Performance of HPV DNA Testing with Hybrid Capture 2 in Triaging Women with Minor Cervical Cytologic Abnormalities (ASC-US/LSIL) in Northern Thailand

  • Background: Minor cervical cytologic abnormalities include atypical squamous cells of undetermined significance (ASC-US) and low-grade squamous intraepithelial lesion (LSIL). Approximately 10-20% of women with minor cytologic abnormalities have histologic high-grade squamous intraepithelial or worse lesions (HSIL+). In Thailand, women with minor cytologic abnormalities have a relatively high risk of cervical cancer, and referral for colposcopy has been suggested. A triage test is useful in the selection of women at risk for histologic HSIL+ to reduce the colposcopy burden. The aim of this study was to assess the performance of high-risk HPV DNA test in triage of women with minor cytologic abnormalities in northern Thailand. Materials and Methods: All women with ASC-US/LSIL cytology who were referred to our colposcopy clinic from October 2010 to February 2014 were included. HPV DNA testing was performed using Hybrid Capture 2 (HC2). All patients received colposcopic examination. Accuracy values of HC2 in predicting the presence of histologic HSIL+ were calculated. Results: There were 238 women in this study (121 ASC-US and 117 LSIL). The HC2 positivity rate was significantly higher in the LSIL group than in ASC-US group (74.8% versus 41.0%, p<0.001). Histologic HSIL+ was detected in 9 women (7.4%) in the ASC-US group and 16 women (13.7%) in the LSIL group (p=0.141). There was no histologic HSIL+ detected among HC2-negative cases (sensitivity and negative predictive value of 100%). The performance of HC2 triage was highest among women aged >50 years with ASC-US cytology. An increase in the cut-off threshold for positive HC2 resulted in a substantial decrease of sensitivity and negative predictive value. Conclusions: HPV DNA testing with HC2 shows very high sensitivity and negative predictive value in triage of women with minor cervical cytologic abnormalities in northern Thailand. An increase of the cut-off threshold for HC2 triage is not recommended in this region.

A STUDY ON THE EFFECT OF DENTIN BONDING AGENTS APPLIED OVER ENAMEL ABOUT THE BOND STRENGTH OF COMPOSITE RESIN (접착강화제가 치아경조직과의 접착강도 변화에 미치는 영향에 관한 연구)

  • The purpose of this study was to investigate the effect of dentin bonding agents on the bond strength of composite resin restorations in case of applying the dentin bonding agents to acid etched enamel surfaces. Freshly extracted 364 bovine anterior teeth were selected as a adherents. 320 enamel specimens were divided into two groups(unetched group (1) and etched group (2) for testing the shear bond strength, 40 specimens were used for the hardness testing, and 4 specimens of rest were to observe the resin-tag formation into etched enamel surfaces. All surfaces of enamel specimens were polished with 320~1500 SiC paper under continuous running water. In Group (1), 100 enamel specimens were polished and unetched. 220 polished enamel specimens in Group (2) were etched with 37 % phosphoric acid solution for 60 seconds, washed with water for 20 seconds, and dried with a light air pressure for 60 seconds. Three kinds of dentin bonding agents(Gluma, Prisma, Scotchbond 2) were evaluated the effect on the bond strength to conditioned enamel surfaces. Shear bond strengths were measured on the three cases such as a coating of primer only, a coating of sealer only, and a sequential coating of primer and sealer to acid etched enamel surfaces were compared with the bond strengths measured by the coating of enamel bonding agent followed by the bonding of composite resin (Photo clearfil bright, Kuraray, Japan) to unetched and acid etched enamel surfaces. In addition, the hardness tested on the adhesive fractured surface between composite resin enamel as a mean of evaluation of a factor whether the mechanical bond strengths were affected and the penetration of dentin bonding agents into etched enamel surfaces was also observed. Bond strengths were measured using the method of shear bond strength by a universal testing machine (Instron-4467, USA), statistical test were applied to the results using a one way analysis variance(ANOVA), and hardness was measured by the Vicker's Hardness Tester(MHT-i, Matsuzawa, Japan) and the penetration of the resins were observed by the SEM (Hitachi, S-2300, Japan). The following conclusions were drawn; 1. Enamel bonding agent showed to affect the improvement of bond strength of composite resin to enamel surface both unetched and etched. 2. Dentin bonding agents could be resulted in increase of bond strength to unetched enamel surface, but there were no statistical significances. 3. Bond strengths to etched enamel surface were significantly decreased with a coating of dentin primer only. 4. Coating of sealer only and coating of primer and sealer noticed the similar bond strengths of composite resin to etched enamel using the enamel bonding agents. 5. The applying method proved to be more effective than the kinds of dentin bonding agents on the bond strength of composite resin to etched enamel than the kind of dentin. 6. Vicker's hardness numbers of dentin bonding agents were lower than that of composite resin, but the degree of penetration of dentin bonding agents into etched enamel surfaces was excellent.

