Diagnostic Accuracy of Clinical Test for Anterior Cruciate Ligament Injury: Systematic Review

  • Purpose: The aim of this study was to conduct a systematic review of randomized controlled studies from 2012 to present that explore the diagnostic accuracy of clinical tests used for diagnosing anterior cruciate ligament (ACL) injury. Methods: Study design: Systematic review. Literature search of the PubMed and Scholar databases was conducted using keywords related to diagnostic accuracy of clinical tests for ACL injury. The PRISMA Guidelines were followed to conduct this study. The Cochrane Risk of Bias Tool was utilized to assess the quality of each included study. Results: As a result, 8 studies were included, and 6 clinical tests used in ACL tears were evaluated for diagnostic accuracy. The pivot shift test was reported as having the highest +LR (29.5) value with a sensitivity of 59% and a specificity of 98%. However, the test with the lowest -LR value was the lever test, and the values were as follows: -LR (0.08), +LR (4.7), specificity (80%), sensitivity (94%). Conclusion: In this study, it was concluded that a single clinical test is not sufficient to determine the presence of ACL injury. Test combinations have a higher diagnostic accuracy than a single test. In this study, the accuracy of the clinical tests was examined without considering the amount of ACL rupture and acute-chronic condition. Further research is required to examine the impact of these two factors on diagnostic accuracy of clinical test.

Effect of the Scapular Retraction Exercise on the Subacromial and Costoclavicular Space According to a Thera-Band Resistance Intensity

  • Purpose: This study investigated effect of the scapular retraction exercise on the subacromial and costoclavicular space using different thera-band intensities. Methods: Thirty-six healthy adults participated in this study, and the subjects were randomly divided into three groups (low, moderate, and high-intensity groups). The exercise was conducted twice a week for three weeks. The subacromial and costoclavicular space were measured before and after the intervention via ultrasonography. Data analysis was performed using the two-way Analysis of Variance with repeated measures and the paired t-test. Results: The effect of the scapular retraction exercise on the subacromial and costoclavicular space using a thera-band after a 60° shoulder abduction has significant difference in all groups (p<0.05). However, there was no significant difference between the groups, and the values in the groups before the exercise did not differ significantly from those after the exercise (p>0.05). Conclusion: Our results indicate that the scapular retraction exercise after a 60° shoulder abduction can be used to widen the subacromial and costoclavicular spaces.

정상 성인의 가로막 이동거리 및 폐활량에 대한 고유수용성신경근촉진법 Bilateral pattern with Spiral pattern의 즉각적 효과 (Immediate Effects of Bilateral Pattern with Spiral pattern of PNF on Diaphragm Excursion and Vital Capacity in Normal Adults)

  • Purpose: This study aimed to examine the immediate effects of different breathing training techniques on diaphragm excursion and vital capacity in healthy adults. Specifically, the study focused on comparing respiratory exercise without PNF, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Methods: Twenty-seven healthy adults in their 20s participated in the study. Diaphragm excursion and vital capacity were evaluated under three different conditions. A one-way repeated ANOVA was used to analyze the differences in diaphragm excursion and vital capacity among the interventions. Results: Statistically significant differences were observed in diaphragm excursion among the interventions, comparing respiratory exercise without PNF, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Similarly, statistically significant differences were found in vital capacity among the interventions without PNF respiratory exercise, bilateral pattern respiratory exercise, and bilateral pattern with spiral pattern respiratory exercise. Conclusion: The study demonstrated that incorporating the spiral technique in respiratory exercise led to increased diaphragm excursion and lung capacity compared to other interventions. These findings suggest that PNF respiratory exercise combined with the spiral pattern may have clinical implications for the treatment of respiratory diseases. Further research is warranted to explore the long-term effects and clinical application of these approaches.

