Expression of Hepatitis B Virus S Gene in Pichia pastoris and Application of the Product for Detection of Anti-HBs Antibody

  • Antibody to hepatitis B surface antigen (HBsAb) is the important serological marker of the hepatitis B virus (HBV) infection. Conventionally, the hepatitis B surface antigen (HBsAg) obtained from the plasma of HBV carriers is used as the diagnostic antigen for detection of HBsAb. This blood-origin antigen has some disadvantages involved in high cost, over-elaborate preparation, risk of infection, et al. In an attempt to explore the suitable recombinant HBsAg for the diagnostic purpose, the HBV S gene was expressed in Pichia pastoris and the product was applied for detection of HBsAb. Hepatitis B virus S gene was inserted into the yeast vector and the expressed product was analyzed by sodium dodecyl sulphate polyacrolamide gel electrophoresis (SDS-PAGE), immunoblot, electronic microscope and enzyme linked immunosorbent assay (ELISA). The preparations of synthesized S protein were applied to detect HBsAb by sandwich ELISA. The S gene encoding the 226 amino acid of HBsAg carrying ahexa-histidine tag at C terminus was successfully expressed in Pichia pastoris. The His-Tagged S protein in this strain was expressed at a level of about 14.5% of total cell protein. Immunoblot showed the recombinant HBsAg recognized by monoclonal HBsAb and there was no cross reaction between all proteins from the host and normal sera. HBsAb detection indicated that the sensitivity reached 10 mIu (micro international unit)/ml and the specificity was 100% with HBsAb standard of National Center for Clinical Laboratories. A total of 293 random sera were assayed using recombinant S protein and a commercial HBsAb ELISA kit (produced by blood-origin HBsAg), 35 HBsAb positive sera and 258 HBsAb negative sera were examined. The same results were obtained with two different reagents and there was no significant difference in the value of S/CO between the two reagents. The recombinant HBV S protein with good immunoreactivity and specificity was successfully expressed in Pichia pastoris. The reagent for HBsAb detection prepared by Pichia pastoris-derived S protein showed high sensitivity and specificity for detection of HBsAb standard. And a good correlation was obtained between the reagent produced by recombinant S protein and commercial kit produced by blood-origin HBsAg in random samples.

Effect of a Gluten Free Diet on Hepatitis B Surface Antibody Concentration in Previously Immunized Pediatric Celiac Patients

  • Purpose: To evaluate the effect of gluten-free diet (GFD) on hepatitis B surface antibody (HBsAb) concentrations among previously immunized pediatric celiac disease (CD) subjects. Methods: We retrospectively evaluated pediatric CD subjects in serological remission who were previously immunized for hepatitis B virus as infants. The temporal relationship between HBsAb concentration, the amount of time on a GFD, and age were evaluated. Results: Overall, 373 CD subjects were analyzed: 156 with HBsAb sampled prior to GFD initiation and 217 after initiation of a GFD and in serological remission. Median age at HBsAb concentration measurement for those before and after GFD initiation was 5.3 years (interquartile range [IQR], 3.1-9.2 years) and 7.6 years (IQR, 5.4-10.9 years), respectively (p<0.001). There was no sex difference between the groups. The median time of HBsAb measurement was 2 months (IQR, 0-5.7 months) before and 12.8 months (IQR, 5.3-30.3 months) after initiation of GFD. The HBsAb concentration was low in 79 (50.6%) and 121 (55.7%) subjects before and after GFD initiation, respectively (p=0.350). Age was inversely associated with low HBsAb concentrations. Neither being on a GFD nor sex was associated with low HBsAb concentrations. Conclusion: Adherence to a GFD does not affect HBsAb concentration in children with CD. Age is inversely associated with HBsAb concentration.

생체 부분 간이식에서 De Novo Hepatitis B에 대한 B형 간염 면역글로불린의 예방적 효과 (Efficacy of Hepatitis B Immune Globulin for Prevention of De Novo Hepatitis B in Living-related Liver Transplantation)

