The structure of an LCD backlight unit (BLU) was simplified by installing a micro reverse-pyramid array (MRPA) or a micro reverse-cone array (MRCA) on the top surface of the light guide panel (LGP) in order to eliminate the conventional diffuser sheet and the prism sheet. The optimum conditions of the MRPA and the MRCA in the new light guide panel were obtained through optical simulation. The change of the luminance and view angle that depend on the side angle, the height, and the length at the top of the MRPA and MRCA were studied. The optimized side angle and the view angle of the MRPA and MRCA were 59 and 57 degrees for the side angles and 68 and 64 degrees for the view angles, respectively.
Purpose : The purpose of this study is to develop a surgical stent for dental implant procedure that can be easily applied and affordable by using cone beam computerized tomography (CBCT). Materials and Methods : Aluminum, Teflon-PFA (perfluoroalkoxy), and acetal (polyoxymethylene plastic) were selected as materials for the surgical stent. Among these three materials, the appropriate material was chosen using the CBCT images. The surgical stent, which could be easily placed into an oral cavity, was designed with chosen material. CBCT images of the new surgical stent on mandible were obtained using Alphard-3030 dental CT system (Asahi Roentgen Co., Ltd., Kyoto, Japan). The point of insertion was prescribed on the surgical stent with the multiplanar reconstruction software of OnDemand3D (CyberMed Inc., Seoul, Korea). Guide holes were made at the point of insertion on the surgical stent using newly designed guide jig. CBCT scans was taken for the second time to verify the accuracy of the newly designed surgical stent. Results : Teflon-PFA showed radiologically excellent image characteristics for the surgical stent. High accuracy and reproducibility of implantation were confirmed with the surgical stent. Conclusion : The newly designed surgical stent can lead to the accurate implantation and achieve the clinically predictable result.
Purpose: The aim of this study was to evaluate image artifacts in the vicinity of dental implants in cone-beam computed tomography (CBCT) scans obtained with different spatial orientations, tube current levels, and metal artifact reduction algorithm (MAR) conditions. Materials and Methods: One dental implant and 2 tubes filled with a radiopaque solution were placed in the posterior region of a mandible using a surgical guide to ensure parallel alignment. CBCT scans were acquired with the mandible in 2 spatial orientations in relation to the X-ray projection plane (standard and modified) at 3 tube current levels: 5, 8, and 11 mA. CBCT scans were repeated without the implant and were reconstructed with and without MAR. The mean voxel and noise values of each tube were obtained and compared using multi-way analysis of variance and the Tukey test(α=0.05). Results: Mean voxel values were significantly higher and noise values were significantly lower in the modified orientation than in the standard orientation (P<0.05). MAR activation and tube current levels did not show significant differences in most cases of the modified spatial orientation and in the absence of the dental implant (P>0.05). Conclusion: Modifying the spatial orientation of the head increased brightness and reduced spatial orientation noise in adjacent regions of a dental implant, with no influence from the tube current level and MAR.
This study focuses on the numerical simulations of the cone penetration tests in a sand ground. The mechanical responses of sand were described using the modified Mohr Coulomb plasticity model based on the critical state soil mechanics. In the plasticity model, the dilatancy angle was not a constant, but a function of the distance to the critical state line from the current state of void ratio and mean effective stress. To simulate cone penetration tests numerically, this study relied on Lagrangian finite element method under the axisymmetric condition. To enable penetration of the cone penetrometer without tearing elements along the symmetric axis, the penetration guide concept was adopted in this study. The results of numerical simulations on the calibration chamber cone penetration tests had good agreement with the experimental results.
Purpose: This study developed a convolutional neural network (CNN) model to diagnose maxillary sinusitis on panoramic radiographs(PRs) and cone-beam computed tomographic (CBCT) images and evaluated its performance. Materials and Methods: A CNN model, which is an artificial intelligence method, was utilized. The model was trained and tested by applying 5-fold cross-validation to a dataset of 148 healthy and 148 inflamed sinus images. The CNN model was implemented using the PyTorch library of the Python programming language. A receiver operating characteristic curve was plotted, and the area under the curve, accuracy, sensitivity, specificity, positive predictive value, and negative predictive values for both imaging techniques were calculated to evaluate the model. Results: The average accuracy, sensitivity, and specificity of the model in diagnosing sinusitis from PRs were 75.7%, 75.7%, and 75.7%, respectively. The accuracy, sensitivity, and specificity of the deep-learning system in diagnosing sinusitis from CBCT images were 99.7%, 100%, and 99.3%, respectively. Conclusion: The diagnostic performance of the CNN for maxillary sinusitis from PRs was moderately high, whereas it was clearly higher with CBCT images. Three-dimensional images are accepted as the "gold standard" for diagnosis; therefore, this was not an unexpected result. Based on these results, deep-learning systems could be used as an effective guide in assisting with diagnoses, especially for less experienced practitioners.
