Indoor PM2.5 Concentration Distribution and Health Risk Assessment according to the Implementation of a Seasonal Management System (미세먼지 계절관리제 시행 여부에 따른 실내 PM2.5 농도 분포 및 노출에 따른 건강위해성 평가)

  • Background: Since 2019, the Ministry of Environment has implemented a seasonal fine dust management system from December to March, targeting high PM2.5 levels with the aim of reducing PM2.5 concentrations and protecting public health. The focus of improving the seasonal management system lies in the atmospheric PM2.5 levels. Considering the primary goal of protecting public health, it is necessary to analyze the policy effects from an exposure perspective rather than a concentration-based approach. Objectives: This study aims to quantitatively assess the improvement of indoor PM2.5 levels and the health impacts of the seasonal management system by comparing the periods before and during its implementation in residential environments. Methods: PM2.5 concentrations within residential environments in a metropolitan area were measured using an optical particle counter (IAQ-C7, K-weather, Ltd, Korea) at one-minute intervals during the pre-implementation period (November 21~25, 2022) and during the implementation period (December 19~23, 2022). Based on the measured PM2.5 concentrations, a quantitative evaluation of cancer and mortality risks was conducted according to age and gender. Results: The results of comparing indoor and outdoor PM2.5 concentrations before and during the implementation of the seasonal management system showed a decrease of approximately 56.6% and 47.9%, respectively. Health risk assessments revealed that both the safety-limit-based and safety-target-based Hazard Quotients (HQ) exceeded the threshold of 0.1 for children under 19 years of age, both before and after the implementation. The mortality risk decreased by approximately 47.9% after the implementation, with children aged 0-9 showing the highest mortality risk at 0.9%. Conclusions: The findings of this study confirmed the positive health impacts of the seasonal management system across all age groups, particularly children under 19 who are more vulnerable to fine dust exposure.

An Organization Theory Perspective on the Structural Reform of the Health Care Delivery System (의료공급체계 구조의 개혁방향에 대한 조직이론적 시각)

  • There is a general consensus that many health care problems are attributable to the structural defects of the health care delivery system in Korea. The basic policy aimed to address these problems is to reform the delivery system so as that it incorporates two core principles: (1) stratification of medical care institutions into primary, secondary, and tertiary care providers according to the capability to perform specialized and complex services; (2) patients seeking care starting from the primary care provider and, if necessary, to be referred to the other provider step by step. This policy has been consistently pursued for about 30 years, but the achievement is far from success. Thus it is believed that the feasibility of the policy should be questioned. Starting from this question, based upon the observation of the current structure of the delivery system and its expected changes, the reform policy was discussed focusing on the assessment of its feasibility from both practical and theoretical viewpoints. The discussion leads to cast doubt on the policy for its possibility of making planned changes and producing expected desirable effects. Therefore it is advisable to investigate a wide range of alternative strategies and models for improving health care delivery.

A Qualitative Study on Public management of the Admission and Discharge of mental hospital (정신장애인의 정신의료기관 입·퇴원과 국가관리에 관한 질적 연구)

  • The purpose of this study is to understand the context of mental hospital admission and discharge related with the stakeholder and to find out issues about public management on mental health disabilities. To this process, the more effecive alternative policies for mental health will be offered. As a research tool, the qualitative study was used, and the 6 case were analyzed. Through this study, we find 5 theme such as admission type occurred outside public management, uneffective public management in mental hospital treatment, long-term treatment mechanism occured in mental hospital, helplessness and role distortion of public follow-up system. According to the mental health law, mental hospital admission and discharge on mental health disabilities is to managed public system. But public management with mental health disabilities did not work on. In this results, we are find infringement on mental health disabilities. According to these findings, several suggestions are offered for better policies about admission and discharge procedures managed by public system.

Health and Safety Management Issues of Migrant Workers in Manufacturing Industries and Workplace Enhancement (제조업사업장 외국인근로자 안전관리 실태와 개선방안연구)

  • With the shortage of labor force in small and medium-size industries, the Korean Employment Permit System (EPS) continues to bring migrant workers. However, the question remains of the health and safety management of their migrant workers. Considering that a significant part of safety and health problems are caused by migrant workers due to a lack of information on identifying risk and inexperience in the field. Telephone interviews were conducted to investigate the migrant workers' perspectives on workplace safety practices. The survey focused on workers' accident histories, understanding of safety practices, and responding to industrial accidents. A total of 66 accidents occurred among 30 participants. 10 accidents were not reported to the employer (company), which the foreign workers judged to be minor injuries that should not be reported as accidents. 80.0% of the participants expressed dissatisfaction with the PPE they were using. Among various reasons lack of understanding due to language barriers, lack of awareness of the importance of safety, communication difficulties were major reasons for the caused accidents and potential health issues. It is necessary to improve the safety and health support system by providing practical support through specialized educational institutes with experts in the field. A legal framework and a proper safety management system need to be maintained. This study suggests changing the current policy to improve the health and safety of migrant workers by, establishing specialized educational institutes with locally grown foreign experts in the field.