  • 목 적: HBcAb 양성인 공여자로부터 생체 부분 간이식을 시행할 때 HBsAg 음성인 수혜자가 B형 간염 바이러스에 감염될 가능성이 높다. 본 연구에서는 삼성서울병원의 소아 생체 부분 간이식에서 능동 면역을 주로 사용하였던 초창기와 Hepatitis Bimmunoglobulin (HBIg) 단독 요법을 사용하였던 후기를 비교 분석하여 간이식 후 de novo hepatitis B발생에 대한 HBIg 단독 요법의 예방 효과에 대해 조사해 보고자 하였다. 방 법: 1996년 5월부터 2002년 6월까지 시행한 생체 부분 간이식에서 공여자가 HBcAb 양성이었으며 수혜자가 HBsAg 음성인 소아는 15명이었다. 다른 이유로 사망한 2명을 제외한 13명 중 11명은 HBsAb 양성, 2명은 naive (HBsAb 음성, HBcAb 음성)였다. 모든 환자는 간이식 전 B형 간염 바이러스 예방 접종을 실시하였다. 초기 단계에는(1997년 1월~1997년 11월, 3명) 수혜자가 HBsAb 양성인 경우 간이식 후 B형 간염 추가접종을 실시하였다. 후기 단계에서는(1997년 12월 이후, 10명) 간이식 전후에 모두 B형 간염 예방 접종을 실시한 이후 항체 양성인 수혜자에게 HBsAb 항체가를 200 IU/L 이상 유지하기 위해 HBIg를 단독 유지요법으로 사용하였다. HBIg의 심한 부작용으로 인해 1명의 경우 Lamivudine을 사용하였다. De novo hepatitis B의 예방효과를 병력 고찰을 통하여 후향적으로 분석하였다. 결 과: 13명 중 3명(23.1%)에서 de novo hepatitis B가 발생하였다. 능동 면역만을 시행한 초기 단계에서 3명 중 3명 모두 7~19개월에 HBsAg 양성으로 혈청 변환을 하였다. 1명은 간이식 전 naive 혈청 소견이었고 2명은 HBsAb 양성인 상태였다. HBIg를 사용한 후기 단계에서는 10명 모두 관찰 기간 7~55개월 동안 HBsAg 음성으로 남아 있다. 결 론: HBIg 단독 요법은 HBcAb 양성인 공여자의 간을 이식받은 HBsAg 음성 수혜자에서 de novo hepatitis B를 예방하는데 효과적이다.

B형간염바이러스 및 C형간염바이러스의 표식자 양성율과 원발성 간세포 암의 연관성에 대한 환자-대조군 연구 (A Case-Control Study on Association Between Hepatocellular Carcinoma and Infection of Hepatitis B and Hepatitis C Virus)

  • To investigate the association between hepatocellular carcinema(HCC) and infection of hepatitis B virus(HBV) and hepatitis C virus(HCV) in an HBV endemic area, a case-control study of 254 patients with HCC and of 1,270 age and sex matched health control subjects was done. Among the 254 HCC patients 166(65.4%) were positive for hepatitis B surface antigen(HBsAg), 49(19.3%) were positive for HCV antibody (anti-HCV Ab). The crude odd ratio of patients with HBsAg was 36.1(95% CI :22.4-58.2) and with anti-HCV Ab was 9.0(95% CI :5.5-14.6). In an analysis, which HBsAg(-), HBcAb(-), anti-HCV Ab(-) group was chosen as referent group, odd ratio of HBsAg(+) group was 14.4(95% CI: 7.2-28.9) and of anti- HCV Ab(+) was 10.7(95% CI: 2.9-40.0). odd ratio of anti-HCV Ab(+), HBsAg(+) group and anti-HCV Ab(+), HBsAg(-), HbcAb(+) group for HCC were elevated to 27.3(95% CI : 9.0-82.9), 15.9(95% CI:7.1-35.8) respectly, The odd ratio of anti-HCV Ab(-), HBsAg(-), HBcAb(+) group was 2.4(95% CI : 1.1-5.0). These result suggested that HBV and HCV were associated with HCC. In HBV endemic area patients with HBcAb alone should be considered risk group for HCC.