'Crossed occlusion' is the condition in which occlusal intercuspation is lost when several teeth on upper and lower jaw remain. This report describes a clinical case in which a patient had two upper-left posterior teeth and two lower-right posterior teeth; typically known as left-right crossed occlusion. Considering the patient's general condition and financial situation, the treatment plan included placement of two implants on each jaw against the remaining teeth using surgical guide. To find out the ideal position of implants, digital diagnostic wax-up was preceded by superimposing the cast and cone beam computed tomography image, which was aided with radiographic stents. The consequent surveyed implant bridge provided stable vertical stop for fabrication of the implant assisted removable partial dentures. The patient was satisfied with the functionality and esthetics of definitive prosthesis.
Purpose: To examine the danger zone of mesial root of mandibular first molar of patient without extraction using CBCT (cone-beam computed tomography) to avoid the risk of root perforation. Materials and Methods: 20 mandibular first molars without caries and restorations were collected, CT images were obtained by CBCT ($PSR9000N^{TM}$, Asahi Roentgen Co., Japan), reformed and analyzed by V-work 5.0 (CyberMed Inc., Korea), Distance between canal orifice and furcation was measured. In cross sectional images at 3, 4 and 5 mm below the canal orifice, distal wall thickness of mesiobuccal canal (MB-D), distal wall thickness of mesiolingual canal (ML-D), distal wall thickness of central part (C-D), mesial wall thickness of mesiobuccal canal (MB-M) and mesial wall thickness of mesiolingual canal (ML-M) were measured, Results: The mean distance between the canal orifice and the furcation of the roots is 2.40 mm, Distal wall is found to be thinner than mesial wall. Mean dentinal wall thickness of distal wall is about 1 mm, The wall thickness is thinner as the distance from the canal orifice is farther. But significant differences are not noted between 4 mm and 5 mm in MB-D and C-D, MB-D is thinner than ML-D although the differences is not significant. Conclusion: The present study confirmed the anatomical weakness of distal surface of the coronal part of the mesial roots of mandibular first molar by CBCT and provided an anatomical guide line of wall thickness during endodontic treatment.
In this study pressure and shaft torque pulsation were measured with variation of head and flow during the model test for a 15 MW Francis Turbine. Pressure pulsations were measured at the inlet of the spiral casing and 4 points in the cone of the diffuser and shaft torque pulsation at the upper position of the turbine. The maximum amplitude of pressure pulsation appeared 2.03% of the maximum rated head with the frequency of 25% of the rated revolution and at the guide vane opening of $10^{\circ}$. Shaft torque pulsation appeared 0.01% of the rated shaft torque, fairly low value. Air was admitted through the cone and pressure pulsation gradually decreased with increase of air flow and kept nearly constant after 5% of the rated flow. A new Francis turbine of which specific speed is 115 m-kW had been designed to rehabilitate the old one and the model test was performed at EPFL. The commercial code, STAR-$CCM^+$ was used for numerical simulation of flow.
Purpose: Special care is necessary to avoid invading important anatomic structures during surgery when presurgical planning is made based on radiographs. However, none of these types of radiography represents a perfect modality. The purpose of this study was to determine the reliability of presurgical planning based on the use of two types of radiographic image (digital panoramic radiography [DPR] and cone-beam computed tomography [CBCT]) by beginner dentists to place implants, and to quantify differences in measurements between radiographic images and real specimens. Methods: Ten fresh cadavers without posterior teeth were used, and twelve practitioners who had no experience of implant surgery performed implant surgery after 10 hours of basic instruction using conventional surgical guide based on CBCT or DPR. Two types of measurement error were evaluated: 1) the presurgical measurement error, defined as that between the presurgical and postsurgical measurements in each modality of radiographic analysis, and 2) the measurement error between postsurgical radiography and the real specimen. Results: The mean presurgical measurement error was significantly smaller for CBCT than for DPR in the maxillary region, whereas it did not differ significantly between the two imaging modalities in the mandibular region. The mean measurement error between radiography and real specimens was significantly smaller for CBCT than for DPR in the maxillary region, but did not differ significantly in the mandibular region. Conclusions: Presurgical planning can be performed safely using DPR in the mandible; however, presurgical planning using CBCT is recommended in the maxilla when a structure in a buccolingual location needs to be evaluated because this imaging modality supplies buccolingual information that cannot be obtained from DPR.
Apical surgery for a mandibular molar is still challenging for many reasons. This report describes the applications of computer-guided cortical 'bone-window technique' using piezoelectric saws that prevented any nerve damage in performing endodontic microsurgery of a mandibular molar. A 49-year-old woman presented with gumboil on tooth #36 (previously endodontically treated tooth) and was diagnosed with chronic apical abscess. Periapical lesions were confirmed using cone-beam computed tomography (CBCT). Endodontic microsurgery for the mesial and distal roots of tooth #36 was planned. Following the transfer of data of the CBCT images and the scanned cast to an implant surgical planning program, data from both devices were merged. A surgical stent was designed, on the superimposed three-dimensional model, to guide the preparation of a cortical window on the buccal side of tooth #36. Endodontic microsurgery was performed with a printed surgical template. Minimal osteotomy was required and preservation of the buccal cortical plate rendered this endodontic surgery less traumatic. No postoperative complications such as mental nerve damage were reported. Window technique guided by a computer-aided design/computer-aided manufacture based surgical template can be considerably useful in endodontic microsurgery in complicated cases.