Effect of Post-Activation Potentiation according to Sequence of Velocity Using Isokinetic Device on Short-Term Performance of Lower Extremity: Taekwondo Athletes and Healthy Adults

  • Purpose: The purpose of this study was to figure out how PAP (Post-Activation Potentiation) phenomenon affects short-term performance efficiently. Methods: This study was conducted with 18 Taekwondo athletes and 16 healthy adults. By using isokinetic dynamometer, two different intervention, TDP (Top-down program) and BUP (Bottom-up program), were performed to measure isokinetic parameter; (peak torque: PT, total work: TW, average power: AP, and average torque: AT) of knee extensor for intragroup, intergroup comparison and two-way ANOVA. Results: The Taekwondo athletes group showed statistically significant differences in all isokinetic parameters PT, TW, AP, and AT after TDP (p<0.05). However, in the healthy adult group, the difference in isokinetic parameters according to the exercise sequence was not statistically significant. (p>0.05). PT and TW at TDP were statistically significant (p<0.05) when the rate of change in TDP and BUP was compared and analyzed considering the difference in physical ability between the Taekwondo athlete group and the healthy adult group. However, AP and AT were not statistically significant. Finally, when examining the interaction between the two groups and two exercise sequence according to isokinetic parameters, only TW (p<0.05) showed a statistically significant interaction, while PT (P=0.099), AP (P=0.103), and AT (P=0.096) did not. This study suggests that short-term performance can be improved through the PAP phenomenon when TDP is applied to the Taekwondo group. Conclusion: According to our result, for Taekwondo athletes, if the goal is to improve short-term performance just before the main game, we suggest a training program through TDP.

Effects of Elastic Taping and Non-elastic Taping on Static Balance Control Ability, Dynamic Balance Control Ability, and Navicular bone Drop in Young Adults

  • PURPOSE: This study examined the effects of the low dye taping technique on the static and dynamic balancing ability and navicular bone drop when the low dye taping technique was divided into elastic and non-elastic taping. METHODS: The subjects of the study were 31 volunteers without musculoskeletal disorders. The length (L) and anterior (A), posteromedial, and posterolateral values of the arch in the NO (normal eyes open), NC (normal eyes closed), PO (pillow with eyes open), and PC (pillow with close eyes closed) states were evaluated when barefoot and when Kinesio tape and non-elastic tape were applied. The measurements were analyzed using repeated ANOVA and an independent t-test. Post hoc tests were performed using a Fisher's LSD. RESULTS: A significant difference was found in the arch L and A values using a foot scanner (p < .05). In addition, there was a significant difference in dynamic balance in the three directions (p < .05), and no difference was found in the case of static balance. As a result, non-elastic tape application helps improve the dynamic balance ability and arch of the foot. CONCLUSION: The non-elastic tape technique is helpful for the foot arch function, and there is no difference in the static balance ability between Kinesio tape and non-elastic tape. Nevertheless, non-elastic tape is more helpful for the dynamic balance ability than Kinesio taping.

Selection of Multienzyme Complex-Producing Bacteria Under Aerobic Cultivation

  • The selection of multienzyme complex-producing bacteria under aerobic condition was conducted for improving the degradation of lignocellulosic substances. The criteria for selection were cellulase and xylanase enzyme production, the presence of cellulose-binding domains and/or xylan-binding domains in enzymes to bind to insoluble substances, the adhesion of bacterial cells to insoluble substances, and the production of multiple cellulases and xylanases in a form of a high molecular weight complex. Among the six Bacillus strains, isolated from various sources and deposited in our laboratory, Paenibacillus curdlanolyticus B-6 strain was the best producer of cellulase and xylanase enzymes, which have both cellulose-binding factors (CBFs) and xylan-binding factors (XBFs). Moreover, multiple carboxymethyl cellulases (CMCases) and xylanases were produced by the strain B-6. The zymograms analysis showed at least 9 types of xylanases and 6 types of CMCases associated in a protein band of xylanase and cellulase with high molecular weight. These cells also enabled to adhere to both avicel and insoluble xylan, which were analyzed by scanning electron microscopy. The results indicated that the strain B-6 produced the multienzyme complex, which may be cellulosome or xylanosome. Thus, P. curdlanolyticus B-6 was selected to study the role and interaction between the enzymes and their substrates and the cooperation of multiple enzymes to enhance the hydrolysis due to the complex structure for efficient cellulases and xylanases degradation of insoluble polysaccharides.