HBsAb와 HBcAb가 양성인 장기 공여자의 간조직에서 Hepatitis B Virus DNA의 발현 (Detection of Hepatitis B Virus DNA in Liver Grafts Obtained from HBsAb and HBcAb Positive Organ Donors)

  • 목 적: 최근 HBsAb 및 HBcAb 양성인 공여자의 간이 식편을 이식 받은 수혜자에서 신생 B형 간염이 발생하는 것이 보고 되고 있으며, 저자들도 약 40%에서 신생B형 간염이 발생하는 것을 보고하였다. 한국인에서의 HBcAb 양성률은 50%가 넘는 것으로 보고하고 있는데, 이는 임상 간이식의 걸림돌이 될 수 있으며 저자들은 이를 예방하기 위한 일환으로써 본 연구를 시행하였다. 방 법: 1997년 11월부터 1998년 11월까지 12개월 동안 서울 아산병원에서 생체 간이식 공여자가 과거 B형간염과 C형 간염 감염의 증거가 없으면서 HBsAg 음성이면서 HBsAb 양성, HBcAb 양성인 성인 공여자 6명을 대상으로 하였다. 간이식 수술 시 동결 생검을 위하여 채취한 절편의 일부를 보관하여 실험에 사용하였다. 동결 절편 조직에서 DNA를 분리하여, HBV DNA의 표면 구역과 핵심 구역에 대한 시발체를 이용하여 이중 중합효소 연쇄 반응을 시행하여 검사를 시행하였다. 결 과: 공여자 6명의 조직에서 표면 구역이 모두 양성으로 관찰되었으며, 핵심 구역은 4명에서 양성으로 관찰되었다. 그 중 4명의 간을 이식받은 소아 수혜자는 모두 예방법을 시행하면서, 신생 B형 간염의 발생은 관찰되지 않고 있다. 결 론: 본 결과는 간이식 후 발생하는 신생 B형 간염의 원인으로 HBcAb 양성이 위험 인자임을 지지하고 있다. HBcAb 양성 공여자의 간이식편에서 핵심구역은 66%에서 양성으로 보여 이식 후 잠재 HBV 감염 혹은 신생 B형 간염의 발생을 막기 위해 예방적 치료가 필요할 것으로 사료된다.

Clinical Analysis of Stages of HBV Infection in 100 Cases of Lymphoma

  • Objective: HBV infection may cause damage to the immune system and induce lymphomas as a result. Some scholars have indicated that HBsAg(+) reflecting HBV infection may have a relationship with lymphoma development. This study was designed to find out the specific stage of HBV infection which may be related to lymphoma. Methods: HBV serum markers, including HBsAg, HBsAb, HBeAg, HBeAb, HBcAb were tested among 100 lymphoma patients and 100 other patients who were diagnosed with non-lymphoma diseases in the First Hospital of Jilin University from 2010.1.1 to 2012.12.31. Three subgroups were established depending on different combinations of HBV serum markers. Subgroup 1 was HBsAg(+) representing the early stage of HBV infection. Subgroup 2 was HbsAb(+) representing convalescence and Subgroup 3 was "HbsAg and HbsAb negative combined with other positive markers" representing the intermediate stage of HBV infection. Chi square tests were used to compare the rates of three subgroups in lymphoma and control groups. Results: The rates of Subgroup were 13% and 5% respectively, an association between HBsAg and lymphoma being found (P<0.05). There was no difference between rate of Subgroup 2 of lymphoma group (15%) and that of control group (16%). In lymphoma group and control group, the rate of Subgroup 3 was different (12% vs 4%). This evidence was not specific to T cell lymphoma, B cell lymphoma or Hodgkin's lymphoma. Conclusions: Among serum markers of HBV, the combination of serum markers representing the early stage and intermediate stage of HBV infection have a relationship with lymphoma. Convalescence from HBV infection appears to have no relationship with lymphoma.

A Study on the Trend of Changes in the Positive Rate of Hepatitis B Surface Antigens and Antibodies over the Last 9 Years in One Health Care Center

  • The purpose of this study was to provide basic data on hepatitis B infection control in the community through the results of the hepatitis B surface antigen and antibody tests conducted at the Cheongyang-County Health Medical Center. From 2012 to 2020, we retrospectively analyzed the HBsAg, HBsAb, HBeAg, HBeAb, and HBV DNA results of 7,329 hepatitis B-related testers. Among 7,329 subjects, the HBsAg positivity rate was 1.7%, and the positivity rate according to age was the highest at 4.4% in their 30s, 4.2% in their 40s, 4.1% in their 50s, 2.0% in their 60s, 1.9% in their 70s and over, and 10 it was shown in the order of 0.3% from less than large. The HBsAb positivity rate was 43.1% for men, 38.2% for men, and 46.7% for women (P<0.001). To summarize the above results, for infection control of hepatitis B in Cheongyang-County, hepatitis surface antigen proton management is required for those in their 30s or older, and it is thought that efforts to acquire immunity are necessary for those in their 20s or younger.

