Purpose: The Oral-anal Transit Test (OTT) is a simple method of obtaining information about colonic transit. We aim to assess the correlation of OTT with the neuromuscular integrity of the colon determined by colonic manometry (CM). Methods: All patients who had OTT followed by CM were evaluated. Less than 6 of 24 markers remaining on OTT was considered normal. CM was performed per previously published guidelines. A normal CM was defined as at least one High Amplitude Propagating Contraction progressing from the most proximal sensor through the sigmoid colon. Results: A total of 34 patients underwent both OTT and CM (44% male, age 4-18 years, mean 11.5 years, 97% functional constipation +/- soiling, Hirschsprung's Disease). Of normal and abnormal OTT patients, 85.7% (6/7) and 18.5% (5/27) respectively had normal CM. When all markers progressed to at least the sigmoid colon, this was 100% predictive against colonic inertia. Greater than 50% of patients with manometric isolated sigmoid dysfunction had markers proximal to the recto-sigmoid. Conclusion: OTT and CM are both valuable studies that assess different aspects of colonic function. OTT can be used as a screening test to rule out colonic inertia. However, the most proximal extent of remaining markers does not predict the anatomical extent of the manometric abnormality, particularly in isolated sigmoid dysfunction.

Background/Aims Functional dyspepsia (FD) and irritable bowel syndrome (IBS) are common gastrointestinal (GI) disorders and these patients frequently overlap. Trimebutine has been known to be effective in controlling FD co-existing diarrhea-dominant IBS, however its effect on overlap syndrome (OS) patients has not been reported. Therefore, we investigated the effect of trimebutine on the model of OS in guinea pigs. Methods Male guinea pigs were used to evaluate the effects of trimebutine in corticotropin-releasing factor (CRF) induced OS model. Different doses (3, 10, and 30 mg/kg) of trimebutine were administered orally and incubated for 1 hour. The next treatment of $10{\mu}g/kg$ of CRF was intraperitoneally injected and stabilized for 30 minutes. Subsequently, intragastric 3 mL charcoal mix was administered, incubated for 10 minutes and the upper GI transit analyzed. Colonic transits were assessed after the same order and concentrations of trimebutine and CRF treatment by fecal pellet output assay. Results Different concentrations (1, 3, and $10{\mu}g/kg$) of rat/human CRF peptides was tested to establish the OS model in guinea pigs. CRF $10{\mu}g/kg$ was the most effective dose in the experimental OS model of guinea pigs. Trimebutine (3, 10, and 30 mg/kg) treatment significantly reversed the upper and lower GI transit of CRF induced OS model. Trimebutine significantly increased upper GI transit while it reduced fecal pellet output in the CRF induced OS model. Conclusions Trimebutine has been demonstrated to be effective on both upper and lower GI motor function in peripheral CRF induced OS model. Therefore, trimebutine might be an effective drug for the treatment of OS between FD and IBS patients.

Obejectives : The purpose of this study is to find out the efficacy of pacemaker potentials of interstitial Cells of Cajal (ICC) by Fermented Lotus Root (FLR) in small intestine. Methods : Enzyme digestions were used to separate the ICC. Using electrophysiological methods, pacemaker potentials were measured and intestinal transit rates (ITR) experiments were conducted to identify in vivo gastrointestinal motility. Results : 1. FLR (0.5-10 mg/ml) caused membrane depolarization by electrophysiological methods. 2. In the case of pretreatment with a Ca2+ free solution and thapsigargin, the pacemaker potential disappeared and in this case, FLR did not have a membrane depolarization reaction. 3. Lowering the concentration of extracellular Na+ concentration stoped the pacemaker potentials and inhibited the reaction caused by FLR. Flufenamic acid also inhibited the reaction by FLR. 4. In mice, ITR was increased by FLR. Conclusions : This study shows that FLR can control ICC by an internal/external Ca2+ and Na+. It also shows that FLR can be a good candidate for gastrointestinal motility medication development.
