아동의 만성질환이 환아 및 환아 가족의 일상생활에 미치는 영향 (The Impact of Chronic Illness on Children and their Families)

  • The aim of this descriptive study was to identify the impact of chronic illness on children and their families. Mothers of 177 children with chronic disease were recruited from aged one to 17 years hospitalizing the pediatric nursing unit at one university hospital. 28 questions. a self-complete maternal questionnaire developed by researchers based on literatures and 'The Impact of Childhood Illness Scale' (Hoare & Russell, 1995) assess four aspects of the child's and family's lifestyle with two dimensions for each question. the frequency of the problem and its importance of concern that it cause. In the data analysis. Pearson correlation coefficients and analysis of variance were used to test any association and a statistical comparisons. between individual variables and the impact on child's and family's lifestyle. The results were as follows: 1) There were the higher mean scores on the importance dimension than on the frequency dimension on total impact on child's and family's lifestyle. On the importance dimension. mothers had much higher levels of concern about impact on child's development and adjustment among subscales. On the frequency dimension, mothers had much higher levels of concern about impact on the family daily living on subscales. 2) The impact on child's and family's lifestyle related on individual variables were: (1) On the importance dimension. there was statistically significant low in the impact on child's development and adjustment among subscales in older parent group compared with it of younger parent group. (2) On the frequency dimension. there were statistically significant high in the impact on child's development and adjustment among subscales in the children with leukemia or cancer compared with it of the other children who had another chronic illnesses. And there were statistically significant high in the impact on child's development and adjustment among subscales in less than high school educated mothers compared with it of mothers who was educated college level. (3) On the frequency dimension. There were statistically significant high in the impact on family's daily living among subscales and total impact on child's and family's lifestyle in female children. (4) There was positive correlation between impact on parents daily living and frequency of child's hospitalization on the frequency dimension. but there was negative correlation between the two variables on the importance dimension. (5) There was positive correlation between impact of chronic illness and its treatment and duration of child's hospitalization on the frequency dimension. In conclusion. these results were reflected by the high level of concerns about impact on child's development and adjustment although mothers had frequently responded about the impact on the family daily living. We suggest consistent intervening program that help the family empowering to make children live with and adjust to their chronic illnesses from the point of diagnosis and after discharge from the hospital. We also suggest these results can be utilized as a useful data in the practice of home health nursing for children who have chronic illnesses and their families.

일부 대학생의 외모관리 행동이 구강건강 행동에 미치는 영향 (The Impact of Appearance Management Behavior on Oral Practice Behavior of Some College Students)

  • Purpose : The college student period is pivotal for establishing oral health management behaviors and habits that persist in adulthood; it is necessary to develop the correct behaviors. Appearance management behavior not only involves the management of an individual's appearance but also ensures a healthy quality of life. Additionally, it is related to health and oral health behavior. The purpose of this study was to determine how college students' appearance management behavior affects their oral health management behavior and to provide basic data for developing policies appropriate for the lifestyle of college students. Methods : A self-administered questionnaire was completed by 450 university students in Gyeonggi, Choung-nam, Daejeon on April 27, 2022. The survey comprised 5 questions about general characteristics, 18 about appearance management behavior, and 8 about oral health behavior. The sum of the practice questions were calculated, with higher scores indicating better oral health behavior. Results : The oral health management behavior exhibited was generally high. Notably, the health-related group (61.2 %) demonstrated significantly higher levels of clothing management (60.5 %), makeup management (56.6 %), and skincare (56.6 %) behaviors, whereas body management behavior (61.2 %) was deemed 'normal' (p<.05). The results of the stepwise multiple regression analysis examining the impact of appearance management behavior on oral health management indicated that clothing management (β=0.17) and skincare behaviors (β=0.12) were influential factors. Among these, clothing management behavior was the most influential. Conclusion : To promote oral health practices among college students, this study incorporated factors related to appearance management into oral health education to increase participation and interest. We intend our findings to serve as foundational data for developing oral health education programs that integrate elements of appearance management.