치위생과 학생들의 B형 간염 바이러스의 표지자와 인지도에 관한 연구 (A Study on occurrence rates from serologic marker of hepatitis B virus and on perception in dental hygiene students)

  • This study is aimed at offering basic data about prevention of Hepatitis B virus and infection control to dental hygiene students who will work mostly at dental offices which are characteristically exposed to a high risk of HBV infection. For this study, surveyed were 188 sophomores and juniors of the dental hygiene department who volunteered to undergo the tests of HBsAg and HBsAb. The examination of the blood collected from the samples and analysis of their perception about hygiene resulted in following conclusion: Positive HBsAg Four was found in 4 testees(2.1%) while 118(62.8%) revealed positive HBsAb. Juniors showed higher positive rate in HBsAg whereas sophomores had higher positive rate in HBsAb. But such difference has statistically no significance. Generally, students seemed to pay little attention to HBV, judging from the survey of their perception about the present state of HBsAg and HBsAb(p>0.05), conduct of infection(p<0.05), completion of 3 requested vaccinations(p>0.05), and formation of antibody(p>0.05). With regard to the infection routes of HBV, most students(92.4%) replied "through blood", which is statistically insignificant, though. Next ratio goes to the reply "through contaminated injectors". This reply came more from sophomores than from juniors, a difference which is statistically significant(p<0.05). The lowest rate of possible infection(29.2%) was thought to be "through breast-feeding of a positive mother"(p<0.05). In general, it turned out that sophomores had more knowledge about the infection routes of HBV than juniors. In terms of clinical history among family members, 6(3.1%) reported that some of their family members are currently suffering from a liver complaint, 3(1.6%) replied their family members were once afflicted, and 4(2.1%) said their members died of hepatitis. Except 10(4.7%), all the surveyees replied that their states of health are better than normal. Generally, sophomores are healthier than juniors except for the very health case, a difference which is statistically of no value.

사업장 보건관리에 따른 A형, B형 간염 항체 양성률 (Seroprevalence of Hepatitis A and B Virus Antibody of Employees among Three Companies with Different Health Policy)

  • Objectives: We checked hepatitis A virus antibody(anti-HAV IgG) and hepatitis B virus antibody(HBsAb) in three large companies. The result could be a guideline to establish proper health policy for Hepatitis A and B virus preventive plan in company. Methods: We investigated the seroprevalence of anti-HAV IgG and HBsAb, and demographic characteristics of employee volunteer in three companies in southern area of Korea. Company was divided three according to health policy for hepatitis A and B. Results: The seroprevalence rate of anti-HAV IgG in company A, B, and C was 53.6%, 25.8%, and 17.7%(P<0.001), respectively. The seroprevalence rate of HBsAb in workplace A, B, and C was 79.7%, 82.4%, and 70.9%(P<0.001), respectively. Anti-HAV IgG showed more considerable difference among the companies. Conclusions: The results confirmed that low rate of IgG anti-HAV and HBsAb, particularly in the company that had low level of hepatitis education and vaccination program. This study was important for establishing hepatitis education policies, to prevent and control outbreaks in companies.

B형 간염 예방접종 전후 연령별 B형 간염 표면항체 양성률에 대한 융합적 연구 (A convergence study on the Positive Rate of Hepatitis B Surface Antibody by Age before and after HBV Vaccination)

  • B형 간염 예방접종 전후 연령별 B형 간염 표면항체 양성률을 분석하기 위하여 제주지역 종합병원 검사의학과에 B형 간염 표면항체 검사가 의뢰된 13,855명의 혈청 검체를 대상으로 CMIA법으로 조사하였다. HBs Ab 양성률은 5,176건으로 37.36%이었으며, 성별에 따른 양성률은 여자가 40.13%로 남자의 34.77%보다 높게 나타났다. HBs Ab 측정 평균값이 가장 높은 연령군은 40대(399.86 mIU/mL)이었으며, 가장 낮은 연령군은 90대(211.50 mIU/mL)로 나타났다. HBs Ab 양성률이 가장 높은 연령군은 30대이며, 10대의 양성률이 가장 낮은 분포를 보인 것은 주목할 만하다. 10대~20대의 B형 간염 표면항체 양성률이 현저히 감소하는 경향을 보였으며, 특히 15, 18, 19세가 통계적으로 유의하였다. 이에 예방접종의 역가의 측정을 고려하고 추가접종 시기를 명확히 해야 한다고 사료된다.
