.

국민건강보험공단 건강검진에 대한 만족도 연구 -부산시의 건강검진수검자를 대상- (A study on the satisfaction of Health Examination for National Health Insurance service -Target of medical examinee in Busan-)

  • 이 연구는 최근 들어 국민소득이 늘어나고 생활수준이 향상됨에 따라 국민들의 의료에 따른 기대와 욕구의 증가로 인한 건강검진의 확실한 서비스가 필요로 되었으며 국민 입장에서 본 건강보험 공단의 건강검진에 대한 만족도를 알아보기 위해서 시행하였다. 2013년 3월부터 6월까지 부산시의 건강검진 수검자를 대상으로 건강검진에 대한 만족도와 인식조사를 자기기입식 설문조사로 시행했으며, 총 300부중 221부를 SPSS를 사용하여 교차분석, T-test, Anova 분석을 시행하였다. 결과로는 일반적 특성에 따른 건강검진 이용유무, 1개월전 의료기관의 이용유무, 하루 담배 흡연량의 차이, 술을 마시는 횟수에서 통계적으로 유의하였다. 또한 검진기관 만족도는 $3.22{\pm}0.52$, 검진서비스에 대한 의견은 $3.31{\pm}0.55$(5점 척도), 건강검진 태도는 $3.73{\pm}0.54$로 나타났다.

  • 미래 사회로 접어들면서, 건강한 삶의 증대를 위한 노력은 현대인들의 주요 관심 분야이다. 특히, ICT 기술과 경쟁력 있는 의료산업 환경을 융합하여 건강한 삶을 위한 기술 개발은 차세대 성장 동력으로 자리잡고 있다. 따라서, 본 논문에서는 건강 검진 프로세스에서 검진 결과에 대한 인공지능 기반의 데이터 분석을 적용하여 종합 판정의 신뢰성을 향상시킬 수 있는 지식 베이스 모델링을 구축하는 연구를 수행하였다. 이를 위해, 딥러닝 분석을 통한 알고리즘을 설계하여 검사 결과지수를 산출, 검증하고, 판정 지식을 통한 종합 검진 정보를 제공하는 모델링을 연구하였다. 제안한 모델링의 적용을 통해, 국민 건강에 대한 빅데이터 분석, 활용이 가능하여 의료비 절감 및 건강 증대의 효과를 기대할 수 있다.

  • 네트워크와 IT기술의 발달로 실시간 가용 데이터의 수요가 촉진되었다. 특히 소득 증가와 고령화 사회 진입에 따른 의료서비스의 질적 향상으로 개인 건강관리를 위한 데이터 수요가 확대되어 의료기관과 관련기업은 다양하고 진화된 의료서비스를 제공하려는 시도를 하고 있다. 그 중에서도 건강검진 결과의 표현은 가장 기본적인 의료 서비스로서, 개인의 건강 기록을 데이터화하여 수검자에게 자신의 건강상태에 대한 이해가 되도록 표현되어야 한다. 그러나 건강검진 결과 표현을 위한 체계적인 프레임워크와 시각화 모델의 부재로 인하여 직관적인 건강데이터의 표현은 사용자에 의한 이해가 어려운 실정이다. 본 연구는 수검자에게 일관된 수치적 데이터 형식으로 제공되었던 기존방식을 지양하면서 사용자 개인의 건강검진 결과에 맞춘 다양한 데이터 표현방식을 정립하여 개발된 맞춤형 데이터 시각화의 모델과 프레임워크를 제안한다.