Effects of Koryo Hand Therapy on Menopausal Symptoms and Blood Lipid Levels in Middle-aged Women (중년여성의 폐경증상과 혈청지질대사에 대한 수지요법의 효과)

  • Objectives: The purpose of this study was to examine the effects of Koryo Hand Therapy(KHT) on menopausal symptoms and lipid levels in middle-aged women. Methods: The design of this study was a nonequivalent control-group pretest-posttest design. There were 32 subjects, 40-60 years of age, and assigned to two group. There were 16 members in the experimental group and 16 members in the control group. KHT was applied three times a week, for a total of 8weeks to the experimental group. The instruments used in this study to measure the effects of KHT were the menopausal symptoms scale and clinical laboratory testing(Blood Lipid Levels). Data was analyzed by Fisher's Exact Test, Mann-Whitney U-test, and ANCOVA, using SPSS/Win PC 13.0. Results: There was a statistically significant decrease in the menopausal symptoms, and levels of the total cholesterol and LDL cholesterol in the experimental group compared to the control group. Conclusions: KHT was effective in decreasing menopausal symptoms in middle-aged women. Therefore KHT is recommended as a nursing intervention strategy for middle-aged women.

The Effects of Music Therapy in the Reduction of Anxiety and Discomfort on Patients Undergoing Gastrofibroscopy (음악요법이 위내시경 검사자의 불안과 불편감 감소에 미치는 효과)

  • Purpose: This study was to examine the effects of music therapy on the levels of anxiety and discomfort in patients undergoing gastrofibroscopy. Method: This study was equivalent control group pre-post test design. The subjects were 61 patients who visited H hospital internal medicine department for gastrofibroscopy ; 31 were randomly selected for the control group and the remaining 30, for the experimental group. Result: 1. The experimental group who used music therapy showed a significantly lower level of anxiety during gastrofibroscopy than the control group. 2. The experimental group revealed a lesser change in pulse rate between pre and post gastrofibroscopy than the control group. 3. There was a significant difference in the levels of subjective and objective discomfort during gastrofibroscopy between the two groups. Conclusion: The results suggested that music therapy may help relieve anxiety and discomfort for patients undergoing gastrofibroscopy. These results showed that the use of music aided in the reduction of anxiety and discomfort during the unpleasant diagnostic procedure and, testing the effectiveness of music therapy deserves further study in other hospital settings.

An Analysis of $H^*$ Production by Korean Learners of English according to the Focus of English Sentences in Comparison with Native Speakers of English and Its Pedagogical Implications (영어 원어민과 비교한 한국인 학습자의 영어 문장 초점에 따른 영어 고성조 구현의 분석과 억양교육에 대한 시사점)

  • Focused items in English sentences are usually accompanied by changes in acoustic manifestation. This paper investigates the acoustic characteristics of $H^*$ in English utterances produced by natives speakers of English and Korean learners of English. To obtain more reliable results, the changes of the acoustic feature values (F0, intensity, syllable duration) were normalized by a median value and a whole duration of each utterance. Acoustic values of sentences with no focused words were compared with those of sentences with focused words within each group (Americans vs. Koreans). Sentences with focused words were compared between the two groups, too. In the instances in which a significant Group x Focus Location (initial, middle and final of a sentence) interaction was obtained, further analysis testing the effect of Group on each Focus Location was conducted. The analysis revealed that Korean learners of English produced focused words with lower F0, lower intensity and shorter syllable duration than native speakers of English. However, the effect of intensity change caused by focus was not significant within each group. Further analysis examining the interaction of Group and Focus Location showed that the change in F0 produced by Korean group was significantly lower in the middle and the final positions of sentences than by American group. Implications for the intonation training were also discussed.