젊은 성인의 근 피로가 발생된 무릎관절 폄근에 냉 찜질과 온 찜질의 적용이 균형, 고유수용성감각 및 근력에 미치는 영향 (Effect of Ice and Hot packs on Balance, Proprioception and Muscle Strength in Young Adults with Knee Extensor Muscle Fatigue )

  • PURPOSE: The purpose of this study was to evaluate the effects of ice and hot packs on proprioception, balance, and muscle strength in knee extensor muscle fatigue. METHODS: A total of 31 male and female students in their twenties from a university in A, Chungnam, Korea, were selected as participants. Three experiments were conducted to assess static balance, dynamic balance, proprioception, and muscle strength before and after induction of muscle fatigue, and following intervention. RESULTS: In the case of stability typical (ST), a significant difference was observed in pillow with eye open (PO) when a Hot pack was applied (p < .05). The weight distribution index (WDI), showed significant differences in normal eye open (NO) and Normal eye closed (NC) tests when ice packs and hot packs were applied (p < .05). In the dynamic balance assessment using Y-balance, significant differences were observed in all values except for pre- and post-intervention in the medial and lateral directions (p < .05). The recovery of proprioceptive sensation showed a significant difference when ice packs were applied (p < .05). In muscle strength, significant differences were observed in all comparisons between measurement time points (p < .05). CONCLUSION: Rest was most effective for static balance, and cold and warm compresses were most effective in recovering dynamic balance. For proprioception, cold compresses were most effective. Muscle strength had a positive effect on recovery in all three intervention methods. These results show that cold and warm compresses can be useful in the recovery of various functions related to muscle fatigue.

한국판 아동용 예일-브라운 강박 척도 ; 신뢰도와 타당도 연구 (DEVELOPMENT OF KOREAN FORM OF CHILDREN'S YALE-BROWN OBSESSIVE-COMPULSIVE SCALE(CY-BOCS):A RELIABILITY AND VALIDITY STUDY)

  • 목 적:본 연구는 소아, 청소년의 강박 증상을 평가하기 위한 도구인 아동용 예일-브라운 강박척도 (Children's Yale-Brown Obsessive Compulsive Scale:Y-BOCS)의 한국판을 제작하여 그 신뢰도와 타당도를 검증하고자 시행되었다. 방 법:강박장애로 진단된 42명의 아동과 청소년들을 대상으로 CY-BOCS를 시행하였다. 평가자간 신뢰도를 측정하기 위하여 20명의 환자를 2명의 평가자가 동시에 면담하였다. 수렴타당도와 변별타당도의 검증을 위해 총괄적인 임상 인상척도-강박장애(Clinical Global Impression-Obsessive Compulsive Disorder:CGI-OCD), 아동용 Leyton 강박 척도(Leyton Obsessional Inventory-Child Version:LOI-CV), 소아우울척도(Child Depression Inventory:CDI), 소아 상태-특성 불안척도(State-Trait Anxiety Inventory for Children:STAI-C)를 시행하였다. 결 과:Cronbach's ${\alpha}$값으로 평가한 내적 일치도는 0.86으로 높게 나타났다. CY-BOCS 전체 점수, 강박사고, 강박행동 소척도 점수에 대한 군내 내적일치도는 각각 0.94, 0.94, 0.84로 우수한 평가자간 일치도를 보였다. CY-BOCS 전체 점수와 CGI-OCD 점수간의 상관관계는 매우 높게 나타났고(r=0.88), LOICV 총점과도 통계적으로 유의한 상관관계(r=0.51)가 관찰되었다. CY-BOCS 전체 점수와 STAI-C의 상태불안 점수간에는 상관관계가 나타나지 않았고(r=0.25), 특성불안 점수(r=0.43) 및 CDI 점수(r=0.49) 와는 유의한 상관관계를 나타냈다. 결 론:본 연구결과, 한국판 CY-BOCS의 높은 타당도 및 신뢰도가 입증되었으며, 향후 강박 증상의 객관적인 평가 및 정량화를 위해 유용하게 사용될 수 있을 것이다.

