Developing a Community Capacity Builded Exercise Maintenance Program for Frail Elderly Women (지역사회 역량강화 전략을 이용한 재가 허약여성노인의 운동유지 프로그램 개발)

  • Purpose: This study was conducted to develop a community capacity builded exercise maintenance program for frail elderly women. Methods: As a guideline to develop the exercise maintenance program, the intervention mapping framework, including needs assessment, setting program goals, selecting theory-informed intervention methods, producing program components, planning program implementation and evaluation, was used. Focus group interviews with public health nurses and frail elderly women were conducted for needs assessment. Intervention strategies and components were formulated based on community capacity theory. Results: The developed exercise maintenance program consisted of strategies focusing on leadership development, partnership construction, organization development, community systematization of dimension of community capacity. A exercise maintenance program using health leader, health contract, exercise pocketbook, rhythmic activity suiting song and self-help group was included. Conclusion: The intervention mapping method was found to be useful to develop theory-based valid and community capacity builded exercise maintenance strategies for frail elderly women.

Barriers to measles mumps rubella vaccine acceptance in the three southern border provinces of Thailand

  • Purpose: This qualitative study utilizing phenomenological methodology aimed to depict parental measles mumps rubella (MMR) vaccine acceptance through the work experiences of health personnel. Materials and Methods: Twenty-two public health workers working as vaccination providers in the three southern border provinces of Thailand were recruited. In-depth, face-to-face, semi-structured interviews were conducted. Data were analyzed using thematic analysis. Results: Four main themes emerged: (1) religious beliefs, (2) personal disagreements, (3) fear and mistrust regarding potential vaccine side effects, and (4) misperceptions about the potential severity of measles. Four subthemes were identified: (1) haram (prohibited), (2) the will of Allah, (3) spousal disagreement, and (4) disagreement from a religious leader. The results of this study indicated that perceived religious prohibition was the most important reason for refusing to vaccinate among Muslim parents. Vaccine-hesitant parents were concerned that the vaccine might contain gelatin derived from pig products. Also, halal certification of the vaccine was required from Muslim parents to ensure that vaccine has been approved for Muslims. Meanwhile, a lack of knowledge and positive attitudes concerning immunizations of vaccine-hesitant parents were also found as predominant reasons for incomplete childhood immunizations in the deep south of Thailand. Conclusion: Health education and engagement by religious leaders to endorse the vaccination and bridge the gap between religious beliefs and vaccine acceptance is needed to overcome this issue. This study findings could be effectively applied to improve vaccination uptake in a Muslim majority context.

Antiobesity Effect of Mixture of Black Garlic and Garsinia cambogia Extracts in 3T3-L1 Adipocytes and L6 Skeletal Muscle Cells

  • The antiobesity effect of the mixture of black garlic and Garsinia cambogia extracts (BGG) was investigated by measuring the Oil red O staining and the expressions of adipogenic genes during preadipocyte differentiation by real-time PCR in the 3T3-L1 adipocytes. BGG reduced contents of Oil red O dye in the 3T3-L1 adipocytes. mRNA expression levels of SREBP1c, C/EBPa, aP2/FABP4, and $PPAR{\gamma}$ which are adipogenic transcription factor, in cells treated with BGG were also significantly down regulated. Also, the phosphorylation of AMP-activated protein kinase (AMPK) in L6 cells was more increased by BGG. These results indicate that BGG seems to be more attractive compound for application of industry than individual extracts such as black garlic and Garsinia cambogia, considering it has two effects not only inhibit the preadipocyte differentiation but also activate the phosphorylation of AMPK unlike other two compound.

The Effects of Job Demand and Job Resources on Burnout and Work Engagement of Hospital Nurse Administrators (직무요구와 직무자원이 병원행정직 간호사의 소진과 조직몰입에 미치는 영향)

  • Purpose: This study aims to investigate the degree of job demand, job resources, burnout, and the organizational commitment of administrative nurses based on the job demands-resources model. Further, it seeks to confirm the influencing factors affecting nurses' burnout and organizational commitment. Methods: The participants were 188 administrative nurses working at hospitals (one tertiary hospital and six general hospitals) located in D City. The collected data were analyzed with IBM SPSS Statistics 23.0 using frequency, percentage, mean, standard deviation, t-tests, ANOVA, Pearson's correlation coefficient, and multiple regression analysis. Results: The influential factors of burnout were role conflict (β=.50), job demand (β=.18), job position (β=-.17, team leaders and above), and social support (β=-.15). The regression model had an explanatory power of 59%. The influential factors of organizational commitment were appropriate rewards (β=.59), job position (β=.15, team leader or above), working department (β=.14, referral center and health screening administration department), and social support (β=.18). The regression model had an explanatory power of 59.5%. Conclusion: The results support the job demands-resources model, and interventions should be developed to decrease job demand and provide sufficient job resources.