A Study on Disease, Self-Esteem, Social Support and Health Management Behavior in Blind & Visually Handicapped Students (시각장애 학생들의 원인질환 및 사회적 지위, 자아존중감, 건강관리 행위에 관한 연구)

  • Blindness or visually handicaps are found to be an important factor destroying lifestyles as well as a physical problem. According to the data of handicapped person's(v.h.p) registered in Korea, the number of v.h.p. has not decreased. Especially, visual handicapped adolescents (v.h.a.) suffer from an insufficiency of social support under the current education system. There is a special education system for v.h.a. in Korea, and, as a result v.h.a.'s are educated separately from general students and communites. Compared with the self-conception of general students, v.h.a.'s have a more negative self-conception. Among former studies 51.7% of v.h.p's are not in good health. This, of course, means that their handicap impacts on other health problems. The first purpose of this study is to find out the causes of disease and the degree of social support, self-esteem, health management behavior provided and then to analyze the effect of self-esteem and social support on their behavior. To accomplish these purposes, visually handicapped the middle and high school students in Seoul city and Kyung-gi province were selected and 300 students sampled. The main results are as follows : 1) The highest specific handicap is impaired lenses (19.7%). 2. There were high correlations between health management behavior, self-esteem, and social support. 3. Social support is a more important variable than self-esteem on health management behavior. 4. Age of entry and length of stay in their special dormitories, and teacher's support account for 16% of psychological health management behavior 5. Age of entry in their special dormitory and total degree of social support account for 12% of preventive health behavior.

Reviewing the Operation of the Navy Safety Management System and Its Direction (해군 안전관리체계 운영에 대한 검토 및 발전 방향 고찰)

  • Purpose: In order to enhance safety, the Korea Navy is making efforts to introduce a safety and health management system and to develop a risk assessment system, while safety accidents keeps occurring. A development plan was proposed through an inspection of the Safety Management System(SMS). Method: To diagnose the Korea Navy's safety management system, changes in the safety environment, documentation system, and safety and health management system operation were reviewed, points to be improved were found, and improvement directions were derived. Result: There is a need for an SMS standard that can present the Korea Navy's safety policy and safety management direction, and a safety program that provides guidelines for implementing it in a standardized manner. To this end, we were able to confirm the feasibility of linking the requirements of the US Navy's SMS and the international standard ISO 45001 standard together with the Korea Navy's safety management system. Conclusion: The Korea Navy's safety management system needs to clearly present safety policies and standards, and have public confidence and effectiveness in safety work through a standard program that can implement them, and for this, the development of a Navy SMS is necessary.

  • The purpose of this study to develop the Hospital Management Award(HMA) programme which would be a basic information for advancing hospital industry in Korea. HMA programme in the study are designed with general census building method of researchers and with the data of professions opinion which was survey with structured questionnaire Major results of this study are as follows. First, Hospital Management Award(HMA) programme is perceived very necessary and useful for advancing hospital industry of Korea (positive response rate: 93.0%). Second, Criteria, Classification of hospitals may be the function, size and ownership of hospital. Third Evaluating area of hospital management, examining method and evaluation marking points are designed on the basis of hospital management information system. Forth, this study suggests same strategies for execution of the programme. On the basis of the study results this study suggests that Hospital Management Award(HMA) programme would rather actualized in advance by formal institute and/or association. And some in depth studies are required mainly for development of the action plan of HMA programme.

  • IT governance in order to achieve the strategic goals of the organization, leadership, organizational structure, and the process is defined. In order to achieve IT governance, organizational structure, leadership for spindle formation, maintenance, and standardized processes to specific and an effective ITIL (Information Technology Infrastructure Library) management model is needed. In this study, the K Authority Management Model of IT Governance ITIL - based research on service delivery and customer satisfaction through the effective information for ITSM (Information Technology Service Management, IT Service Management) building measures, information for business planning and performance management PMS (Project Management System, project Management System), survey on ICT service levels established through the process of planning and construction management models such as ITIL future research on the public portion of the proceeds from the ITIL management model can be utilized as a guideline when establishing a theoretical presents a base.

A Study on The Determinants of the Medical Expenses in the Health Insurance System in Korea (의료보험 진료비의 결정요인에 대한 연구)

  • Since the inauguration of the medical insurance system in 1977, the increasing medical expenses which can be menace to tile finance of the medical insurance system, have become major concern in the medical insurance field In Korea. This study focuses on the determinants of the medical expenses in the health insurance in Korea and analyzes the impact of these factors on the increase in the medical expenses. The empirical work is done using the pooled cross-section and time-series data of the medical insurance for the self-employeds and the industrial workers from the year 1995 to 1997. The result of this study shows that the main determinants of the medical expenses in the health insurance are the ratio of the population of the aged to the total population, the frequency of the utilization, number of doctors per capita and the regime changes. Although the increasing trend in the medical expenses seems to be unavoidable, we probably need to add some efficiency to the medical expenses by suppressing the supply and the utilization of the unnecessary medical services. The fee-for-service reimbursement system of today can't suppress the supply of the unnecessary medical services effectively. So we need to convert the present fee-for-service system into DRG's which is known to reduce the medical costs. The increase in the medical expenses comes from a lot of factors. Therefore, we should develop more systematic and comprehensive measures to control the soaring medical expenses in consideration of the various factors such as demand, supply, and the organizational side of the medical system.