광주 도시텃밭 토양 환경의 건강성 조사 (A Research of Soil Environmental Health in Urban Garden, Gwangju)

  • BACKGROUND: The objective of this study is to evaluate the soil quality to cultivate crops in urban garden.METHODS AND RESULTS: During the period of six month from March to August 2015, measured eight heavy metals, seven Organic items and Fluorine on seventeen urban sites and thirty-one suburban sites in Gwangju city.METHODS AND RESULTS: The average concentration and range of heavy metal in soil are 0.15 mg/kg(ND-0.6) for Cd, 14.9 mg/kg(1.5-33.3) for Cu, 4.4 mg/kg(0.4-71.8) for As, 0.05 mg/kg(ND-1.366) for Hg, 24.7 mg/kg(13.1-62.7) for Pb, 102.5 mg/kg(49.1-276.4) for Zn and 9.2 mg/kg(ND-90.1) for Ni but Cr6+ is not detected. The average value and range of soil fertility items are 253.5mg/kg(76.6-1766.0) for fluorine, 6.4(4.8-7.7) for pH, 20.3 g/kg(5.0-44.0) for orangic matters, 562.7 mg/kg (28.0-1672.0) for available phosphate, 0.6 cmol+/kg (0.1-2.3) for K, 9.7 cmol+/kg (2.7-22.0) for Ca, 3.0 cmol+/kg (0.9-7.4) for Mg, 1.0 ds/m(0.2-2.9) for conductivity. The concentration of Hg in Suburban area is 0.005 mg/kg lower than 0.134 mg/kg in urban area. Also, the concentration of As, Cd, Ni and Zn is lower than urban area as 32%, 37%, 51%, 71% respectively.CONCLUSION: According to SPI index of soil contamination, 39 sites are first degree and 8 sites are second degree and 1 site(41th) is fourth degree. Pb and Cd are not detected and As is detected tiny amounts in plants grown polluted soil, so heavy metals have not moved to plants.

진단용 X-선 촬영시 부가 필터 및 노출의 변화에 따른 피폭선량 및 영상 화질 비교 연구 (A Comparative Study of Image Quality and Radiation Dose according to Variable Added Filter and Radiation Exposure in Diagnostic X-Ray Radiography)

  • 본 연구는 진단용 X-선 촬영에서 부가필터 두께에 따라 피폭 선량 및 의료영상저장전송시스템에 저장된 영상을 비교 평가하여 환자에게 가장 적은 방사선 피폭을 주면서 진단에 적절한 영상을 얻을 수 있는 촬영 조건을 알아보고자 하였다. 그 결과 면적선량($mGy{\cdot}cm^2$)과 입사표면선량($mGy$)은 1 mmAl+0.2 mmCu의 부가필터에서 가장 낮은 피폭 선량값을, 그리고 0 mmAl (No Filter)에서 가장 높은 피폭 선량 값을 보였다. 그러나 의료영상저장전송시스템으로 전송된 영상을 수치 해석 소프트웨어(MATLAB)로 구현한 히스토그램 결과 영상의 질에는 크게 영향을 미치지 않았다. 따라서 진단용 X-선 촬영 장치를 이용한 영상의학적 검사에서 영상의 질에 영향을 미치지 않고 피폭선량을 경감할 수 있는 최적의 부가필터를 사용함으로서 환자의 피폭 선량을 예측하고 검사 조건을 조절하는 데 있어 유용하게 이용될 수 있을 것으로 판단되었다.