The Effects of Critical Thinking Disposition and Self-esteem to Self-leadership of Nursing Student (간호대학생의 비판적 사고성향과 자아존중감이 셀프 리더십에 미치는 영향)

  • Objectives : The purpose of this study was to evaluate the effect of critical thinking disposition and self-esteem to self-leadership in nursing students. Methods : In this study, 273 data was collected from nursing students of K-university in South Korea from Sep. 2015 to Nov. 2015, and the collected data was analyzed using SPSS 22.0 Statistics Program. Results : The results showed that both of critical thinking disposition and self-efficacy according to school year are significant to self-esteem. It was found that critical thinking disposition, self-esteem and self-leadership has a positive correlation. Critical thinking disposition and self-esteem were predictors of self-leadership with 46% of influence. Conclusions : The nursing training program needs to be developed to promote critical thinking and self-esteem in the nursing curriculum, so that the nursing students can demonstrate the ability as a nursing leader in the various fields of clinical practice and health care.

A Meta-analysis of Factors Related to Organizational Effectiveness of Healthcare Employees (의료기관 종사자의 조직효과성 관련요인에 관한 메타분석)

  • Objectives : The aim of this meta-analysis was to draw overall conclusions on the correlation between the organizational effectiveness (OE) of healthcare employees and related factors by summarizing recent studies and to provide a baseline for future research. Methods : A literature search for original articles published from 2005 to 2014 including doctoral theses and KCI journal articles was done in KERIS' RISS, etc. in a systematic manner. From 77 studies which met the inclusion criteria 190 effect sizes (ESs) were calculated for the analysis. A meta-analysis was done with CMA 3 (Biostat company, USA). Results : The overall ES was moderate for all, positively and negatively related factors. For all related factors, job characteristics, group-leader relations, personal and organizational characteristics had a moderate ES. The ES for work experiences was small. Conclusions : Although the results of this study were meaningful it is necessary to perform follow-up analyses (1) based on a longer data period and more articles, (2) including moderating variables with higher explanatory power and (3) adopting a study model with more specific and simplified variables.

The farmers consciousness of Health status and food habits in Korea (농업인의 식습관과 건강상태의 관련성 연구)

  • The purpose of this study was to compare the consciousness health status and food habit for farmers in Korea. Eight hundred persons were surveyed by rural leader of RDA, composed with 35.1% of male and 64.9% of female. Data collection includes height weight and blood pressure by measuring method and personal & family situation by interview for influential factor of health status and food habit. The questionnaire was included dietary habit of salt, sugar, dietary fiber and fat intake and other behavior questions related with diet. The subjects were composed 32.5% of elementary school graduated and 50% of middle size farming. Of the all subjects, 54% of were rice planted farming mainly and 19 were over 65 year-aged group. In the concept of disease prevalence. gender difference was exist, liver and heart complains were more frequency in male and waist pain and urinary complain in female, shoulder pain and nervousness were frequently prevalent to all group of farmers. The hypertension prevalence of farmers were 16% by measuring blood pressure, but consciousness of hypertension was very low (5%). The correlation of health status with personal factors were analysed, body weight, number of family, education level, and working level were significant with CMI score and Farmers' syndrome score. The food habit status was negatively correlated with weight and BMI, age was negatively correlated with CMI score and farmers' syndrome. The consciousness of health status and food habit score were significantly correlated with smoking and alcohol drinking. It concludes that good food habit contribute wellness of health. Nutrition education for better food habit would be helpful on health status.

An Assessment of Notice Exposure by Job and Dosimeter Parameters Setting in Automobile Press Factory (자동차 프레스 공정에 있어서 직무 및 누적소음기 설정치 차이에 따른 작업자의 소음노출 평가)