A new in vitro method for evaluating the antimicrobial activity of toothpaste

  • The purpose of this study was to introduce a new in vitro method for evaluating the antimicrobial activity of toothpaste, reflecting the actual toothbrushing time and the dilution of toothpaste by salivation. We designed three experimental groups and one negative control group. The experimental groups were (1) 90 μL of toothpaste + 10 μL 1X phosphate-buffered saline (PBS, 9/10 dilution group), (2) 50 μL of toothpaste + 40 μL 1X PBS (1/2 dilution group), and (3) 25 μL of toothpaste + 65 μL 1X PBS (1/4 dilution group). During toothbrushing, saliva is continuously secreted into the oral cavity and the toothpaste concentration is diluted over time during toothbrushing. Therefore, the 1/2 and 1/4 dilution experimental groups were added. The negative control group was toothpaste diluted 20,000-fold with 1X PBS. Miracle Fresh Doctor toothpaste and Streptococcus mitis KCOM 1350, Prevotella intermedia KCOM 1107, Fusobacterium nucleatum subsp. polymorphum KCOM 1322, and Aggregatibacter actinomycetemcomitans KCOM 1306 were used as the toothpaste and target bacterial strains, respectively. The number of bacterial cells plated on agar plates in the negative control group was 1,000 CFU. If the number of colonies on the experimental group plate was less than one, the treatment was considered to have > 99.9% bactericidal activity. These results suggest that this new in vitro method for antimicrobial evaluation could be used as the standard method for testing the antimicrobial activity of toothpaste.

A Design of Mobile App Test System Using Web Socket (웹소켓을 이용한 모바일 앱 테스트 시스템 설계)

  • In this paper, we propose a testing method of the quality of device application effectively reflecting the necessity of increasing mobile device application quality test, and designed and developed it as a web system. We have studied traditional software testing methods for designing and developing a system. We have developed a next-generation alternative technology that can analyze and compensate for the weaknesses of the web and weaknesses of the Android in order to realize the access system and the responsive web system. As an example applied to the system, we analyzed the requirements of the system implementation and expressed the functions of the system through the use case diagram. The test scenarios were conducted by connecting 5,000 devices to each system using the existing HTTP polling method and the proposed web socket method. After setting the comparative group, the result of the proposed web management system was observed and compared through repeated tests. The research shows that the proposed web socket system has better performance than HTTP polling. The more the number of simultaneous processing of the devices, the lower the server stability but the better the time efficiency. In conclusion, the more the number of devices to be tested is, the more the number of devices can be effectively tested using the proposed web management system.

  • Perfluorinated chemicals are highly diverse and widely used. More than 160 substances are pre-registered under REACH and approximately 140 substances are in the existing chemicals list of Korea from this chemical group. Chemical structures of PFCs that are globally prohibited and still in uses are identified with OECD's classification of PFCs with an overall review on their uses in consumer products including textile products. Case examples for current domestic situation on use of PFCs as a major component of water-repelling agents in textile products as well as a brief summary of eight major PFC manufacturers' situation are presented from our survey study along the supply chains and the most recent report of EPA stewardship programme, respectively.

A Comparative Study of Oswestry Back Pain Disability Questionnaire Versus Computer Adaptive Testing for Measuring Back Pain

  • The aim of the present study was to compare measurement precisions of the Oswestry Back Pain Disability Questionnaire (ODQ) and a computer adaptive testing (CAT) method. The ODQ has been regarded as one of the most reliable condition-specific measure for back pain for decades. Cross-sectional study was carried out with two independent convenient samples from two out-patient rehabilitation clinics for back pain ($n_1=42$) and non-back pain group ($n_2=42$). Participants were asked to fill out the ODQ and CAT of International Classification of Functioning, Disability and Health-Activity Measure (ICF-AM). A series of Rasch analyses were performed to calculate person ability measures. The CAT measures had greater relative precision in discriminating the groups than did the ODQ measure in comparisons of the relative precision. The CAT measure appears to be more effective than did the ODQ measure in terms of measurement precision. By administering test items calibrated in a way, CAT measures using item response theory may promise a means with measurement precision as well as efficiency.

  • Multiple spurious operations (MSOs) mean multiple fire induced circuit faults causing an undesired operation of one or more systems or components. The Nuclear Energy Institute (NEI) of the United States published NEI 00-01 as guidelines for solving MSOs. And this guideline includes MSO scenarios of pressurized water reactor (PWR) and boiling water reactor (BWR). Nuclear power plant operators in U.S. analyzed MSOs under MSO scenarios included in NEI 00-01 and operators of PWRs in Korea also analyzed MSOs under the scenarios of NEI 00-01. As there are no pressurized heavy water reactors (PHWRs) in the United States, MSO scenarios of PHWRs are not included in the NEI 00-01 and any feasible scenarios have not been developed. This paper developed MSO scenarios which can be applied to PHWRs by reviewing the 63 MSO scenarios included in NEI 00-01. This study found that seven scenarios out of the 63 MSO scenarios can be applied and three more scenarios need to be developed.