대사증후군 대상자들의 건강코칭프로그램 평가 (The Evaluation of a Health Coaching Program on Metabolic Syndrome Patients)

  • Objectives: We assessed the feasibility of health coaching for health coaching program on metabolic syndrome. Methods: We developed a 6 month health coaching program on metabolic Syndrome. We recruited people with metabolic syndrome according to modified NCEP-ATP III. The participants were 9 men over 30 years of age who had taken a health screening at general hospital. We collected data such as demographics, BMI, body fat, blood pressure, HDL-cholesterol blood sugar and triglyceride. The program was analyzed by using Wilcoxon signed rank test. Results: Participants showed significantly decreased BMI, weight, waist circumference, body fat after 6 month program. They talked the awareness about their own behavior. They changed into better for eating habits, physical activities, and self management. Their discipline increased and eating habits became regular. They were satisfied to this program and showed strong confidence about their own change. Conclusions: Coaching did not direct certain behavioral change but guided self awareness and practice. Health coaching program showed long maintained effect to participants. We suggested health coaching as a helpful individual program to intervene risky health behavior especially for metabolic Syndrome.

전문대학생 정신건강의 영향 요인에 관한 연구 (A Study on Effectiveness Factors for Mental Health : Focus on College Students)

  • Objective : The objective of this study is to analyze various factors of how mental health, self esteem, and stress affect health living conditions aimed at college students who have a low level of self-awareness. This study can provide college students the vitality lifestyle by developing and applying customized healthy living program. Furthermore, it can benefit preventing disease and improving health condition. Method : This study is aimed at college students in Daegu. The research subjects' distributions consist of male of 138 and female of 144, which totaled 282. The collected data was analyzed by SPSS 18.0, statistical program.4 Results : The result of the study indicated that the main factors that affect self esteem, stress, and mental health condition are allowance and part time jobs. It shows that health characteristic doesn't have relationship with self esteem. However, stress are mental health have a close relationship with drinking, health conditions, sleeping hours, and exercise. Conclusion : College Students should manage and lead healthy living style as a role model of family and society; as a result, healthy living style in the college becomes very important time period in the life time. Therefore, college students need to improve self esteem and reduce stress in order to benefit their mental health. College faculty members also need to put an effort on educational health programs regarding the practice of healthy life such as campaign for antismoking, moderating in drink, and exercising.

심혈관질환 위험인자를 가진 중소규모 사업장 남성 근로자의 모바일헬스 앱 활용 건강파트너십 프로그램의 효과: 무작위 실험연구 (Effects of a Health Partnership Program Using Mobile Health Application for Male Workers with Cardiovascular Risk Factors in Small and Medium Enterprises: A Randomized Controlled Trial)

  • Purpose: This study aimed to apply a health partnership program using commercially available mobile health apps to improve cardiovascular risk factors in male employees and verify its effectiveness. Methods: Using a randomized control group pretest-posttest design, male employees with cardiovascular risk factors from five small and medium-sized workplaces were randomly assigned to an experimental group (n = 32) and a control group (n = 31). The experimental group was encouraged to use three mobile health apps for 12 weeks to acquire the necessary cardiovascular disease-related information and practice strengthening training, walking, and diet management appropriate to their level. They also received feedback on their weekly activities and motivational text messages from health partners. Hypotheses were tested using the SPSS WIN 22.0. Results: The experimental group showed a significant difference compared to the control group in terms of their perception of mobile health app (p < .05), self-efficacy for exercise and diet, self-management partnership, and cardiovascular disease prevention health behavior (p < .001). In particular, there were significant decreases in the body mass index, ratio, serum fasting blood sugar, total cholesterol, and triglyceride in the experimental group (p < .001); however, there was no significant difference in high-density lipoprotein-cholesterol. Conclusion: Intervention using mobile apps based on partnership with health managers is effective in improving the objective cardiovascular risk index in male employees; therefore, such intervention should be continuously used as a useful lifestyle modification strategy in the workplace.

세대간의 웰빙인식정도와 웰빙관련 실천행동의 비교 (Comparison of Awareness and Practice on Well-being Life and Related Behaviors According to Generations)