  • Noise-induced hearing loss(NIHL) was the highest rate (43.5%~58.5% from 1996 to 1998) of positive findings through specific medical program in Korea. There were much more NIHL at workers of automobile manufacturing factories than other manufacturing factories. The specific aim of the present study was to determine the noise exposure of automobile press lines, according to their job titles, press line types(auto, semiauto), dosimeter parameters setting. There were a total 11 press lines sampled at a automobile manufacturing company. Among those press lines, 10 press lines were autolines with acoustic enclosure, one semiauto press line was no aucostic enclosure Noise exposure data were sampled for an work shift using noise dosimeter, which recorded both time-weighted average(TWA) and 1-min average. The mean OSHA TWA(Korea TWA with threshold 90) was $80.7dB(A){\pm}4.7dB(A)$ for leader, $82.8dB(A{\pm}4.5dB(A)$ for pallette man, $76.7dB(A){\pm}4.3dB(A)$ for press operators, $76.6dB(A){\pm}5.6dB(A)$ for crane operators, $77.1dB(A){\pm}2.8dB(A)$ for forklift drivers, whereas the mean NIOSH TWA was $88.9dB(A){\pm}1.7dB(A)$ for leader, $89.6dB(A){\pm}2.1dB(A)$ for pallette man, $86.7dB(A){\pm}1.8dB(A)$ for press operators, $88.5dB(A){\pm}2.0dB(A)$ for crane operators, $87.7dB(A){\pm}1.0dB(A)$ for forklift drivers. While L10 for NIOSH TWA samples was 84.8 dB(A) ~ 87.3 dB(A), L10 for OSHA TWA samples was 69.5 dB(A) ~ 77.4 dB(A). L10 means that the TWA for 90% of the samples exceeded L10. Among OSHA TWA(Korea TWA with threshold 90) samples for pallette man, 7.7 % exceeded 90 dB(A), the OSHA permissible exposure level, but OSHA TWA samples for the other job titles didn't. Among NIOSH TWA samples, the samples over 85 dB(A), the NIOSH recommended exposure limit, was 100% (leaders), 83.3 %(operators), 97.4%(palletteman), 100%(forklift drivers), 91.7 %(crane operator). The results of One-way random effects analysis of variance models shows that the difference between job titles was significant by OSHA TWA(p<0.05), but not significant by NIOSH TWA(p>0.05). NIOSH TWA samples were significantly higher than OSHA TWA samples(P<0.05). Regression analysis was used to obtain relationships between OSHA TWA samples and NIOSH TWA samples. In this case the coefficient of determination = 0.90, which shows the high degree association between two methods. Regression equation, NIOSH TWA = 0.552 * OSHA TWA + 42.13 dB(A), shows that if OSHA TWA is known, NIOSH TWA can be predicted by the equation. The mean TWA difference between threshold 80 dBA and 90 dBA was significant(p<0.01). While the TWA noise exposures were 7.7% above the Korea(OSHA) PEL, they were more than 83.3% over NIOSH REL. Automobile workers were exposed to noise level that could be potentially damaging to their hearing. It found that there is approximately 25% excess risk of hearing loss even if a worker is protected to the PEL in according to NIOSH study.

Convergence Study on the Experience of a Community Based Participatory Research leader using the BeHaS Program for Elderly Living Alone (독거노인을 위한 베하스 프로그램을 활용한 지역사회기반 참여연구 리더 경험에 관한 융합적 연구)

  • This study aimed to confirm the nature of the experiences of elderly living alone who participated in the BeHaS program using community-based participatory research. 6 elderly living alone with metabolic syndrome participated as participants, and they played a role as a leader when conducting the 12 weeks BeHaS program as a community-based participatory study. Data was collected using focus group interview. As a result, Six categories were derived; 'the burden of becoming a research participant', 'knowing a new program', 'getting into the BeHaS program', 'having a new leadership', 'knowing about metabolic syndrome health care', 'Feeling worth through a changed self'. Therefore, this study has identified the nature of the experience of participating in the BeHaS program for metabolic syndrome through a community participation-based study targeting the elderly living alone, and it will be able to provide basic data for the operation and program settlement strategies in the Community of the BeHaS program in the future.

Implementation and Results of a Survey on Safe Community Programs in Gangbuk-gu, Korea: Focusing on Participants at a Local Public Health Center

  • Objectives: The purpose of this study was to investigate the current status of and problems with the Safe Community Programs in Gangbuk-gu, one of the 25 districts of the capital city of Korea. Methods: The study subjects were 396 individuals who were involved in Safe Community Programs between 2009 and 2011. We examined the effectiveness and willingness of respondents to participate as a safety leader of the Safe Community Program with a questionnaire. We examined the injury death rates of Gangbuk-gu by using of the death certificate data of Korea's National Statistical Office. Descriptive statistics and chi-squared tests were used. Results: The effectiveness of programs did not differ but active participation differed significantly among subjects (p<0.05). The injury death rate of Gangbuk-gu as a whole increased during the implementation period. However, senior safety, in particular, may be a helpful program for reducing injuries in Gangbuk-gu. Conclusions: This study suggests that the lack of active participation may be a major problem of Safe Community Programs in Gangbuk-gu. Therefore, Safe Community Programs should be expanded to the entire district of Gangbuk-gu and more active participation programs should be developed.
