Health Management Experience of Residents through Participation in the Community Based Hypertension and Diabetes Registry Program (지역사회 고혈압·당뇨병등록관리사업 참여를 통한 주민의 건강관리경험)

  • Purpose: This study was conducted to explore the healthcare experience of residents participating in the Community-Based Hypertension and Diabetes Registry Program. Methods: This study used qualitative content analysis. Data were collected from in depth interviews of fifteen participants, who either participated in the program hosted by the Training Center for Hypertension and Diabetes for more than two years or participated in an intensive one-on-one education program. Results: The results showed that the core theme in healthcare of participants was 'Proactive healthcare with attention and systematic guidance of experts'. The positive reinforcement of staffs and the improvement of self-efficacy through effective close management were found to be positive influencing factors of participation. The theme consisted of the following categories: 'Participation in the program with positive expectations on healthcare', 'Interest in body and health', 'Vitality in life', 'Confidence in dealing with disease', 'Thankfulness for close care and attention', and 'Desire for continuous care'. Conclusion: When establishing policies on managing chronic diseases, a strategy that reflects the results of the study is required. It is necessary to build up emotional relationship among stakeholders, and expand opportunities for close healthcare such as one-on-one customized training, and mutual cooperation with various organizations in the local community.

Hypertension and Diabetes mellitus Management Effects of Community based Chronic Disease Management Program (일 지역 만성질환관리 건강포인트사업이 고혈압 및 당뇨병관리 지식에 미치는 효과)

  • Purpose: The aim of this study is to evaluate the effects of community based chronic disease management program for managing hypertension and diabetes mellitus. Method: The subjects of this study were 1,000 clients, who participated in a city community based chronic disease management program. The data were collected by a phone survey directly, and selected from DB of community based chronic disease management program, indirectly. The data were analyzed by the STATA program, using descriptive statistics, t-test, and Multiple regression analysis. Results: The mean score of knowledge of hypertension was 4.33 and diabetes mellitus was 2.52. The affecting factors of knowledge of hypertension were the clinic visiting point and blood sugar. However, the affecting factors of knowledge of diabetes mellitus was core lab point, diastolic blood pressure, and blood sugar. Conclusion: the results of this study showed that knowledge of hypertension increased with higher level of education, and younger ages, while knowledge of diabetes mellitus increased with higher level education, more core lab point, younger ages, and in males. Therefore, to control the chronic diseases more effectively in a community, it is important to develop a more systematic education and self- managing programs, using collaboration of health centers and local clinics.

  • The National Health Promotion Act passed in 1995 was a milestone for initiating a national and local health promotion program in Korea. And since then local governments and health centers have been developing and providing health promotion programs for the community population. To apply the effectiveness of community health promotion program, it is important to understand the key issue related to health education and the role of health education personnel. The purpose of this study was to define the responsibility and competency of health education specialist, and to develop the activity areas of health promotion program in Korea. Those who provide the service for health promotion and health education should be properly qualified and professionally trained. However, the skills and responsibilities of those who are in charge of providing health education program have not yet been clearly defined in Korea because the areas of health promotion and health education are composed of multi-academic fields. In case of United States, health education specialist is being developed through professional preparation in colleges and graduate schools, and certified through the examination. Also health education specialist is in charge of the planing, implementing and evaluation of health education program in school, hospital, health center, workplace and health food company. Therefore it is important to develop the programs to train and certify health education specialist. Also to extend the activity areas, the government should support continuously program development for health promotion and health education personnel.

Home visiting oral health program of Longterm home care service and the change of some pathogenic microorganism counts in denture (재가장기요양기관의 방문구강보건교육 전·후 의치의 일부 병원성 미생물의 집락수 변화)

  • Purpose: This study is a repetitive comparative analysis of a qualitative case study that carried out a home visiting oral health education program. Method: This study conducted an interview survey through medical examination by interview before and after the home visiting oral health education and measured some pathogenic microorganism counts in the denture. Results: There was a positive change in the self oral care ability of the elderly at home after home visiting oral health education, including the behavior of self management of dentures, and some pathogenic microorganism counts in the dentures. Conclusion: The home visiting oral health education of home care service centers can improve oral health care for the elderly at home.

A Study on the Stress, Self-Esteem, and Mental-Health in Nursing Students (간호대학생의 스트레스, 자아존중감, 정신건강에 관한 연구)

  • This study is intended to identify relationship between stress, self-esteem and mental health in nursing students. The data were collected from 448 nursing students using self-report questionnaire. The data were analyzed by frequencies, t-test, ANOVA, Pearson's correlation coefficients and Stepwise multiple regression using the SPSS 19.0 program. The result are as followed; First, the average of stress, self-esteem, and mental health was $1.25{\pm}0.55$, $3.33{\pm}0.42$, and $2.79{\pm}1.20$ point. Second, mental health was correlated with stress positively and self-esteem negatively. Third, the influencing factor in mental health of the participant were interpersonal relationship and clinical based stress, $R^2=.561$. Thus, development of guidance program to improve mental health of nursing students is recommended.