  • The purpose of this study was to investigate the awareness and practice on well-being life and well-being related behaviors, and the various factors affecting well-being related behaviors such as purchasing food materials, food habits, eating out and daily routine activities. A survey was conducted by questionnaire and on a 5-point Likert scale. The subjects of this study were composed of 221 students and their 102 parents who were over 40 years residing in the Ulsan area. The results of this study are summarized as follows: Almost half of the subjects (47.4%) responded that they have good health conditions; to keep a good health condition, 41.2% of the subjects were exercising regularly and 20.4% of them kept diet control. In regard to the meaning of well-being, 66.6% of the subjects thought it is the lifestyle for physical and mental richness (children: 70.6%, parents: 57.8%). 30.3% of the subjects answered that the most important part of well-being was food related. The importance order was mental richness, food related things, physical health for children, and for the parents, it was food related things, physical health, mental richness. Most of population (45.8%) answered that they have a willingness for the pursuit of a well-being life. Among the well-being related behaviors, 69.7% of subjects have purchased items (children: 61.5%, parents: 87.3%). 37.2% of the subjects have acquired information from TV. The average well-being practice score was $61.01{\pm}10.36$. Children's scores were significantly lower than the parent's scores (p < 0.001). And the average practice score of 'purchasing food materials,' 'eating out,' 'food habits,' 'daily routine activities' were $15.3{\pm}3.3,\;15.5{\pm}3.1,\;16.8{\pm}3.3$ and $13.4{\pm}3.5$, respectively. Among five types of purchasing food materials, 'purchasing domestic agricultural food' was greatest ($3.64{\pm}0.91$) and 'purchasing of organic or low agricultural chemical food products' was lowest ($3.15{\pm}0.91$). In regard to food habits, 'eating rice and bread made of mixed grains' was greatest ($3.46{\pm}1.12$) and 'eating uncooked food or zen food' was lowest ($2.46{\pm}0.99$). The practice scores were significantly affected by gender (p < 0.05), monthly income (p < 0.01), educational level (p < 0.01), presence of disease (p < 0.05), subjective health condition (p < 0.05), well-being awareness (p < 0.001) and concern with well-being (p < 0.001). Well-being awareness scores and well-being practice scores are related positively. Therefore various programs in well-being education should be necessary in order to boost the authentic perceptions of well-being and well-being oriented behaviors in any socioeconomic situation, such as different generations; industrial companies producing well-being goods for consumer's needs and satisfaction; and government and local community create various conditions for well-being oriented behavior.

고등학교 청소년의 건강증진행위 경험: 근거이론 적용 (The Experience of Health-Promotion Behavior in Adolescents: A Grounded Theory Approach)

  • 본 연구는 우리나라 고등학생의 건강증진행위 실천 경험을 이해하고 그 의미를 밝히는 것이다. 연구대상자는 자신의 경험을 충분히 표현할 수 있는 고등학생 12명을 편의표집 하였다. Strauss와 Corbin(1998)의 근거이론방법을 적용한 질적 연구이며, 참여자 진술로부터 312개의 의미 있는 문장을 도출하여 22개의 주제로 구분하여 13개로 범주화하였다. 연구결과, 중심현상은 '건강증진이 배제된 라이프스타일'이었으며, 이를 조율해 나가는 중재조건은 '부모님의 생활습관 지도', '건강행위를 함께 할 수 있는 친구', '운동시설 접근의 제약'이었으며 이를 대처하도록 고안된 작용/상호작용전략은 '건강증진 실천과 유지를 위한 의지 다지기', '멋진 몸매를 가진 친구를 모방하기', '부모님의 건강행동 배우기'로 나타났다. 이상의 결과들은 청소년의 건강증진행위 과정에 대한 본질을 이해하는데 도움이 될 것이며, 이를 통해 고등학생의 건강증진행위 실천을 위한 건강증진 프로그램 개발의 기초자료가 되길 기대한다.

Identifying psychological predictors of adherence to a community-based lifestyle modification program for weight loss among Chinese overweight and obese adults

  • BACKGROUND/OBJECTIVES: Existing evidence on lifestyle modification programs for weight loss is limited by the high attrition rate of such programs. Identifying predictors of adherence to a lifestyle modification program could result in program improvement. However, little is known about behavior-specific adherence and its psychological predictors. This study aimed to examine the psychological predictors of adherence after one-month participation in a community-based lifestyle modification program among Chinese overweight and obese adults in Hong Kong. SUBJECTS/METHODS: A total of 205 Chinese overweight and obese adults aged $38.9{\pm}10.5years$ completed the study. Data were collected at baseline and after one month using self-reported questionnaires, which assessed knowledge (self-developed scale), motivation (Treatment Self-Regulation Questionnaire), stage of change (Stage of Exercise Scale) and self-efficacy (Self-Rated Abilities for Health Practices Scale). At one month, a 4-day dietary recall and the International Physical Activity Questionnaire-Short Form were used to assess dietary and physical activity (PA) adherence. Food and PA diaries were examined to indicate self-monitoring. Program attendance was tracked between baseline and one-month follow-up. RESULTS: After one month, participants reported high dietary adherence, attendance, and adherence to self-monitoring but low PA adherence. Multiple regression analyses suggested that diet self-efficacy (baseline) and nutrition knowledge (one-month change) were independent predictors of dietary adherence score at one month, whereas autonomous PA motivation (baseline) and PA self-efficacy (both baseline and one-month change) were independent predictors of PA adherence score at one month. No significant psychological predictor was identified for attendance or self-monitoring. CONCLUSIONS: The results suggest that the effect of psychological factors on adherence differs between diet and PA adherence outcomes. To promote adherence, practitioners should assess self-efficacy, knowledge, and motivation at the beginning of a weight-loss program and explore behavior-specific strategies to improve knowledge and self-efficacy. The results of this study have direct implications for program improvements.