Probiotics are generally excreted within a few days if their ingestion in feces at the same rate as or even more quickly than a transit marker (meaning not clear). Ability of probiotics to adhere to intestine prolongs their persistence in gastrointestinal tract, allowing them to exert healthful effects longer. Hydrophobicities, zeta potentials, Alcian blue-binding capacities, and sedimentation profiles of Pichia farinosa SKM-1, P. anomala SKM-T, and Galactomyces geotrichum SJM-59 were determined to evaluate characteristic properties of cell surfaces responsible for adhesion. Results of intestinal Caco-2 cell line in vitro and murine intestine in vivo studies revealed these strains exhibit adhesive properties regardless of their cell surface hydrophobicity.
This study was conducted to investigate the effets of chitosan and beef tallow at different level on glucose and lipid metabolism in rats. Dietary fot level was 20% and 40%, and chitosan was given at levels of 0%, 3%, and 5%(wt/wt) of diet. Chitosan supplement tended to decrease the serum total lipids, total cholesterol, and triglycerides. HDL cholesterol and HDL cholesterol : total cholesterol ratio tended to increase with 5% chitosan supplementation. LDL cholesterol and VLDL triglyceride tended to decrease with chitosan supplementation. Lipid concentration of liver and epididymal fat pad(EEP) tended to decrease with medium dietary fat and chitosan treatment. fecal excretion of total lipid and triglyceride exhibited a tendency to increase with high fat levels and chitosan. Length of small intestine and gastrointestinal transit time were not affected by dietary fit levels or chitosan supplements. Motilitone$^{(R)}$ (DA-9701) is a new herbal drug that was launched for the treatment of functional dyspepsia in December 2011 in Korea. The heterogeneous symptom pattern and multiple causes of functional dyspepsia have resulted in multiple drug target strategies for its treatment. DA-9701, a compound consisting of a combination of Corydalis Tuber and Pharbitidis Semen, has being developed for treatment of functional dyspepsia. It has multiple mechanisms of action such as fundus relaxation, visceral analgesia, and prokinetic effects. Furthermore, it was found to significantly enhance meal-induced gastric accommodation and increase gastric compliance in dogs. DA-9701 also showed an analgesic effect in rats with colorectal distension induced visceral hypersensitivity and an antinociceptive effect in beagle dogs with gastric distension-induced nociception. The pharmacological effects of DA-9701 also include conventional effects, such as enhanced gastric emptying and gastrointestinal transit. The safety profile of DA-9701 is also preferable to that of other treatments.
Purpose: The aim of this study was to evaluate the correlation among descriptions regarding one's stool, Bristol stool form scale and colon transit time (CTT) in children with gastrointestinal symptoms, along with the clinical significance of Bristol stool form scale. Methods: 489 patients treated in the pediatric department of Severance hospital with gastrointestinal symptoms between May 2002 to May 2004 were included. We analyzed their age, sex, verbal descriptions of stool, Bristol stool form types, and CTT measured by Metcalf's method. Results: 116 children were under 5 years of age, 202 children between 5.1~10, and 171 children 10 years of age or older. Their mean age was $8.2{\pm}3.9years$. Stools were described as loose in 65 children (13.3%), normal in 221 (45.2%), hard in 188 (38.4%), and mixed (loose+hard) in 15 (3.1%). According to Bristol stool form scale, 57 children(11.7%) were classified as type 1, 66 (13.5%) as type 2, 203 (41.5%) as type 3, 109 (22.3%) as type 4, 36 (7.4%) as type 5, 18 (3.7%) as type 6, and 1 (0.2%) as type 7. Their mean CTT was checked $35.9{\pm}19.5hours$. Though no significant relationship was observed between age and CTT (p=0.4), a significant relationship was noted among patient's stool description, Bristol stool form scale and CTT (p<0.001). However, concordance between stool description and Bristol stool form was relatively low in the loose stool group (29%) and normal stool group (37%) while high in the hard stool group (87%). Conclusion: Bristol stool form scale could be used in the estimation of CTT in clinical practice.

