  • Long-term care insurance has been introduced in Korea a year ago, and we are in a stage requiring to set principles regarding the generosity of coverage and how to gradually extend the coverage. This study empirically analyzes how the long-term care insurance in Korea is operated. Special attention is given to who is the main beneficiary of the long-term care insurance introduction, and what is the factors influencing the elderly's decision to apply for or use long-term care services. Use of a detailed information of individuals' public health insurance and long-term care insurance from administration data made it possible to control for health status, socioeconomic status including family type, housing tenure, income level. Logit models were employed to analyze the effects of various socioeconomic factors on the likelihood of applying and using long-term care services. Also, this study employed a survey questioning whether to ever willing to take other option as a alternative to residential care or home-care and the level of cash benefit for which they are willing to replace the formal care with informal care. The result indicated that although the poorest elderly population groups are in the greatest need for the long-term care service, they are in difficulty using the service due to economic burden. This implies the copayment amount needs to be adjusted in order for the poor elderly group to be able to get the benefit of the long-term care service.

Interview data were collected from 280 elders and analyzed. Using SPSS/WIN 21, $x^2$-test and t-test were conducted to analyze the relationship between the subjects' characteristics and their suicidal ideation, and logistic regression was done to identify factors affecting suicidal ideation. Results: Of the subjects, 48.6% had suicidal ideation for the last one year. Self-evaluated psychological status, quality of sleep, depression, and loneliness were affecting factors to the subjects' suicidal ideation. Conclusion: Psychological and social factors are strongly related to the suicidal ideation of the elderly who are living alone in metropolitan areas. Therefore, home visiting services should supply physical, psychological and social cares continuously to the low-income elderly, especially those who live alone in metropolitan areas.

  • The purpose of this study is to present our elderly welfare policy and welfare facilities for the elderly and to propose the future direction of policy for the elderly with comparison to that of Japan. The comparative study results of Korea and Japan are as follows: As the basis of elderly welfare, care insurance for the elderly is now fixated in the two countries. On the other hand, for quantitative expansion in Korea, the quality level has been gone down. Due to qualitative evaluation, the growth of facilities has slowed and material welfare providers was also a decline in the number of operators. Two countries in the field of community welfare in recent years has been actively promoting. It is the most important to solve the social welfare problem with local councils who know the area, which is a part of the future to focus and to support. Due to huge construction investment of facility, there is the burden of operators. This facility is appropriate for an extension of the home. However, it is in the hard economic times. Until now, the number of people in a room is at least four according to the legal standard of Korea, there are a few private rooms. On the other hand, in Japan, unit-care was phased since 2002 and private rooms have been expanded. In Japan, health center for the elderly plays an important role with special elderly nursing home among facilities for the elderly. Health center provides services that are recovering, nursing, and caring to support independence for the elderly after acute phase of treatment. As treatment and care, the aim of health center is to return the elderly to home. On the other hand, there are a few health centers for returning the elderly to home in Korea. Furthermore, in Japan, a project for preventive care has been begun. The project is needed in Korea.

  • This study is about major symptoms of elderly and medical services for elderly in long-tenn care facilities. The subject of this study was 298 patients over 00 years old staying in two geriatric hospitals and two nursing homes. The symptoms and medical services were level of patient classification from RUG(Resource Utilization Group)-III which is applied for both Medicare and Medicaid for skilled nursing facilities reimbursement system in US and designed for measuring patient characteristics and medical staff time. This classification is explained by each patient resource(staff time) utilization level which is called CMI(Case-Mix Index). In this study, the symptoms and services were compared by facility type and they were categorized by level and compared by CMI. Major findings are as follows; 1. There were more elderly who have cognitive function problems in nursing homes than patients in geriatric hospitals. There were more patients with behavioral problems in geriatric hospitals than residents in nursing homes. These results were both statistically significant. 2. The patients in geriatric hospitals received significantly more nursing rehabilitation services, rehabilitation services and extensive services than residents in nursing homes. Other hands, special care services were provided significantly more to residents in nursing homes than elderly in geriatric hospitals. 3. ADL and depression variables had higher CMI when the symptoms were heavier condition. The CMI were not matched with levels of cognitive function problems and behavioral problems. 4. The CMI matched well significantly with levels of nursing rehabilitation services, special care services, and clinically complex services provided for the patient in geriatric hospitals and only nursing rehabilitation services in nursing homes. The CMI for rehabilitation services level and extensive services had regular trends. From the result of this study, the resource utilization level and services provided for elderly in each long-term care facilities were figured out. For the further study, it needs to have more concern about RUG-ill which classification variables were just analyzed.

METHODS: A total of 1027 male/female undergraduate students majoring in physical therapy were selected to examine the knowledge of the elderly. The knowledge of physical, psychological, and social aspects were investigated and measured by FAQ I of palmore (1988) and a scale developed by Yunjin et al. (1982). T-test, One-way-ANOVA and Scheff test were used. RESULTS: The percentage of the students who correctly answered the questions on the elderly was 53.6%. In particular, the higher error rates were seen in the social (56.9%) and psychological (57.6%) aspects compared in the physical (33.7%) aspects. Those who answered the questions correctly were over 30s (63.69%, p=0.000), seniors (57%, p=0.000)and grown in cities (53.91%, p=0.001). In addition, students who took lessons on the elderly (57.71%, p=0.032)and had experiences in voluntary services (54.09%, p=0.023) were significantly higher in answering the questions correctly. CONCLUSION: When students learned about the elderly, social and psychological aspects should be reinforced. The knowledge levels of the elderly can be elevated though voluntary services. It is thought that a follow-up study would be continuously neede to improve physical therapists' knowledge on the elderly and quality of available services.