The interaction of Apolipoprotein A5 gene promoter region T-1131C polymorphism (rs12286037) and lifestyle modification on plasma triglyceride levels in Japanese

  • BACKGROUND/OBJECTIVE: Apolipoprotein A5 gene promoter region T-1131C polymorphism (APOA5 T-1131C) is known to be associated with elevated plasma TG levels, although little is known of the influence of the interaction between APOA5 T-1131C and lifestyle modification on TG levels. To investigate this matter, we studied APOA5 T-1131C and plasma TG levels of subjects participating in a three-month lifestyle modification program. SUBJECTS/METHODS: A three-month lifestyle modification program was conducted with 297 participants (Age: $57{\pm}8years$) in Izumo City, Japan, from 2001-2007. Changes in energy balance (the difference between energy intake and energy expenditure) and BMI were used to evaluate the participants' responses to the lifestyle modification. RESULTS: Even after adjusting for confounding factors, plasma TG levels were significantly different at baseline among three genotype subgroups: TT, $126{\pm}68mg/dl$; TC, $134{\pm}74mg/dl$; and CC, $172{\pm}101mg/dl$. Lifestyle modification resulted in significant reductions in plasma TG levels in the TT, TC, and CC genotype subgroups: $-21.9{\pm}61.0mg/dl$, $-20.9{\pm}51.0mg/dl$, and $-42.6{\pm}78.5mg/dl$, respectively, with no significant differences between them. In a stepwise regression analysis, age, APOA5 T-1131C, body mass index (BMI), homeostasis model assessment-insulin resistance (HOMA-IR), and the 18:1/18:0 ratio showed independent association with plasma TG levels at baseline. In a general linear model analysis, APOA5 T-1131C C-allele carriers showed significantly greater TG reduction with decreased energy balance than wild type carriers after adjustment for age, gender, and baseline plasma TG levels. CONCLUSIONS: The genetic effects of APOA5 T-1131C independently affected plasma TG levels. However, lifestyle modification was effective in significantly reducing plasma TG levels despite the APOA5 T-1131C genotype background.

Demographic and lifestyle factors and selenium levels in men and women in the U.S.

  • Selenium is an antioxidant trace element linked to cardiovascular disease and cancer. Although diet is a major source, relatively little else is known about independent determinants of selenium levels in free-living humans. In this study, we aimed to investigate the independent demographic. lifestyle, and dietary determinants of selenium levels in 1,997 men and 1,905 women in two large prospective U.S. cohorts. Toenail selenium levels were quantified using neutron activation analysis. Diet, geographic residence, demographic, and environmental factors were assessed by validated self-administered questionnaires. Multivariate generalized linear models were conducted to assess the independent relations of these factors with toenail selenium levels, correcting for measurement error in the diet. In multi variable-adjusted analyses, independent predictors of higher selenium were male gender (6.3% higher levels); living in West and Northern-Midwest U.S. regions (8.9% and 7.4% higher than Southern-Midwest regions, respectively); consumption of beef and bread products (between 0.7 - 2.5% higher per daily serving); and selenium supplement use (6.9% higher than non-users); whereas cigarette smoking (5-10% lower than never smokers), older age (0.6% lower per 5 years), and consumption of eggs, white rice, dairy products, coffee, and alcohol (between 0.1 to 2.0% lower per daily serving) were associated with lower selenium. Multiple dietary and non-dietary factors independently predicted selenium levels, suggesting that both consumption and non-dietary processes (e.g.. related to oxidant status) may affect levels. Significant geographic variation in selenium levels exists in the US.