The Possibility of Expanding Pay-for-Performance Program as a Provider Payment System (성과연동지불제도의 확대 가능성 고찰)

  • This paper investigates the possibility of expanding pay-for-performance (P4P) program as a provider payment system, in terms of financial, economical, and political sustainability. In order to expand the sustainable P4P, P4P should have usefulness in terms of economic value as well as efficiency in the financial aspects of health care. More importantly, the P4P would be politically sustainable only when both providers and consumers can accept. Korea's healthcare system seems to have logical ground for the P4P program financially and economically. However, how well the P4P can work remains to be proven in its implementation. After 43 tertiary hospitals applied the P4P program for acute myocardial infarction (AMI) and C-section in 2007, the number of hospitals adopting the P4P program for AMI and C-section has increased to 316 in 2011, and an incentive for hospitals applying the P4P has risen to 2% from 1% of health insurance benefits. This shows that the P4P program introduced by Health Insurance Review and Assessment Service is quite successful. In addition, people are aware of the need for improved P4P program and policy alternatives have been already made. Therefore, it is very important to come up with politically supportable strategies that can make providers and consumers accept the P4P program while maintaining the governance of the existing health insurance policy. To this end, there are some tasks to be considered. First, the expansion of the P4P program should be placed on the agenda of the Health Insurance Policy Review Committee, the highest decision-making body, and a separate agency for P4P planning should be established. Second, for more efficient P4P program, the processes of review and assessment, currently carried out separately, should be integrated into a single process. Third, infrastructure to measure the quality of medical services should be sharply expanded. Fourth, the current paradigm for the assessment should be changed. Lastly, a P4P program for consumers should be considered. Given that the consumers in Korea can use medical services freely, the National Health Insurance Corporation could initiate the P4P program for consumers as a means of controlling excessive use of medical services and adjusting consumer's moral hazard.

Long-term Policy Development for the Aged on Medical and Health Care Security (노인의료보장 및 건강관리를 위한 장$cdot$단기 대책)

  • The ageing problem of the population has been emerging in Korea since 1970's so that it is expecting the elderly 65 years and over among the total population from $4.5\%$ in 1988 to be $6.3\%$ in year 2000. This study was conducted to provide secure policy development in coming years for the aged on medical and health care aspects based on the examining current status of the aged problems and health care policies and systems. The study divided into four parts; The first part examined the medical insurance program and public assistance program of the health services in relation to the aged. The second part emphasized on reduction of medical care cost for the aged. The third part studied the regular health check-up program and health education for the aged. The fourth part examined the chronic disease management programs for the aged and strategies of the health care service quality improvement and specialized programs. The following recommendations made as the results of the study. 1. At present, the medical insurance program and public assistance program for the medically indigent is not appropriate to the elderly because it is a part of general medical insurance program so that Health Security Law for the Aged is proposed. 2. Medical cost will be increased due to the high occupancy rate of hospital beds and long stay of the elderly so that it is recommended to develop an early discharge program, home health care program, Health hospice and an althernative programs. 3. At present, a regular health check-ups for the elderly is not included in medical insurance program so that it is recommended to be included in the insurance program and at the same time health education program thoroughly developed for the aged. 4. To make proper medical and nursing services on chronic diseases for the elderly, it is recommended manpower development, specialized clinics or hospitals, nursing homes and an equivalent long term care facilitices should be established on the community based and a research institutions also to be related to supper the care programs.

The Effect of Case Management Program for Hypertensive Patient (보건소 등록 고혈압 환자에게 실시한 사례관리 프로그램의 효과)

  • Purpose: The purpose of this study was to investigate the effects of case management service for hypertensive elderly patients through the customized visiting home health care program. Method: Non-equivalent control group pretest-posttest design was employed. Data were collected from April 2007 to January 2009. Among a total of fifty subjects, 25 subjects were belonged to the case management group and the others were belonged to the general management group. Results: 1) In experimental group, significant increasing rate of number of people within normal blood pressure by time, but no significant increasing rate of number of people within normal blood neutral fat, total cholesterol, HDL, & LDL. 2) In experimental group, the score of knowledge about disease and healthy life practice were increased on 8 week period but decreased on follow up period. Number of people within normal blood HDL and mean score of healthy life practice were significantly higher than control group on follow up period. Conclusion: Case management service was effective on controlling blood pressure but partly effective on blood lipid, knowledge about disease, and healthy life practice in hypertensive patients. Further study is needed to conduct a periodic re-evaluation of the effect of case management and to verify an optimal interval to provide the case management service.

Effects of the Weight Management Program Based Self-Efficacy for Body Composition, Blood Lipid Profile, Weight Self-Efficacy Lifestyles, Depression in Middle-Aged Obese Women

  • Purpose. This study was done to determine the effects of weight management program using self-efficacy in middle-aged obese women. The study also attempted to measure the effects of the program on the weight efficacy lifestyle, body composition, and depression. Method. The research design of this study was a nonequivalent control group pretest-posttest design. The experimental group consisted of 21 middle-aged obese women and another 21 middle-aged obese women in the control group. The women in the experimental group participated in the weight management program for 12 weeks using self-efficacy. The weight management program using self-efficacy included education on effects of exercise for weight control, aerobic exercise program, muscle training and counseling through the telephone. Results. After 12 weeks of participation in the program, BMI (p <.0001), body fat % (p <.0001), abdominal fat (p <.0001), in the experimental group were significantly decreased compared to the control group. Weight self-efficacy lifestyle (p <.0001) and depression (p =.006) in the experimental group were significantly improved after the program compared to the control group. According to these findings, weight management program self-efficacy for middle-aged obese women could increase weight efficacy lifestyle, and decrease depression, BMI, body fat, and abdominal fat. Conclusion. The result also suggested that the increasing weight efficacy and lifestyle help the obese women to perform and continue exercise. This program could be used in the community such as public health center for weight care and mental health promotion of middle-aged obese women.