Probiotics is known improve the microenvironment of colon; however, the metagenomic DNA study of its lactic acid bacteria in constipation induced by loperamide is not clearly understood. In the present study, we investigated the reduction of the lactic acid bacteria in case of constipation, in normal and loperamide-induced rat. Lactic acid powder (lactic acid bacteria 19) was prepared from Chong Kun Dang Pharmaceutical Corporation. After 2 weeks of oral administration, the group treated with the higher concentration of lactic acid bacteria ($10^9CFU/mL$ per kg of body weight) following loperamide treatment was the most effective in increasing number, weight, and water content of feces. A similar but significant increase was found in the group treated with lower concentration of lactic acid bacteria ($10^7CFU/mL$ per kg of body weight) after loperamide treatment. The concentrations of acetic acid and propionic acid in feces in the loperamide-induced rat with high concentration lactic acid, were significantly higher than that of others. Furthermore, gastrointestinal transit ratio as well as the length and area of intestinal mucosa were significantly increased after treatment with lactic acid bacteria in loperamide-induced rat. Metagenomics DNA analysis indicated that the microorganism homology in cecum was similar between the groups of normal (NOR) and HIG. Our results show that lactic acid bacteria were effective in improving the constipation.
Dried Citrus unshiu peels (Aurantii Nobilis Pericarpium; ANP) are used as a traditional folk medicine for the treatment of gastrointestinal (GI) motility disorders in East Asia, including Korea. In the present study, an ethanolic extract of ANP (ANP-E) exhibited no significant toxicity in mice, even at an oral dose of 5 g/kg. The effects of ANP-E on GI motor function were investigated by measuring the intestinal transit rate (ITR) of Evans blue in normal mice and mice with experimental GI motility dysfunction (i.e., peritoneal irritation by acetic acid; PIA). In normal mice, ANP-E significantly increased the ITR in a dose-dependent manner. The ITR in the PIA mice was significantly retarded compared to that in the normal mice. However, ANP-E significantly inhibited this retardation in a dose-dependent manner. Furthermore, in all the models, the potency of ANP-E appeared to be same or higher than that of cisapride, which was used predominantly for the treatment of various GI motility disorders in humans in the 1900s but was removed from the market in 2000 due to fatal side effects. The results suggest that ANP-E has potential as a new prokinetic agent that could be used as a substitute for cisapride.

Aqueous extract of antidiarrhoeal formulation (ADF) containing Holarrhena antidysentrica, Aegle marmelos and Punica granatum was investigated for antidiarrhoeal activity against charcoal-induced gut transit, serotonin-induced diarrhoea and $PGE_2-induced$ small intestine enteropooling in rats. The control, standard and test groups of experimental animals were administered with normal saline (p.o.), diphenoxylate hydrochloride (5 mg/kg, p.o.) and ADF (250 mg and 500 mg/kg, p.o.) respectively except the control group of $PGE_2-induced$ small intestine enteropooling which received only 5% ethanol in normal saline (i.p.). Charcoal (10 ml/kg, p.o.) and serotonin $(600\;{\mu}g/kg,\;i.p.)$ were administered after 30 min, while $PGE_2\;(100\;{\mu}g/kg,\;p.o.)$ was administered immediately afterwards. The distance traveled by charcoal in small intestine was measured after 15 and 30 min of charcoal administration, diarrhoea was observed every 30 min for six hour after serotonin administration and the volume of intestinal fluid was measured after 30 min of $PGE_2$ administration. Oral administration of ADF significantly inhibited the frequency of defaecation and decreased the propulsion of charcoal meal through the gastrointestinal tract, reduced the wetness of faecal dropping in serotonin-induced diarrhoea and also reduced the $PGE_2-induced$small intestine enteropooling. ADF may have potential to reduce the diarrhoea in rats.
